    Diagnoserelaterte grupper (DRG) er et pasientklassifiseringssystem hvor sykehusopphold eller polikliniske konsultasjoner klassifiseres i grupper som er medisinsk meningsfulle og ressursmessig tilnærmet homogene. DRG-systemet kan danne grunnlag for beskrivelse, styring og finansiering av spesialisthelsetjenesten. DRG-grupperingen baserer seg i hovedsak på opplysninger om pasientens tilstand, utførte prosedyrer, kjønn, alder og utskrivningsstatus. For pasienter som har mottatt livreddende og ressurskrevende behandling i en intensivenhet er det flere indikasjoner på at en DRG-logikk som baserer seg på de nevnte kriterier, ikke godt nok ivaretar kompleksiteten eller forskjellene medisinsk og ressursmessig sett. I denne masteroppgaven er det derfor sett nærmere på hvordan slike opphold grupperes i dag, og om bruk av skår for graden av organsvikt hos pasienten kan inngå som kriterium i DRG-systemet for å bedre den medisinske relevansen og den ressursmessige homogeniteten i DRG-gruppene. Til dette er det benyttet SOFA-skår (Sequential Organ Failure Assessment). SOFA gir en samleskår av funksjonene til seks organsystemer (respirasjon, sirkulasjon, koagulasjon, lever, nyrer og sentralnervesystemet), og summen av den daglige SOFA-skår dekker både dimensjonen alvorlighet, ressursbruk og varighet på intensivoppholdet. På bakgrunn av analyser av data fra Haukeland universitetssjukehus og St. Olavs hospital (Universitetssykehuset i Trondheim) er det beskrevet fire nye DRG-grupper for denne type behandling. De nye gruppene baserer seg på ett utvalg utførte prosedyrekoder som indikerer intensivbehandling og informasjon om sum av SOFA-skår fra pasientens opphold i intensivavdelingen. DRG-ene differensierer på SOFA-skår slik at pasienter innenfor samme intervall av SOFA-skår samles i de enkelte DRG-ene. Analyser på tilgjengelig datamateriale viser at disse nye gruppene framstår som mer homogene enn dagens grupper. Oppgaven gir også en oppsummering av hvordan intensivbehandling er ivaretatt i andre lands case-mix system.     Background: Endovenous Steam Ablation (Steam) of Vena Saphena Magna (VSM), due to venous insufficiency, is a new treatment believed to reduce time of convalescence compared with High Ligation and Stripping (Stripping). However, new treatment methods often increases costs due to the price of new equipment. Purpose: The purpose of this master thesis is to compare differences in convalescence and treatment costs between Steam and Stripping, and to our knowledge this has not previous been done. Material and Methods: Eighty tree patients previously assigned to outpatient steam ablation (46) or day-surgery stripping (37) was included. Stripping vs. Steam respectively: Age 51.5 vs. 53.7 years. Female 67.6 % vs. 71.7 %. Classification C2 (varices) 51.4 % vs. 56.5 %. Working 78.4 % vs. 65.2 %. Patients were contacted every second week until they resumed all expected activities such as daily activities, training/ sport and work and no longer had limitations in daily life. We used a micro-costing approach to calculate costs including staff salary, disposable and reusable equipment, cleaning, room, medication and food. We used mean procedure and operation room times as units and assumed equal treatment effects when comparing costs and outcomes. Results: Steam vs. stripping patients resumed daily activities after median (IQR) 0 (0-2) vs. 4 (1,5-6,5) days, training/ sport after 4 (2-9) vs. 11 (3-19) days and had limitations in daily life for 2 (0-3,3) vs. 10 (3,5-15,5) days, and had 2 (0-5) vs. 14 (6-21) days with sick leave. Stripping and steam costed respectively NOK 10 109 and NOK 5 972, which made steam NOK 4 137 less expensive. Conclusion: Steam patients recovered faster than patients receiving stripping did. Our results indicate that steam ablation can be offered at a lower cost, and with shorter convalescence than stripping. This indicate that steam may be cost effective compared to stripping. Relevance: The convalescence results in this study can be used to inform patients what to expect when seeking one of the two treatment strategies for venous insufficiency. The cost results is a relevant information for hospitals and departments that are treating patients with steam ablation or stripping with high ligation. Departments that want to calculate their own treatment costs can replace our results with their own due to our detailed description

    The burden of hip fractures in elderly patients: Changes in health-related quality of life one year following hip fracture.

