The synthesis of monodisperse alkanes with long chains
This thesis discusses reasons for the interest in monodisperse long chain alkanes and describes attempts, past and present, to synthesise such molecules. Chapter 1 discusses why the synthesis of such molecules are important and the objectives of this project. Chapter 2 reviews the methods previous groups have devised to prepare pure samples of long chain alkanes. In particular, work carried out by Whiting et al. at Bristol, whose scheme formed the basis of the early work in Durham. Chapter 3 describes the work in Durham and improvements which were made to Whiting's method, allowing the synthesis of longer chain lengths and greater quantities of materials to be achieved. Chapter 4 provides a summary of the practical work carried out by the author. Chapter 5 gives experimental details of the work described in Chapter 4

Ext. 86-6 "Worksheets for Calculating Breakeven Bids for the Milk Production Termination Program". Farm information for I. May Bidd is on the next page. A breakeven bid is calculated for each of the three situations, described in detail in the aforementioned bulletin. Briefly, the three situations differ with respect to whether the dairy farmer plans to leave dairy production soon regardless of MPTP and whether or not he plans to return to milk production at the end of MPTP. The same basic data will be used for all three situations but we have changed I. May Bidd's name to fit the situation
Oral vs intravenous paracetamol for lower third molar extractions under general anaesthesia: is oral administration inferior?
BACKGROUND: /st>Paracetamol formulations provide effective analgesia after surgery [Duggan ST, Scott LJ. Intravenous paracetamol (acetominophen). Drugs 2009; 69: 101-13; Toms L, McQuay HJ, Derry S, Moore RA. Single dose oral paracetamol (acetaminophen) for postoperative pain in adults. Cochrane Database Syst Rev 2008: CD004602]. I.V. paracetamol is superior to oral for pain rescue (Jarde O, Boccard E. Parenteral versus oral route increases paracetamol efficacy. Clin Drug Invest 1997; 14: 474-81). By randomized, double-blinded trial, we aimed to determine whether preoperative oral paracetamol provides inferior postoperative analgesia to preoperative i.v. paracetamol. METHODS: /st>One hundred and thirty participants received either oral paracetamol and i.v. placebo (Group OP), or oral placebo and i.v. paracetamol (Perfalgan™) (Group IP). Oral preparations were given at least 45 min before surgery; i.v. preparations after induction of anaesthesia. Pain was assessed by a 100 mm visual analogue scale (VAS) 1 h from the end of surgery. Rescue analgesia was given on request. RESULTS: /st>A total of 128 patients completed the study. There were no significant differences in baseline characteristics or intraoperative variables between the groups. The study was designed to reveal whether OP is inferior to IP, with an inferiority margin of 20%. The number of patients reporting satisfactory analgesia at 1 h with VAS ≤30 mm were 15 (OP) and 17 (IP), respectively. The secondary outcome measure of the mean (standard deviation) VAS (mm) for the whole of each group was 52 (22) for OP and 47 (22) for IP. Analysis of confidence intervals indicates that oral paracetamol is not inferior to i.v. paracetamol. The median survival (90% CI) to rescue analgesia request was 54.3 (51.2-57.4) min in Group OP and 57.3 (55.4-59.2) min in Group IP; there was no significant difference in this measure. The effect of intra-operative passive movement therapy on non-surgical site pain after breast reconstructive surgery: a preliminary study
Pain distant to an operative site is under-reported but sometimes more severe than pain from the surgical site. Intra-operative passive movement could possibly reduce this pain. This preliminary study was designed to assess the practicalities of conducting a randomised controlled trial of this therapy in anaesthetised patients. The study design was pragmatic. Forty-two patients undergoing breast reconstruction were randomly assigned to receive either intra-operative passive movement or standard care. Twenty-four hours after surgery, median (IQR [range]) morphine consumption was 33 (11-42 [0-176]) mg in the passive movement group compared with 74 (15-118 [0-238]) mg with standard care (p = 0.126), while participants reported median (IQR [range]) visual analogue scores in areas distant from the surgical site of 0 (0-4 [0-34]) mm in the passive movement group compared with 10 (2-30 [0-57]) mm in those receiving standard care (p = 0.002). Multi-candidate immunohistochemical markers to assess radiation response and prognosis in prostate cancer: results from the CHHiP trial of radiotherapy fractionation.
BACKGROUND: Protein markers of cellular proliferation, hypoxia, apoptosis, cell cycle checkpoints, growth factor signalling and inflammation in localised prostate tumours have previously shown prognostic ability. A translational substudy within the CHHiP trial of radiotherapy fractionation evaluated whether these could improve prediction of prognosis and assist treatment stratification following either conventional or hypofractionated radiotherapy. METHODS: Using case:control methodology, patients with biochemical or clinical failure after radiotherapy (BCR) were matched to patients without recurrence according to established prognostic factors (Gleason score, presenting PSA, tumour-stage) and fractionation schedule. Immunohistochemical (IHC) staining of diagnostic biopsy sections was performed and scored for HIF1α, Bcl-2, Ki67, Geminin, p16, p53, p-chk1 and PTEN. Univariable and multivariable conditional logistic regression models, adjusted for matching strata and age, estimated the prognostic value of each IHC biomarker, including interaction terms to determine BCR prediction according to fractionation. FINDINGS: IHC results were available for up to 336 tumours. PTEN, Geminin, mean Ki67 and max Ki67 were prognostic after adjusting for multiple comparisons and were fitted in a multivariable model (n = 212, 106 matched pairs). Here, PTEN and Geminin showed significant prediction of prognosis. No marker predicted BCR according to fractionation. INTERPRETATION: Geminin or Ki67, and PTEN, predicted response to radiotherapy independently of established prognostic factors. These results provide essential independent external validation of previous findings and confirm a role for these markers in treatment stratification. An archaeology of Irish cinema: Ireland's subaltern, migrant and feminist film cultures (1973-87)
This thesis examines the development of an Irish film avant-garde, from the mid 1970s to the late 1980s. The thesis argues that this period was marked by an historically specific intersection between Irish and international film cultures, which can be traced through contemporary film theory, cultural policy and critical practice. This period witnessed a revitalisation of indigenous production, and new initiatives in Irish arts policy, but many important Irish filmmakers trained or began their careers in London and New York, while others were supported by cultural and political agencies outside the state. The thesis focuses on the work of five filmmakers (Bob Quinn, Joe Comerford, Thaddeus O’Sullivan, Vivienne Dick and Pat Murphy) and on three key areas of intersection between Irish and international film culture, associated with the ‘subaltern’, migration and feminism. Through close readings of specific films, supported by interviews with selected filmmakers, distributors and archivists, the thesis develops an expanded model of practice, which extends beyond production to address issues of distribution and exhibition. This archaeology of Irish cinema is informed by post-structural critiques of the archive, as well as theories of the avantgarde, and it argues that the reception of Irish avant-garde film has been structured by the institutional discourses of the museum and the academy
