Squamous Cell Carcinoma In Situ—The Importance of Early Diagnosis in Bowen Disease, Vulvar Intraepithelial Neoplasia, Penile Intraepithelial Neoplasia, and Erythroplasia of Queyrat

    No full text
Cutaneous squamous cell carcinoma (cSCC) is the second-most-prevalent malignancy in humans. A delayed diagnosis of cSCC leads to heightened invasiveness and positive surgical margins. Bowen’s disease (BD) represents an early form of cSCC and presents as a small erythematous, photo-distributed, psoriasiform plaque. Although certain dermoscopy features in BD are quite characteristic, histopathology remains the gold standard for diagnosis and provides a severity-scoring system that assists in guiding appropriate treatment strategies. The classification of precancerous lesions of the vulva and penis has undergone multifarious transformations due to variations in clinical and histopathological characteristics. Presently, erythroplasia of Queyrat is categorized as a clinical variant of penile intraepithelial neoplasia (PeIN). The diagnoses of vulvar intraepithelial neoplasia (VIN) and PeIN present significant challenges and typically necessitate one or more biopsies, potentially guided by dermoscopy. Aceto-white testing demonstrates a notably high negative predictive value for genital precancerous lesions. Histopathological examination represents the gold-standard diagnosis in VIN and PeIN, while p16 and p53 immunostainings alongside HPV testing provide crucial diagnostic clues. The histopathologic features, degree of differentiation, and associations with lichen planus, lichen sclerosus, and HPV guide the selection of conservative treatments or surgical excision

In the late 1980s-early 1990s, Moldavian identity and language were seen as failed constructions from the Soviet period. Twenty years after the independence of the Republic of Moldova (1991), one can still observe the complexity of representations and identity claims of its citizens, which is bound up with the complexity of representations and linguistic claims. The author uses the example of the Autonomous Moldavian Soviet Socialist Republic (amssr), in order to question the notion of language in its widest sense and to observe how language contributes to imagine communities in different ways.Scurtu Natalia. Thanking, belonging to expressive speech acts, according to the classification by Searle (1969), can be regarded as worth exploring due to the fact that gratitude, as a component of value systems, is as the fundamental emotion underlying social relations, and thus reflected in the language, especially fixed expressions, crucial for linguistic politeness. The aim of the present research is to compare explicit and implicit formulas (as referred to by Kerbrat-Orecchioni 2001) of the act of thanking in French and Polish. The examined material is composed of exemplary utterances retrieved from blogs, social networking sites, and internet fora. The methodology used includes studies on speech act theory (Austin 1962), typology of speech acts (Searle 1969), as well as research papers on language politeness by Kerbrat-Orecchioni (1996, 2001) and gratitude (SieradzkaBaziur 2017). Moreover, the author refers to comparative studies on politeness acts conducted up to now (Romero 2000, Rădulescu and Scurtu 2003, Perez 2005, Grossmann and Krzyżanowska 2018, Meng 2012). The author classifies collected explicit and implicit utterances, juxtaposing French and Polish examples and providing an analysis based on pragmatic parameters. As a result, the author creates a corpus of utterances used for expressing gratitude along with a study on diverse thanking formulas in both languages.Thanking, belonging to expressive speech acts, according to the classification by Searle (1969), can be regarded as worth exploring due to the fact that gratitude, as a component of value systems, is as the fundamental emotion underlying social relations, and thus reflected in the language, especially fixed expressions, crucial for linguistic politeness. Biophilic Dynamic Light Projections: A proposal for the revitalization of socially-inactive urban spaces

