Kidney stones diseases and glycaemic statuses: focus on the latest clinical evidences
Diabetes and obesity are already recognized as potential risk factors for nephrolithiasis, especially for uric acid stones. Insulin resistance and hyperinsulinemia actively contribute to impaired ability to excrete an acid load and altered ammonium production, leading to a lower urinary pH compared to non-diabetic controls. All these electrolytic disorders play an important role in stone formation and aggregation, especially in uric acid stones. There are still missing points in scientific evidence if the increased risk in stone formation is already existing even in the prediabetic statuses (isolated impaired glucose tolerance, isolated impaired fasting glucose, and associated impaired glucose tolerance/impaired fasting glucose) as well as it is worth to consider the same level of risk. Urolithiasis is the most frequent urological cause of hospitalization in diabetic patients and its cost is usually higher compared to non-diabetic patients, but less is known in others altered glycaemic diseases. The aim of this review article is to focus on the association between stone formation and altered glycaemic statuses, beyond the already known link between nephrolithiasis and diabetes mellitus
Structural characterization of carboxyatractyloside and acaricidal activity of natural ent-kaurene diterpenoids isolated from Chamaeleon gummifer against Tetranychus urticae
Plant-borne secondary metabolites are attracting high interest for their potential use in agricultural applications, with special reference to the control of arthropod pests. In the present work, the structural elucidation of glycosylated diterpenoid carboxyatractyloside (2) isolated from the roots of Chamaeleon gummifer Cass. (Asteraceae) is reported by means of spectroscopic and spectrometric techniques. Complete identification occurred thanks to one- and two-dimensional NMR experiments, assigning the single protons and carbons, and the stereochemistry by the NOESY correlations. Carboxyatractyloside (2), together with two ent-kaurenes atractyloside (1) and atractyligenin (3), extracted from the roots of C. gummifer, have been tested for their acaricidal and oviposition inhibition activity against the two-spotted spider mite, Tetranychus urticae Koch (Acari: Tetranychidae) Notably, compounds 1–3 were toxic to T. urticae, leading to significant mortality, oviposition inhibition, reduced hatchability of eggs, and natality inhibition. However, at the lowest dose (12.5 μg cm−2) compound 2 was the most effective, leading to mortality > 60% after 5 days exposure, inhibiting oviposition by > 70% and egg hatching by 33%; it also reduced natality by 80%. Overall, these compounds represent valuable candidates to develop novel acaricides for crop protection. Further research on how to develop stable formulations for field use, as well as on non-target effects of these compounds on pollinators and mite biocontrol agents, is ongoing

diets in an untrawled area and in two trawled areas off northern Sicily (western Mediterranean): the Gulf of Castellammare (which hosts a 200 km2 no-trawl area) and the gulfs of Termini Imerese and Sant’Agata. Scaldfish were collected in May-June 2005 using a commercial otter trawler between 40 and 80 m on muddy bottoms. Our data show that scaldfish is a selective feeder and consumes similar prey items in the three gulfs, although between-site differences in the relative prey abundance in the stomachs were detected. In the three gulfs scaldfish fed mainly on the crab Goneplax rhomboides and on the fish
Lesueurigobius suerii, but there was an increase in the consumption of L. suerii and the shrimp Alpheus glaber in the trawled areas. Ontogenetic changes were evident in the diet of scaldfish, with juveniles preying mainly upon suprabenthic species, e.g. mysids and small decapods. While significant differences occurred in the diet of juveniles among all areas due to natural spatial variability, variations in the diet of adults, which feed on benthic prey, occurred between untrawled and trawled areas. These variations were clearly due to trawling disturbance, and no significant difference was detected in the diet of adults between the two trawled areas. On the contrary, the trophic level (δ15N values) of scaldfish did not vary between trawled and untrawled areas and between juveniles and adults. Metabolic syndrome and uric acid nephrolithiasis: insulin resistance in focus
Uric acid nephrolithiasis (UAN) is an increasingly common disease in ethnically diverse populations and constitutes about 10% of all kidney stones. Metabolic syndrome and diabetes mellitus are accounted among the major risk factors for UAN, together with environmental exposure, individual lifestyle habits and genetic predisposition. The development and overt manifestation of UAN appears to stem on the background of insulin resistance, which acts at the kidney level by reducing urinary pH, thus hampering the ability of the kidney to generate renal ammonium in response to an acid load. Unduly acidic urinary pH and overt UAN are both considered renal manifestations of insulin resistance. The mechanisms underlying increased endogenous acid production and/or defective ammonium excretion are yet to be completely understood. Although the development of UAN and, more in general, of kidney stones largely recognizes modifiable individual determining factors, the rising prevalence of diabetes, obesity and accompanying metabolic disorders calls for the identification of novel therapeutic approaches and intervention targets. This review aims at providing an updated picture of existing evidence on the relationship between insulin resistance and UAN in the context of metabolic syndrome and in light of the most recent advancements in our understanding of its genetic signature
Perioperative and mid-term results of trans-graft embolization of the hypogastric artery for treatment of type II endoleaks after endovascular aortic repair with off-label use of re-entry catheters
Type II endoleaks after endovascular aortic repair are a common scenario that, although infrequently, may sometimes require secondary interventions when leading to significant enlargement of the aneurysm sac. Herein, we present the perioperative and mid-term results of one of our endovascular aortic repair cases with type II endoleak from the hypogastric artery, whose ostium was covered by the prior stent graft limbs and that were successfully treated with a novel technique employing re-entry catheters in an off-label fashion. This technique may represent a valid alternative solution when conventional access between artery and prosthesis is laborious or impossible to achieve
