Going Beyond Counting First Authors in Author Co-citation Analysis
The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation
counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings
are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that
only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into
account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed. Variations on the Author
“Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. Appropriate Similarity Measures for Author Cocitation Analysis
We provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature. Prospective analysis of consecutive lung function tests at a large teaching hospital
Diese Studie untersuchte 1008 konsekutive Patienten, die zur Durchführung einer Lungenfunktionsuntersuchung in einem großen Lehrkrankenhaus angemeldet wurden. Dies geschah mit dem Ziel eine Deskription des Patientenkollektives zu liefern und obstruktive Atemwegserkrankungen in der präoperativen Diagnostik zu identifizieren. Neben den Lungenfunktionsparametern wurden weitere Informationen zu den Patienten erhoben und mit der Allgemeinbevölkerung verglichen.1008 consecutive lung function tests, performed at a university hospital were analysed. Dispelling the Myths Behind First-author Citation Counts
We conducted a full-scale evaluative citation analysis study of scholars in the XML research field to explore just how different from each other author rankings resulting from different citation counting methods actually are, and to demonstrate the capability of emerging data and tools on the Web in supporting more realistic citation counting methods. Our results contest some common arguments for the continued
use of first-author citation counts in the evaluation of scholars, such as high correlations between author rankings by first-author citation counts and other citation
counting methods, and high costs of using more realistic citation counting methods that are not well-supported by the ISI databases. Target controlled infusion of propofol in combination with entropy monitoring for sedation during flexible bronchoscopy
Hintergrund: Für die zunehmende Anzahl diagnostischer und interventioneller Techniken in der flexiblen Bronchoskopie sind entsprechende Vorangehensweisen für die Sedierung notwendig, um sowohl für den Untersucher, als auch für die Patienten sichere und praktikable Bedingungen zu schaffen. Die Target Controlled Infusion (TCI) von Propofol ist eine neue Technik, deren Anwendung auch zur Sedierung in der flexiblen Bronchoskopie bereits beschrieben wurde. Zudem wurden EEG-basierte Methoden entwickelt, welche die Sedierungstiefe während einer Untersuchung visualisieren können. In der vorliegenden Studie soll untersucht werden, ob die Kombination aus TCI und EEG-basiertem Monitoring der Sedierungstiefe die Sedierungsqualität bei der Untersucher geführten Sedierung verbessern kann.
Methoden: Insgesamt wurden 51 Patienten mit einer TCI von Propofol sediert und eine diagnostische oder interventionelle Bronchoskopie durchgeführt. Die Steuerung einer TCI nach der Effektkompartiment-Konzentration anhand des Modells von Schnider wurde bei 43 Patienten angewendet, die Steuerung einer TCI nach der Plasmakompartiment-Konzentration anhand des Modells von Marsh wurde bei 8 Patienten eingesetzt. Der Zeitpunkt, zu dem der Patient nicht mehr auf verbale Aufforderungen reagierte, wurde als Bewusstseinsverlust festgehalten und entsprach dem Beginn der Prozedur.
Die Anwendung des Entropie-Monitorings während der Untersuchung, ermöglichte die Aufzeichnung der Sedierungstiefe anhand der State-und Response-Entropie. Weiterhin wurden die Biosignale, die Effektkompartiment- bzw. Plasmakompartiment-Konzentration von Propofol sowie unerwünschte Ereignisse aufgezeichnet.
Nach der Untersuchung bewertete der Untersucher die Sedierungstiefe, aufgetretene Hustenepisoden und Untersuchungsbedingungen auf einer fünfstufigen Likert-Skala als sehr gut, gut, normal, schlecht oder sehr schlecht.
Ergebnisse: Während der Untersuchung betrug der Mittelwert der Effektkompartiment Konzentration (± Standardabweichung) für die TCI nach dem Modell von Schnider 3,89 µg/ml (± 1,05 µg/ml) Propofol und die mittlere Plasmakompartiment Konzentration für die TCI nach dem Modell von Marsh betrug 3,06 µg/ml (± 0,8 µg/ml) Propofol. Ohne zusätzlichen Sauerstoff zeigten alle Patienten eine mittlere Sauerstoffsättigung (± Standardabweichung) von 91,37 % (± 4,60 %). Während der Prozedur erhielten alle Patienten zusätzlichen Sauerstoff und die mittlere Sauerstoffsättigung betrug 93,75 % (± 5,01 %). Aus Sicherheitsgründen wurde bei 16 Patienten vorübergehend ein Endotrachealtubus eingelegt.
Während dem Bewusstseinsverlust wurde eine State-Entropie von 70,59 (± 18,4) und eine Response Entropie von 78,24 (± 19,7) gemessen. Sowohl State-als auch Response-Entropie zeigten vom Beginn der Sedierung bis zum Beginn der Prozedur einen relativ konstanten Verlauf. Während der Bronchoskopie wurden sowohl State-als auch Response-Entropie stark, sowohl durch Untersuchungsereignisse als auch durch Husten beeinflusst und entsprachen nicht mehr der klinischen Einschätzung der Sedierungstiefe.
Der Mittelwert (± Standardabweichung) der Bewertung durch den Untersucher betrug 2,06 (± 0,81) für die Sedierungstiefe, 2,35 (± 0,74) für Hustenereignisse und 1,92 (± 0,72) für Untersuchungsbedingungen.
