Saintly doctors : the early iconography of SS. Cosmas and Damian in Italy
The Italian iconography of the doctor saints Cosmas and Damian reflects
fluctuations in the fortunes of the cult of those saints with significant variations in
appearance and meaning being tied to changes in the position of the saints with
respect to function, as miraculous healers, as representatives of professional
doctors and as patrons of a powerful family.
This study considers the development of the iconography of the doctor
saints Cosmas and Damian in Italy, beginning with the emergence of images in
the late antique period. These early representations are explored within the
context of the historic and liturgical origins of the cult of SS. Cosmas and
Damian with particular attention paid to the hagiography and more specfically the
miracle stories which provide a significant amount of information about the role
of images in a Christian healing cult. Evidence that sheds light on the early
development of the iconography of the saints reflecting their position within the
broader context of the establishment of Christian healers in direct opposition to
their popular pagan counterparts.
In the fourteenth century the appearance of SS. Cosmas and Damian was
transformed mirroring the appearance of contemporary doctors, which in turn
reflected the professionalisation of medicine and the role of the saints as patrons
to members of that profession. This iconographic development is considered in
the context of sources such as university statutes and civic sumptuary regulations
that helped to shape the environment of increasing specialization that resulted in the necessity of a distinctive costume for qualified professionals. At the same
time there remained continuity in the position SS. Cosmas and Damian inhabited
in the popular imagination with images of the saints continuing to be associated
with their traditional role as miraculous healers.
Finally the large number of images commissioned by Cosimo de’ Medici
in Florence in the first half of the fifteenth century are examined. At this time the
position of the saints, as intercessors for and protectors of the Medici family
allowed them to appear in unfamiliar locations granting them a civic and political
relevance not achieved before in the history of the cult. The clear identification of
the saints with the family allowed them to act as a reminder of the family’s
position in Florence and for a time the doctors were known as family patrons
rather than solely as doctors and healers
Integrative Approaches in the Management of Hypertrophic Cardiomyopathy: A Comprehensive Review of Current Therapeutic Modalities
Hypertrophic cardiomyopathy (HCM) is often associated with left ventricular outflow tract (LVOT) obstruction, which affects a substantial proportion of patients. This obstruction results from a range of anatomical abnormalities involving both the valvular and subvalvular structures. Pharmacological therapies play a pivotal role in the management of LVOT obstruction, with a range of drug classes exhibiting distinct mechanisms of action. Beta-blockers, including atenolol and nadolol, are considered the first-line treatment due to their ability to reduce heart rate and myocardial contractility and enhance diastolic filling. Non-dihydropyridine calcium channel blockers, such as verapamil and diltiazem, are utilized as second-line agents when beta-blockers are ineffective or contraindicated. Disopyramid, a Class 1A antiarrhythmic agent, is employed for patients who do not respond to initial therapeutic interventions and can reduce LVOT gradients. Recent advancements in cardiac myosin modulators, such as Mavacamten and Aficamten, offer targeted therapies by modulating myosin–actin interactions to reduce LVOT gradients and improve symptoms, with promising results from clinical trials. Although gene therapy is still in its nascent stages, it has the potential to address the genetic basis of HCM by employing techniques such as genome editing, gene replacement, and the modulation of signaling pathways. For patients exhibiting severe symptoms or demonstrating unresponsiveness to medical treatment, invasive therapies, such as septal reduction therapy and alcohol septal ablation, are considered. Ultimately, the treatment and prevention of atrial fibrillation and sudden cardiac death are two key points of HCM management in both obstructive and non-obstructive forms. Recherche-action et promotion écocentrée : expériences de refondation mémorielle et de construction territoriale à partir d’une écriture située
