Ionic Hydrogel Monotherapy and in Combination with Antiviral-Antibiotic Prophylaxis, in the Post-procedure Management of Fractional Laser-Treated Patients
Objective: The pre- and postoperative care for fractionated laser resurfacing is still controversial, especially in regard to the use of antibiotics to prevent bacterial infection and potential consequences. Recently, an ionic hydrogel has shown to be useful in the postoperative treatment of minor burns. The aim of this study is to evaluate the use of this hydrogel after fractional laser treatments targeting photoaging and chronoaging damage to skin. Design: A randomized prospective study. Setting: one plastic surgeon private practice. Participant: Fifty patients with chronoaging and photoaging cosmetic issues were enrolled in two different post-treatment regimens: ionic hydrogel alone and ionic hydrogel in combination with antibiotics. Measurements: Patients were evaluated for healing time, complications, and postoperative pain, the latter assessed with a 10-point visual analogue score. A questionnaire to investigate how patients managed through the postoperative phase was also provided to each patient. Results: No significant differences between the two groups were observed in regard to healing time, postoperative pain, complications, and patient satisfaction. Conclusion: Ionic hydrogel alone has shown to provide adequate skin care support in the postoperative phase of fractional-laser-resurfacing-treated patients
Il trattamento chirurgico delle complicanze da tatuaggio
colori della dermatologia: Il volume è dedicato a tutti gli appassionati di tatuaggi che desiderano conoscere l'origine e le tecniche di questa antica arte. I tatuaggi sono diventati un vero fenomeno di massa e, con l'aumento dei tatuati appartenenti a diverse fasce di età, si assiste anche a un incremento delle reazioni avverse associate a tale pratica. Sia in dermatologia che in medicina estetica, verranno descritti i tatuaggi comuni e i tatuaggi estetici con esperienze dermatologiche, cosmetologiche e con tecniche moderne di tatuazione. Viene affrontato il significato psicologico che si cela dietro la scelta di un tatuaggio e l'importanza di affidarsi a centri di massima sicurezza in termini igienici e sanitari con conseguente descrizione delle norme europee vigenti con l'Istituto superiore di sanità. In ambito legale, sono descritti limiti e divieti prima dell'esecuzione di un tatuaggio e i danni morali nel caso di pazienti di età inferiore di 18 anni oltre al tema dell'idoneità ai concorsi militari in caso di concorrenti tatuati. Sono infine descritte le metodiche di rimozione dei tatuaggi con tecniche di chirurgia plastica e i risultati ottenuti grazie a laser sempre più sofisticati

Skin cancers are a common disease, but unusual
is their association with other tumors. The
Authors have studied the patients presenting
skin cancers (BCC and SCC) in association with
a cancer not involving the cutaneous district,
with the purpose to evaluate ali the possible
ethiologies of the skin cancer. This retrospective
research has included all the people treated for
BCC and/or SCC by the Plastic Surgery Unit of
the University of Rome «La Sapienza», during
36 months (March 1987-May 1990). In this
time 433 people have been treated for skin
cancers, among them nine (7 males, 2 females)
presented also another neoplastic disease. We
have observed three kinds of links among the
neoplastic associations: the cutaneous neoplasy
may rise as a consequence of an organic-immunologic
deficit in the patient, may be caused by
the treatment underwent to heal from the first
cancer, or the association seems to happen by

La responsabilità professionale del chirurgo plastico nella mastoplastica additiva.
