Gastric Perforation by Ingested Rabbit Bone Fragment
The majority of accidentally ingested foreign bodies is excreted from the gastrointestinal (GI) tract without any complications. Sometimes sharp foreign bodies – like chicken and fish bones – can lead to intestinal perforation and may present insidiously with a wide range of symptoms and, consequently, different diagnoses. We report the case of a 59-year-old woman presenting with fever and a 1-month history of vague abdominal pain. Computed tomography (CT) showed the presence of a hyperdense linear image close to the gastric antrum surrounded by a fluid collection and free peritoneal air. At laparotomy, a 4-cm rabbit bone fragment covered in inflamed tissue was detected next to a gastric wall perforation. Rabbit bone fragment ingestion, even if rarely reported, should not be underestimated as a possible cause of GI tract perforation
The arthroereisis procedure in adult flexible flatfoot grade IIA due to insufficiency of posterior tibial tendon
Background: To report on the functional, biomechanical, and radiographic results of patients who had undergone arthroereisis plus tensioning of the posterior tibial tendon for flexible flatfoot. The hypothesis is that arthroereisis associated to a tensioning of the posterior tibial tendon give a good correction with great satisfaction in patients with flexible flatfoot in grade IIA. Methods: We evaluated 29 patients (31 feet), mean age of 46.4 years, who had been surgically treated for adult flatfoot grade IIA according to Myerson. Mean follow-up was 34.15 months. For clinical evaluation, the AOFAS hindfoot and VAS-FA scores were used. Results: Postoperative results showed significant increases in both AOFAS and VAS-FA scores: 54.2–81.9 and 61.5–83.2 points, respectively. For the X-ray parameters, we observed a significant variation in the talo-first metatarsal angle, from 13.8° in pre-op to 7.4° in post-op. In lateral view, Djian Annonier angle was improved from 146.6° to 134.1°. The Meary's angle, compared to an average of 8.8° in pre-operative stage improved to 4.3° in the post-operative stage. Postoperative satisfaction was excellent-good according to 23 patients (79.4%). Pain in the tarsal sinus was reported in 5 out of 31 feet (16.1%) for the first three months after surgery. Conclusions: Arthroereisis and tensioning of the posterior tibial tendon provided good functional outcomes for patients under 60 years of age having stage IIA flexible flatfoot without arthritic manifestations
IgG4-RELATED CRANIO-SPINAL HYPERTROPHIC PACHYMENINGITIS INVOLVING THE INTERNAL AUDITORY CANAL
Immunoglobulin G4 (IgG4)-related hypertrophic pachymeningitis, which is a focally or diffusely thickened dura mater and lymphoplasmacytic infiltration with increased IgG4 bearing plasma cells, is a rare disease, and cases involving the whole cervical spine are even rarer. Here, we describe a case of probable IgG4-related hypertrophic pachymeningitis involving the whole cervical spine and the auditory canals in an 18-year-old male. The patient, who had a history of paresthesia and had previously experienced weakness, presented with generalized tonic seizures. A decompressive laminectomy on cervical vertebrae was performed as a matter of urgency, removing intradural fibrous material. The patient responded well to treatment and was discharged walking independently, with no strength deficit to any of the 4 limbs, and with normal blood tests

Con Il Libro pel bambino, Don Giulio Tarra risultò il vincitore di due concorsi indetti dal Congresso Pedagogico Italiano nel 1861: quello per un libro di lettura destinato ai bambini sordomuti e l’altro per un libro destinato alle scuole rurali. Fra le opere del Tarra pubblicate da Paolo Carrara è menzionato il volumetto “Lezioni in famiglia, versione da M. Carraud”. Questo contributo intende offrire alcuni elementi d’informazione su Zulma Carraud, autrice delle Historiettes véritables pour les enfants de quatre à huit ans, titolo originale della raccolta di racconti tradotta da Don Giulio. Il libro era uscito a Parigi nel 1864 per i tipi di Hachette. In precedenza lo stesso editore aveva accolto le prime opere di Zulma nelle collane destinate alla scuola primaria. A pochi anni di distanza l’uno dall’altra, la Carraud nella Francia del II Impero e il Tarra nell’Italia postunitaria, entrambi hanno contribuito con i loro “libri di lettura” allo sviluppo dell’editoria per la scuola e per l’infanzia.

With the volume entitled Il Libro pel bambino, Don Giulio Tarra won two literary competitions held in 1861 by the Congresso Pedagogico Italiano: the first was dedicated to books for deaf-mute chidren, the second to books for rural schools. Among Tarra’s works published by Paolo Carrara we find “Lezioni in famiglia, versione da M. Carraud”. This article provides some information about Zulma Carraud, the author of the collection of novels entitled: Historiettes véritables pour les enfants de quatre à huit ans published by Hachette in 1864 and translated into Italian by Don Giulio. Previously the same publisher had collected Zulma’s first works in a series to be used in primary schools. Carraud, during the Second Empire period in France, and Tarra, in post-Unity Italy, both contributed to the development of the publishing industry for schools and children with their educational and entertaining books
Acute hematoma expansion after spontaneous intracerebral hemorrhage: risk factors and impact on long-term prognosis
Introduction: Hematoma expansion (HE) after intracerebral hemorrhage (ICH) is associated with short-term mortality, but its impact on long-term prognosis is still unclear. The aim of this study was to evaluate the impact of HE on long-term survival and functional status after spontaneous ICH. Methods: Consecutive patients admitted with spontaneous ICH were prospectively enrolled and followed up for a minimum of 2 years. We compared short-term (< 30 days) and long-term survival and functional status between ICH patients with HE (HE+) and those without (HE-). Main outcomes were mortality and poor outcome, defined as modified Rankin Scale ≥ 3. Secondary outcomes included recurrent ICH, admission to institutionalized care, and ischemic events (stroke, myocardial infarction, and systemic embolism). Results: Overall, 140 patients were included (mean age 74.9 years, male 59.3%) and followed up for a mean of 2.25 years. HE+ patients (25.7%) had larger hematoma volume at admission (23.8 ml vs 15.3 ml, p < 0.05), higher NIHSS score (14.6 vs 10.5, p < 0.05) and higher cumulative mortality (59.3% vs 39.2%, p < 0.05) compared to HE- patients. Survival analysis showed that HE+ confers higher mortality and worse functional status at all time points. HE did not associate with secondary outcomes. Discussion: HE translates into higher mortality and functional dependence over long-term follow-up. Strategies limiting HE might benefit long-term functional status
