    Mide kanseri şüphesi olan hastaların çok dedektörlü bilgisayarlı tomografi ile değerlendirilmesi

    Amaç: Dicle Üniversitesi Tıp Fakültesi Gastroenteroloji bölümünde mide kanseri öntanısı ile endoskopi eşliğinde biyopsi yapılan hastaların, patoloji sonuçları ile çekilen çok kesitli bilgisayarlı tomografi (ÇKBT) bulgularını retrospektif olarak değerlendirerek, ÇKBT'nin mide kanseri tanısına katkısını araştırmak istedik. Materyal ve metod: Bu çalışmaya Eylül 2008- Aralık 2014 tarihleri arasında Dicle Üniversitesi Tıp Fakültesi Gastroenteroloji Endoskopi Ünitesine başvurup endoskopi yapılan ve biyopsi alınan hastalar dahil edildi. Aynı hastaların batın pelvik ÇKBT çekimleri değerlendiridi. Çekimler 64 dedektörlü Philips Brillance ve 16 dedektörlü Toshiba Activion cihazlarıyla; ksifoid proçesten başlayarak simfisiz pubise kadar inguinal kanal orifislerini de içine alacak şekilde nefes tutturularak yapıldı. Radyolojik bulgular, patoloji verileri altın standart kabul edilip istatistiksel veriler ile karşılaştırıldı. Bulgular: Patoloji sonucu malign bulunan 129 hastanın 125 'inde (%96) BT bulguları malign bulgular lehine yorumlandı. Patoloji sonucu benign bulunan 22 hastanın 7 'sinde (%30) BT bulguları benign bulgular lehine yorumlandı. BT ' nin mide kanseri saptama sensitivitesi %96 olarak bulundu. BT ' nin spesifite değeri %30 olarak bulundu. Opere edilen 28 hastanın 22 ' sinde (%78) perigastrik bölge lenf nodu metastazı mevcuttu. Lenf nodu metastazı mevcut hastaların ÇKBT 'de perigastrik bölgedeki ölçülen LAP 'ların kısa çapları en düşük 6 mm en yüksek 22 mm olarak bulundu . Sonuç: Mide kanseri ön tanısı ile başvuran hastaların endoskopik biyopsi sonuçları ile % 96 oranında ÇKBT bulguları arasında korelasyon bulunmaktadır. Mide kanserini saptama duyarlılığı yüksek olmasına rağmen lenf nodu metastazlarında aynı başarı mevcut değildir. ÇKBT ' nin yüksek değerlere varan mide kanseri saptama oranları, özellikle endoskopi gibi bazı hastalarca travmatik bulunan işlemler yerine kullanılabilinmesinin önünü açmaktadır. Anahtar kelimeler : ÇKBT, mide kanseri, patolojiAim: To retrospectively assess multidetector computed tomography (MDCT ) findings of the subjects admitted to Dicle University Faculty of Medicine and underwent endoscopic investigation at Department of Gastroenterology with preliminary diagnosis of gastric cancer, and thus to determine the role of MDCT in diagnosing gastric cancer. Material-Methods: Subjects that underwent gastric endoscopy and biopsy procedure at department of Gastroenterology between September 2008 and December 2014 were included In this study. All abdominopelvic MDCT studies were done at. All CT studies were carried out at CT suits (Brilliance 64 detector and Toshiba Activion 16 detector multidetector CT systems ) in our Department of Radiology. Cases were scanned from xiphoid process through symphisis pubis including inguinal orifice with inspiratory breath holding technique. Pathological results were taken as gold standart and were statistically compared with radiological findings. Findings: CT findings of 125 (96%) cases out 129 subjects with pathologically proved malignant pathology were interpreted in favor of malignant findings. sensitivity of CT in detection of gastric cancer was 96% . CT findings of 7 (30%) cases out of 22 subjects with pathologically proved benign lesions were interpreted in favor of benign findings. CT's specificity value was found to be 30%. Perigastric lymph node metastases were present in 22 (78%) cases out of 28 subjects who underwent surgical operation. On MDCT studies of those with lymph node metastases, perigastric LAP' s short diameter was ranged between 6 - 22 mm. Results: The correlation between endoscopic biopsy and MDCT findings of patients admitted with preliminary diagnosis of gastric cancer was 96%. Despite the high sensitivity in detection of gastric cancer the same success rate was not reached in detecting the lymph node metastases. In some cases, MDCT can replace endoscopical investigation which found to be traumatic by some individuals. Keywords : MDCT , gastric cancer, patholog

