Untersuchung von Schäden des Fesseltragapparates in Zusammenhang mit dem Fesselgelenkswinkel beim Islandpferd
The aim of the present study was to investigate a correlation between the dorsal fetlock joint angle and the angle of the sesamoid bones in relation to the canon bone in connection with the occurrence of damage to the suspensory ligament in Icelandic horses. In addition, whether the horses in this study were ridden four gaited or five gaited, was investigated in this context. For this purpose, 22 lateromedial radiographs of the toe or the fetlock joint of 9 matched pairs were measured and assessed. Through research, the 18 Icelandic horses were previously selected and paired with each other in the best possible way. A matched pair consists of one Icelandic horse with damage in the suspensory ligament and one matched Icelandic horse without such a diagnosis. The criteria for a matched pair were as follows: There had to be a lateromedial radiograph of the toe or fetlock of the limb with the diagnosed suspensory ligament damage or the leg that was to be compared, the age of the horses in the matched pair had to be as close as possible, also the sex and and the gait use had to be identical. To categorize the horses in gait use categories, the information was taken from the online database WorldFengur. The horses were divided into three categories: “four gait use” when they were ridden in the basic gaits walk, trot, canter and tölt, “five gait use” if they were ridden in the basic gaits, tölt and flying pace, and the category “no specification” for horses which gait use could not be taken from WorldFengur. The dorsal fetlock joint angle and the angle of the sesamoid bones were measured three times by the author and three times in a blinded procedure by the supervisor for all available radiographs. The generated data were calculated using the statistical software SPSS from IBM. A significantly difference in the dorsal fetlock joint angle and the angle of the sesamoid bones was found in the diseased horses compared to the control group. Thus, the first hypothesis can be considered confirmed. The calculations for the correlation between the ridden gaits and the dorsal pastern angle did not show a significant correlation. Thus, this hypothesis could not be confirmed. A larger number of subjects is needed for studies of this kind and further research in this area. The present study provides information on the correlation between dorsal fetlock joint angle, the angle of the sesamoid bones in relation to the canon bone and the damage to the fetlock suspensory apparatus and can be considered as a basis for further studies with this topic.Diplomarbeit - Veterinärmedizinische Universität Wien - 2022Ziel der vorliegenden Arbeit war es, eine Korrelation zwischen dem dorsalen Fesselgelenkswinkel und dem Gleichbeinwinkel im Zusammenhang mit auftretenden Schädigungen des Fesselträgers beim Islandpferd darzustellen. Zusätzlich wurde die Gangnutzung, ob die Pferde viergängig oder fünfgängig genutzt wurden, in diesem Zusammenhang untersucht. Dazu wurden 22 lateromediale Röntgenbilder der Zehe oder des Fesselgelenks von 9 Matched Pairs ausgemessen und beurteilt. Durch eine Recherche wurden zuvor die 18 Islandpferde ausgewählt und miteinander bestmöglich verpaart. Ein Matched Pair besteht aus einem Islandpferd mit der Diagnose Fesselträgerschaden und einem Islandpferd ohne solche Diagnose. Die Kriterien für ein Matched Pair waren wie folgt: Es musste ein lateromediales Röntgenbild der Zehe oder des Fesselgelenks der Gliedmaße mit diagnostiziertem Fesseträgerschaden bzw. dem Bein, dass verglichen werden soll, vorhanden sein, das Alter der Pferde des Matching Pair sollte möglichst nah beieinander liegen, und das Geschlecht und die Gangnutzung müssen miteinander übereinstimmen. Die Gangnutzung wurde dabei aus der Online-Datenbank WorldFengur entnommen. Die Gangnutzung wurde in drei Kategorien aufgeteilt: „viergängige Nutzung“ mit den Grundgangarten Schritt, Trab, Galopp und Tölt, „fünfgängige Nutzung“ mit den Grundgangarten, Tölt und Rennpass, und die Kategorie „keine Angabe“ bei Pferden, bei denen die Gangnutzung nicht aus WorldFengur zu entnehmen war. Der dorsale Fesselgelenkswinkel und der Gleichbeinwinkel wurde bei allen vorhandenen Röntgenbildern drei Mal von der Diplomandin und drei Mal in einem verblindeten Verfahren von der Betreuerin ausgemessen. Die generierten Daten wurden mithilfe der Statistiksoftware SPSS von IBM geprüft. Es konnte ein signifikanter Unterschied im dorsalen Fesselgelenkswinkel und im Gleichbeinwinkel bei den erkrankten Pferden im Vergleich zur Kontrollgruppe festgestellt werden. Somit kann die erste Hypothese als bestätigt angesehen werden. Die Berechnungen für den Zusammenhang zwischen Gangnutzung und den dorsalen Fesselgelenkswinkel ergab keine signifikante Korrelation. Damit konnte diese Hypothese nicht bestätigt werden. Hierbei bedarf es einer größeren Menge an Probanden für Studien dieser Art und weitere Forschung auf diesem Gebiet. Die vorliegende Arbeit liefert Informationen über den Zusammenhang zwischen dorsalem Fesselgelenkswinkel, Gleichbeinwinkel und einer Schädigung des Fesseltrageapparates und kann als Grundlage für weitere Studien mit dieser Thematik angesehen werden