    No full text
The notion of “biophilia” refers to our innate and genetically determined affinity to nature. Despite having numerous benefits on our well-being and on how we perceive our environments, facts proven by a significant amount of scientific research, biophilia in the sphere of lighting design is a relatively untapped niche. Therefore, this thesis seeks to investigate the combination of biophilic patterns with modern technology with the intention to implement them in socially-inactive urban spaces as a mean of fast and effective revitalization. The overall goal is to establish new knowledge on creating, implementing and fine-tuning biophilic dynamic light projections. Through an extensive literature & case study review, design experimentation and atmosphere perception test, valuable knowledge on the notions of “preference for biophilic patterns”, “visual complexity”, “speed of change” and “value-add of interactivity” was developed. It is concluded that biophilic patterns are more positively perceived than their non-biophilic counterparts. Moreover, overall preference for complexity lies at a low to medium degree, while with speed of change, it is overall determined that a low value is inherently more pleasant, with negative consequences on the perception of atmosphere if increased. Additionally, adding the possibility to interact with the biophilic pattern provides overall better perception of safety and atmosphere. On the creation of patterns, the author recommends solutions based on the size of the space: with video projectors and the software Processing as ideal for small-scale implementation, while gobo projectors are recommended for large-scale projects

Stabilitätsanalyse und Numerische Simulation der Nichtnewtonschen Flüssigkeiten von Oldroyd Art

In this thesis, a numerical method was introduced for solving non-Newtonian viscoelastic fluid models of Oldroyd type, based on the finite element spatial discretization and on the fractional step theta-scheme time discretization used as operator splitting method. For the velocity and pressure fields, the stable Taylor-Hood element was used, whereas the stress field was discretized using discontinuous elements which satisfy an inf-sup condition in relation to the velocity space. Due to the mixed hyperbolic-parabolic character of the time-dependent system of equations governing the motion of an Oldroyd fluid, the basic idea in the present numerical approach was to decouple the calculation of the velocity and pressure fields from that of the stress field. By the operator splitting algorithm, one reduces the Oldroyd system to three simple subproblems: a Stokes like problem, a Burgers like one and a stress transport problem. A comprehensive stability analysis was given in chapter 5, for the Oldroyd system of equations starting from the continuous case, where already stability limits occurred. Further on, the semi-discretized in time Oldroyd system was analyzed and also the stability of the fractional step theta-scheme coupled with the finite element approximation applied to the linearized Oldroyd problem was investigated. The spectral analysis of the theta-scheme applied to the linearized Oldroyd system, by neglecting the nonlinear terms, was showing good stability properties and second order accuracy of the time discretization scheme. For the linear Oldroyd system no restrictions for the problem parameters were found. Considering the pure stress equation without the stress transport term and with a given stationary velocity field, a stability limit can exist. More precisely, if their exist a point or a region in the computational domain where the determinant of the velocity gradient is negative then for a=1 the stress equation is stable until a critical value of the Weissenberg number. In the case of a=0, no stability limit was found. Considering the stress constitutive equation with given velocity field, the stress will be transported along the characteristics which were the streamlines. Along streamlines which leave the computational domain, no perturbation arise. But, if there exist a stagnation point of the flow in the region with negativedeterminant of the velocity gradient, or if the streamlines are closed curves which intersect such a region, then instability of the stress components along the streamlines arises for Weissenberg numbers over the critical value. For a=0 the pure stress equation was not affected, but in the full Oldroyd system stability limits exists. For the Oldroyd-B system at least the stability limits arising in the pure stress constitutive equation exists. The numerical tests were confirming this stability limits. Two benchmark problems were studied: the lid driven cavity and the four-to-one planar contraction problem. Very good agreement with the results from the literature were obtained in the convergence range of our code. The numerical implementation of the Oldroyd system was based on the Navier-Stokes solver incorporated in the program package Albert. The principal personal contribution was the implementation of the stress tensor field with the corresponding routines for assembling the Oldroyd system and solving the stress transport equation by the discontinuous Galerkin method. Although the author largely focus in this work on the Oldroyd fluid model, the numerical algorithm can readily be extended to other differential and rate type non-Newtonian fluid models.