Prognostic Impact of Anemia and Blood Transfusions on Cardiovascular Outcomes in Patients Undergoing Vascular Surgery: A Scoping Review
Objective: Prior studies suggest an association of anemia and blood transfusion with increased morbidity and mortality in patients undergoing cardiac surgery. However, the impact of perioperative anemia and blood transfusion on clinical outcomes in patients undergoing major vascular surgery has been poorly defined yet. The primary objectives of this scoping review were to determine the extent of the evidence base that links anemia and blood transfusions to mortality and cardiovascular outcomes in patients undergoing major vascular surgery, and identify recurring themes or gaps in the literature to guide future research. Methods: A scoping review of the literature from PubMed, Cochrane, and EMBASE databases was conducted up to December 2023 to identify articles related to the impact of anemia and blood transfusions on postoperative cardiovascular outcomes on patients undergoing vascular surgery. Methodology followed the PRISMA Protocols Extension for Scoping Reviews. Results: Twenty-two articles met the inclusion criteria, including 15 retrospective and 6 observational prospective studies. Anemia definition varied across studies, mainly based on hemoglobin cut-off levels. An association with older age, coronary artery disease, hypertension, diabetes, and other comorbidities was reported. Particularly in peripheral and endovascular aortic surgery, anemia was linked consistently with higher mortality, major adverse cardiac events, and other postoperative complications, such as respiratory and renal issues, surgical site infections, and longer hospital stays, depending on hemoglobin levels. Anemia itself is an important predictor of transfusions. Transfusions in anemic patients were associated with increased mortality, postoperative complications, and increased need for major amputation. Conclusions: The weight of the evidence suggests that anemia carries a substantial burden of cardiovascular complications, mortality, and multiorgan complications, resulting in increased health care costs. Peripheral and endovascular aortic surgery are affected deeply by the impact of anemia. Anemia itself stands out as a crucial predictor for requiring transfusions. In turn, the effect of transfusion of blood products is associated with worse outcomes and complications
Turning casualty into opportunity: fragmenting dislodged colonies is effective for restoring reefs of a Mediterranean endemic coral
Within the framework of ecosystem-based management, restoration appears as a sensible option to counteract the global decline of coral reefs. Several techniques involving sexual and asexual coral propagules have been used for the restoration of reefs. Culturing of fragments has proved fruitful since it takes advantage of the capability of corals to asexually reproduce, providing a number of novel colonies that can be replanted. This method however, when using fragments detached from a colony, might be stressful for the wild donor. Astroides calycularis is an endemic and endangered Mediterranean scleractinian coral forming massive colonies mostly at shallow depth. It is subject to anthropogenic impact, particularly from damage due to accidental contacts by SCUBA divers, and it is expected to suffer from sea storms of increasing power under the projected climate change scenarios. Corals of opportunity (i.e. dislodged colonies found alive on the seabed) may be a useful resource for the restoration of A. calycularis reefs, given that the fragment-based transplant technique is effective for this species as it is for other massive corals. A one-year transplant experiment was carried out along an exposed rocky shore in NW Sicily (Mediterranean Sea) to test the feasibility of using fragments of corals of opportunity for restoration purposes. The transplants revealed high survival rates and higher number of new polyps than in control colonies. The original size of transplanted fragments did not influence their capability to bud new polyps and was not related to their survival rate. The applied technique provides the opportunity to restore rocky reefs, even the very shallow ones, through direct transplant of coral fragments, thus making reef restoration a feasible option in ecosystem-based management plans for this species
Sizing up the role of predators on Mullus barbatus populations in Mediterranean trawl and no-trawl areas
Fishing leads to drastic changes in ecosystems with a net loss of predatory biomass. This issue has been evidenced from historical ecological studies and from the evaluation of the effects of effective and large marine protected areas. In two fishery reserves off the northern Sicily coast the red mullet Mullus barbatus underwent an impressive biomass increment and a few piscivores fish species recovered after a trawl ban. The red mullet, more than 20% of all demersal fish in the untrawled areas, represented a huge food resource to its potential predators. By contrasting two trawled and two untrawled gulfs we figured out predator - prey relationships through the use of a combined approach based on trophic niche, stable isotopes data and biomass. We show that the white grouper Epinephelus aeneus and the black-bellied anglerfish Lophius budegassa are the most important predators of the red mullet with higher biomass in the untrawled areas. We also found a potential re-establishment of the trophic role for white grouper inside the untrawled areas. Our study highlights some of the benefits obtained from the use of large offshore marine protected areas as a tool for ecosystem restoration
Symptomatic Deep Femoral Artery Pseudoaneurysm Endovascular Exclusion. Case Report and Literature Review
Deep femoral artery pseudoaneurysms (DFAPs) are rare and generally occur after penetrating trauma or surgical procedures. A 36-year-old obese man presented with pain in correspondence of the anterior-lateral thigh after 6 months from gunshot wound. Duplex and computed tomography (CT) showed a bilobed right DFAP (maximal diameter 12.9 cm). The patient was managed urgently, under local anesthesia, by placement in the distal DFA of a Viabahn 8 × 100-mm stent graft (W L Gore & Associates, Inc). The postoperative course was uneventful, and the 24-month CT showed regular stent-graft patency and 20-mm DFAP shrinkage. The literature review reported 8 cases of DFAPs; of these 6 were managed by endovascular mean (3 stent-graft implantations and 3 coil embolization). The remaining 2 cases were managed surgically (one of these after failed coil embolization). In conclusion, the use of covered stent graft was effective to treat a DFAP localized in the medium DFA. This tool allowed maintaining the native DFA patency and the preservation of its main branches