Evaluation of a newly described rostrotentorial craniotomy approach in cats for consistency in size and shape

For this purpose, all data were uniformly integrated into a coordinate system and the brains were isolated from the surrounding soft tissue. In addition, the brain surface and the gyri located on it were marked and color coded. Subsequently, the brains were anatomically accurately merged with the CT images, resulting in 3D models of cat skulls with anatomically accurately embedded brains. Subsequently, the virtual access was performed on the obtained 3D models with predefined drilling points, considering the vascular, nervous, and muscular structures. It was investigated which gyri could be accessed through this approach and to what extent. The results have shown that through this approach, in each cat (100 %), the Gyrus ectomarginalis could be reached at an average of 93.6 %-95.4 %. In each cat (100 %), the Gyrus ectosylviuss could be reached at an average of 88.3 %-89.6 %. In each cat (100 %), the Gyrus intersylvius could be reached at an average of 76.5 %-84.6 %. The Gyrus occipitalis could be reached on the left side in all cats (100 %) and on the right side in five of six cats (83.3 %) at an average of 56.6 %-73 %. Zu diesem Zweck wurden alle Daten einheitlich in ein Koordinatensystem integriert und die Gehirne von den umgebenden Weichteilen isoliert. Zusätzlich wurden die Hirnoberfläche und die darauf befindlichen Gyri markiert und farblich kodiert. Anschließend wurden die Gehirne anatomisch genau mit den CT-Bildern zusammengeführt, so dass 3D-Modelle von Katzenschädeln mit anatomisch genau eingebetteten Gehirnen entstanden. Anschließend wurde der virtuelle Zugang an den erhaltenen 3D-Modellen mit vordefinierten Bohrpunkten unter Berücksichtigung der vaskulären, nervösen und muskulären Strukturen durchgeführt. Es wurde untersucht, welche Gyri auf diese Weise zugänglich sind und in welchem Umfang. Die Ergebnisse haben gezeigt, dass mit diesem Zugang bei jeder Katze (100 %) der Gyrus ectomarginalis zu durchschnittlich 93,6 %-95,4 % erreicht werden konnte. Bei jeder Katze (100 %) konnte der Gyrus ectosylvius im Durchschnitt zu 88,3 %-89,6 % erreicht werden. Bei jeder Katze (100 %) konnte der Gyrus intersylvius im Durchschnitt zu 76,5 %-84,6 % erreicht werden. Der Gyrus occipitalis konnte auf der linken Seite bei allen Katzen (100 %) und auf der rechten Seite bei fünf von sechs Katzen (83,3 %) zu durchschnittlich 56,6 %-73 % erreicht werden. Der Gyrus compositus caudalis konnte auf der rechten Seite bei allen Katzen (100 %) und auf der linken Seite bei fünf von sechs Katzen (83,3 %) mit durchschnittlich 14,8 %-15,8 % erreicht werden. Der Gyrus sylvius wurde bei fünf von sechs Katzen (83,3 %) mit durchschnittlich 8,5 %-16,6 % auf beiden Seiten erreicht. Der Gyrus praecruciatus wurde nur bei einer Katze (16,7 %) auf der rechten Seite zu 2,4 % und bei zwei Katzen (33,3 %) auf der linken Seite zu durchschnittlich 9,5 % freigelegt. Der Gyrus compositus rostralis konnte auf beiden Seiten bei zwei von sechs Katzen (33,3 %) mit durchschnittlich 4,9 %-7,8 % erreicht werden. Der Gyrus proreus konnte bei keiner Katze erreicht werden. Ein transfrontaler Zugang wäre für diesen Zweck besser geeignet. Da die Zahl der brachycephalen Katzen so gering war, konnte kein statischer Vergleich zwischen mesocephalen und brachycephalen Katzen vorgenommen werden
Vergleich der transfrontalen Zugänge nach Glass et al. (2000) und nach Kostolich und Dulisch (1978) mit dem Zugang von de Wet et al. (1982) bei brachyzephalen und mesozephalen Katzen
Diplomarbeit - Veterinärmedizinische Universität Wien - 2021Für diese Diplomarbeit wurden Schädel CTs und MRTs von vier mesozephalen und zwei brachycephalen Katzen verwendet, die von der Abteilung für Bildgebende Diagnostik der Veterinärmedizinischen Universität Wien, zur Verfügung gestellt wurden. Drei unterschiedliche, bereits bei Hunden beschriebene transfrontale Kraniotomiezugange, wurden virtuell untersucht: Der Zugang von Glass et al. (2000), von Kostolich/Dulisch (1987) und von de Wet et al. (1982). Die Verarbeitung der CT- und MRT-Bilder erfolgte mithilfe der Software Amira. Die Modelle wurden einheitlich ausgerichtet, um für die weiteren Arbeitsschritte vergleichbar zu sein. Auf den Schädelmodellen wurden Markierungen gesetzt, um die Schädel einheitlich vermessen zu können. Das Gehirn wurde aus den MRT Bildern extrahiert und