    Hintergrund und Ziele Morbus Crohn und Colitis ulcerosa sind zwei chronisch entzündliche Darmerkrankungen (CED), bei denen der anti-α4β7 Integrin-Antikörper Vedolizumab (VDZ) in Phase III Studien bereits eine gute Wirksamkeit zeigte und auch unter realen Bedingungen schon erfolgreich eingesetzt wurde. Vereinzelt wurde VDZ auch bei anderen Formen der CEDs verwendet, wie zum Beispiel bei der Behandlung von mikroskopischer Kolitis (MC). Allerdings wurden die Wirkmechanismen von VDZ bei MC noch nicht untersucht. Daher war es das Ziel dieser Arbeit, im Rahmen einer Kohortenstudie mit MC-Patienten, die am Universitätsklinikum Erlangen in Behandlung sind, die Expression und Funktion von gut-homing-Integrinen, insbesondere des α4β7- Integrins, zu erforschen. Methoden Wir untersuchten mithilfe von Durchflusszytometrie die Expression von gut-homing- Integrinen auf T-Zellen von Patienten mit MC und verglichen diese mit gesunden Kontrollen. Um die Funktion von α4β7-Integrins bei MC und das Potenzial von VDZ dieses zu blockieren zu analysieren, verwendeten wir dynamische Adhäsionsassays und Transmigrationsassays. Außerdem beschreiben wir zwei klinische Fälle von Patienten mit MC, die mit VDZ behandelt werden. Ergebnisse und Beobachtungen Patienten mit MC zeigen ein spezifisches Profil an gut-homing-Markern auf T-Zellen. α4β7-Integrin auf T-Zellen von Patienten mit MC führt zu einer festen Adhäsion der T- Zellen an Mucosal addressin cellular adhesion molecule-1 (MAdCAM-1) und unterstützt die Transmigration. VDZ kann diese beiden Prozesse unterbinden. In zwei Fällen von MC beobachteten wir einen Rückgang der klinischen Symptome sowie eine Verbesserung der histologischen Befunde unter VDZ-Therapie. Schlussfolgerungen Unsere Daten weisen darauf hin, dass α4β7 das Homing von T-Zellen in den Darm bei MC vermittelt und dass VDZ auf Zellebene die Funktion von α4β7 bei MC blockiert. Somit liefern wir mechanistische Hinweise, die den Einsatz von VDZ als Therapieoption bei MC unterstützen

    Entschlüsselung der Rolle von NFATc1 in allergischem Asthma: Eine molekulare und immunologische Perspektive mit Fokus auf CD4 und CD11c Zellen

    Asthma is a chronic lung disease that has become a significant public health problem worldwide. Dendritic cells and T cells play a pivotal role in regulating immune responses in asthma. However, the function of NFATc1 within these cells in the context of asthma immunity remains underexplored. This study investigated NFATc1's role in CD4+ T cells and CD11c+ dendritic cells in allergic asthma utilizing a house dust mite (HDM)-induced asthma model and a human adult asthmatic cohort. In the HDM-induced asthma model, I found that Nfatc1 deletion in T cells attenuated Th2-driven eosinophilic inflammation, IgE levels, and mast cell presence in asthmatic mice. Inter-estingly, this was accompanied by an increased Th1 and Th17 response, indicating a shift to-wards a neutrophilic inflammation and suggesting Nfatc1's critical role in balancing Th2 and Th17 pathways. Additionally, an upregulation of regulatory T cells (Tregs) was observed, pointing to an adaptive mechanism aimed at controlling exacerbated inflammation. Conversely, in CD11c+ dendritic cells, Nfatc1 deficiency was associated with reduced airway hyperrespon-siveness and diminished eosinophilic inflammation, underscoring its pivotal role in dendritic cell maturation and function. This led to impaired antigen presentation and a consequent de-crease in T cell activation, highlighting the significance of Nfatc1 in dendritic cells in maintain-ing the eosinophil-dominated inflammatory response during asthma. Further aligning with these mouse model findings, translational research in humans has re-vealed significant associations between CD4+NFATc1+ and CD11c+NFATc1+ cell popula-tions and eosinophil levels in asthmatic patients, as opposed to healthy controls. This under-scores the vital role of NFATc1 in both T cells and dendritic cells in modulating Th2-mediated eosinophilic inflammation in allergic asthma. Overall, my thesis highlights NFATc1's significant role in T cells and dendritic cells, particu-larly within the context of Type 2 high allergic asthma. It reveals how NFATc1 in both cell types modulates eosinophilic inflammation and IgE-mediated allergic reactions, offering a deeper understanding of asthma pathogenesis and potential therapeutic targets for asthma treatment.Asthma ist eine chronische Lungenerkrankung, die weltweit zu einem bedeutenden Problem der öffentlichen Gesundheit geworden ist. Dendritische Zellen und T-Zellen spielen eine zent-rale Rolle in der Regulierung der Immunantwort bei Asthma. Die Funktion von NFATc1 in diesen Zellen im Kontext der Asthma-Immunität bleibt jedoch untererforscht. Diese Studie un-tersuchte die Rolle von NFATc1 in CD4+ T-Zellen und dendritischen Zellen bei allergischem Asthma unter Verwendung eines durch Hausstaubmilben induzierten Asthmamodells und einer Kohorte von erwachsenen Asthmatikern. Im Hausstaubmilben-induzierten Asthmamodell führte die Deletion von Nfatc1 in T-Zellen zu einer abgeschwächten Th2-getriebenen eosinophilen Entzündung, niedrigeren IgE-Spiegeln und verminderter Mastzellpräsenz bei asthmatischen Mäusen. Interessanterweise war dies von einer erhöhten Th1- und Th17-Antwort begleitet, was auf eine Verschiebung hin zu neutrophi-ler Entzündung hinweist und die kritische Rolle von Nfatc1 bei der Ausbalancierung der Th2- und Th17-Antwort unterstreicht. Zusätzlich wurde eine Hochregulierung von regulatorischen T-Zellen (Tregs) beobachtet, was auf einen adaptiven Mechanismus hindeutet, welcher auf die Kontrolle der verschärften Entzündung abzielt. Im Gegensatz dazu wurde eine Nfatc1-Deletion bei CD11c+ dendritischen Zellen mit reduzierter Atemwegshyperreaktivität und verminderter eosinophiler Entzündung in Verbindung gebracht, was seine zentrale Rolle bei der Reifung und Funktion dendritischer Zellen unterstreicht. Dies führte zu einer beeinträchtigten Antigenprä-sentation und einer daraus resultierenden Abnahme der T-Zell-Aktivierung, was die Bedeutung von NFATc1 in dendritischen Zellen bei der Aufrechterhaltung der für Asthma charakteristischen eosinophilen Entzündungsreaktion hervorhebt. In Einklang mit diesen Befunden aus dem Mausmodell zeigen die translationalen Daten signi-fikante Assoziationen zwischen CD4+NFATc1+ und CD11c+NFATc1+ Zellpopulationen und Eosinophilenspiegeln bei Asthmapatienten im Vergleich zu gesunden Kontrollen. Dies unter-streicht die entscheidende Rolle von NFATc1 sowohl in T-Zellen als auch in dendritischen Zellen bei der Modulation der durch Th2-vermittelten eosinophilen Entzündung bei allergi-schem Asthma. Insgesamt hebt diese Arbeit die bedeutende Rolle von NFATc1 in T-Zellen und dendritischen Zellen, insbesondere im Kontext von Typ-2-hohem allergischem Asthma hervor. Sie zeigt, wie NFATc1 in beiden Zelltypen die eosinophile Entzündung und IgE-vermittelte allergische Re-aktionen moduliert und bietet ein tieferes Verständnis der Asthma-Pathogenese und potenziel-ler therapeutischer Ziele für die Asthmabehandlung

