Kinetic analysis of knee joint during single leg tasks of asymptomatic young men and women
Objetivos: verificar se há diferença no torque da articulação do joelho entre as tarefas de descida de degrau, single leg step down, e agachamento unipodal. Ainda, investigar se há diferença entre os sexos na realização destas tarefas e a relação do torque com variáveis cinéticas e cinemáticas das articulações adjacentes. Métodos: a estimação do torque articular foi realizada por meio da dinâmica inversa em 30 voluntários (15 do sexo feminino e 15 do sexo masculino) durante as tarefas unipodais de descida de degrau, single leg step down, e agachamento unipodal. Foram utilizados 42 marcadores anatômicos retro reflexivos para o registro das tarefas utilizando o Sistema VICON MOTION ANALYSIS SYSTEM, composto de oito câmeras com quatro megapixels de resolução espacial e com uma frequência de amostragem de 250 Hz, sincronizado com duas plataformas de força BERTEC e AMTI Accugait, ambas com frequência de amostragem de 2000 Hz. O processamento dos dados cinemáticos foi realizado com o software VICON NEXUS 1.8.5 onde todos os marcadores foram tridimensionalmente reconstruídos. Em seguida, foi calculado o torque articular do quadril, joelho e tornozelo utilizando a dinâmica inversa durante toda a tarefa de interesse pelo software Visual3D. Para a análise dos dados, dois eventos relacionados à articulação do joelho foram selecionados: o pico de deslocamento angular e o pico de torque gerado nos planos sagital e frontal, além disso, as tarefas foram exploradas em sua totalidade, tendo como enfoque a análise dos picos de torque e picos angulares de todas articulações no decorrer das tarefas. Como análise estatística foi utilizado o SPSS 21.0 e adotado um nível de significância de 0,05. Foi confirmado o padrão de distribuição normal dos dados pelo teste de Shapiro-Wilk. Para variáveis de caracterização da amostra foi utilizado o teste t de Student, e para a comparação das três tarefas nas variáveis cinéticas e cinemáticas, foi aplicado o teste de Friedman, com nível de significância de 0,05 e, como post hoc, foi aplicado o teste de Wilcoxon com correção de Bonferroni por multiplicidade (p<0,017). Para comparar os sexos, foi aplicado um teste Mann-Whitney em cada uma das tarefas. Resultados: no plano sagital no momento do pico de torque do joelho não houve diferença entre as tarefas e sexos na articulação do joelho, já no momento do pico angular do joelho, o agachamento unipodal gerou um maior torque articular em relação à descida de degrau. Uma maior flexão do joelho foi observada no single leg step down em relação às outras tarefas no momento do pico de torque do joelho e na descida de degrau em relação ao agachamento unipodal no momento de pico angular do joelho. Já no plano frontal, o agachamento unipodal foi a tarefa que gerou o maior torque em relação às outras tarefas, e o single leg step down foi a tarefa que gerou maior angulação em varo em relação à descida de degrau tanto no momento de pico de torque do joelho quanto no momento de pico angular do joelho. Conclusão: Quando se considera o torque no pico de deslocamento angular no plano sagital e frontal verifica-se uma ordem progressiva de torque entre as tarefas, sendo o agachamento unipodal a tarefa que gera maior torque do joelho seguida pelo single leg step down e descida de degrau, e o sexo não aparenta influenciar no torque do joelho em nenhum plano.Objectives: To verify if there is difference in the knee joint torque between the tasks of descending of step, single leg step down, and unipodal squatting. Also, investigate if there is difference between the sexes in the accomplishment of these tasks and the relation of the joint torque with kinetic and kinematic variables of the adjacent joints. Methods: the estimation of joint torque was performed by means of the inverse dynamics in 30 volunteers (15 females and 15 males) during the single leg step down single-legged and onelegged squatting tasks. We used 42 retro reflective anatomical markers to record the tasks using the VICON MOTION ANALYSIS SYSTEM (Centennial, CO, USA), composed of eight cameras (MX-T-40S) with four megapixels of spatial resolution and a sampling frequency of 250 Hz, synchronized with two BERTEC (Columbos, OH, USA) and AMTI Accugait (Waterloo, MA, USA) power platforms, both with 2000 Hz sampling frequency. Kinematic data processing was performed with VICON NEXUS software 1.8.5 where all markers were reconstructed three-dimensionally. Then, the joint torque of the hip, knee and ankle was calculated using the inverse dynamics during the entire task of interest for Visual3D software (version 6.01.22, C-Motion Inc). For the analysis of the data, two events related to the knee joint were selected: the angular displacement peak and the peak torque generated in the sagittal and frontal planes, and the tasks were fully explored, focusing on the analysis of the