die Auflösung digital erhöht. Auf den Gehirnen wurden weiter die Gyri eingezeichnet und farblich markiert. Danach wurden die drei Zugange virtuell an allen Schädeln simuliert und die Kraniotomieflachen sowie die freigelegte Fläche des Gehirns und der Gyri ausgemessen. Weiterführend wurden die drei Zugange miteinander verglichen und statistisch ausgewertet. Die Ergebnisse zeigen, dass man mit dem Zugang nach Glass die größte Kraniotomieflache erzielt. Der Zugang nach Glass bietet ebenfalls die beste Übersicht über den Frontallappen. Des Weiteren zeigen die Ergebnisse, dass der Zugang zum Bulbus olfactorius am besten mit dem Zugang nach Kostolich/Dulisch gelingt. Mit dem Zugang nach de Wet bekommt man keinen Zugang zum Bulbus olfactorius oder den kranialen Gyri am Gehirn. Außerdem gibt es keine signifikanten Unterschiede zwischen mesozephalen und brachyzephalen Katzenrassen. Der Vergleich aller drei Zugange miteinander war sehr schwer, da sich der Zugang nach de Wet sehr von den anderen unterscheidet. Ein weiterer großer Nachteil war, dass die Gruppe der brachyzephalen Katzen nur aus zwei Tieren bestand und sich somit eine statistische Vergleichbarkeit von brachyzephalen und mesozephalen Katzen als unmöglich herausstellte.For this diploma thesis, skull CTs and MRIs of four mesocephalic and two brachycephalic cats were used, which were provided by the Department of Diagnostic Imaging of the University of Veterinary Medicine Vienna. Three different transfrontal craniotomy approaches, already described in dogs, were examined virtually: The approach of Glass et al. (2000), of Kostolich/Dulisch (1987) and of de Wet et al. (1982). The CT and MRI images were processed using Amira software. The models were aligned uniformly in order to be comparable for the further work steps. Markings were placed on the skull models in order to be able to measure the skulls uniformly. The brain was extracted from the MRI images and the resolution was digitally increased. The gyri were further marked on the brains and colour coded. Afterwards, the three approaches were virtually simulated on all skulls and the craniotomy areas as well as the exposed area of the brain and the gyri were measured. The three approaches were then compared with each other and statistically evaluated. The results show that the largest craniotomy area is obtained with the Glass approach. The Glass approach also offers the best overview of the frontal lobe. They also show that the best approach to the olfactory bulb is the Kostolich/Dulisch approach. The de Wet approach does not provide access to the olfactory bulb or the cranial gyri on the brain. Furthermore, there are no significant differences between mesocephalic and brachycephalic cat breeds. Comparing all three approaches with each other was very difficult, as the de Wet approach is very different from the others. Another big disadvantage was that the group of brachycephalic cats consisted of only two animals and thus a statistical comparability of brachycephalic and mesocephalic cats turned out to be impossible
Evaluation of the described rostrotentorial craniotomy in five mesaticephalic dogs (25-35 kg) for consistency in size and shape

Anatomic- al landmarks are sparse and regions of the brain accessed are not specified in detail. The study evaluated the rostrotentorial approach after the description published in the book ‘Small Animal Surgery’ by Fossum et al. (3rd and 4th edition). The aim was to get a comparison in consistency in size and shape and to further discuss the outcome and difficulties that may occur due to given instructions. The clinical study was performed on five cadavers by the institute of small animal surgery, University of Veterinary Medicine in Vienna. The surgeons were not familiar with the craniotomy approach and had to follow the given instructions. The five specimen were mesaticephalic only and were no longer deceased than 36 hours. The rostrotentorial appoach was performed on both sides by different surgeons. Questions, comments and difficulties during the trial were noted in protocols. Afterwards CT- And MRI- scans were performed and the data was sent to the imaging facility, at VetMedUni Vienna. The images were further evaluated by using the 3D animated data program Amira 6.4.4 (Thermo Fischer Scientific) The created three dimensional models were then used to create virtual craniotomy areas, accessible cortex areas and the Gyrus areas in detail. The data was further measured and analysed. By using the rostrotentorial approach by Fossum et al. the study showed that the areas of the Lobus frontalis, parietalis and occipitalis were partly accessible as described, showing a tendency to specific Gyri and the likelihood of reaching them. Parts of Gyrus suprasylvius rostralis, Gyrus suprasylvius medius, Gyrus ectomarginalis, and Gyrus marginalis