Iridocyclectomy for Ciliary Body Melanoma - Unique Approach and Surprising Result
Introduction: We present a rare clinical case of ciliary body melanoma managed with eye-saving operation.Materials and Methods: A 50-year-old woman was diagnosed with increased IOP, very low visual acuity and cataract. On a visit for a second opinion a melanocytic lesion of the iris with a massive `sentinel` vascularization in the entire upper quadrant was noticed. Ultrasound investigation was very suggestive of ciliary body melanoma. The patient rejected the possibility of enucleation and after assessment of risk factors underwent an open-sky iridocyclectomy. She was left aphakic. After surgery, the patient had hemophthalmos for 20 days, after the absorption of which a zone of excised ciliary body was clearly visible on gonioscopy. Histology investigation confirmed the spindle-cell variant of the tumor and subsequent studies (PET scan), three months after surgery, rejected local and distant metastases. Second operation was performed in 6 months and a lens with artificial iris was sutured horizontally.Results: She is currently presenting with vision of 0.8, normal pressure, and unaffected other ocular structures.Conclusion: Ciliary body melanoma is a life-threatening condition that usually requires enucleation as a treatment of choice. In our case, by performing a novel surgery, we managed to preserve the patient`s eye and thereby provide good quality of life
Cell and Tissue Therapy of the Anterior Ocular Surface // Клетъчна и тъканна терапия на предна очна повърхност
Трансплантацията в офталмологията е област с нови и бързо развиващи се перспективи, което ще позволи приложение на хибридни технологии и тъканно инженерство с цел постигане на по-добри оптични резултати и хомеостаза на предната очна повърхност. Целта на разработката е да се направи анализ на възможностите за клетъчна и тъканна терапия на предна очна повърхност, като се приложат иновативни технологии и се тестват методики за персонализирано лечение.
Анализът на въпросниците за тенденциите на трансплантацията в България (38 правилно попълнени от 100 изпратени) показва, че роговичните трансплантации в над 90% от случаите се извършват с класически техники. Проспективният анализ на 186 трансплантации, от които 28 – трансплантации на роговица в един център са сравнени ретроспективно с 2050 трансплантации, извършени от един опитен хирург. Трансплантацията на лимбални стволови клетки е тествана при 88 пациенти. Част от проучването е посветено на ролята и мястото на иновативните технологии, като триизмерната микроскопия и интраоперативната оптична кохерентна томография (iOCT). Ретроспективният анализ на видеозаписите показва относително еднаквото време за извършване на операцията, което заедно с подобрените клинични резултати е в полза на 3D технологията.
В процеса на подготовка на този труд е разработена концепция за иновативна очна банка с възможност за надграждане с нови технологии. Разработени са стандартни оперативни процедури за вземане, обработка, преработка, съхранение, разпространение и транспорт на роговица и склера, както и за пръв път в България – процедури за вземане, обработка, преработка, съхранение, разпространение и транспорт на лимбални стволови клетки.
Офталмологията е високотехнологична специалност и за хирургическите интервенции и особено за трансплантациите е необходимо приложение на методики, позволяващи микроструктурен анализ в реално време и непрекъснат визуален контрол от целия екип.Transplantation in ophthalmology is an area with new and rapidly evolving perspectives, which will allow the application of hybrid technologies and tissue engineering to achieve better optical results and homeostasis of the anterior ocular surface. The development aims to analyse the possibilities for cellular and tissue therapy of the anterior ocular surface, applying innovative technologies and testing methods for personalised treatment. Analysis of the questionnaires shows that corneal transplants in over 90% of cases are performed with classical techniques. The prospective analysis of 186 transplants, of which 28 -corneal transplants in one centre were compared retrospectively with 2050 transplants performed by one experienced surgeon. Limbal stem cell transplantation was tested in 88 patients. Part of the study is devoted to the role and place of three-dimensional microscopy and intraoperative optical coherence tomography. The retrospective analysis of the videos shows the relatively uniform time for the operation, which, together with the improved clinical results, favours the 3D technology. An innovative eye bank has been created with the possibility of upgrading with new technologies. Standard operating procedures have been developed for the collection, processing, storage, distribution, and transport of the cornea and sclera and limbal stem cells. Ophthalmology is a high-tech speciality, and for surgical interventions and especially for transplants, it is necessary to apply methods that allow microstructural analysis in real-time and continuous visual control by the whole team
Going Beyond Counting First Authors in Author Co-citation Analysis
The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation
counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings
are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that
only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into
account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed. Reasons for these effects are discussed
Variations on the Author
“Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship
Appropriate Similarity Measures for Author Cocitation Analysis
We provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature. Dispelling the Myths Behind First-author Citation Counts
We conducted a full-scale evaluative citation analysis study of scholars in the XML research field to explore just how different from each other author rankings resulting from different citation counting methods actually are, and to demonstrate the capability of emerging data and tools on the Web in supporting more realistic citation counting methods. Our results contest some common arguments for the continued
use of first-author citation counts in the evaluation of scholars, such as high correlations between author rankings by first-author citation counts and other citation
counting methods, and high costs of using more realistic citation counting methods that are not well-supported by the ISI databases. We have done our best to complete the author checklist relating to the use of animals in the hut study. Note that the objective for the hut study was to evaluate the IRS treatment applications for residual efficacy against Anopheles mosquitoes, including the local An. coluzzii mosquito population. Cows were only used to attract mosquitoes into the huts and no tests were carried out directly on the cows. The author checklist is intended for use with studies where experiments are carried out on animals, which is why we have had such difficulty in completing this for the hut study, as many of the questions do not relate to how the cows were used