    Bakgrunn: Påvirkningen av et hoftebrudd på kvalitetsjusterte leveår er ikke grundig studert blant norske hoftebruddpasienter. Målet med denne studien var å 1) kvantifisere endringen i helserelatert livskvalitet (HRQoL) det første året etter gjennomgått hoftebrudd, og 2) undersøke hvilke prefrakture faktorer som er assosiert med endringen i HRQoL. Materiale og metoder: Vi gjennomførte en longitudinell kohortstudie hvor 387 hoftebruddpasienter ble inkludert. Data på HRQoL ble innhentet gjennom spørreskjemaet EQ-5D-3L ved 1-, 4- og 12 måneder etter hoftebruddet. Prefrakture data på HRQoL ble estimert ved hjelp av en publisert mapping-algoritme og data på prefraktur Barthel Index. En tidligere publisert konstant ble brukt som proxy for data på HRQoL i den umiddelbare perioden etter hoftebruddet oppstod. QALYs ble beregnet ved bruk av arealet under kurvemetoden. Multivariat regresjonsanalyse ble utført for å undersøke hvilke faktorer som var assosiert med tapet av QALYs. Resultater: Gjennomsnittlig HRQoL før hoftebruddet var 0,64. HRQoL falt til 0,27, 0,44, 0,51 og 0,53 henholdsvis umiddelbart etter brudd, 1-. 4- og 12 måneder etter brudd. Gjennomsnittlig tap i QALY var 0,15. HRQoL før brudd, ADL-funksjon før brudd og komorbiditeter ved bruddtidspunkt var assosiert med QALY-tapet. Konklusjon: Et hoftebrudd har en dramatisk påvirkning på pasientenes HRQoL, og forringelsen vedvarte ett år etter bruddet. QALY-tapet representerer en betydelig påvirkning på disse pasientene når det gjelder forringet HRQoL og økt dødelighet. Våre funn understreker viktigheten av å forebygge hoftebrudd og optimalisere både pre- og postoperativ behandling for å forhindre tap av QALYs.Background: The impact of a hip fracture on quality-adjusted life years has not been thoroughly studied in Norwegian hip fracture patients. The aim of this study was to 1) quantify the change in health-related quality of life the first year following hip fracture, and 2) investigate which pre-fracture factors are associated with the change in HRQoL. Material and Methods: Longitudinal cohort study including 387 hip fracture patients. Data on HRQoL was obtained using the EQ-5D-3L questionnaire at 1-, 4-, and 12-months post fracture. Pre-fracture data on HRQoL were estimated using a published mapping algorithm and data from the Barthel Index, and a published constant was used as a proxy for data on HRQoL immediate post-fracture period. QALYs were calculated using the area under the curve method. Multivariate regression analysis was performed to investigate predictors of QALY loss. Results: Mean pre-fracture HRQoL for all participants was 0.64. HRQoL declined to 0.27, 0.44, 0.51 and 0.53 immediately after fracture, 1-. 4-, and 12-months post fracture, respectively. The mean loss in QALY was 0.15. Pre-fracture HRQoL, pre-fracture ADL and present comorbidities were predictors of the QALY loss. Conclusions: A hip fracture has a dramatic impact on the patients´ HRQoL, and the deterioration sustained one year after the fracture. The QALY loss represents a substantial impact on these patients in terms of poor HRQoL and premature mortality. Our findings emphasize the importance of preventing hip fracture and optimizing both pre- and postoperative care in order to prevent loss of QALYs

    Occupational rehabilitation and readiness for return to work in individuals with musculoskeletal complaints and mental health disorders: A randomized clinical trial

    Background Musculoskeletal complaints and mental health disorders are the two leading causes of sickness absence in Norway. Despite increasing documentation of overlap in symptoms between these diagnoses groups, most occupational rehabilitation programs described in the scientific literature are diagnosis-specific. In Norway, inpatient occupational rehabilitation programs including different diagnostic groups in the same groups has been done for decades, but such programs have never been evaluated with a rigorous study design. Aims The main aim of this thesis was to assess the effects of a 4+4 days multicomponent inpatient occupational rehabilitation program compared to a less comprehensive outpatient program on sickness absence and somatic and mental health in persons with musculoskeletal- or mental health disorders. A secondary aim was to explore the usefulness of the Readiness for Return to work scale, a questionnaire developed to evaluate where sick listed individuals are in their return to work (RTW) process, by assessing the association between the scale and RTW, and comparing the scale to a single question assessing participants` expectations about length of sick leave. Methods In a randomized trial with parallel