    Serebral vasküler varyasyonların 64 kesitli ÇKBT ile görüntülenmesi

    Amaç: Herhangi bir sebeple, ileri teknoloji ürünü olan 64 dedektörlü bilgisayarlı tomografi cihazıyla beyin BT anjiyo incelemesi yapılan hastalar- da serebral vasküler varyasyonları incelemek, bu varyasyonların sıklık oranlarını saptamaktır. Gereç ve Yöntem: Dicle Üniversitesi Tıp Fakültesi Hastanesi Radyoloji Ünitesinde Nisan 2008 – Mart 2010 tarihleri arasında çekilen 500 hasta (253 kadın,247 erkek) çalışmaya dahil edilmiştir. Beyin BT anjiyo protoko- lü ile çekilen hastalar, iş istasyonu üzerinden multiplanar, MİP ve VR görüntüler yardımıyla değerlendirilmiştir. Bulgular: Serebral vasküler varyasyonlar açısından 500 hasta incelendi. Bu hastaların 253’ ü (% 50.6) kadın, 247’ si (% 49.4) erkekti. 500 hasta- nın 73’ ünde (% 14.6) herhangi bir varyasyona rastlanmadı. 500 hastada toplam 773 varyasyon saptandı. Varyasyonlar Willis poligonundaki varyas- yonlar, fenestrasyonlar ve diğer varyasyonlar şeklinde sınıflandırıldı. Willis poligonunda sık varyasyon görülmekte olup, en sık rastlanan varyasyon PCOA hipoplazisi olarak izlendi. Sonuç: Serebral vasküler varyasyonlar sıkça karşılaştığımız, herhangi bir hastalık nedeniyle çekilen beyin BT anjiyografi incelemeleri sırasında tesadüfen saptadığımız olgulardır. Bu anomalilerin bilgisayarlı tomografi görünümlerinin bilinmesi doğru tanımlanmasını sağlar. Doğru tanımlama ayırıcı tanı açısından önemlidir, ayrıca cerrahi sırasında varyasyonların bilinmesi majör komplikasyonların önlenmesi için de gereklidir.Objective: The objective of this study is to investigate cerebral vascular variations in patients observed with BT angio for any reason using computed tomography with 64 detectors which is a high tech product. Material and Methodology: 500 patients (253 women, 247 men) had CT scan in the period of April 2008- March 2010 at Dicle University Medicine Faculty Hospital Radiology Unit were surveyed in this study. Patients had CT scan with brain BT angio protocol were evaluated using multiplanar, MİP and VR images in work station. Findings: 500 patients were surveyed in terms of cerebral vascular variations. 253 of them were women (50,6%) and 247 of them were men (49,4%). Any variation could not detected among 73 (14,6%) of 500 patients. 773 variations were totally detected among 500 patients. Variations were categorized as Willis polygon, fenestration and other variations. Frequent variations were detected at Willis polygon and PCOA hypoplasia was detected as prevalent variation. Result: Cerebral vascular variations are phenomenon frequently met and detected during cerebral BT angiography screening scanned for any illness. Analyze of computed tomography images of these anomalies lets correct definition of these anomalies. Correct definition is important in the sense of differential diagnosis, and detection of variations during surgery is necessary for preventing major complication

    Going Beyond Counting First Authors in Author Co-citation Analysis

    The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed. Reasons for these effects are discussed

    “Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship

    Appropriate Similarity Measures for Author Cocitation Analysis

    We provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature.     Dispelling the Myths Behind First-author Citation Counts

    We conducted a full-scale evaluative citation analysis study of scholars in the XML research field to explore just how different from each other author rankings resulting from different citation counting methods actually are, and to demonstrate the capability of emerging data and tools on the Web in supporting more realistic citation counting methods. Our results contest some common arguments for the continued use of first-author citation counts in the evaluation of scholars, such as high correlations between author rankings by first-author citation counts and other citation counting methods, and high costs of using more realistic citation counting methods that are not well-supported by the ISI databases. It is argued that increasingly available digital full text research papers make it possible for citation analysis studies to go beyond what the ISI databases have directly supported and to employ more sophisticated methods

    We have done our best to complete the author checklist relating to the use of animals in the hut study. Note that the objective for the hut study was to evaluate the IRS treatment applications for residual efficacy against Anopheles mosquitoes, including the local An. coluzzii mosquito population. Cows were only used to attract mosquitoes into the huts and no tests were carried out directly on the cows. The author checklist is intended for use with studies where experiments are carried out on animals, which is why we have had such difficulty in completing this for the hut study, as many of the questions do not relate to how the cows were used