et endomarginalis were accessible in every single craniotomy approach. The craniotomy areas and accessible areas on the brain cortex are of similar sizes, but the accessibility of the brain regions seem to differ in the five specimen. Evaluating the data obtained it can be said that the instructions for the approach showed to be indecisive, giving the surgeons difficulties to proceed a secure and consistent way to per- form in this field.Die in der veterinärmedizinischen Literatur beschriebenen Kraniotomie Zugänge bei Hund und Katze sind bis heute nicht einheitlich definiert. Anatomische Orientierungspunkte sind spärlich und die zugänglichen Hirnbereiche nicht im Detail beschrieben. Diese Studie unter- suchte den rostrotentorialen Zugang, anhand der Beschreibung des Buches ‘Small Animal Surgery’ von Fossum et al. (3. und 4. Edition). Ziel dieser Studie war es, die Reproduzierbarkeit des Zuganges in Bezug auf Größe und Ausbreitung zu evaluieren, die Ergebnisse und Problemstellungen anhand der vorgelegten Anleitung zu diskutieren und zu vergleichen. Die klinische Studie wurde an fünf Hundeleichen am Institut für Kleintierchirurgie an der Veterinärmedizinischen Universität Wien durchgeführt. Die Chirurgen waren mit dem rostrotentorialen Zugang nicht vertraut und hatten den vorgelegten Instruktionen zu folgen. Alle fünf Hunde entsprachen dem mesaticephalen Schädeltyp und die Kadaver waren nicht älter als 36 Stunden. Der Zugang wurde auf beiden Schädelseiten von jeweils unterschiedlichen Chirurgen durchgeführt. Fragen, Kommentare und Probleme in der Ausführung wurden protokolliert. Die anschließend vorgenommenen CT und MRT Scans des Kopfes wurden an die ‘Imaging facility’ der VetMedUni Wien übertragen. Zur weiteren Bearbeitung und Evaluierung fand das 3D-Bildverarbeitungsprogramm Amira 6.4.4. (Thermo Fischer Scientific) Verwendung. Anhand der erstellten dreidimensionalen Modelle konnten virtuell die Kraniotomie Flächen, die zugänglichen Bereiche und die Gyri im Detail dargestellt werden. Die Messungen wurden anschließend analysiert und ausgewertet. Durch den rostrotentorialen Zugang konnten Teilbereiche des Lobus frontalis, parietalis und occipitalis wie im Buch beschrieben freigelegt werden. Teile des Gyrus suprasylvius rostralis, Gyrus suprasylvius medius, Gyrus ectomarginalis, und Gyrus marginalis et endomarginalis konnten in jedem Versuch dargestellt werden. Obwohl die Kraniotomie Flächen und freigelegten Areale eine vergleichbar große Fläche zeigten, konnte man Unterschiede in ihrer Position erkennen. Vergleicht man die gewonnenen Daten, kann man sagen, dass durch den beschriebenen rostrotentorialen Zugang ähnliche Hirnbereiche dargestellt werden konnten. Virtual assessment of the accessible frontal lobe in brachycephalic and mesaticephalic dogs
Diploma thesis - University of Veterinary Medicine Vienna - 2021A transfrontal craniotomy is defined as an opening of the skull from cranially. This can be done for decompression or to extract neoplasms. (Kramer et al. 2007) Certain reference points and anatomical structures need to be taken into consideration. Given that the skulls of our dogs differ more in shape and size than any other species, the reference points may vary. (Drake and Klingenberg 2010) For this thesis CT and MRI scans from eight dogs were obtained from the Department of diagnostic imaging of the University of Veterinary Medicine Vienna. The CT and MRI scans were visualized using Amira 2019.1-2019.4, and 3D-Models of the skull as well as the brain were created. The dogs were categorized into two groups, brachycephalic and mesaticephalic, according to their Cephalic Index, using the method of Roberts et al (2010). The group was splitted at the median and then the dogs were categorized into “High CI” and “Low CI”. For the plotting of the Gyri, the brain was halved, and the Gyri marked individually in a strict order and a specific colour pattern. For the olfactory bulb, another brain surface was created. The approaches of Kostolich et al (1987) as well as Glass et al (2000) were executed on the skull surfaces. The accessible brain area was then marked by rotating and turning the skull. This was done to get the results as close as possible to a real surgical environment. This can be done for decompression or to extract neoplasms. (Kramer et al. 2007) Certain reference points and anatomical structures need to be taken into consideration. Given that the skulls of our dogs differ more in shape and size than any other species, the reference points may vary. (Drake and Klingenberg 2010) For this thesis CT and MRI scans from eight dogs were obtained from the Department of diagnostic imaging of the University of Veterinary Medicine Vienna. The CT and MRI scans were visualized using Amira 2019.1-2019.4, and 3D-Models of the skull as well as the brain were created. The dogs