joint torque peaks and angular peaks of all joints during tasks. Statistical analysis was performed using SPSS 21.0 (IBM Corporation, Armonk, NY) and a significance level of 0.05 was adopted. The pattern of normal data distribution was confirmed by the Shapiro-Wilk test. Student\'s t test was used to characterize the sample, and for the comparison of the three tasks in the kinetic and kinematic variables, the Friedman test was applied, with a significance level of 0.05 and as a post hoc Wilcoxon test with Bonferroni correction by multiplicity (p <0.017). To compare the sexes, a Mann-Whitney test was applied to each task. Results: in the sagittal plane at the moment of the knee torque peak there was no difference between the tasks and sexes in the knee joint, whereas at the moment of the knee angular peak, the single leg squat generated a greater joint torque in relation to the step descent. Higher knee flexion was observed in the single leg step down compared to the other tasks at the moment of knee peak torque and the step descent in relation to the single leg squat at the moment of knee angular peak. In the frontal plane, single leg squat was the task that generated the highest torque in relation to the other tasks, and the single leg step down was the task that generated the highest varus angle in relation to the step descent at both, moment of knee peak torque and the moment of the knee angular peak. Conclusion: When considering the torque at the angular displacement peak in the sagittal and frontal plane, there is a progressive order of torque between the tasks, with single leg squat being the task that generates the highest knee torque followed by single leg step down and lower step, and gender does not appear to influence knee torque in any plane
Effects of combining manual therapy, neck muscle exercises, and therapeutic education pain neuroscience in patients with migraine: a 3-armed randomized clinical trial
A fisioterapia representa uma abordagem não farmacológica para o manejo da migrânea, com potencial para melhorar os desfechos clínicos dos pacientes. No entanto, a eficácia comparativa de uma abordagem multimodal, que combina terapia manual, exercícios cervicais específicos e educação terapêutica em neurociência da dor, versus o uso isolado dessas técnicas ainda não foi estabelecida. Objetivo: Este ensaio clínico randomizado visa avaliar a eficácia de um protocolo multimodal em comparação com o uso isolado da terapia manual ou da educação em neurociência da dor em pacientes com migrânea. Métodos: Foi conduzido um ensaio clínico randomizado, com três braços, paralelo, simples-cego, com um protocolo de tratamento de 12 semanas. Os participantes foram avaliados antes do tratamento, durante o tratamento (4, 8 e 12 semanas) e após o tratamento, nas avaliações de acompanhamento após 1, 2 e 4 meses. Os participantes foram randomizados em três grupos: Grupo de Educação Terapêutica em Neurociência da Dor (TEG, n=25); Grupo de Terapia Manual (MTG, n=25); e Grupo Multimodal (MUG, n=25). O desfecho primário foi o Headache Impact Test (HIT-6™). Os desfechos secundários incluíram frequência, intensidade e duração das cefaleias e da dor cervical; Neck Disability Index (NDI); 12-item Allodynia Symptom Checklist (ASC-12); Tampa Scale for Kinesiophobia (TSK); Pain Catastrophizing Scale (PCS); European Quality of Life Five Dimension with Five Levels (EQ-5D-5L); Dizziness Handicap Inventory (DHI); Patient Global Impression of Change Scale (PGIC); palpação articular cervical; limiar de dor à pressão (LDP) cervical; amplitude de movimento (ADM) cervical; força muscular cervical; Flexion Rotation Test (FRT); e Craniocervical Flexion Test (CCFT). Modelos mistos foram utilizados no SPSS para analisar os efeitos dos grupos, os efeitos ao longo do tempo e as interações grupo-tempo, com um nível de significância de 0,05. Resultados: Melhorias significativas nos escores do HIT-6 foram observadas em todos os grupos ao longo do tempo (p<0,001). O MUG demonstrou uma melhora maior no impacto das cefaleias em comparação com o MTG no acompanhamento. Da mesma forma, o TEG apresentou maior melhora em comparação ao MTG ao final do tratamento. Interações significativas grupo-tempo, indicando desfechos favoráveis para o tratamento multimodal, foram notadas para frequência e intensidade das cefaleias, frequência da dor cervical, ASC-12, NDI, EQ-5D-5L e FRT (interação grupo-tempo; p < 0,01). Mudanças clinicamente relevantes foram observadas no NDI, PCS e EQ-5D-5L em todos os grupos, no ASC-12 nos grupos MUG e MTG, e no TSK apenas no TEG. Conclusão: O tratamento multimodal demonstrou resultados superiores na redução do impacto das cefaleias em pacientes com migrânea. Embora todos os grupos tenham mostrado melhora no impacto das cefaleias ao longo do tempo, a abordagem multimodal revelou maior eficácia na redução da frequência e intensidade da migrânea, alodinia, incapacidade