Le nævus épidermique verruqueux inflammatoire linéaire
Le nævus épidermique verruqueux inflammatoire linéaire (NEVIL) est une affection rare correspondant à des hyperplasies épidermiques bénignes qui se présentent cliniquement sous forme de lésions linéaires unilatérales hyperkératosiques. Une adolescente âgée de 16 ans, sans antécédents, présentait depuis la naissance une lésion, papuleuse brunâtre au niveau l'hémiface antérieure droite du cou avec une distribution Blaschko linéaire (a); et depuis 2 ans une lésion nodulaire kératosique verruqueuse, prurigineuse, grisâtre, d'aspect psoriasiforme par endroits au niveau de la région pariéto-occipitale droite du cuir chevelu (b). Le diagnostic de NEVIL était évoqué puis confirmé par la biopsie cutanée. Les dermocorticoïdes entraînaient une amélioration modeste. Les lésions cutanées résultent d'un phénomène de mosaïcisme génomique. Elles ont l'aspect de papules kératosiques verruqueuses, disposées en traînées, parfois reliées entre elles. Elles peuvent aussi se développer sur plusieurs territoires cutanés et sont parfois congénitale. L'évolution est chronique entrecoupée de poussées exsudatives et prurigineuses. Une extension secondaire des lésions est aussi possible comme chez notre patiente. Des anomalies diverses peuvent être associées comme l'aplasie d'une partie de membre. La distinction avec un psoriasis linéaire peut être difficile histologiquement. Les corticoïdes locaux sont peu efficaces, et l'exérèse chirurgicale est parfois pratiquée. Rhomboid glossitis caused by candida?
Rhomboid glossitis (RG), first described by Brocq in 1914, is an uncommon benign abnormality of the tongue, most frequently affecting men. His etiology is unknown, although it has been proposed that it may be derived from chronic candidiasis, or that it may be of embryological, inflammatory, or even immunological origin. It appears as a rounded or rhomboid painless plaque with well-defined margin, intense reddish or pinkish in colour due to atrophy or depapillation, and firm to palpation. The diagnosis of rhomboid glossitis is largely based on clinical examination, though histopathological studies of biopsies may be required if clinical appearance does not allow other possible diagnoses to be ruled out. No treatment is required for asymptomatic cases. We report a case of a female patient aged 24 years, consulted for a painless plaque in dorsum of the tongue. Clinical examination indicated an erythematous plaque, located anterior to the lingual "V" in left paramedial position, elongated and slightly fissured, about 2 x 0.5 cm in size. Candida culture tested positive. The lesion was thus diagnosed as RG with presence of C. albicans. Treatment was with fluconazole for 2 weeks at 50 mg per day. A complete disappearance of the lesion was noted. After 12 months follow-up, no recurrence was reported.Pan African Medical Journal 2016; 2
Une dermatophytie en "rosette"
Dermatophyties are diseases caused by keratinophilic fungi. We here report the case of a 8-year old vaccinated female child having extremely itchy vesicular-erosive lesions grouped in rosette-like pattern at the level of the tail of the left eyebrow (A), the nostrils, the extensor surfaces of the forearm and the upper part of the chest and back associated with hypochromic scars (B), without digestive symptoms since 3 months. Moreover skin appendage examination showed alopecia of the scalp(C) with shaved head. According to child’s mom this condition had started one year before and evolved since then. Skin lesions primarily evoked dermatitis herpetiformis, but the presence of patchy alopecia and the wish to avoid skin biopsy, which would have been an invasive procedure for the child, led clinicians to perform mycological examination which was in favor of hair and skin parasitism caused by Microsporum Canis. Treatment with topical and systemic griseofulvin at a dosage of 20 mg/kg/day for 6 weeks was established with very good clinical outcome. Dermatophyties are diseases caused by microscopic filamentous keratinophilic fungi. Clinically, skin lesions usually appear as single or multiple, rounded or polycyclic patches with extremely evocative erythematous scaly vesicular edge, centrifugal extension and central healing. Sarcome fibromyxoïde de localisation inhabituelle
Le sarcome fibromyxoïde de bas grade est une tumeur rare des tissus mous. Nous rapportons l'observation d'un patient de 54 ans consultant pour une tumeur du dos du pied droit évoluant depuis 2 ans. L'examen dermatologique notait la présence d'une tumeur bourgeonnante sous cutanée prenant la quasi-totalité du dos du pied droit, d'environ 10 cm de grand axe, douloureuse à la palpation et saignant au moindre contact. Le reste de l'examen clinique était sans particularités. L'étude histologique et immuno-histochimique d'une biopsie cutanée objectivait un processus tumoral d'allure sarcomateuse à cellules fusiformes en faveur d'un sarcome fibromyxoïde de bas grade anti-Vimentine+ anti CD34+ anti PS100-anti EMA- anti-Bcl2-. Le patient a été adressé en chirurgie et a bénéficié d'une amputation de l'avant pied de type Charcot. Le sarcome fibromyxoïde de bas grade a été décrit pour la première fois en 1987 par Evans. Moins de 150 cas sont décrits dans la littérature. Il s'agit d'une tumeur sous-diagnostiquée, souvent confondue avec d'autres entités, principalement localisée dans les tissus mous profonds des régions proximales des membres et du tronc. Histologiquement, cette variété se caractérise par l'alternance de zones myxoïdes et fibreuses constituées de cellules fusiformes. L'immunohistichimie est primordiale pour confirmer le diagnostic. Cette tumeur a un potentiel de récidive locale et de métastases même après plusieurs années, d'où l'intérêt d'un suivi à long terme. Notre observation clinique est particulière par la rareté du sarcome fibromyxoïde de bas grade ainsi que par sa localisation inhabituelle distale au niveau du pied.Pan African Medical Journal 2015; 2
Going Beyond Counting First Authors in Author Co-citation Analysis
The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation
counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings
are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that
only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into
account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed. Reasons for these effects are discussed
Variations on the Author
“Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship
Appropriate Similarity Measures for Author Cocitation Analysis
We provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature. Dispelling the Myths Behind First-author Citation Counts
We conducted a full-scale evaluative citation analysis study of scholars in the XML research field to explore just how different from each other author rankings resulting from different citation counting methods actually are, and to demonstrate the capability of emerging data and tools on the Web in supporting more realistic citation counting methods. Our results contest some common arguments for the continued
use of first-author citation counts in the evaluation of scholars, such as high correlations between author rankings by first-author citation counts and other citation
counting methods, and high costs of using more realistic citation counting methods that are not well-supported by the ISI databases.