were categorized into two groups, brachycephalic and mesaticephalic, according to their Cephalic Index, using the method of Roberts et al (2010). The group was splitted at the median and then the dogs were categorized into “High CI” and “Low CI”. For the plotting of the Gyri, the brain was halved, and the Gyri marked individually in a strict order and a specific colour pattern. For the olfactory bulb, another brain surface was created. The approaches of Kostolich et al (1987) as well as Glass et al (2000) were executed on the skull surfaces. The accessible brain area was then marked by rotating and turning the skull. This was done to get the results as close as possible to a real surgical environment. Die häufigsten Einsatzgründe dafür sind einerseits die Dekompression, bei erhöhtem Hirndruck (Kramer et al. 2007), sowie auch die Extraktion von Tumoren aus dem vorderen Bereich des Gehirns. Für die Trepanation gibt es verschiedene anatomische Bezugspunkte, auf die man zurückgreifen kann, jedoch erschwert die große Diversität der Hundeschädel, gerade in Bezug auf die Größe und Form die Sache. (Drake and Klingenberg 2010) Für diese Diplomarbeit wurden Schädel CTs und MRTs von acht Hunden verwendet, die wir von der Abteilung für Bildgebende Diagnostik der Veterinärmedizinischen Universität Wien, bekommen haben. Diese Daten wurden dann weiter in Amira 2019.1-2019.4 visualisiert und für die weiteren Schritte prozessiert. Für die weiteren Schritte wurden 3D Modelle von sowohl den Schädeln als auch den Gehirnen erstellt. Die Modelle wurden einheitlich ausgerichtet, um für die weiteren Arbeitsschritte vergleichbar zu sein. Auf den Schädelmodellen wurden Markierungen gesetzt, um die Schädel einheitlich vermessen zu können. Die genaue Auflistung der 34 gemessenen Punkte ist im Appendix in Table 31 zu finden, und ist von Drake und Klingenberg (2010). Das Gehirn wurde aus den MRT Bildern extrahiert und die Auflösung digital erhöht. Auf den Gehirnen wurden weiters die Gyri eingezeichnet und farblich markiert (Table 8). An den Schädelmodellen wurden die zwei Zugänge von Kostolich et al. (1987) sowie Glass et al. (2000) durchgeführt. Durch die Öffnungen im Schädel wurde die erreichbare Fläche auf dem Gehirn markiert. Dies geschah auch unter Drehung und Wendung des Schädels, um eine möglichst realistische Darstellung der erreichbaren Fläche zu kreieren. Die Hunde wurden aufgrund ihrer Schädelmaße in zwei Gruppen aufgeteilt: „High CI“ und Low CI“. Dies erfolgte nach den Richtlinien von Roberts et al. (2010), wobei der Median herangezogen wird und dadurch die Gruppen geteilt werden. Diese Gruppen unterschieden sich statistisch signifikant in der Gesichtsschädellänge (p=0.006), der Schädellänge (p=0.026), der Schädelbasislänge (p=0.019), dem Cranialen Index (p=0.020) und dem Cephalischen Index (p=0.000). Kein signifikanter Unterschied konnte für die Kraniotomiefläche sowie die erreichbare Hirnfläche evaluiert werden. Aber die deskriptive Statistik gibt Anhaltspunkte darüber, dass mit dem Zugang nach Kostolich, bei brachycephalen Hunden, mehr Hirnfläche zu erreichen war als mit dem Zugang nach Glass. Das Gegenteil war der Fall bei den mesaticephalen Hunden. Dennoch schien der Zugang nach Glass einen besseren Zugang zum Lobus Frontalis zu gewähren, 44.84%, als der Zugang nach Kostolich mit 33.00%. In der brachycephalen Hundegruppe war der olfaktorische Bulbus nicht darstellbar mit dem Glass Zugang (p=0.029). Auch in der Gruppe der mesaticephalen Hunde war der Zugang nach Glass dem Kostolich Zugang unterlegen, wenn es um die Erreichbarkeit der ventralen Anteile sowie des olfaktorischen Bulbus ging
Evaluation of a newly described rostrotentorial craniotomy approach in brachycephalic dogs for consistency in size and shape in comparison with mesaticephalic breeds
Diplomarbeit - Veterinärmedizinische Universität Wien - 2021Ein bereits beschriebener rostrotentorialer Kraniotomiezugang bei mesaticephalen Hunderassen (Steyrer 2018) wird auf die anatomischen Gegebenheiten und Besonderheiten von brachycephalen Hunden angepasst um auch bei diesen einen möglichst sicheren und dennoch zweckdienlichen chirurgischen Zugang zu erhalten. Dazu wurden im Vorfeld Schädelknochenpräparate von brachycephalen Hunderassen des Instituts für Topographische Anatomie der VUW auf allfällige Besonderheiten in deren Anatomie untersucht und größere Auffälligkeiten festgehalten. In der Folge stellte die klinische Abteilung für Bildgebende Diagnostik der VUW Ihre Datenbank für den eigentlichen Versuch zur Verfügung. Alle seit 2010 verfügbaren CT und MRI Scans physiologischer Schädel von mesati- und brachycephalen Hunden wurden in die Studie inkludiert. Die Verarbeitung dieser Scans erfolgte mithilfe des Programms Amira (Thermo Fischer Scientific) in der Version 2019.01. Die Schädelknochen wurden in Amira vermessen und auf Basis