The Value of Baseline [18F]FDG-PET in Predicting the Progression-Free Survival in Patients with Thymic Epithelial Tumours: A Systematic Review and Meta-Analysis

No full text
Background/Objectives: [18F]FDG-PET is often used for staging thymic epithelial tumours (TETs). However, its prognostic role remains uncertain. The aim of this present systematic review and meta-analysis is to assess the prognostic value of baseline [18F]FDG-PET-derived semiquantitative parameters in predicting progression-free survival (PFS) in patients with TETs. Methods: A systematic review and meta-analysis were conducted according to PRISMA guidelines. PubMed, Embase, and Scopus databases were searched up to 30 May 2025. Studies evaluating the prognostic impact of [18F]FDG-PET parameters on PFS in TETs were included. Pooled hazard ratios (HRs) with 95% confidence intervals (CIs) were calculated. Results: Six retrospective studies involving 593 patients were included. Maximum standardized uptake value (SUVmax), analysed as a continuous variable in four studies, significantly predicted worse PFS (HR: 1.18, 95% CI: 1.08–1.29, p < 0.001), with high inter-study heterogeneity (I2 = 79.7%). When dichotomized (two studies), higher SUVmax was associated with significantly poorer PFS (HR: 9.00, 95% CI: 2.93–27.71). Similarly, mean SUV (SUVmean) as a continuous predictor was also significantly associated with impaired PFS (HR: 1.41, 95% CI: 1.25–1.59), but only two studies assessed this parameter. Conversely, metabolic tumour volume (MTV) and total lesion glycolysis (TLG), both assessed as continuous prognosticators, did not show a significant prognostic value. Notably, in both MTV and TLG analyses, two studies contributed a weight of 0%, reflecting limited precision and highlighting the need for larger data. Conclusions: Baseline [18F]FDG-PET parameters such as SUVmax and SUVmean showed a potential prognostic value in patients with TETs. However, these results are based on a limited number of retrospective studies with significant heterogeneity. Prospective multicentre investigations are necessary to confirm the potential role of [18F]FDG-PET for risk stratification in TETs

MyoData: An expression knowledgebase at single cell/nucleus level for the discovery of coding-noncoding RNA functional interactions in skeletal muscle

No full text
Non-coding RNAs represent the largest part of transcribed mammalian genomes and prevalently exert regulatory functions. Long non-coding RNAs (lncRNAs) and microRNAs (miRNAs) can modulate the activity of each other. Skeletal muscle is the most abundant tissue in mammals. It is composed of different cell types with myofibers that represent the smallest complete contractile system. Considering that lncRNAs and miRNAs are more cell type-specific than coding RNAs, to understand their function it is imperative to evaluate their expression and action within single myofibers. In this database, we collected gene expression data for coding and non-coding genes in single myofibers and used them to produce interaction networks based on expression correlations. Since biological pathways are more informative than networks based on gene expression correlation, to understand how altered genes participate in the studied phenotype, we integrated KEGG pathways with miRNAs and lncRNAs. The database also integrates single nucleus gene expression data on skeletal muscle in different patho-physiological conditions. We demonstrated that these networks can serve as a framework from which to dissect new miRNA and lncRNA functions to experimentally validate. Some interactions included in the database have been previously experimentally validated using high throughput methods. These can be the basis for further functional studies. Using database information, we demonstrate the involvement of miR-149, -214 and let-7e in mitochondria shaping; the ability of the lncRNA Pvt1 to mitigate the action of miR-27a via sponging; and the regulatory activity of miR-214 on Sox6 and Slc16a3. Murder Most Foul. Storia, memoria e mito nella produzione tarda di Bob Dylan

No full text
While many of Dylan's songs became historic anthems of the civil rights movement, their author never misses a chance to remind us that his interest in historical events lies in the possibility of turning them into metahistorical tales. In Chronicles (2004), he writes: "The madly complicated modern world was something I took little interest in. It had no relevancy, no weight. What was up to date for me was stuff like the Titanic sinking, the Galveston flood, John Henry driving steel, John Hardy shooting a man on the West Virginia line. This was the news that I considered". However, in Rough and Rowdy Ways, Dylan taps into contemporary history more than in any other album, clearly stating his intent in "Mother of Muses": "I'm falling in love with Calliope | She don't belong to anyone, why not give her to me?". By invoking such an elitist relationship with the muse of epic poetry, Dylan begins a raging reflection on the meaning of time, history, and memory that culminates in a seventeen-minute track about Kennedy's assassination. References to facts, people, films, and songs from different places and times merge into masterpieces that, reliving memories, reshape history. The present paper examines these themes and mechanics and the labile boundary between memory and history, between written and oral cultures, in the 2016 Nobel laureate's late narrative songs

Congenital Esophageal Atresia Long-Term Follow-Up{ extemdash}The Pediatric Surgeon's Duty to Focus on Quality of Life

No full text
Esophageal atresia (EA) is the most common congenital esophageal malformation. An improvement in survival led to a focus on functional outcomes and quality of life (QoL). We analyzed the long-term outcomes and QoL of patients submitted to surgery for EA. Perinatal characteristics, surgical procedures, gastrointestinal and respiratory current symptoms and QoL were investigated. Thirty-nine patients were included. Long Gap patients had a higher rate of prematurity and low birth weight. The prevalent surgical procedure was primary esophageal anastomosis, followed by gastric pull-up. Twenty-four patients had post-operative stenosis, while gastroesophageal reflux (GER) required fundoplication in eleven cases. Auxological parameters were lower in Long Gap patients. The lowest scores of QoL were in the Long Gap group, especially in younger patients, which was the group with the highest number of symptoms. In the long term, the QoL appeared to be more dependent on the type of esophageal atresia rather than on associated malformations. Surgical management of GER was indicated in all patients with Long Gap EA, supposedly due to the prevalence of gastric pull-up for this type of EA. The assessment of QoL is part of surgeon’s management and needs to be performed in each phase of a child’s development

Pediatric burns represent a significant public health problem. We analyzed the characteristics of pediatric burns in a reference center, in order to identify better strategies for prevention and care. Burn patients admitted to the pediatric departments of our hospital from January 2020 to June 2022 were retrospectively evaluated. Age, gender, the etiology of injuries, the total burn surface area (TBSA), the degree of burns, the length of hospital stay (LOS), concomitant SARS-CoV-2 infection, and burn surface microbial colonization were analyzed. Forty-seven patients were included in the analysis (M:F = 1:0.67). Most of the cases involved patients between 0 and 4 years of age (83%). Hot liquid burns accounted for 79% of cases, flame burns for 9%, thermal burns for 6%, scald burns for 4% and chemical burns for 2%. Mean TBSA was 14 ± 11%. A second-degree lesion was detected in 79% of patients and third-degree in 21%. Mean LOS was 17 days. No additional infection risks or major sequelae were reported in patients with SARS-CoV-2 infection. Fifteen different species of bacteria plus C. parapsilosis were isolated, while no anaerobic microorganisms were detected. In the light of our experience, we recommend a carefully planned and proactive management strategy, always multidisciplinary, to ensure the best care for the burned child