groups, individuals on sick leave with musculoskeletal complaints or common mental health disorders were randomized to the inpatient program or the outpatient program. The inpatient program consisted of Acceptance and Commitment Therapy (ACT), physical training and work-related problem-solving including creating a RTW plan and a workplace visit if considered relevant. The outpatient program consisted primarily of ACT (6 sessions during 6 weeks). The primary outcome was cumulated number of sickness absence days at 6 and 12 months follow-up based on registry data. Secondary outcomes were time until sustainable RTW and different somatic and mental health outcomes measured by questionnaires up to 12 months follow-up (paper 1 and 2). In a prospective cohort study with 9 months follow-up participants on sick leave with musculoskeletal complaints or common mental health disorders who took part in two randomized clinical trials were included. Associations between the Readiness for RTW scale and RTW was analysed using linear and logistic regression. The Readiness for RTW scale was compared to a self-reported question assessing participants` expectations about length of sick leave using adjusted/pseudo R2. Questionnaires were answered at the start and the end of rehabilitation programs and sick leave was measured using registry data (paper 3). Results 168 individuals were randomized to the 4+4 days multicomponent inpatient program (n=92) or the outpatient program (n=76) (paper 1 and 2). There was no statistical difference between the programs in median number of sickness absence days at 6 and 12 months follow-up. The hazard ratio for sustainable RTW was 0.74 (95% CI 0.48-1.32, p=0.165), in favor of the outpatient program. There were no statistically significant differences between the programs in health outcomes, except for slightly more reduced pain after the outpatient program. For participants not working at the end of rehabilitation (n=96), high scores on two dimensions (Prepared for action-self-evaluative and Prepared for action-behavioral) were associated with a higher probability of RTW and more working days (paper 3). For those working (n=121), high scores on the Uncertain maintenance dimension was associated with a lower probability of RTW and less working days. Stage allocation, allocating participants to the dimension with the highest score, was problematic due to several tied scores between (not necessarily adjacent) dimensions. Generally, models including the Readiness for RTW dimensions were not as good at explaining work outcomes as models including a single expectation question. Conclusions There was no difference between the inpatient program and the outpatient program on sick leave or sustainable RTW. There was no difference in self-reported health measures between the programs, except for slightly more reduced pain after the outpatient program. Therefore, this study presents no support that a 4+4 days inpatient multicomponent occupational rehabilitation program is superior to a less comprehensive outpatient program. Three of the Readiness for RTW dimensions were associated with RTW, but only to a lesser or similar extent than a single expectation question. Furthermore, several weaknesses with the Readiness for RTW scale were established and particularly the stage allocation approach cannot be recommended for clinical use in its current form

    Health-related quality of life in Young People with Chronic fatigue syndrome/ Myalgic encephalomyelitis

    Summary Background Chronic fatigue syndrome/Myalgic encephalomyelitis (CFS/ME) is a disease that affects people of all ages. CFS/ME significantly limits the activity level of those affected, including in relation to physical activity, schooling, occupational life and social life. High levels of school absence among young people with CFS/ME result in loss of important learning and social development among peers. As such, there is increasing uncertainty about their future, and personal and socio-economic consequences could put them at risk of becoming disabled at a young age. Measurements of health-related quality of life (HRQoL), including being able to function in school, have shown that young people with CFS/ME score lower than their counterparts without CFS/ME. Aims The overall aim of this project was to explore HRQoL among young people with CFS/ME, including the factors associated with HRQoL in relation to school and everyday life. More specifically, the aim was to firstly (Study1) examine HRQoL, including factors that are positively or negatively associated with HRQoL, in a cohort of young people with CFS/ME. Study 1, along with the previous literature, provided