der anschließenden Berechnungen in zwei Gruppen (mesati- und brachycephal) eingeteilt. Zusätzlich wurden sie auf anatomische Variationen innerhalb des Schädelknochens untersucht. Ein wesentlicher Unterschied der beiden Schädelformen ist das Vorhandensein und die Ausprägung des beidseitigen Sinus frontalis sowie der medianen Crista sagittalis externa und der Protuberantia occipitalis. Um einen sicheren Zugang zu gewährleisten wurde die Maximalausdehnung der Sinus frontales, der Protuberantia occipitalis und der Crista sagittalis externa in Amira ausgemessen und der von Steyrer (2018) beschriebene Zugang unter Bedachtnahme auf entsprechende Sicherheitsabstände abgeändert. Danach wurden sowohl der bereits beschriebene als auch der neu konzipierte Kraniotomiezugang virtuell an allen Schädeln simuliert und die Kraniotomieflächen sowie die freigelegte Fläche des Gehirns und der Gyri ausgemessen. Weiterführend wurden die beiden Zugänge miteinander verglichen und statistisch ausgewertet. Die Ergebnisse zeigen, dass der neu angepasste Kraniotomiezugang bei brachycephalen Hunderassen eine Vielzahl an Gyri freilegt. Der Großteil des Gyrus ectosylvius rostralis, Gyrus ectosylvius caudalis, Gyrus suprasylvius rostralis, Gyrus suprasylvius medius, Gyrus suprasylvius caudalis und der gesamte Gyrus ectosylvius medius werden dargestellt. Die folgende Table 15 vergleicht die durchschnittlich zugängliche Fläche jedes einzelnen Gyrus in Prozent von beiden Zugängen und Schädelformen.Die Vergleichbarkeit mit dem Zugang von Steyrer (2018) ist vorsichtig zu beurteilen, da dieser bei strikter Ausführung bei brachycephalen Hunden zu nicht geeigneten Kraniotomien führt welche ein Risiko der Verletzung des Sinus sagittalis dorsalis bergen. Weiters stimmt diese Diplomarbeit mit vorherigen Berichten einer Inkonsistenz der Sinus frontales bei brachycephalen Hunden überein. Der „Approach Mesaticephalic“ öffnete mindestens einen Sinus frontalis bei zwei von acht Hunden, beide mesaticephal. Der „Approach Brachycephalic“ führte zu keiner Öffnung bei brachycephalen, aber zur Öffnung des Sinus frontalis von drei mesaticephalen Hunden. Daher kann man sagen, dass beide Zugänge sicher für brachycephale, aber nicht für mesaticephale Hunde sind. Zusammenfassend kann man sagen, dass der neu beschriebene „Approach Brachycephalic“ aufgrund der höheren Gefahrlosigkeit und Reproduzierbarkeit bei brachycephalen Hunden besser geeignet ist und daher bei diesen bevorzugt werden sollte. Der „Approach Mesaticephalic“ erzielt bei mesaticephalen Hunden gute Ergebnisse und sollte deswegen auch bei diesen angewandt werden.Diploma thesis - University of Veterinary Medicine Vienna - 2021A rostrotentorial craniotomy approach in brachycephalic dog breeds is described with its anatomical features and circumstances of brachycephalia and then compared to an already described approach in mesaticephalic dogs (Steyrer 2018) regarding its safety and consistency. For this, the Institute of Topographic Anatomy of the VUW provided specimen of brachycephalic dog breeds prior to the study which were examined for possible anatomical differences. Afterwards the Clinical Unit of Diagnostic Imaging of the VUW made the database available for the study. All mesati- and brachycephalic dogs with available CT and MRI scans of the skull since the year 2010 without pathologies of the brain were included in this study. The processing of these scans was done in Amira (Thermo Fischer Scientific) in the version 2019.01. The skulls were measured in Amira and based on follow-up calculations split into two groups (mesati- and brachycephalic). Additionally, they were scanned for anatomical variations inside the skull. A substantial difference in these two skull shapes is the presence and extent of the bilateral sinus frontalis as well as the median crista sagittalis externa and the protuberantia occipitalis. To ensure a safe surgical approach, the maximal expansion of the sinus frontales, the protuberantia occipitalis and the crista sagittalis externa were measured and the approach from Steyrer (2018) under consideration of appropriate safety margins was amended. Subsequently the already described as well as the newly designed craniotomy approaches were simulated virtually on all skulls. The craniotomy area together with the according gyri area was measured and furthermore both approaches were compared and statistically analysed. The results show that the newly adapted craniotomy approach in brachycephalic dogs exposes a variety of gyri. The majority of the gyrus ectosylvius rostralis, gyrus ectosylvius caudalis, gyrus suprasylvius rostralis, gyrus suprasylvius medius, gyrus suprasylvius caudalis and entire gyrus ectosylvius media are exposed. The following Table 14 compares the mean accessible area of each gyrus in percent of both approaches and both skull shapes.
