Enzalutamide in prostate cancer after chemotherapy.
No full textIn the randomized trial reported
by Scher et al. (Sept. 27 issue),1 the administration
of enzalutamide, an androgen-receptor–signaling
inhibitor, was associated with an increased incidence
of headache, as compared with placebo, in
patients with castration-resistant prostate cancer.
Androgens are implicated in the pathogenesis of
migraine; thus, it is important to understand
whether the headache associated with enzalutamide
could be classified as migraine. Moreover,
a deeper understanding of the pathophysiology
of this side effect is clinically relevant, since a
relationship between migraine and the presence
of either hot flashes or seizures has been described
previously.2,3 Migraine and epilepsy have
pathophysiological characteristics in common,3
so patients who have migraine during enzalutamide
therapy may be at increased risk for seizures.
Notably, headache was not reported as a
clinically relevant symptom in patients with castration-
resistant prostate cancer who received
abiraterone,
4 a drug that deeply reduces circulating
androgen levels. Abiraterone was administered
in association with prednisone, and glucocorticoids
are efficacious in preventing migraine.5 It
would be interesting to know whether glucocorticoids
were used in the management of enzalutamide-
induced headache and whether they
were effective
Preliminary results from pore pressure monitoring on an unstable clay slope.
No full textWe monitored positive pore water pressure in a clay slope in the Apennines to investigate how rainfall influences the pore
pressure distribution along the soil profile and, consequently, affects slope stability conditions. Data refers to the first season of
monitoring activities (August 2001–June 2002). The study site is located at the head of a complex landslide, where incipient
shallow failures are visible along the slope.
The preliminary analysis of pressure data allowed to point out consistent trends, which manifested throughout 11 months.
During the cold and wet season, saturation always extends close to the ground surface ( < 1 m) and pore water pressure
measurements indicate a strong downward component of the flow, which is largely dominant over the downslope component, in
any condition. Response to rainfalls at shallow depth (1 to 3 m) generally consists in a pressure pulse whose delay is relatively
short if compared to low permeability of the soil and whose nature appears clearly transient. Although no failure occurred along
the slope during the observation period, it is very likely such transient response would mimic the pore water pressure pattern
capable to cause sliding.
The presence of prefertial flow path in the sub-surface can only explain anomalous response of some sensors or substantial
differences between buried at similar depths

Relation between body mass index and endothelium-dependent vasodilatation in healthy postmenopausal women.
No full textOBJECTIVE: To evaluate whether endothelium-dependent vasodilatation is related to anthropometric parameters in 105 healthy postmenopausal women 47-68 years of age. METHODS: Flow-dependent, endothelium-dependent vasodilatation was considered as the maximal dilatation following deflation of a cuff placed on the forearm and inflated to supra-systolic blood pressure values for 4 min. Endothelium-independent vasodilatation was considered as the maximal dilatation induced by sublingual nitroglycerine (400 microg). RESULTS: Among parameters such as height, weight, body mass index (BMI), waist, hip, waist/hip ratio, lipids, glucose or insulin, only BMI, an indirect index of adiposity, was independently and directly related to baseline brachial artery diameter (b = 0.042, r = 0.269, p = 0.0055) and flow-mediated endothelium-dependent vasodilatation either expressed as net (b = 0.034, r = 0.315, p = 0.001) or percentage (b = 0.376, r = 0.202, p = 0.039) change. Stratification for BMI categories showed that women with BMI < 22 kg/m(2) had an endothelium-dependent vasodilatation, significantly lower than that of women with BMI >or= 30 kg/m(2) (0.711 +/- 0.076 mm vs. 1.107 +/- 0.141 mm; p = 0.0114). BMI was not related to endothelium-independent vasodilatation. CONCLUSIONS: Present results show that, in healthy postmenopausal women, endothelium-dependent vasodilatation is related to BMI, arteries of slender women dilating less than those of their heavier counterparts. A low BMI does not appear to be beneficial for artery vasodilatation in healthy postmenopausal women