RIASSUNTO
Il tema della responsabilità professionale del medico è apparso nel tempo in continua evoluzione in virtù dei continui indirizzi giurisprudenziali, sia civili che penali, che hanno determinato un incessante rimaneggiamento della dottrina in merito alle problematiche connesse all’operato medico. Negli ultimi anni infatti la classe medica è stata oggetto di innumerevoli battaglie legali inoltrate da pazienti insoddisfatti delle prestazioni mediche ottenute: prima fra tutte la branca della chirurgia plastica. Con riferimento a tale disciplina, il seguente lavoro trae spunto da una personale casistica inerente 44 richieste di risarcimento del danno, in seguito a presunta condotta medica erronea, da parte di donne sottoposte ad intervento chirurgico di mastoplastica additiva. Vengono così poste in risalto, le cause di insoddisfazione delle pazienti, gli errori professionali ricorrenti ed analizzati gli elementi costituenti l’errore, al fine di fornire al chirurgo plastico un’ampia prospettiva circa le motivazioni che inducono l’avvio di un’istanza risarcitoria. In relazione alla giurisprudenza attuale in tema di responsabilità professionale e consenso e dall’analisi degli errori intercorsi nella relazione medico- paziente si sottolineano alcuni aspetti da curare nella compilazione della cartella clinica.
SUMMARY
The jurisprudential evolution on matter of sanitary responsibility in civil and criminal worlds had produce continuous change of judge’s point of view of the problems connected to the doctor’s action. Often medical doctors are sued to the Court by unhappy patients, asking to justify their professional actions: among the specialists the plastic surgeons are frequently involved in medical litigation cases.
Aim of this paper is to present the Authors experience with 44 claims evaluated after an augmentation mammaplasty procedure. The unsatisfactory reasons leading to medical litigation are analyzed. In this article we report the recurrent causes of dissatisfaction of the patients and the professional errors when occurred with an analysis of the constituent elements of the error. The aim of this study is to supply to the plastic surgeon a wide perspective of the motivations that induce the patients to start the judicial procedure , as well as highlighting some elements asking for attention in the doctor- patient relationship
Medical dermal filler procedures pre questionnaire
Introduction: Dermal filler complications can arise due to unpredictable causes, operator-dependent causes, or at other times are caused by unwanted reactions related to the patient's pre-existing clinical conditions or co morbidities that favor an unsatisfactory outcome of the aesthetic procedure.
The aim of this study is to formulate, based on a critical review of the literature and the personal experience of the authors, an anamnestic questionnaire to be completed by patients requiring treatment with dermal filler, in order to facilitate the identification of patients not suitable for the procedure.
Materials and Methods: Medline and PubMed databases were searched on April 25th, 2021 using the terms: “Anamnesis”, “Complications”, “Medical history” and “Dermal filler”. The search covered the 2011-2021 decade.
From the publications found, following the reading of the abstract, those which did not meet the inclusion criteria were eliminated. From the ones remaining, the entire article was analyzed in order to highlight the presence of anamnestic indications that contraindicated the aesthetic procedure. Only articles available in English were considered for this review.
Results: The PubMed search showed 478 results for "dermal filler" and "complications", 0 for "dermal filler" and "anamnesis", 26 for "dermal filler" and "medical history". A total of 458 articles were excluded based on analyses of abstracts and full texts. A total of 20 articles were eligible for the study. Active or chronic infections, autoimmune diseases, previous treatments with unknown filler, history of allergies, conditions that cause Koebner response, coagulopathies, and some medical treatments represent known contraindications.
Conclusions: Following the literature review a pre-treatment questionnaire is proposed to help doctors screen out patients who may present contraindications to dermal filler treatment
Effects of traditional and ultrasonic liposuction on adipose tissue: a biochemical approach.
Little is known about the interaction of ultrasonic liposculpture with fat tissue. The surgical technique is well established and its clinical effects are satisfactory. However, the in vivo effects on adipose tissue remain to be determined. Previous studies have shown that ultrasound waves break fat cells. The purpose of this study was to ascertain whether ultrasound waves can cause the release of fatty acids from the molecular structure of triglycerides. A double-blind study was designed with samples obtained from traditional and ultrasonic liposuction of an equivalent area in the same patient. Samples were checked for triglycerides and for free fatty acids. Triglyceride values were always higher in the sample that had undergone ultrasonic procedure. No significant differences were observed between the free fatty acid chromatograms of the two kinds of samples analyzed. Data showed that no changes occurred in the triglyceride molecule when using ultrasound waves in the experimental condition