The comparability to the approach from Steyrer (2018) is to be taken cautiously, because strictly following the guide leads to an impossible craniotomy in brachycephalic dogs which would risk a laceration of the sinus sagittalis dorsalis. Furthermore, this study agrees with previous findings, that the sinus frontalis is inconsistent in brachycephalic dogs. The “Approach Mesaticephalic” opened at least one sinus frontalis in two out of eight dogs, both mesaticephalic. The “Approach Brachycephalic” lead to trepanation of no brachycephalic, but three mesaticephalic dogs. This means that in this study both approaches were safe regarding opening of the sinus frontales in brachycephalic, but not for mesaticephalic dogs. In conclusion the newly designed “Approach Brachycephalic” is more feasible in brachycephalic dogs due to its safety and reproducibility and the “Approach Mesaticephalic” is best for mesaticephalic dogs
Virtual suboccipital craniotomy approach in cats

Bony landmarks, that enable a maximum expansion of the standard suboccipital approach without insulting vital structures, were described. Also, the difference in accessibility of the cerebellum between a standard suboccipital approach and a rostrolaterally extended approach on the left and on the right side, respectively, was captured. Using the Amira® software to process the imaging data, 3D models of the feline skull and brain were created. Craniotomy windows were virtually cut and the accessible underlying area of the cerebellum and its parts was measured. The measurements were then put into relation with the previously measured entire surface of the cerebellum and its parts. The results show that an average of 26.5 % of the occipital bone is removed for the maximum extension of the standard approach. Furthermore, they show, that only 24.6 % of the cerebellar surface is visible through the standard approach. A rostrolateral extension to the left side increases the accessibility of the cerebellum by 73.6 % and 80.6 % to the right side compared to the standard suboccipital approach. The access to the ipsilateral hemisphere is even increased by more than 200 %. Es wurden knöcherne Orientierungspunkte beschrieben, die eine maximale Ausdehnung des subokzipitalen Standardzugangs ermöglichen, ohne vitale Strukturen zu verletzen. Auch der Unterschied in der Zugänglichkeit des Kleinhirns zwischen einem suboccipitalen Standardzugang und einem rostrolateral erweiterten Zugang auf der linken bzw. rechten Seite wurde erfasst. Mit Hilfe der Amira®-Software zur Verarbeitung der Bildgebungsdaten wurden 3D-Modelle des felinen Schädels und Gehirns erstellt. Die Kraniotomie-Fenster wurden virtuell ausgeschnitten und die zugängliche Oberfläche des Kleinhirns und seiner Teile wurde vermessen. Die Messungen wurden dann mit der zuvor gemessenen Gesamtoberfläche des Kleinhirns und seiner Teile in Beziehung gesetzt. Die Ergebnisse zeigen, dass bei der maximalen Ausdehnung des Standardzugangs durchschnittlich 26,5 % des Hinterhauptbeins entfernt werden. Außerdem zeigen sie, dass nur 24,6 % der Kleinhirnoberfläche durch den Standardzugang sichtbar ist. Eine rostrolaterale Erweiterung auf der linken Seite erhöht die Zugänglichkeit des Kleinhirns um 73,6 % und um 80,6 % auf der rechten Seite im Vergleich zum subokzipitalen Standardzugang. Der Zugang zur ipsilateralen Hemisphäre wird sogar um mehr als 200 % verbessert. Angesichts der deutlich verbesserten Sichtbarkeit sollte bei schwer zugänglichen interkranialen Tumoren ein Verschluss des transversalen Sinus, der bei einer rostrolateralen Erweiterung unumgänglich ist, in Betracht gezogen werden
Evaluation of a pterional approach in brachycephalic and mesaticephalic dogs

The extraction of tumors, as well as decompression in the case of increased intracranial pressure (Kramer et al. 2007) are the most common indications. Specific bone points serve as orientation for the performance of the craniotomies, but this can be due to morphological differences due to the breed and diversity of the animal canine skull be complicated. (Drake and Klingenberg 2010) The aim of the study, to find out which of the gyri can be reached, was achieved. To find out whether reaching the gyri applies to the entire population, a larger study is required. The hypothesis that larger portions of the frontal lobe are accessible in brachycephalic dogs due to the shape of the brain was disproved. The olfactory bulb could not be reached either. As part of this diploma thesis, CTs and MRIs of a total of eight dogs, four of them phenotypically belonging to the mesatcephalic breeds