After an editorial that motivates the symposium on the book by Arrigo Opocher and Ian Steedman, there are comments by five scholars (in alphabetical order). These deal with capital theoretic issues (Bellino), the labour demand curve (Bidard), the zero-excess-profit position in alternative theories of value (Fratini), the role of intermediate as opposed to final products in the setting of prices (Harcourt) and the role of time in the analysis (Yoshihara). Exchange and Production Equations: Understanding a Debate between Harrod and Sraffa
Harrod’s claim, that the relative prices that solve Sraffa’s “no-surplus” equations reflect the ratios of the amounts exchanged, deserves more than the scanty tribute paid by the author of Production of Commodities by Means of Commodities. Not only, in fact, does this claim hold true in the simple sense referring to direct, bilateral exchange when there are two commodities (and, in special cases, when there are more than two), but it holds true generally, in the more elaborate sense referring to amounts directly and indirectly exchanged by pairs of industries

Abstract
It was demonstrated that patients who have undergone the Fontan operation can safely undertake exercise training and that this results in an improvement in aerobic capacity. These findings suggest that aerobic training could be useful in the long-term management of these patients to optimize their cardiovascular fitness for more active lives.

Clinical and surgical features of lower brain stem hemangioblastomas in von Hippel-Lindau disease
BACKGROUND: In the context of von Hippel-Lindau disease (VHL), the medulla oblongata is a relatively frequent site of growth of hemangioblastomas, posing related clinical and surgical difficulties. Their management requires a close correlation between clinical evolution and morphological surveillance. In order to describe their clinical and surgical features, we reviewed our experience in the treatment of these lesions.
METHODS:
Between 2001 and 2009, 14 patients (9 female and 5 male, mean age 34 years) underwent removal of 15 lower brain stem hemangioblastomas. Based on the review of the clinical records and outpatient long-term follow-up visits, their clinical course was analyzed. Functional evaluation was measured with the Karnofsky Performance Scale (KPS) on admission, at discharge and at the last follow-up. The mean follow-up period was 30.8 months (range 4-99).
RESULTS:
All the operated hemangioblastomas were located in the dorsal medulla oblongata, in the context of multiple lesions, cerebellar and/or spinal. In ten patients hemangioblastomas were located in a median position at the obex area; in four cases a lateral location was observed. Cystic component was absent in two cases. Clinical onset preceded surgery by a mean of 8.5 months. Preoperatively three patients showed a KPS lower than 80, ten patients between 80 and 90, and one patient scored 100 (asymptomatic). There was no surgical mortality. Nine out of 14 patients showed a temporary surgical morbidity. One patient required a tracheostomy. At follow-up ten patients scored a KPS better than before the operation, while the other four patients remained stable. Permanent morbidity was observed in three patients.
CONCLUSIONS:
Lower brain stem hemangioblastomas in the context of VHL show an often gradual onset of signs and symptoms except for patients who develop an obstructive hydrocephalus. Although transient surgical complications are possible, surgery provides favorable long-term results

The increasing aging of the population points out the problem of an evidence-based approach to the elderly (≥ 65 years old) in the setting of adjuvant chemotherapy, whose benefit is established only in younger patients. Currently the cornerstone trials in these fields are planned for patients aged less than 65 and the extrapolation of these results to elderly patients is not methodologically correct. On the other hand, the search for a survival benefit should be balanced by the acknowledgement that aging gradually reduces the functional reserves of the body, so such treatments could be harmful. The favorable outcomes were mostly related to breast cancer, due not only to its high prevalence and social impact, but also to the dual opportunity of chemotherapy and hormonal treatment. Chemotherapy has also proved effective in colorectal cancer and to a lesser extent in lung cancer, despite the limits of such studies. Some recent clinical trials have however demonstrated that fit elderly patients may show similar outcomes in comparison with their younger counterparts. These results then display again the need for a comprehensive geriatric assessment in treatment planning, also in the adjuvant setting, in order to balance the benefit of this treatment with the possible harms linked to age-associated disorders
