    Quality and user satisfaction scores for prosthetic limbs provided in a fitting camp, Cambodia

    Objective: prosthetic limbs can enable mobility, independence and social participation. Fitting camps provide devices to many individuals in intensive sessions. This study addressed concerns regarding device design and fabrication at a fitting camp for 525 people in Cambodia.Methods: we assessed device quality and satisfaction at prosthetic limb provision, and device usage and preference at 3-month follow-up.Findings: many devices failed to meet International Society for Prosthetics and Orthotics standards for usage, workmanship, and client satisfaction. Interviews revealed dissatisfaction with function (26%), workmanship (34%), and fit (56%). At follow-up, 36% of clients reported discomfort or pain. 80% with a previous prosthesis preferred it. 81% reported not very often, rarely, or never using their new device, whereas 87% of clients with a previous device reported often or always using it. 29% were using a previous device observed at the camp as unused, broken, painful, or poorly fitting, suggesting many rely on an inadequate or potentially dangerous prosthesis.Conclusion: these findings indicate that shortcomings in quality and satisfaction of the studied prosthesis system persist, and raise new questions about inclusiveness of patient selection and effectiveness of funding use in the intensive delivery format. Inadequate training and aftercare may exacerbate these issues, burdening local rehabilitation services or leaving vulnerable clients without support. Camps may be more appropriate when need is clearly demonstrated, should integrate fully with existing services, and provide adequate materials and components for repairs. Screening patients for need is essential, as is engaging the practitioners who will continue with their care

    Understanding maintenance, repair and replacement of prosthetic limbs using routinely-collected data: a retrospective study over three decades in Cambodia

    Prosthetic limbs deliver major quality of life and socioeconomic benefits for people with amputation, particularly in low-resource settings. The value of administrative data analysis to enable sustainable health care improvement is established, but there has been limited research into the failure, repair, and replacement of prosthetic limbs. Survivorship data is sparse and highly variable, and rarely addresses differences between demographic groups.Therefore, we investigated the distribution of time between device delivery, repair and replacement for a Cambodian cohort, considering the influence of a range of service delivery, user demographic and health characteristics. We conducted Kaplan-Meier survival analysis, and a Cox model to compare repair and replacement likelihood between groups.The study explored 14,822 device deliveries to 6,986 clients, with median 3 devices/person (interdecile range IDR 1–9), and 22,878 repairs, median 1 repair/device (IDR 0–4). The median device survival before repair was 237 days (IDR 38–854), and replacement was 727 days (IDR 208–2154). Devices used by people in more active occupations were repaired earlier, and devices were replaced earlier when used by children, replaced later for upper- than lower-limb devices, and replaced earlier for volume change than for wear and tear. Several less intuitive trends were revealed such as different preferences or capacities between clinics for device repair vs. replacement, and earlier device repair and replacement for women than men.Prosthetic limb repair and replacement is influenced both by the device’s durability and the user’s access to well-resourced physical rehabilitation services. A worn-out device may indicate poor quality, or the opposite: that it fitted well and enabled great mobility. However, such analysis may enable us to identify groups who are less well-served by current devices or rehabilitation models, and contribute to cost effectiveness analysis of current services. Furthermore, the findings represent benchmark data against which engineers could measure new technologies, to ensure that innovation justifies its inherent risk by offering a genuine improvement which balances functionality, cost, and durability

    Going Beyond Counting First Authors in Author Co-citation Analysis

    The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed.     “Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship

    Appropriate Similarity Measures for Author Cocitation Analysis

    We provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature.     Dispelling the Myths Behind First-author Citation Counts

    We conducted a full-scale evaluative citation analysis study of scholars in the XML research field to explore just how different from each other author rankings resulting from different citation counting methods actually are, and to demonstrate the capability of emerging data and tools on the Web in supporting more realistic citation counting methods. Our results contest some common arguments for the continued use of first-author citation counts in the evaluation of scholars, such as high correlations between author rankings by first-author citation counts and other citation counting methods, and high costs of using more realistic citation counting methods that are not well-supported by the ISI databases. It is argued that increasingly available digital full text research papers make it possible for citation analysis studies to go beyond what the ISI databases have directly supported and to employ more sophisticated methods