cervical, catastrofização, na melhoria da qualidade de vida e da mobilidade cervical superior.Physical therapy represents a non-pharmacological approach for the management of migraine, with the potential to improve clinical outcomes in patients. However, the comparative efficacy of a multimodal approach-combining manual therapy, specific cervical exercises, and therapeutic education in pain neuroscience-versus the isolated use of these techniques has not yet been established. Objective: This randomized clinical trial aims to evaluate the efficacy of a multimodal protocol in comparison to the isolated use of manual therapy or pain neuroscience education in patients with migraine. Methods: A three-arm, parallel, single-blind, randomized clinical trial was conducted, with a 12-week treatment protocol. Participants were evaluated at three time points: at the baseline, during treatment (at weeks 4, 8, and 12), and after treatment (at follow-up assessments conducted at 1-, 2-, and 4-months post-treatment). Participants were randomized into three groups: Pain Neuroscience Education Group (TEG, n=25); Manual Therapy Group (MTG, n=25); and Multimodal Group (MUG, n=25). The primary outcome was the Headache Impact Test (HIT-6™). Secondary outcomes included headache frequency, intensity, and duration; neck pain frequency, intensity, and duration; Neck Disability Index (NDI); 12-item Allodynia Symptom Checklist (ASC-12); Tampa Scale for Kinesiophobia (TSK); Pain Catastrophizing Scale (PCS); European Quality of Life Five Dimension with Five Levels (EQ-5D-5L); Dizziness Handicap Inventory (DHI); Patient Global Impression of Change Scale (PGIC); neck joint palpation; cervical pressure pain threshold (PPT); cervical range of motion (ROM); cervical muscle strength; Flexion Rotation Test (FRT); and Craniocervical Flexion Test (CCFT). Mixed models were used in SPSS to analyze group effects, time effects, and group-time interactions, with a significance level of 0.05. Results: Significant improvements in HIT-6 scores were observed in all groups over time (p<0.001). The MUG demonstrated greater improvement in headache impact compared to the MTG at follow-up. Similarly, the TEG showed greater improvement compared to the MTG at the end of treatment. Significant grouptime interactions, indicating favorable outcomes for the multimodal treatment, were noted for headache frequency and intensity, neck pain frequency, ASC-12, NDI, EQ-5D-5L, and FRT (group-time interaction; p < 0.01). Clinically relevant changes were observed in NDI, PCS, and EQ-5D-5L in all groups, in ASC-12 in the MUG and MTG groups, and in TSK only in the TEG. Conclusion: The multimodal treatment demonstrated superior results in reducing the impact of headaches in patients with migraine. Although all groups showed improvement in headache impact over time, the multimodal approach proved more effective in reducing migraine frequency and intensity, allodynia, cervical disability, catastrophizing, and in improving quality of life and upper cervical mobility
Effect of exercise protocol for strengthen cervical muscles in migraineurs compared with placebo group - randomized controlled trial
A presença de alterações musculoesqueléticas em pacientes com migrânea, bem como a alta prevalência de dor cervical, já estão bastante elucidadas na literatura. Apesar da indicação de tratamentos não farmacológicos estar crescendo, um protocolo de exercícios direcionado especificamente para a região cervical ainda não teve sua eficácia avaliada. Dessa forma, o objetivo desse estudo foi verificar a eficácia de um protocolo de exercícios craniocervicais na redução da frequência e intensidade das crises de migrânea. Métodos: Nesse ensaio clínico randomizado de dois braços e grupos paralelos com acompanhamento de 3 meses, 42 voluntários com diagnóstico de migrânea foram avaliados e randomizados em dois grupos, intervenção e placebo. Os voluntários randomizados para o grupo intervenção (n=21) realizaram durante oito semanas um protocolo de exercícios craniocervicais, enquanto que os voluntários do grupo placebo (n=21) receberam aplicações de ultrassom terapêutico desligado na porção média do músculo trapézio superior, também durante oito semanas. Os desfechos primários avaliados foram frequência e intensidade das crises de migrânea, registrados em diários de dor mensais. Como desfechos secundários, foram verificadas a incapacidade relacionada a migrânea e a dor cervical, a percepção global de mudança e satisfação com o tratamento, a amplitude de movimento cervical, o limiar de dor a pressão dos músculos craniocervicais, o craniocervical flexion test (CCFT), a força e a resistência dos músculos cervicais. Além disso, a eletromiografia de superfície foi utilizada durante os últimos três testes. Resultados: Não foram observadas diferenças estatisticamente significativas para os desfechos primários após as oito semanas de tratamento e após os três meses de acompanhamento (p>0,050). Em relação aos desfechos secundários, observamos no grupo intervenção uma melhora da sensibilidade do músculo frontal (p=0,040), além de redução na amplitude de ativação dos músculos escaleno anterior e trapézio superior nos últimos estágios do CCFT (p≤0,000) e um aumento no slope da frequência mediana do músculo esplênio da cabeça, durante o teste de resistência dos flexores cervicais (p=0,014) e do músculo escaleno anterior durante o teste de resistência dos flexores e extensores cervicais (p0.050). Regarding secondary outcomes, we observed an improvement in frontal muscle sensitivity in the intervention group (p=0.040), in addition a reduced amplitude of muscle activity of the anterior scalene and upper trapezius in the last stages of CCFT (p ≤0.000) and an increase in the slope of the median frequency of the splenius capitis muscle, during the cervical flexor resistance test (p=0.014) and of the anterior scalene muscle during the cervical flexor and extensor resistance test (p<0.000; p=0.045 ; respectively). Conclusion: We conclude that the exclusive performance of a craniocervical exercise protocol was not enough to reduce the frequency and intensity of migraine attacks, as well as improve the performance of the cervical muscles and reduce the disability caused by migraine and neck pain. However, the same exercise protocol promoted an improvement in the activation of the cervical muscles during the performance of the CCFT and resistance test in patients with migraine
Cross-cultural adaptation and reliability of the 12 item Allodynia Symptom Checklist for the Brazilian population
O objetivo foi realizar a adaptação transcultural do 12 item Allodynia Symptom Checklist para a população brasileira e testar a confiabilidade dessa versão, já que não havia nenhum questionário de avaliação da alodínia cutânea em migranosos, sintoma que tem sido apontado como sinalizador da sensibilização central e como fator de cronificação dessa doença. A adaptação transcultural foi realizada em seis estágios: tradução, síntese, retrotradução, revisão pelo comitê de especialistas na área, pré-teste e submissão dos documentos ao comitê de especialistas. No estágio do pré-teste, foram aplicados 30 questionários em migranosos de ambos os sexos, que relataram dificuldades de compreensão e, dessa maneira, uma segunda versão foi criada e aplicada em mais 30 sujeitos, para a qual não foram relatadas dificuldades adicionais. O tempo médio de preenchimento foi de 336 e a consistência interna foi considerada adequada demonstrando uma boa correlação entre os itens ( de Cronbach = 0,77). Para testar a confiabilidade, outros 30 sujeitos preencheram o questionário, em dois momentos, sendo que para 15 deles o questionário foi autoaplicável e para os outros 15 a avaliação foi feita no formato de entrevista. A confiabilidade foi classificada como moderada (kappa ponderado linear = 0,58) e excelente ( kappa ponderado linear =0,84) respectivamente. Portanto, foi possível através desse estudo, disponibilizar em língua portuguesa um questionário válido, confiável e de fácil aplicação para a avaliação da alodínia em migranosos da população brasileira.Since there was no portuguese questionnaire to evaluate cutaneous allodynia in migraine population, which has been pointed out as a central sensibilization flag and as risk factor of migraine chronification we aimed to perform the cross-cultural adaptation of the 12 item Allodynia Symptom Checklist for the Brazilian population and to test its reliability. In the pretest stage, the questionnaire was applied to 30 migraineurs of both sexes, who had some difficulty in understanding it. Thus, a second version was applied to 30 additional subjects, with no others difficulties being reported. The mean filling out time was 336and the internal consistency was considered adequate showing a good interrelation between items (Cronbachs = 0,77). To test reliability, 30 other subjects filled out the questionnaire at two different times, 15 of them in a self-report administration and 15 in a structured interview. It was classified as moderate (linear weighted kappa = 0.58) and as excellent (linear weighted kappa = 0.84) respectively. In this study, it was possible to make available in portuguese a valid, reliable and easy tool for cutaneous allodynia assessment in Brazil
Avaliação da força máxima e do controle motor da coluna cervical pela dinamometria e pela eletromiografia de superfície em mulheres com migrânea e migrânea crônica: estudo controlado