Schlussfolgerung: TCI von Propofol ist eine praktikable Möglichkeit, um gute Untersuchungsbedingungen zur Sedierung während der flexiblen Bronchoskopie zu ermöglichen. Die Sicherheit dieses Konzeptes sollte in größeren Studien überprüft werden um Komplikationen zu vermeiden, insbesondere, wenn unbeabsichtigt eine zu tiefe Sedierung herbeigeführt wird. Das Entropie Monitoring während der flexiblen Bronchoskopie stellt einen vielversprechenden Ansatz zur Überwachung der Sedierungstiefe da. Aufgrund der Artefaktanfälligkeit ist sowohl bei der State-Entropie als auch bei der Response Entropie aber die Aussagekraft während der Prozedur eingeschränkt.Background: The increasing development of diagnostic and interventional techniques in flexible
bronchoscopy requires viable sedation concepts to create safe and practicable conditions for the
patient as well as for the endoscopist. Target controlled infusion (TCI) of propofol is a possible technique to support sedation in flexible bronchoscopy. In addition, EEG-based methods have been developed to monitor sedation levels during the procedure. In this study we questioned, whether TCI in combination with the quantitative measurement of sedation depth can optimize sedation quality as a novel concept in
bronchoscopist-administered sedation.
Methods: 51 patients were sedated with TCI of propofol for diagnostic or interventional bronchoscopies. Effect-site TCI by Schnider was used in 43 patients and plasma-site TCI by Marsh was used in 8 patients. In addition, entropy monitoring was utilized to quantify sedation depth. The procedure was started after clinical loss of consciousness (LOC), defined as loss of verbal commands. During the examination, the resulting state entropy and response entropy values were mapped. Biometric data, effect-site / plasma-site concentrations of propofol and adverse events were also recorded.
After the procedure the endoscopists graded sedation depth, coughing and examination conditions with a five point Likert scale.
Results: During procedure, the mean effect-site concentration (± standard deviation) for TCI by Schnider was 3,89 µg/ml (± 1,05 µg/ml) of propofol and the mean plasma-site concentration for TCI by Marsh was 3,06 µg/ml (± 0,8 µg/ml) of propofol. Without supplementary oxygen, the mean oxygen saturation (± standard deviation) was 91,37 % (± 4,60 %) for all patients. During procedure all patients received supplementary oxygen and the mean oxygen saturation was 93,75 % (± 5,01 %). For safety reasons, 16 patients received a temporary endotracheal tube.
During LOC the observed state and response entropy was 70,59 (± 18,4) and 78,24 (± 19,7).
SE and RE showed a stable and concordant course during the induction period until LOC. During bronchoscopy, entropy levels were predominantly a function of arousals and facial muscle activity (e.g. induced by cough events) which confounded the correlation between entropy and sedation depth.
Sedation depth, coughing and examination conditions were graded (± standard deviation) 2,06 (± 0,81), 2,35 (± 0,74) and 1,92 (± 0,72), respectively.
Conclusion: TCI of propofol is feasible and provides good examination conditions for sedation during flexible bronchoscopy, although, the safety of this concept should be further evaluated, especially when sedation gets deeper than initially intended.
Entropy monitoring supporting TCI of propofol for sedation in flexible bronchoscopy is an achievable
measure to quantify sedation quality during the induction period, however, different stimuli
affect the EEG-based measure and limit its use while the procedure is in progress
Screening of patients with connective tissue disease and rheumatic diseases for pulmonary arterial hypertension and interstitial lung disease: a prospective study.
Die Mitbeteiligung der Lunge bei Erkrankungen des rheumatischen Formenkreises ist ein erheblicher prognostischer Parameter und daher entscheidend für die Morbidität und Mortalität der Patienten. In dieser prospektiven Beobachtungsstudie untersuchten wir 39 Patienten mit rheumatischen Erkrankungen auf das Vorliegen einer pulmonal arteriellen Hypertonie (PAH) und einer interstitiellen Lungenerkrankung (ILD) im Sinne einer Lungenfibrose. Als Screening-Untersuchungen wurden nicht-invasive Methoden eingesetzt. Die primären Endpunkte der Studie für die Diagnose der Lungenfibrose bzw. der pulmonalen Hypertonie konnten nur in einem Fall bestätigt werden. Im gesamten Kollektiv zeigten sich jedoch indirekte Zeichen einer PAH und ILD sowie Einschränkungen der Belastbarkeit, Lebensqualität und Rechtsherzbelastungszeichen. Somit sind Anzeichen von Lungenfunktionseinschränkungen und Lungenerkrankungen bei Patienten mit rheumatischen Erkrankungen ernstzunehmende BegleiterscheinungenThe involvement of the lung in rheumatic diseases is a prognostic factor for morbidity and mortality. In this prospective study we examined 39 patients with rheumatic diseases for pulmonary arterial hypertension and interstitial lung disease as in a pulmonary fibrosis. Only non-invasive screening methods were applied. The primary endpoints of this study for pulmonary hypertension and pulmonary fibrosis were only detected in one patient. Although in the cohort as whole, a reduction of quality of life, right heart function and physical fitness were seen. Cross-Sectional Survey on Bronchoscopy in Germany – The Current Status of Clinical Practice
Wir führten eine nationale Umfrage zum aktuellen Stand der Bronchoskopie durch. Es wurden 301965 Bronchoskopien von 2158 Ärzten erfasst, was es zur größten Umfrage zu diesem Thema macht. In 88% werden die Patienten sediert, vorzugsweise mit Propofol und Midazolam. 74% machen TBB bei ASS-Einnahme und 8% bei dualer Plättcheninhibition. 9% der Untersuchungen werden mithilfe von EBUS durchgeführt. Im Vergleich zu früheren Umfragen hat die Erfahrung und der Umgang mit interventionellen Methoden zugenommen.We conducted a nation-wide survey on the current status of bronchoscopy in Germany.