International audienceThe aim of this contribution is to review the design and realization phases of the literary walk La battuta di caccia a Monte Mei: tra cascate, boschi e assaggi di miniera sulle tracce di Angelo Uras, developed by Maria Carmela Aru, Cosimo Frigau and the author of this essay during the summer of 2021 and inaugurated during the 36th edition of the Giuseppe Dessì Literary Prize (Villacidro, Sardinia). Based on an analysis of the research carried out in the village and the literary animation that followed, the aim is to demonstrate the analytical validity and the applicative potential of literary action-research (particularly within material ecocriticism) in a regional and peri-urban promotional context, within the broad methodological and operational horizon of parks and literary walks. Our aim is to examine the recreative and socio-territorial function of an eco-centric promotion of literature, in order to reactivate conscious ways of experiencing and re-appropriating the text with a communitarian intention, through the prism of an ecological discourse close to the narrations of the habitat.Cette contribution vise à reparcourir les phases de conception et de mise en place de la randonnée littéraire « La battuta di caccia a Monte Mei : tra cascate, boschi e assaggi di miniera sulle tracce di Angelo Uras », élaborée par Maria Carmela Aru, Cosimo Frigau et l’autrice de cet essai pendant l’été 2021 et cr ée lors de la 36e édition du Prix Littéraire Giuseppe Dessì (Villacidro, Sardaigne). À partir de l’analyse de la recherche de terrain menée dans le village et de l’animation littéraire qui a suivi, il sera question de montrer la validité analytique et le potentiel applicatif de la recherche-action en littérature (au sein de l’écocritique matérielle notamment) dans un contexte promotionnel régional et périurbain, à l’intérieur de l’horizon méthodologique et opérationnel ample des parcs et des promenades littéraires. Nous tâchons d’interroger, par cela, la fonction récréative et socio-territoriale d’une promotion de la littérature écocentrée, en mesure de réactiver des modalités d’expérience et de réap-propriation du texte à vocation sensible et communautaire, au prisme d’un discours éco-logique proche des narrations de l’habitat
Sutureless and Rapid Deployment Prosthesis in Redo-Bentall Endocarditis
Aortic valve replacement (AVR) in a patient with a bio-Bentall conduit can be very challenging, especially if there was a previous endocarditis process for significant morbidity and mortality. We report a case of sutureless AVR in an old patient with a bio-Bentall conduit (Carpentier–Edwards Perimount Magna Ease 25 aortic valve and Hemashield 30 aortic conduit), who developed an endocarditis on aortic prosthesis valve. We believe that sutureless AVR is the best option for redo-operation in older patients with a high surgical risk because it allows for easy rapid deployment implantation, avoids anchoring sutures on a fragile aortic anulus, and reduces cardiopulmonary and aortic cross-clamp times. In this setting, it should be considered as a safe and valid alternative not only to traditional prosthesis but also in selected cases to transcatheter valve-in-valve solutions
Enhancing Safety in Regional Anesthesia: Guidelines from the Italian Society of Anesthesia, Analgesia, Resuscitation and Intensive Care (SIAARTI)
Abstract Background Regional anesthesia techniques have become integral to modern perioperative care, offering enhanced pain management and recovery outcomes. However, their application in patients with specific conditions, such as anticoagulation therapy or preexisting comorbidities, raises concerns regarding safety and efficacy. Current guidelines addressing these issues are fragmented, necessitating comprehensive, evidence-based recommendations. Methods A multidisciplinary panel of experts in anesthesiology and pain management was convened under the auspices of the Italian Society of Anesthesia, Analgesia, Resuscitation, and Intensive Care (SIAARTI). The guidelines presented herein were developed according to the GRADE system (Grading of Recommendations of Assessment Development and Evaluations), in compliance with the methodological manual for the production of clinical practice guidelines published by the National Center for Clinical Excellence, Quality, and Safety of Care, Italian National Institute of Health. Results The guidelines encompass recommendations on neuraxial blocks in anticoagulated patients, the dual guidance use in peripheral nerve blocks, the role of sterile field preparation, and post-procedural monitoring. Evidence from meta-analyses and large-scale observational studies supported most recommendations, though limitations in study heterogeneity were noted. Conclusions These guidelines provide a structured framework for clinicians to enhance patient safety and procedural efficacy in regional anesthesia. Further research is encouraged to address identified gaps, particularly regarding specific patient subgroups and novel regional anesthesia techniques
