    Survival of the Birmingham Hip Resurfacing in young men up to 13 years post-operatively

    The aim of this study was to determine the medium to long term survivorship of the Birmingham Hip Resurfacing prosthesis in an independent series of young men. The medical records and radiographs of all men aged 55 and under who had undergone Birmingham hip resurfacing by the senior author were reviewed. Patients who had not attended recent follow-up or had been previously discharged were contacted by telephone. Between 1999 and 2011 a total of 147 hips were resurfaced in 155 patients (mean age 47 years (19 to 55)) with minimum 5 year follow-up (mean 8.2, range 5 to 13.9 years). Eleven hips were revised giving overall cumulative survival of 88.8% at 13 years. In conclusion, this independent, single surgeon series demonstrates acceptable survival of the Birmingham Hip resurfacing in young men. It remains a valid option in certain cases but we believe alternative bearings are more suitable for most patients

    Digital media are quite unlike the 'mechanical' reproduction media that figure so often in the literature of cultural and media studies. Both the interaction between author and user through the digital medium, and a number of other intrinsic qualities of many digital media lead to novel interpretations of the concepts of "copy" and "authenticity" in the digital world. Digital media re-introduce some aspects of authenticity that were lost through the use of purely mechanical media. Digital media also lead to new conceptions of authenticity and power, related to shifts of control from author to reader and from publisher to author, and a shift from mechanical reproduction to digital re-production. the paper concludes that digital media rsult in a reduction of commonality and shared reading experiences, and a a re-definition of the concept of authenticity in terms of readership rather than authorship

    Performance nutrition to support athletes and aging

    Skeletal muscle plays a critical role in several metabolic processes that support both physical performance and metabolic health. Performance nutrition is an emerging field that aims to identify optimal nutrition and exercise strategies for physical performance, which includes the maintenance and/or promotion of muscle mass. The studies reported in this dissertation investigated aspects of performance nutrition, including carbohydrate and protein metabolism, in order to support both endurance and strength training performance. In Study 1, carbohydrate manipulation was investigated to identify a novel nutritional approach to support endurance performance. We demonstrated that potato ingestion exhibited similar performance improvements over water when compared to the ingestion of carbohydrate gels during prolonged cycling in trained athletes. This work identified potatoes as a high performance food to provide athletes with a cost effective and nutrient dense alternative to carbohydrate sports gels. In Study 2, we evaluated the acute regulation of fed-state muscle protein synthesis rates (MPS) by the primed continuous infusion method to define how rest period frequency and duration impacts the skeletal muscle’s adaptive response to resistance exercise. The measurement of MPS is commonly used in clinical and performance nutrition studies in order to provide an insight into protein remodeling: a necessary process to support skeletal muscle adaptations. Previous work has shown that intra-set rest can increase skeletal muscle contraction velocity, thereby improving power. Our study demonstrated that intra-set rest similarly stimulated MPS compared to a traditional set strategy. This work established intra-set rest as an easily manipulated variable to support both power and hypertrophic protein remodeling. Beyond athletes, the field of performance nutrition also strives to improve skeletal muscle mass in clinical populations. The ability of resistance exercise to promote muscle size and power is not only necessary for power and strength athletes, but also for aging populations to mitigate age-related skeletal muscle loss (i.e., sarcopenia). Moreover, this anabolic stimulus is known to be potentiated when combined with dietary protein. The interaction of dietary protein and resistance exercise is maximized with protein amount, quality, and ingestion timing. In Study 3, we used a less-invasive, free-living measurement of MPS to show that high protein diets do not further augment early resistance training mediated changes in MPS when compared to a moderate protein intake in middle-aged adults. In summary, the studies in this dissertation established novel performance nutrition strategies by showing that ingestion of potatoes matched the performance gains of sports gels. Moreover, it was established that a simple rest between repetitions could be an effective training alternative to stimulate muscle adaptations and finally that protein intake in moderate amounts will lead to similar MPS responses to higher protein intake. Collectively, these studies highlight both the complexity of skeletal muscle and the intricacy of the mechanistic response to nutrition and exercise.     (The) man, his body, and his society: masculinity and the male experience in English and Scottish medicine c.1640-c.1780.

