    THE IDIOLECT OF REVEREND ECONOMOS DIMITAR POPNIKOLOV PETKANOV

    A less studied talented author and brother of the popular fiction writer Konstantin Petkanov, priest Dimitar wrote in the 1950s. This publication analyzes several parameters of his idiolect: phonetic peculiarities (reflex of ѣ, ѫ, ъ; epenthetic [l]; reflexes of the [tʃ] [dʒ] groups; phonetic dialectisms; phonetic doublets); morphological paradigm (case inflections; noun form for number; dualis; nomina collectiva; extended Church Slavonic suffix for adjectives; dialectal and contracted pronoun forms; a temporal system characterized by high frequency of the verbs in terms of origin, distribution and stylistic differentiation); lexical specifics (in terms of origin, distribution and stylistic differentiation). Emphasis is placed on those lexical layers that constitute and ditinguish the author’s language. Hapaxes (produced using morphological and non- morphological methods), rare words, semantic transponents, dialectisms, colloquial vocabulary and foreign words are studied. All linguistic phenomena are examined retrospectively and in comparison with the synchronous linguistic situation before and after the orthographic reform of 1945 in order to highlight the innovative contribution and the mechanisms through which the author enriched the contemporary Bulgarian vocabulary. Occasionalisms, dialectal and archaic units occur more often in Petkanov’s poetic works than in his fiction.     The most common idea in Romance studies is that Romans, as practical people, didn’t conduct theoretical research on their country or their army, but they gradually built them both. Meanwhile, they reformed and upgraded it, so that they could respond to the challenges of their age. Moreover, the basis of their research was not explicit doctrines, or prior concepts, in fact they used their own, or the experience of others, to find concrete solutions to daily problems. Just as the Hellenic romanophile Polibius (200-120 B.C), in his work Historia, asks the crucial question: ”Is it possible to have such an unreliable man who is not interested in how the Romans, with their unicipal structure, managed to conquer the whole world”? - in the same way the author of this paper, as much as its content allows, humbly and unpretentiously tries to answer the crucial question: “What kind of military structure created and defended one of the biggest and most enduring empires in world’s history, and what rules governed it”

    Iridocyclectomy for Ciliary Body Melanoma - Unique Approach and Surprising Result

    Introduction: We present a rare clinical case of ciliary body melanoma managed with eye-saving operation.Materials and Methods: A 50-year-old woman was diagnosed with increased IOP, very low visual acuity and cataract. On a visit for a second opinion a melanocytic lesion of the iris with a massive `sentinel` vascularization in the entire upper quadrant was noticed. Ultrasound investigation was very suggestive of ciliary body melanoma. The patient rejected the possibility of enucleation and after assessment of risk factors underwent an open-sky iridocyclectomy. She was left aphakic. After surgery, the patient had hemophthalmos for 20 days, after the absorption of which a zone of excised ciliary body was clearly visible on gonioscopy. Histology investigation confirmed the spindle-cell variant of the tumor and subsequent studies (PET scan), three months after surgery, rejected local and distant metastases. Second operation was performed in 6 months and a lens with artificial iris was sutured horizontally.Results: She is currently presenting with vision of 0.8, normal pressure, and unaffected other ocular structures.Conclusion: Ciliary body melanoma is a life-threatening condition that usually requires enucleation as a treatment of choice. In our case, by performing a novel surgery, we managed to preserve the patient`s eye and thereby provide good quality of life

