    Quality of Life and Rehabilitation in Patients with Total Hip Replacement // Рехабилитация и качество на живот при тазобедрено ендопротезирани пациенти

    This thesis examines the problems of rehabilitation after hip arthroplasty, following the principles of a holistic, patient-centred approach to health and disease, and the possibilities of assessing the effectiveness of rehabilitation, emphasising the patient's self-assessment in terms of its impact on all spheres of life. The thesis aims to monitor the quality of life before and after an individual rehabilitation program carried out in a hospital environment within a 7-day stay in the Department of Rehabilitation at St. Marina University Hospital – Varna. The representative sample was formed from a heterogeneous group of multimorbid patients (72 in number) in different stages of rehabilitation after endoprosthesis. The study's results reveal the positive dynamics of the biopsychosocial aspects of life after the applied individual rehabilitation program. The correlation of rehabilitation with quality of life is expressed in the patient's self-assessment of the effectiveness of rehabilitation measures. Although the functional assessment carried out by medical specialists reflects the change in the physical aspects of health and life, the main goal of rehabilitation is the positive dynamics in the bio-psycho-social spheres.Настоящият дисертационен труд разглежда проблемите на възстановяването след тазобедрено ендопротезиране, в съответствие с принципите на холистичния, пациент-центриран подход към здравето и болестта и възможностите за оценка на ефективността на рехабилитацията, с акцент към самооценката на пациента по отношение на влиянието ѝ върху всички сфери на живота. Цел на дисертацията е да се проследи динамиката в качеството на живот преди и след индивидуална рехабилитационна програма, изпълнена в болнична среда, в рамките на 7 дневен престой в Отделението по Рехабилитация към УМБАЛ „Св. Марина-Варна“. Представителната извадка се сформира от хетерогенна група полиморбидни пациенти (72 на брой), в различни етапи на рехабилитацията след ендопротезиране. Резултатите от изследването разкриват положителната динамика биопсихоциалните аспекти на живота след приложената индивидуална рехабилитационна програма. Отношението на рехабилитацията към качеството на живот намира израз в самооценката на пациента за ефективността на рехабилитационните мерки. Макар функционалната оценка, извършвана от медицинските специалисти да отразява промяната във физическите аспекти на здравето и живота, главна цел на рехабилитацията е положителната динамика в био-психо-социалните сфери. Следователно търсенето и намирането на възможности за оптимизация на ресурсите на здравната система и реализиране на пациент-центрирания подход се явяват основна концепция, залегнала в стратегията за възстановяването след тазобедрено ендопротезиране

    Prevention and therapy of aging – potential of aesthetic acupuncture and aromatherapy

    Aesthetic medicine is evolving rapidly in response to the increasing life expectancy and the psychosocial need to maintain an attractive appearance. The aim of this article is to examine the scientific literature and world practice regarding the application of aesthetic acupuncture and aromatherapy and their ability to influence the aging process.Material and Methods Documentary content analysis of bibliographic data, scientific literature and good practices concerning the preventive and therapeutic use of aesthetic acupuncture and essential oils.Results Review of the scientific literature and good practices reveals the possibilities of independent application of aesthetic acupuncture (incl. aromatherapy) to influence the aging process. No data has been found in foreign and Bulgarian scientific literature on the combined use of acupuncture and local application of essential oils.Conclusion Aesthetic acupuncture and aromatherapy are well known and represented in scientific knowledge as separate therapies. Practical experience of European integrative clinics in the field of aesthetic acupuncture reveals new guidelines for the development and introduction of the combined methodology „aroma acupuncture“ and its influence on the aging process. The analysis of global science and practice stimulates scientific research in the University Center for Eastern Medicine for new opportunities to complement the prevention and treatment of face aging with aesthetic aroma acupuncture

    Abstract: Gas discharge visualization (GDV) is a method for entire diagnosis, which is based on natural physical phenomena in human body. Since the invention of GVD, till present, there is an inscreased interest in its application in different areas of life. The most important use of GVD is in medicine. GVD has a possibility for  quantitative measurement of human physical and psychological  well-being.  It is used  in medicine, psychology and sport for  diagnosis, prevention and  monitoring.The purpose of  this  article is to summarize the  dynamics of scientific  interest in GDV application in medicine, psychology and sport and to present current concepts and techiques in  the use of this device in alternative medicine.Matherial and methods: research of the  publishing activity in  application of  GVD in medicine, psychology and sport during last 70 years.Results:  We observed an increasement of the scientific intreset in GVD use in medicine, alternative medicine, psychology and sport with progression for each decade, from 1950 - till now. GVD is a focus of large number of  clinical studies in three dimentions of medicine - prevention, diagnosis and treatment, with culmination of publishing activity in all of the fields during last year.  Conclusion: Although GVD is not well known in our country, the results of our survey are in prove of the uniqueness of GVD as a device for diagnosis, prevention and monitoring of illness. The opportunity  for application of this method in conventional medicine,  alternative medicine, pshychology and sport, is a beginning of  new perspectives for personalized healthcare.  