    This thesis examines the relationship(s) between medicine, the body and societal codes of masculinity in England and Scotland between c.1640 and c.1780. It responds to the way in which the men in histories of post-1660 masculinity are often disembodied, and to the comparative absence of men’s gendered experiences from the history of medicine. Its findings show that in both centuries the experience of being a man with a body that was the site of health and sickness was an open, candid, and often communal, one, inside and outside of the formal medical encounter. Thus, and on both sides of 1700, ill men had full freedom in the pursuit and acceptance of medical, familial and social assistance, while their physical suffering, and associated emotional distress, was met with sympathy. With their sick bodies the sites of honest self-examination and open discussion, it was in part this very public nature of their sicknesses that allowed men, as a gender and as individuals, independence and agency in their non-commercial health care. Indeed, later-seventeenth- and eighteenth-century men suffered no constraints in their ability to respond to the vulnerabilities of their bodies, even where this involved behaviours or attributes allegedly associated with women and femininity, or inconsistent with ideals of active, independent, masculinity. These findings indicate, therefore, great continuity across the period 1640-1780, and not only in masculine ideals of and involving the male corporeality. There seems to have been significant consistency across time in men’s social and medical experiences of both sickness and their pre-emptive preparation for it, and in an apparent collective self-confidence concerning their corporeal masculinity, their sex, and, possibly, even their sexual potential. Indeed, these sources suggest that seventeenth- and eighteenth-century men had a resilient sense of self-identity (and personal masculinity), conceptually separable from the corporeal body and its known fragilities

    Radical enhancement as a moral status de-enhancer

    Nicholas Agar, Jeff McMahan and Allen Buchanan have all expressed concerns about enhancing humans far outside the species-typical range. They argue radically enhanced beings will be entitled to greater and more beneficial treatment through an enhanced moral status, or a stronger claim to basic rights. I challenge these claims by first arguing that emerging technologies will likely give the enhanced direct control over their mental states. The lack of control we currently exhibit over our mental lives greatly contributes to our sense of vulnerability. I then argue moral status should be viewed in terms of vulnerability. The enhanced will slowly gain the ability to command their mental states, reducing their vulnerability. These radically enhanced beings will have greater capacities, and possibly an inner life more valuable than our own. They will also be less vulnerable, and as a result, their moral status will be subordinate to our own

    A critical-theoretic study of the South African Truth and Reconciliation Commission : with reference to the work of Jurgen Habermas

    By using the work of Jurgen Habermas as my central focus the objective of this thesis was to judge whether the ideas that he has formulated in different bodies of work illuminate the problems and prospects for sustained democratic development in a country that has been affected by the impact of a series of cumulative civilizing offensives.I argue that the South African Truth and Reconciliation Commission(TRC) must be judged in relation to the outcomes that its commissioners attempted to promote. Following a reading of the works of Habermas I argue that it possible to specify the outcomes that would make it possible for the leaders of a new state to settle their accounts with the violations of the past by paying the fullest settlement of damages. Through the use of a case study methodology I was able to step outside of the realms of pure theory by establishing how some of Habermass ideas do enable us to acquire an understanding of the outcomes that the TRC was and was not able to influence. I use the ideas of Habermas in a critical way in order to judge the consequences of successive truth-telling hearings and the settings in which they were constituted. My methodology was also theoretic in so far as my goal was to establish whether it was possible to identify a characteristic or trait that differentiates a positive and/or a full settlement of damages from a negative and/or an empty settlement of damages. My analysis has demonstrated that the TRC was not able to establish an authoritative record of the perpetrators who committed violations during the mandate period. Therefore, the judgement that successive truth-telling processes made a decisive contribution to the revision of the country's political culture needs to be revised

    Research on target: A collaboration between researchers and practitioners for a target hardening scheme

    In this paper we report on a researcher-practitioner collaboration to deliver a crime reduction initiative across a borough in the West Midlands region of England. The circumstances of the collaboration and the initial analysis are explained. The crime prevention programme, which involved the situational crime prevention technique of target hardening, is described and a brief summary of the evaluation findings are included. We then consider the value of such collaborations for developing evidence-led interventions and discuss a number of issues which researchers embarking upon this type of project should consider: funding, timescales, data access, publication and evaluation. We conclude that, with due regard for these issues, such collaborations can be beneficial to all the parties involved
