Tailoring adjuvant endocrine therapy in early breast cancer: When, how, and how long?
Endocrine therapy represents the gold standard for the adjuvant treatment of luminal-like early breast cancer, but its personalization is still a major point of debate. To define the most appropriate therapeutic strategy, both the patient's menopausal status at the moment of diagnosis and the individual risk of disease recurrence should be taken into account. Five years of therapy with tamoxifen represent the standard of care for low-risk pre/perimenopausal patients, whilst the combination of ovarian suppression with tamoxifen or an aromatase inhibitor should be considered for high-risk patients. Also, to high-risk patients, an extended strategy can be proposed. Postmenopausal patients, instead, should receive an upfront aromatase inhibitor and an extended strategy can be considered for a high risk of disease recurrence. Aim of this review is to set a focus on the major studies investigating the optimal type and duration of adjuvant endocrine therapy and evaluate emerging options
Going Beyond Counting First Authors in Author Co-citation Analysis
The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation
counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings
are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that
only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into
account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed. Reasons for these effects are discussed
Variations on the Author
“Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship
Appropriate Similarity Measures for Author Cocitation Analysis
We provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature. Clinical Perspectives in the Use of Liquid Biopsy in Metastatic Breast Cancer
Precision medicine aims to improve healthcare by identifying subsets of patients that are more likely to benefit from specific treatment strategies. Liquid biopsy offers the unique potential to better understand and monitor disease biology, since it is not burdened by the temporal and spatial limitations of tissue biopsy. CTC (circulating tumor cell) characterization is being explored as an alternative and flexible tool to identify predictive and prognostic biomarkers thanks to its capability of recapitulating a complete biological system. The present chapter will summarize the current clinical applications of CTC enumeration and its combination with other liquid biopsy technologies such as ctDNA. It will, moreover, offer new insights on non-canonical CTCs, which are emerging as a new entity with respect to epithelial circulating cells, and emerging multi-omics essays that may evolve into new comprehensive profiling workflows
Beyond Hormone Receptors: liquid biopsy tools to unveil new clinical meanings and empower therapeutic decision-making in Luminal-like metastatic breast cancer
Immunohistochemical (IHC) tissue profiling is a standard practice in the management of metastatic breast cancer (mBC), that enables the identification of distinct biological phenotypes based on hormone receptors' expression. Luminal-like tumors primarily benefit from a first line treatment strategy combining endocrine therapy and cyclin-dependent kinase 4/6 inhibitors. However, IHC analyses necessitate invasive procedures and may encounter technical and interpretational challenges. In the current era of precision medicine, liquid biopsy holds potential to provide clinicians with additional insights into disease biology, including mechanisms underlying endocrine resistance and disease progression. Several liquid-based biomarkers are entering clinical practice and hold prognostic and predictive values in Luminal-like mBC, while many others are currently being investigated. The present work aims to summarize the current evidence regarding the clinical meanings of hormone receptors and their downstream molecular pathways, alongside their implications for therapeutic decision-making in Luminal-like mBC
