    L’oxymétrie transcutanée permet une analyse objective, non-opérateur dépendant de la présence d’une ischémie de repos ou d’effort. Ce travail vise à développer de nouvelles applications de l’oxymétrie transcutanée en se basant sur des explorations réalisées quotidiennement (TCPO2 d’effort dans la claudication ou TCPO2 de repos dans l’ischémie permanente chronique) mais également en l’appliquant à une nouvelle pathologie (le syndrome du défilé thoraco-brachial). Successivement, il est rapporté l’intérêt de l’analyse du temps de récupération dans les localisations hors mollet chez les patients présentant une artériopathie au stade de claudication d’effort. Secondairement, l’applicabilité et la validation de l’oxymétrie transcutanée versus un gold standard (artériographie) dans le syndrome du défilé thoraco-brachial. Enfin, l’intérêt de l’analyse continue des courbes thoraciques lors de la réalisation d’une oxymétrie transcutanée de repos chez les patients présentant une ischémie critique, en rapportant la présence de mouvements sinusoïdaux et son impact en termes de morbi-mortalité. Ce travail a permis de développer des applications originales de l’oxymétrie transcutanée afin d’améliorer la prise en charge globale de ces patients.Transcutaneous oximetry is an objective and non-operator dependent investigation assessing rest and exercise ischemia. This work aims to focus on new transcutaneous oximetry applications based on daily investigations (as exercise transcutaneous oximetry in peripheral arterial disease or rest transcutaneous oximetry in critical limb ischemia) but also new area of interest (thoracic outlet syndrome). Firstly, we reported non-calf hemodynamic recovery interest in patient with arterial limb claudication. Secondly, we studied the feasibility and reliability of transcutaneous oximetry in thoracic outlet syndrome and its validation versus gold standard (arteriography). Finally, we reported sinusoidal changes in transcutaneous oxygen pressure, suggesting Cheyne-Stockes respiration, appearing to be frequent and of poorer prognosis in critical limb ischemia patients.     Nous avons souhaité caractériser l’influence de la structure et de la fonction microcirculatoire étudiées en préopératoire sur les complications et le devenir postopératoires après une chirurgie cardiaque programmée sous circulation extra-corporelle (CEC). Nous avons conduit une étude de cohorte prospective monocentrique incluant 60 patients de chirurgie cardiaque sous CEC, valvulaire ou coronarienne programmée. La fonction microcirculatoire préopératoire a été évaluée par la mesure de la vaso-réactivité induite par différents tests et mesurée par laser Speckle. Cette étude a montré que certaines caractéristiques de la réponse microvasculaire dépendante de l’endothélium étaient associées à des complications postopératoires. Nos résultats suggèrent une dissociation structure-fonction de la microcirculation dans la période périopératoire. La réserve microcirculatoire préopératoire évaluée par la réponse microvasculaire fournirait des informations pronostiques sur l'évolution postopératoire et pourrait être liée à la pathogenèse des dysfonctions d’organes. Compte tenu de sa faisabilité et de son caractère non invasif, l'évaluation cutanée de la fonction microcirculatoire semble être une technique prometteuse. Les paramètres précis du profil de patients à risque de dysfonctions d’organes devront être définis par une étude de cohorte multicentrique de construction/validation d’un score de stratification du risque. À l'ère de la médecine personnalisée, les données préopératoires sur la microcirculation pourraient être intégrées dans la réflexion sur la stratégie anesthésique et chirurgicale.Our objective was to study the impact of preoperative microcirculatory structure and function on postoperative complications and outcomes following elective cardiac surgery under cardiopulmonary bypass (CPB). We conducted a monocentric, prospective, cohort study including 60 cardiac surgery patients undergoing scheduled valve or coronary arterial by-pass grafting surgery undergoing CPB. Preoperative microcirculatory function was assessed by measuring vasoreactivity induced by various tests examined through laser Speckle. This study showed that certain characteristics of the endothelium-dependent vasodilation were associated with postoperative complications. Our findings suggest a separation between the structure and function of the microcirculation in the perioperative period. Assessing preoperative microcirculatory functional reserve via microvascular reactivity could provide prognostic insight into postoperative outcomes and may be associated with organ dysfunction pathogenesis. Cutaneous microcirculatory function assessment appears to be an encouraging technique due to its feasibility and non-invasive characteristics. The exact parameters of the risk profile for organ dysfunction will need to be defined by a multicenter cohort study to create/validate a risk stratification score. In the era of personalized medicine, preoperative microcirculation data could be incorporated into the anesthetic and surgical plan