    MULTIDISCIPLINARY COOPERATION BETWEEN COMPLEMENTARY AND CONVENTIONAL MEDICINE WITH PATIENT SUFFERING FROM MYOFASCIAL PAIN SYNDROME

    Introduction: Pain and vertigo in the neck zone are a common problem which leads to temporary and even permanent incapacity of work. The problems in the neck deteriorate the quality of life, and that makes people turn to the services of the health care system. The medications prescribed by conventional doctors have a symptomatic effect, which discourages patients and they more often start looking for the methods of complementary medicine as a part of their complex treatment. The cooperation complementary-conventional medicine could contribute to faster improvement with the patient suffering from myofascial pain syndrome. Case report: A 63-year-old female patient in the menopause, with over 30 years length of service as a bank employee, visited The Medical University of Varna and the University centre of East medicine with the following complaints: pain and vertigo in the whole spine for many years, hypertonic crises and tachycardia. She had been prescribed a medication treatment for a long time by a large team of specialists (neurologist, cardiologist, physiotherapist, psychiatrist). It didn’t have a systematic or continuous effect, and that was the reason why the patient was incapable of work. The frequent absence from work and the feeling of inadequacy in life are the reasons for the patient to search for help by means of unconventional methods (complementary medicine), which is practised by members of the University centre of East medicine – Varna. Conclusion: The cooperation between complementary and conventional medicine offers new opportunities for applying the holistical approach in treatment with patients suffering from myofascial pain syndrome

    Physical rehabilitation approach to temporomandibular disorders—a clinical case

    Introduction: Temporomandibular disorders are an umbrella term covering pain and dysfunction of the temporomandibular joint, masticatory muscles and associated structures. These conditions can negatively affect body posture, daily activities, and quality of life. The challenges in the entire diagnostic process and the complex treatment indicate the need of a multidisciplinary approach to temporomandibular disorders.Aim: The aim of this article is to monitor the effectiveness of an individualized rehabilitation plan for temporomandibular dysfunction.Materials and Methods: A 58-year-old woman diagnosed with a severe temporomandibular joint dysfunction underwent a 10-day rehabilitation course, including laser puncture, ultrasound, interference current therapy, manual lymphatic drainage, muscle relaxing techniques, and fitball.Results: We founded a significant improvement in De Fonseca index, total Quality of Life score and range of motion as a result of the treatment.Conclusion: The personalized administration of physical and rehabilitation modalities acts not only as a focused therapy, but determines the long-term effectiveness of the treatment of temporomandibular disorders

    Introduction: The trends for healthy lifestyles and the seeking of opportunities to amplify the main medical therapy for chronic diseases make physicians, pharmacists and patients confront numerous difficulties when using phytotherapy. The question how and who should provide the information about the rational use of phytopreparations and food supplements has to be clarified because of the chronic polymorbidity, the need of complex approach in pharmaceutical care and the potential health risks.Aim: To investigate the need of consulting the patient in choosing phytopreparations and food supplements.Material and methods: Exploring the accessible literature sources in database Google scholar about the difficulties patients face while using phytopreparations and food supplements.Results and discussion: The leading place in making the final choice of phytopreparation is taken by the pharmacist but he also encounters obstacles to individualize the therapeutic approach, as going into the clinical scope. Here comes the need of establishing a narrow collaboration with broad doctor specialty physician to improve the patient’s quality of life.Conclusion: The establishment of multidisciplinary team including the patient and his family, pharmacist and a physician create a favourable environment for effective consultation about choosing a suitable phytopreparation and the achievement of patient satisfaction

    On the need for legal regulation of acupuncture practice in the Bulgarian context

    In the Western world acupuncture, whose homeland is considered to be China, was introduced by Portuguese missionaries in the 16th century, and since then the methods of its application have changed and adapted to the perceptions and expectations of the modern patient. The growing interest in the method presupposes the adaptation of the Bulgarian legislation to the established practice of acupuncture, in addition to the complex therapy.The aim of this article is to study and analyse the legal framework of acupuncture in Bulgarian conditions.Material and methods. Documentary-content analysis of enactments, incl. regulations, medical standards and clinicalpathways, in which acupuncture is mentioned or discussed.Results. Acupuncture does not meet the definition of an unconventional method and its legal status should be reconsideredin a conventional direction. Specialists who can train and practice acupuncture are in an unnecessarily wide range. The method poses risks to human health and should be performed in accordance with the rules for good medical practice by a trained doctor (certified acupuncturist), who has undergone all levels of training in acupuncture.Conclusions. The review of the legal framework of acupuncture reveals contradictions between the separate texts in the enactments in which acupuncture is mentioned. In medicine acupuncture is imposed as a conventional method, and perhaps as more – a conventional physical factor, among other natural and reformed physical factors. To this date the practice requires a change in the legislation on this issue by removing acupuncture from the regulation for unconventional methods

From the analysis of the almost fully resolved rotational structure of the origin of the X~+2B2X~1A1\rm \widetilde{X}^+\,^2B_2\leftarrow\widetilde{X}\,^1A_1 ionizing transition of c-C3_3H4_4, c-C3_3H3_3D, c-C3_3H2_2D2_2, c-C3_3HD3_3 and c-C3_3D4_4 and the observation of several low-lying vibrational levels of the corresponding cations, we conclude that the cyclopropene cation has an equilibrium structure deviating from C2vC_{\rm 2v} symmetry. We believe that the cation is subject to a large-amplitude tunnelling motion along a mode of a2_2 symmetry involving a twist of the CH2_2 group and an asymmetric out-of-plane bending of the CH units.