    Трансплантацията в офталмологията е област с нови и бързо развиващи се перспективи, което ще позволи приложение на хибридни технологии и тъканно инженерство с цел постигане на по-добри оптични резултати и хомеостаза на предната очна повърхност. Целта на разработката е да се направи анализ на възможностите за клетъчна и тъканна терапия на предна очна повърхност, като се приложат иновативни технологии и се тестват методики за персонализирано лечение. Анализът на въпросниците за тенденциите на трансплантацията в България (38 правилно попълнени от 100 изпратени) показва, че роговичните трансплантации в над 90% от случаите се извършват с класически техники. Проспективният анализ на 186 трансплантации, от които 28 – трансплантации на роговица в един център са сравнени ретроспективно с 2050 трансплантации, извършени от един опитен хирург. Трансплантацията на лимбални стволови клетки е тествана при 88 пациенти. Част от проучването е посветено на ролята и мястото на иновативните технологии, като триизмерната микроскопия и интраоперативната оптична кохерентна томография (iOCT). Ретроспективният анализ на видеозаписите показва относително еднаквото време за извършване на операцията, което заедно с подобрените клинични резултати е в полза на 3D технологията. В процеса на подготовка на този труд е разработена концепция за иновативна очна банка с възможност за надграждане с нови технологии. Разработени са стандартни оперативни процедури за вземане, обработка, преработка, съхранение, разпространение и транспорт на роговица и склера, както и за пръв път в България – процедури за вземане, обработка, преработка, съхранение, разпространение и транспорт на лимбални стволови клетки. Офталмологията е високотехнологична специалност и за хирургическите интервенции и особено за трансплантациите е необходимо приложение на методики, позволяващи микроструктурен анализ в реално време и непрекъснат визуален контрол от целия екип.Transplantation in ophthalmology is an area with new and rapidly evolving perspectives, which will allow the application of hybrid technologies and tissue engineering to achieve better optical results and homeostasis of the anterior ocular surface. The development aims to analyse the possibilities for cellular and tissue therapy of the anterior ocular surface, applying innovative technologies and testing methods for personalised treatment. Analysis of the questionnaires shows that corneal transplants in over 90% of cases are performed with classical techniques. The prospective analysis of 186 transplants, of which 28 -corneal transplants in one centre were compared retrospectively with 2050 transplants performed by one experienced surgeon. Limbal stem cell transplantation was tested in 88 patients. Part of the study is devoted to the role and place of three-dimensional microscopy and intraoperative optical coherence tomography. The retrospective analysis of the videos shows the relatively uniform time for the operation, which, together with the improved clinical results, favours the 3D technology. An innovative eye bank has been created with the possibility of upgrading with new technologies. Standard operating procedures have been developed for the collection, processing, storage, distribution, and transport of the cornea and sclera and limbal stem cells. Ophthalmology is a high-tech speciality, and for surgical interventions and especially for transplants, it is necessary to apply methods that allow microstructural analysis in real-time and continuous visual control by the whole team

    Contemporary corneal transplantation – the old school and new perspectives in Bulgaria

    Aim: The aim is to analyze the possibilities of cell and tissue therapy of the anterior ocular surface by implementing innovative technologies and testing methodologies for personalized treatment. Materials and Methods: The methodologies consist of a theoretical analysis of the legislation, clinical analysis, and detailed manuals on performing a transplantation in Bulgaria and the world. A questionnaire focused on testing the trends in performing transplantations in Bulgaria is added. All clinical procedures have been conducted using state-of-the-art innovative technology for microstructural assessment during surgery by means of 3D microscopy and real-time optical coherent tomography (OCT). Results: The analysis of the questionnaires on the transplantation trends in Bulgaria (38 correctly filled out out of 100 sent) shows that, in 90% of the cases, corneal transplantations are performed using classic techniques. The shares of anterior and posterior lamellar keratoplasty are only 2% each. The complex AM transplantation techniques are limited to only 10%. A prospective analysis of 186 transplantations has been conducted. Of them 28 are corneal transplantations. The data have been compared with that of the retrospective analysis of 2050 AM transplantations performed by a single surgeon in the last 10 years. Conclusion: The anterior ocular surface is a unique part of our organism and the visual analyzer. Its health is measured by three simple patient sensations: vision, comfort, and lack of redness. The impairments affecting each of the structures which are part of it lead to a damage to the neighboring structures and this unlocks the vicious circle of the anterior ocular surface disease

    Transplantation in ophthalmic practice in Bulgaria - a study based on a questionnaire survey

    Aim:The purpose is to study transplantation habits in ophthalmology in Bulgaria based on the opinion of experienced surgeons. The pillars are: type of transplant performed, the geographical distribution of the treatment, surgical experience and eye bank utilization.Materials and Methods:For the purpose of the study, a questionnaire was developed and distributed to 42 Bulgarian ophthalmologists with a recognized proficiency in ophthalmology and active records in transplantation based on the Executive Medical Agency.Results:The questionnaire was completed by 38 Bulgarian ophthalmology specialists with a strong interest in corneal and amniotic membrane transplantation. Ophthalmic transplantation in Bulgaria is concentrated in Sofia and Varna - the cities where there are eye and tissue banks. Private hospitals and centers are in serious competition with the specialists from the university centers. In Bulgaria, more specialists perform amniotic membrane transplantation in Varna.Nationwide over 90% of cases performed are penetration corneal grafts, and only 2% are lamellar keratoplasty. Tissue supply and bank utilization are uneven.Conclusion:Bulgaria has the potential to develop corneal and amniotic membrane transplantation in ophthalmology, as well as in the development of bio-analogues. The rapid introduction of new methods for the treatment of the anterior ocular surface is important for improving the quality of life and vision of the patients. The application of new technologies in surgical treatment of the anterior ocular surface provides greater efficiency and better quality of ophthalmic care. Training ophthalmic surgeons in new methodology based on regenerative therapy and transplantation is an important prerequisite for improving the ophthalmic care in the country