Objectives: to verify if women with migraine present altered neck muscles function and altered muscle activity in comparison to non-headache subjects. Moreover, we aimed to identify if there is any relationship between neck muscles dysfunction and the chronicity of migraine attacks. Methods: women with migraine, stratified by episodic and chronic, and age-matched non headache women were the subject of the current thesis. Neck pain relateddisability was assessed by the Neck Disability Index. Neck muscles strength was assessed by a customized hand-held dynamometer. Myoelectrical signals of the sternocleidomastoid, anterior scalene, splenius capitis and upper trapezius were assessed using surface electromyography. Experimental conditions involved maximal isometric voluntary contractions in flexion, extension and lateral bending directions and; craniocervical flexion test, a low load test applied to verify deep cervical flexors ability to supply adequate cervical spine stability. Finally, identification of myofascial trigger points and head and cervical posture photogrammetric evaluation were made in order to be correlated to the muscles electrical activity in the craniocervical flexion test. Results: Although both groups presents high prevalence of neck related disability, subjects with chronic migraine are more likely to report neck related disability, especially the most disabling forms, than episodic migraine. Episodic migraine demonstrated to different neck muscle control only under maximal voluntary tasks. Chronic migraine presented alterations in comparison with controls at maximal conditions, i.e., lower extension force; more time to produce maximal force; greater antagonist coactivation and greater fatigability. During craniocervical flexion test, chronic migraine also differs from control group presenting greater activity of neck extensors. Presence of active trigger points in neck muscles and extended head posture can influence the neck muscles activity during the craniocervical flexion test. Conclusions: migraine is associated to a deterioration of neck muscles functions and to an altered motor control. We can assume an interaction between chronicity of migraine frequency and cervical dysfunction, specially related to neck extensors. More severe disabilities and the majority of the altered function or motor control could only be evidenced in the chronic formObjetivos: verificar se mulheres com migrânea apresenta alteração na função e atividade dos músculos do pescoço quando comparados a mulheres sem cefaleia. Ainda, objetivamos identificar se há relação entre disfunção cervical muscular e a cronicidade da frequência de crises migranosas. Materiais e métodos: mulheres com migrânea, estratificada entre episódica e crônica, e mulheres sem cefaleia com idade semelhante foi o objeto de estudo dessa tese. Foi investigada a incapacidade relacionada à dor cervical pelo questionário Neck Disability Index e a força muscular cervical utilizando um dinamômetro manual customizado. Ainda, a atividade elétrica dos músculos esternocleidomastoideo, escaleno anterior, esplênio da cabeça e trapézio superior foi verificada pela eletromiografia de superfície. Os procedimentos experimentais foram contração isométrica voluntária máxima em flexão, extensão e inclinação lateral; e o teste de flexão craniocervical, que avalia a habilidade dos músculos flexores cervicais profundos em prover adequada estabilidade para esse segmento. Por fim, a identificação de pontos gatilhos nos músculos cervicais e a avaliação da postura da cabeça e do pescoço por foto foram realizadas a fim de correlacionar os achados com a atividade eletromiográfica. Resultados: embora ambos os grupos de migrânea tenham apresentado alta prevalência de incapacidade relacionada à dor cervical, a migrânea crônica apresenta maior risco de relatar incapacidades mais severas em relação a migrânea episódica. O grupo de migrânea episódica apresentou controle muscular cervical alterado em condição de contração voluntária máxima. Já o grupo migrânea crônica apresentou alterações sob condições máximas, como por exemplo, redução da força dos extensores, mais tempo para gerar pico de força em flexão e inclinação lateral, maior coativação do antagonista e maior susceptibilidade à fadiga. Ainda, durante o teste de flexão craniocervical, o grupo migrânea crônica apresentou maior atividade dos extensores indicando alteração no controle motor. A presença de pontos gatilhos nos músculos do pescoço e a postura em extensão de cabeça pode influenciar a atividade elétrica dos músculos cervicais durante a execução do teste de flexão craniocervical. Conclusões: a migrânea está associada a uma deteriorização da função e do controle motor cervical. Podemos assumir ainda que há uma interação entre a cronicidade da frequência das crises migranosas e a disfunção muscular cervical, especialmente relacionada aos extensores. Incapacidades mais severas e a maioria das disfunções musculares e alteração no controle motor só puderam ser evidenciadas para o grupo com migrânea crônic

Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed. Variations on the Author
“Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship
Appropriate Similarity Measures for Author Cocitation Analysis
We provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature.
