    Apport de la tomographie en cohérence optique dans la prise en charge de la maculopathie diabetique

    La maculopathie diabétique est la cause la plus fréquente de baisse d’acuité visuelle chez le patient diabétique. L’ophtalmoscopie, la photographie stéréoscopique du fond d’œil et l’angiographie à la fluorescéine sont les outils usuels du diagnostic de l’œdème maculaire diabétique. Une nouvelle technique d’imagerie récemment développée, la tomographie en cohérence optique, permet la réalisation d’images en coupe de la rétine in vivo de haute résolution ainsi qu’une mesure quantitative de l’épaisseur et du volume rétiniens. But : Mettre en évidence l’apport de la tomographie en cohérence optique dans la prise en charge diagnostique et thérapeutique de la maculopathie diabétique. Méthodes : Chez 15 patients (30 yeux) diabétiques de type 2 avec baisse d’acuité visuelle, on a réalisé un examen ophtalmologique complet incluant la meilleure acuité visuelle, un examen du fond d’œil, une angiographie à la fluorescéine et un examen OCT. Ces examens ont été répétés chez les patients déjà traités. On a comparé les résultats des 3 techniques. Le logiciel de cartographie de l’OCT a permis de quantifier et de comparer les valeurs de l’épaisseur fovéale et du volume rétinien. Résultats : L’OCT a permis d’identifier et de quantifier de manière précise des œdèmes maculaires non diagnostiqués ou suspectés cliniquement. L’OCT a pu identifier des syndromes de traction vitréomaculaire. Grâce aux mesures quantitatives de l’épaisseur et du volume rétiniens fournies par l’OCT, l’évolution de l’œdème maculaire avant et après traitement a pu être suivie et l’efficacité des traitements instaurés évaluée. Enfin, l’OCT a pu objectiver l’aggravation de l’œdème maculaire après chirurgie de la cataracte. Conclusion : L’OCT est une nouvelle technique d’imagerie indispensable pour le diagnostic et la gradation des œdèmes maculaires diabétiques, notamment les œdèmes infracliniques, mais également pour l’analyse de l’interface vitréorétinienne et l’identification des syndromes de traction vitréomaculaire. Le suivi pré et postopératoire de ces œdèmes maculaires repose essentiellement sur les mesures quantitatives précises de l’épaisseur fovéale et du volume rétinien données par le logiciel de cartographie maculaire de l’OCT. L’OCT est un outil précieux complémentaire à l’examen biomicroscopique et à l’angiographie à la fluorescéine dans la prise en charge de la maculopathie diabétique

The research is triggered by the request for replacing standard lead-based piezoelectric ceramics with environmentally friendly lead-free materials and potential deployment of energy-harvesting microsystems in internet of things, internet of health, 'place and leave' sensors in infrastructures and agriculture monitoring. Moreover, futher system miniaturization and co-integration of functions are required in line with a desired possibility to increase the harvested power density per material volume. Thus, further research efforts are necessary to develop more sustainable materials/systems with high-performance. This paper gives a comprehensive overview on the processing and functional testing the lead-free bulk materials and thin films and discusses their potential in the applications in the stress- and strain-driven piezoelectric energy harvesting. This includes the methodology of estimation of the substrate clamping and orientation/texture effects in the thin films, and identification of orientations offering high figure of merit. The ability to control film orientation of different lead-free materials is reviewed and the expected piezoelectric performances are compared with the ones reported in literature

    A gallbladder tumor revealing metastatic clear cell renal carcinoma: report of case and review of literature

    Abstract Metastatic renal cell carcinoma in the gallbladder is extremely rare, with reported frequencies of less than 0.6% in large autopsy reviews. Only 40 cases were reported in the literature. We report a first case of gallbladder polypoid tumor revealing metastatic clear cell renal cell carcinoma, which demonstrates the importance of radiological tests, histology and immunohistochemistry when making a definitive diagnosis. These examinations also allow differentiating metastatic clear cell renal cell carcinoma from other polypoid lesions in the gallbladder with clear cell morphology. Cholecystectomy should be performed to obtain a definitive diagnosis and to improve survival in case of solitary metastatic renal cell carcinoma.     Characterization of granular materials using a blade hardness gauge

    This study was a preliminary investigation of the ability of a Blade Hardness Gauge (BHG) to reveal distinct behaviours in granular materials. A blade hardness gauge was constructed and used to investigate force versus time curves in dry/moist sand, polystyrene beads and snow. The effects of blade details, insertion speed (6-20mm/s), insertion direction (vertical or horizontal) of the blade were examined. There was a small but measurable effect on the blade details. The smaller cross-sectional area resulted in reduced forces. The insertion of blades vertically into dry sand is well described by an exponential force versus depth relationship, and importantly, independent on insertion speed. The vertical insertion into polystyrene beads shows a linear (weak exponential) dependence of the force as a function of the speed. The investigation into moisture laden sand shows an increasing force required for insertion up to about 20% moisture content, then decreasing back to dry sand equivalent for saturated sand (slurry). The use of the blade hardness gauge with snow revealed the force has a sigmoidal increase followed by a plateau, then an unexpected increase again.     Advanced Non-small Cell Lung Cancer: EGFR Mutation Analysis Using Pyrosequencing and the Fully Automated qPCR-Based Idylla System

    Background Epidermal growth factor receptor (EGFR) mutation status is of a major clinical significance in non-small cell lung cancer (NSCLC) management, as it guides therapeutic decision making to target patients for a better response to therapy. This implicates the introduction of EGFR mutation analysis as the standard of care for Moroccan NSCLC patients, which in itself entails the implementation of targeted methods for routine EGFR mutation analysis in our laboratories. In this study, we aimed to present 2 targeted methods for EGFR mutation identification and to determine the prevalence and spectrum of EGFR mutations in NSCLC Moroccan patients. Methods A retrospective investigation of a cohort of 340 patients was undertaken to analyze somatic EGFR mutations in exons 18 to 21 using pyrosequencing and the Idylla TM system. Results Of the enrolled patients, 70.9% were males and 29.1% were females. Predominately, 92% of cases had adenocarcinoma, and 53.7% of patients self-reported a history of smoking. Overall, 73 patients (21.7%) harbored an EGFR mutation, the most prevalent of which were the exon 19 deletions (53.4%) followed by exon 21 substitutions (31%). Exon 18 mutations and exon 20 alterations occurred in 8.1% and 6.7% of the positive EGFR mutation cases, respectively. Of the analyzed cases, all of the EGFR-mutated patients had adenocarcinoma. EGFR mutation prevalence was significantly higher in females (females vs males: 38.4% vs 14.5%, P < .001) and non-smokers (non-smokers vs non-smokers: 36% vs 10.3%, P < .001). The featured pyrosequencing and the Idylla TM system are targeted methods endowed with high sensitivity and specificity as well as other compelling characteristics which make them great options for routine EGFR mutation testing for advanced NSCLC patients. Conclusion These findings underline the imperious need for implementing quick and efficient targeted methods for routine EGFR mutation testing among NSCLC patients, which is particularly useful in determining patients who are more likely to benefit from targeted therapy