the basis for an in-depth study (Study 2) to investigate the positive and negative factors that young people with CFS/ME experience in school and everyday life. Based on the findings from Study 1 and Study 2, a third study (Study 3) was planned to explore teachers’, counsellors’ and school nurses’ experiences with educational and social adaptation at school for young people with CFS/ME. Method To explore HRQoL and the factors associated with HRQoL among young people with CFS/ME (Studies 1 & 2), a cross-sectional survey- and interview-based study was conducted. The participants of the cross-sectional study were recruited to participate in the interview study. To explore the experiences of teachers, counsellors and school nurses with education and social adaptions at school for young people with CFS/ME (Study 3), an interview study was conducted with participants recruited among school personnel and school nurses in secondary school (educating students aged 13-16), high school (educating students aged 16-19) and educational psychological services (EPS). Results A total of 63 participants were included in the cross-sectional study, 18 of whom participated in the interview study. A total of 12 participants were included in the interview study with the teachers, counsellors and school nurses. In the cross-sectional study (Study 1), young people with CFS/ME scored lower on HRQoL than their counterparts who were healthy or had other chronic diseases. Contact with school and teachers was associated with a higher HRQoL among young people with CFS/ME. This association could be due to that more contact resulted from adaptations of education and social life at school, or that fewer health problems due to CFS/ME had abled the young people to maintain the contact with school and teachers. In Study 2, it was found that an adapted plan for education and social life at school for young people with CFS/ME could increase the possibility of continuing schooling with peers. The lack of an adapted plan for education and social life at school could lead to increased school absence as well as loss of education, social contact and development among peers. Subsequently, this could lead to depressive thoughts and worry about the future. The school personnel and school nurses in Study 3 experienced that young people with CFS/ME lost confidence in school. The challenges experienced by school personnel included (1) understanding students’ needs before they received a diagnosis and before school personnel received information from healthcare providers and (2) maintaining the teacher–student relationship and (3) the continuity of teaching. In terms of measures for better management, early problematization of school absence, interdisciplinary collaboration on early measures, ensuring the maintenance of the teacher–student relationship and increasing CFS/ME-related competence in schools were proposed. These measures could contribute to prevent loss of function and school absence among young people with CFS/ME. Conclusion HRQoL among young people with CFS/ME was associated with contact with school and teachers, but a causal relationship could not be proven. Interviews with young people with CFS/ME and school personnel suggested that interdisciplinary strategies for early adaptations to education and social life at school for young people with CFS/ME may benefit education and social development among peers for young people with CFS/ME. Lack of educational and social adaptations at school might lead to loss of education, social life and development among peers.Sammendrag Bakgrunn Kronisk utmattelsessyndrom/myalgisk encefalopati (CFS/ME) er en sykdom som begrenser aktivitetsnivået til de som rammes i betydelig grad. Både fysisk aktivitet, skolegang, arbeidsliv og sosialt liv påvirkes. Høyt skolefravær blant ungdom med CFS/ME gjør at de mister viktig lærdom og utvikling sammen med jevnaldrende. Framtiden til disse ungdommene blir usikker, og personlige og samfunnsøkonomiske konsekvenser kan være at de risikerer å bli uføre i ung alder. Målinger av helserelatert livskvalitet og skolefunksjon har vist at ungdom med CFS/ME scorer lavere enn andre ungdommer. Hensikt Hovedhensikten med dette prosjektet var å undersøke helserelatert livskvalitet blant ungdom med CFS/ME, og samtidig undersøke faktorer i skole og hverdagsliv som kunne være assosiert med helserelatert livskvalitet. I første studie ble helserelatert livskvalitet og assosierte faktorer undersøkt i en kohort av ungdom med CFS/ME. I Studie 2 ble funn fra Studie 1 og tidligere litteratur brukt som utgangspunkt for tema i en fordypnings studie som undersøkte erfaringer blant ungdom med CFS/ME i