Platelet-Rich Plasma (PRP) in Dermatology: Cellular and Molecular Mechanisms of Action

    No full text
Platelet-rich plasma (PRP) therapy has gained attention in the scientific field due to its potential regenerative effects and great benefit–risk ratio. This review extensively explores the most studied mechanisms of this therapy according to the etiopathogenesis of skin diseases: cellular proliferation, matrix formation, regulation of inflammation, angiogenesis, collagen synthesis, and the remodeling of new tissue. Moreover, it draws on newly reported and lesser-known effects of PRP: its anti-apoptotic effects, immunological suppression, decrease in melanin synthesis, anti-microbial effects, overexpression of miR-155, antioxidant effects, and their involved pathways. This work aims to provide a complete update for understanding PRP’s benefits and clinical relevance in wound healing, alopecia, pigmentary disorders, scars, rejuvenation, lichen sclerosus, and other inflammatory dermatoses, based on the current evidence. Furthermore, recent reports with novel indications for PRP therapy are highlighted, and new potential pathways correlated with the pathogenesis of skin diseases are explored

Erythema ab igne—A Potential Cutaneous Marker of Chronic Heat Use in Patients with Endometriosis: A Narrative Literature Review and a Case Report

    No full text
Erythema ab igne (EAI), also known as “hot water bottle rash” or “toasted skin syndrome”, is a benign cutaneous condition caused by chronic exposure to low-level infrared heat. It typically begins as transient erythema and evolves into a reticulated brown pigmentation with telangiectasias. A skin biopsy, ideally taken from the central area of the hyperpigmented lesion, is recommended to exclude differential diagnoses. Although usually benign, EAI has been associated with rare malignant transformations, supported only by low-level evidence. Elimination of the heat source is essential, and topical treatments such as hydroquinone or retinoids may be considered, while agents like 5-fluorouracil or imiquimod are reserved for dysplastic lesions. Women with endometriosis frequently use heating devices to alleviate dysmenorrhea and chronic pelvic pain. However, prolonged or inappropriate heat application can lead to chronic thermal injury, including EAI, and may delay medical consultation. While controlled trials confirm short-term analgesic efficacy of heat therapy, extrapolating these findings to unrestricted home use without standardized safety recommendations can be misleading. EAI illustrates the broader impact of chronic pain in endometriosis, linking cutaneous manifestations with neuroplastic alterations and psychiatric comorbidities. A nuanced approach combining patient education on safe use of heat, close dermatologic monitoring, and multidisciplinary pain management is warranted

Basal cell carcinoma—a clinical indicator of immunosuppression

    No full text
BackgroundBasal cell carcinoma (BCC) and squamous cell carcinoma (SCC) are skin-derived carcinomas. The literature strongly connects SCC with acquired immunosuppression. Current data regarding BCC’s association with immunosuppressive comorbidities are vague. The primary objective of this study was to establish the correlations between BCC and immunosuppressive comorbidities of patients. Materials and methods: We conducted a retrospective cohort study on 275 patients with a histopathological proven diagnosis of BCC from October 2019 to October 2023. Demographic data, BCC characteristics, and patients’ comorbidities were analyzed. Comorbidities were classified as non-immunosuppressant and immunosuppressant (primary and secondary immunodeficiencies).ResultsWe recorded 292 BCCs from 275 patients (142 females, 133 males), with equally distributed skin phototypes. 66.44% of the BCCs were detected in patients with various comorbidities (p < 0.001), of which 81.44% had immunosuppressive comorbidities (p < 0.001). All the immunosuppressive comorbidities were secondary and included diabetes mellitus (47.55%), history of solid or hematogenous cancer in the last 5 years (26.57%), chronic kidney disease (8.39%), chronic infections (9.09%), and antirheumatic immunosuppressive therapies (8.39%) (p < 0.001). BCC patients with immunosuppressive comorbidities did not develop larger BCCs (p = 0.2577) or more aggressive subtypes (p = 0.4269) and BCC did not arise earlier in their life (p < 0.001). BCC on the nasal pyramid was frequent in cancer history patients (p = 0.008). The ulcerated form of BCC is more confined to patients with chronic kidney disease (p = 0.006). Multiple BCCs are more frequent in patients with secondary immunodeficiencies (p = 0.027).ConclusionBCC represents a clinical indicator of secondary immunodeficiency. Further research should establish if cancer screening campaigns may be beneficial in BCC patients
