Impact de l’embolisation des fistules duro-veineuses sur la qualité de vie et les symptômes associés chez les patients souffrants d’hypotension intracrânienne spontanée
No full textBackground: Transvenous Onyx embolization of cerebrospinal fluid-venous fistulas (CSFVF) is an emerging and effective treatment for symptomatic spontaneous intracranial hypotension (SIH). This condition significantly impacts patients' quality of life (QoL) through a variety of debilitating symptoms. Methods: Patients were selected from a prospective database of individuals with CSFVF who underwent transvenous Onyx embolization. All participants completed 13 questionnaires assessing their QoL, before and three months after treatment. Clinical and radiological data were retrospectively collected from the database, and the impact of embolization was evaluated across multiple variables. Correlations and stepwise regression analyses were used to explore relationships between QoL and specific domains including headache, audio-vestibular and psychological symptoms, and spiritual well-being. Results: The study included 30 patients (mean age: 60.4 ± 14.1; female-to-male ratio: 2:1) diagnosed with spontaneous intracranial hypotension and CSFVF, that were treated successfully with Onyx embolization. There was no treatment-related morbidity. All 28 patients with headache reported symptom improvement, with 64% achieving complete resolution. QoL scores related to headache significantly improved post-treatment (HIT-6: p = 0.0119; MSQ: p = 0.0004; MIDAS: p = 0.0236). Psychological symptoms like depression and anxiety significantly decreased, while suicidal ideation resolved when present. Significant audio-vestibular QoL improvements were noted for dizziness (p = 0.002) and hearing disturbances (p = 0.021), but not for tinnitus. MRI findings showed a significant reduction in SIH-related brain abnormalities (mean score: 6.3 ± 1.9 to 1.7 ± 1.5 post-treatment). However, changes in global MRI-SIH scores did not correlate with clinical variables, although brain sagging showed a trend toward correlation with headache intensity reduction (r = 0.37, p = 0.06). Conclusion: CSFVF embolization is associated with significant radiological and clinical improvements, leading to enhanced global quality of life for patients with spontaneous intracranial hypotension.Contexte : L'embolisation transveineuse à l’Onyx des fistules duro-veineuses est une option thérapeutique émergente et fiable en cas d'hypotension intracrânienne spontanée (HIS) symptomatique. Cette pathologie a un impact significatif sur la qualité de vie des patients, avec une multitude de symptômes incapacitants. Méthode : Les patients ont été sélectionnés à partir d'une base de données prospective de personnes atteintes d’HIS et ayant bénéficié d’une embolisation transveineuse à l'Onyx de fistule duro-veineuse. Tous les participants ont rempli un ensemble de 13 questionnaires évaluant leur qualité de vie, avant et trois mois après le traitement. Les données radio-cliniques ont été recueillies rétrospectivement à partir de la base de données, et l'impact de l'embolisation a été évalué en fonction de plusieurs variables. Des corrélations et des analyses de régression pas à pas ont été utilisées pour explorer les relations entre la qualité de vie et des domaines spécifiques, notamment les céphalées, les symptômes audio-vestibulaires et psychologiques, et le bien-être spirituel. Résultats : L'étude a porté sur 30 patients (âge moyen : 60,4 ± 14,1 ; ratio femme/homme : 2:1) chez qui on avait diagnostiqué une hypotension intracrânienne spontanée et une fistule duro-veineuse, et qui ont été traités avec succès par embolisation à l'Onyx. Il n'y a pas eu de morbidité liée au traitement. Les 28 patients souffrant de céphalées ont signalé une amélioration de leurs symptômes, 64 % d'entre eux ayant obtenu une résolution complète. Les scores de qualité de vie liés aux céphalées se sont améliorés de manière significative après le traitement (HIT-6 : p = 0,0119 ; MSQ : p = 0,0004 ; MIDAS : p = 0,0236). Les symptômes psychologiques tels que la dépression et l'anxiété ont diminué de manière significative, tandis que les idées suicidaires ont disparu lorsqu'elles étaient présentes. Des améliorations significatives de la qualité de vie audio-vestibulaire ont été notées pour les vertiges (p = 0,002) et les troubles auditifs (p = 0,021), mais pas pour les acouphènes. Les résultats de l'IRM ont montré une réduction significative des anomalies cérébrales liées à l’HIS (score moyen : 6,3 ± 1,9 à 1,7 ± 1,5 après le traitement). Cependant, les changements dans les scores globaux IRM-HIS n'ont pas été corrélés avec les variables cliniques, bien que l'affaissement du cerveau (brain sagging) ait montré une tendance à la corrélation avec la réduction de l'intensité des maux de tête (r = 0,37, p = 0,06) après embolisation. Going Beyond Counting First Authors in Author Co-citation Analysis

counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings
are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that
only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into
account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed. Reasons for these effects are discussed

Get PDF“Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship
Appropriate Similarity Measures for Author Cocitation Analysis

We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature. Note that the objective for the hut study was to evaluate the IRS treatment applications for residual efficacy against Anopheles mosquitoes, including the local An. coluzzii mosquito population. Cows were only used to attract mosquitoes into the huts and no tests were carried out directly on the cows. The author checklist is intended for use with studies where experiments are carried out on animals, which is why we have had such difficulty in completing this for the hut study, as many of the questions do not relate to how the cows were used