and four to the brachycephalic breeds, were made available by the Department of Imaging Diagnostics at the Vetmeduni Vienna. With the help of the Amira 2019.1-2019.4 program, the data was visualized and processed in further steps. The generated 3D models were then aligned according to the work processes found in material and method. According to Table 4 from Drake and Klingenberg (2010), markings were made on the 3D models to ensure consistent measurements. The existing gyri were drawn uniformly, in a strict order and marked in color (Table 3). These two groups showed significant differences in skull base length (p=0.019), skull length (p=0.026), and facial length (p=0.006). Further significant differences were found in both the Cranial Index (p=0.020) and the Cephalic Index (p=0.000). Only few significant differences were found for the accessible gyri, size of the area in square millimetres and percentage of the accessible area, which can be explained by the small size of the study. Descriptively it can be said that the pterional approach in mesaticephalic dogs primarily allows gyri of the frontal lobe to be reached, which are located in the rostral part of the brain surface. Gyri associated with the temporal lobe are reached at a higher frequency in brachycephalic dogs. Gyri of the parietal lobe are reached with similar frequency in both skull types. The olfactory bulbus and gyrus occipitalis could not be reached in any group.Diplomarbeit - Veterinärmedizinische Universität Wien - 2022Als pterionaler approach bezeichnet man die Trepanation des Gehirnschädels in einem Bereich, bei dem sich mehrere Knochen vereinen zu dem sogenannten Pterion. Die Extraktion von Tumoren, sowie auch die Dekompression bei erhöhten Hirndrücken (Kramer et al. 2007) stellen hierbei die häufigste Indikation dar. Für die Durchführung der Kraniotomien dienen jeweils spezifische Knochenpunkte als Orientierung, jedoch kann dies durch morphologische Unterschiede aufgrund der Rasse und Diversität der Hundeschädel erschwert sein. (Drake and Klingenberg 2010) Das Ziel der Studie, herauszufinden welche Gyri zugänglich sind wurde erreicht. Um Kenntnis darüber zu erlangen, ob das Erreichen der Gyri für die Gesamtheit der Polulation gilt, ist eine Studie mit größerem Umfang erforderlich. Die Hypothese, dass größere Teile des Frontallappens bei brachyzephalen Hunden aufgrund der Form des Gehirns erreichbar sind, wurde widerlegt. Der Bulbus olfactorius konnte ebenfalls nicht erreicht werden. Im Rahmen dieser Diplomarbeit wurden CTs und MRTs von insgesamt acht Hunden, vier davon phänotypisch der mesatizephalen Rassen und vier der brachyzephalen Rassen zugehörig von der Abteilung für Bildgebende Diagnostik der Vetmeduni Vienna zur Verfügung gestellt. Mithilfe des Programms Amira 2019.1-2019.4 wurden die Daten visualisiert und in weiteren Schritten prozessiert. Die generierten 3D Modelle wurden im Anschluss daran gemäß der in Material und Methode zu findenden Arbeitsprozesse ausgerichtet. Entsprechend der Tabelle 4 von Drake und Klingenberg (2010) wurden auf den 3D Modellen Markierungen gesetzt, um einheitliche Messungen zu gewährleisten. Die vorhandenen Gyri wurden wurden einheitlich, in gleicher Reihenfolge eigezeichnet und farblich markiert (Table 3). Die pterionale Kraniotomie wurde an den Schädelmodellen durchgeführt. Die erreichbaren Gyri wurden vorab über die Kraniotomiestelle markiert und im Anschluss nach Entfernung des Schädelmodells auf Plausibilität kontrolliert. Diese beiden Gruppen zeigten signifikante Unterschiede in der Schädelbasislänge (p = 0,019), der Schädellänge (p = 0,026) und der Gesichtslänge (p = 0,006). Weitere signifikante Unterschiede wurden sowohl im Cranial Index (p = 0,020) als auch im Cephalic Index (p = 0,000) gefunden. Signifikante Unterschiede in der Erreichbarkeit der einzelnen Gyri konnten nicht evaluiert werden. Dennoch schien es, dass durch den pterionalen Zugang bei mesatizephalen Hunden primär Gyri des Frontallappens erreicht wurden, die sich im rostralen Teil der Hirnoberfläche befinden. Mit dem Schläfenlappen assoziierte Gyri wurden bei brachyzephalen Hunden häufiger erreicht. Gyri des Parietallappens werden bei beiden Schädeltypen ähnlich häufig erreicht. Going Beyond Counting First Authors in Author Co-citation Analysis
The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation
counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings
are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that
only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into
account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed.