forhold til skole og hverdagsliv. På bakgrunn av funn fra de to første studiene ble det planlagt en tredje studie som undersøkte lærere, rådgivere og helsesykepleieres erfaringer med å legge til rette skolehverdagen for ungdom med CFS/ME. Metode For å undersøke helserelatert livskvalitet og assosierte faktorer blant ungdom med CFS/ME (Studie 1 & 2), ble det gjennomført en tverrsnittsstudie med spørreskjema, og en intervjustudie. Deltagere fra tverrsnittsstudien ble rekruttert til å delta i intervjustudien. For å undersøke erfaringer med å legge til rette skolehverdagen for ungdom med CFS/ME (Studie 3) ble det gjennomført en intervjustudie med deltagere rekruttert blant personale i ungdomsskole, videregående skole, pedagogisk-psykologisk tjeneste (PPT) og skolehelsetjeneste. Resultat Totalt ble 63 deltagere inkludert i tverrsnittsstudien, og 18 av disse deltok i intervjustudien. I intervjustudien med lærere, rådgivere og helsesykepleiere ble 12 deltagere inkludert. I tverrsnittsstudien (Studie 1) skåret ungdom med CFS/ME lavere på livskvalitet enn friske og ungdom med andre kroniske sykdommer. Kontakt med skole og lærere var assosiert med høyere livskvalitet blant ungdom med CFS/ME. Denne assossiasjonen kunne enten skyldes mer kontakt på grunn av tilrettelegging av utdanning og sosialt liv i skolen, eller at færre helseproblemer fra CFS/ME gjorde det mulig for ungdommene å opprettholde kontakten med skole og lærere. I Studie 2 ble det funnet at en tilrettelagt plan for undervisning og sosialt liv i skolen kunne bidra til å øke muligheten for å fortsette skolegangen sammen med jevnaldrende. Mangel på en tilrettelagt plan for skolegang og sosialt liv, kunne føre til høyere skolefravær og tap av lærdom, sosial kontakt og utvikling sammen med jevnaldrende. Dette igjen kunne føre til bekymring for framtida og depressive tanker. I Studie 3 hadde lærere, rådgivere og helsesykepleiere erfart at ungdom med CFS/ME mistet tillit til skolen. De hadde erfart at det var utfordrende å forstå elevens behov før elevene fikk en diagnose og før lærere, rådgivere og helsesykepleiere fikk informasjon fra helsetjenesten om tilrettelegging av undervisning og sosialt liv i skolen. Andre utfordringer var å opprettholde lærer-elevrelasjonen og kontinuiteten i undervisningen. Som tiltak for å forbedre håndteringen av ungdom med CFS/ME i skolen foreslo lærere, rådgivere og helsesykepleiere følgende; å problematisere skolefravær tidlig, ha et tverrfaglig samarbeid rundt tidlige tiltak, sikre opprettholdelse av lærer-elev relasjonen og øke kompetansen om CFS/ME i skolen. Dette kunne bidra til å forebygge funksjonstap og skolefravær blant elever med CFS/ME. Konklusjon Helserelatert livskvalitet blant ungdom med CFS/ME var assosiert med kontakt med skole og lærere, men en årsakssammenheng kunne ikke bevises. Intervju med ungdom med CFS/ME, skolepersonell og helsesykepleiere antyder at tverrfaglige strategier for tidlig tilrettelegging av undervisning og sosialt liv i skolen kan være nyttig for utdanning og sosial utvikling sammen med jevnaldrende for ungdom med CFS/ME.     Background: Hearing impairment is a condition that has a negative impact on communication and is associated with age. The psychosocial consequences of hearing impairment can be debilitating and hinder an individual’s societal participation. There is also an increasing amount of evidence that indicates a link between hearing impairment and more serious conditions like dementia, neck pain and psychological conditions. Purpose: The aim of this master’s thesis is to provide information about the societal cost regarding hearing loss in Norway. And to predict the future cost in 2030 given the increasing growth in the elderly population. Material and methods: This master’s thesis looked at the cost of illness for hearing loss in Norway using a top-down prevalence perspective with a human capital approach. Registry data was collected from the Norwegian patient registry (NPR), control and reimbursement of health-expenses (KUHR) and the employment registry (NAV). Data was also collected through a questionnaire distributed digitally to members of the national association of the hearing impaired (HLF) and from the website of the Norwegian technical audiological society (NTAF) for hearing aids. Results: The societal cost of illness for hearing impairment in Norway was found to be 890 million NOK in 2016 with 712 million of this cost going to health services. For the health services this cost was estimated at 940 million in 2030 based on population growth and cost per individual. Conclusions: The expenditure for the treatment of hearing impairment can be expected to increase in the coming years.