    Type III interferons (IFNs), or IFN-λ, are known to have potent antiviral and antiproliferative activities. It inhibits viral replication and upregulates cytotoxic responses to virally infected cells. Besides these characteristics, IFN-λ also has additional activities in the immune system. In fact, it induces the proliferation of Foxp3-expressing regulatory T cells mediated in part by dendritic cells and inhibit the production of IL-5 and IL-13 in vitro. Regulatory T cells and the Th2 cytokines like IL-5 and IL-13 play important roles in the pathogenesis of allergic asthma. In humans, there seems to be an inverse link between IFN-λ and the severity of allergic asthma and allergic asthma exacerbations. Asthmatic patients, without a detectable viral infection show an inverse correlation between IL-28 and IL-29 mRNA levels and severity of allergic responses in the airways. These additional features of IFN-λ that affect the adaptive immune system make it a potential immunotherapeutic agent for the treatment of allergic asthma

    Asthma bronchiale ist eine chronisch-entzündliche Erkrankung der Atemwege, die weltweit etwa 300 Millionen Menschen betrifft, Tendenz steigend: Es wird erwartet, dass 2025 bis zu 400 Millionen Menschen von dieser Krankheit betroffen sein werden (Masoli et al., 2004). Vor allem Kinder sind betroffen: In Deutschland beträgt die Prävalenz bei Kindern etwa 10%, bei Erwachsenen etwa 5% (Buhl et al., 2017). Die Tatsache, dass fast 250.000 Menschen jedes Jahr vorzeitig an Asthma sterben, unterstreicht die Bedeutung einer adäquaten Asthma-Therapie, denn die meisten dieser Todesfälle sind vermeidbar (Serebrisky and Wiznia, 2019). Das Ziel dieser Arbeit ist es, durch die Untersuchung des Chemokins Regulated on activation, normal T-cell expressed and secreted (RANTES) in sowohl klinischem als auch experimentellem Setting ein umfassendes Bild von dessen Rolle in allergischem Asthma zu schaffen und mögliche therapeutische Eigenschaften von RANTES zu explorieren

    Hintergrund: Asthma bronchiale ist eine chronisch entzündliche Erkrankung der Atemwege. Die Symptome variieren zeitlich und in ihrer Intensität. So gibt es leichte Verläufe mit rezidivierendem Husten oder Räusperzwang, aber auch schwere Verläufe mit lebensbedrohlicher Luftnot sowie Giemen, Brustenge und Husten. Die Genese des Asthma bronchiale ist multifaktoriell. T-Lymphozyten scheinen eine prominente Rolle in der Pathogenese von Asthma bronchiale zu haben, insbesondere die T-Helfer-Zellen und deren Zytokine Interleukin-4, Interleukin-5 und Interleukin-13. Insgesamt kommt es dann zu einer Hyperplasie der Becherzellen, einem Spasmus der glatten Muskulatur und zu einer eosinophilen Entzündungsreaktion. Die klassische Einteilung des Asthma bronchiale erfolgte in allergisches und nicht-allergisches Asthma. Diese Einteilung wurde in den letzten Jahren um weitere Phänotypen ergänzt, so wird nun unter anderem in Type-2-High-Asthma und Type-2-Low-Asthma unterschieden. Wiederholte virale Infekte könnten den sogenannten „immune shift“ zum Type-2-High Phänotypen begleiten und spielen eine wichtige Rolle bei Asthma Exazerbationen. Asthma Exazerbationen werden definiert als Zeiträume einer Abnahme der Lungenfunktion sowie einer progredienten Zunahme der Asthmasymptome. Eine Exazerbation kann lebensbedrohliche Zustände auslösen (R. Buhl 2017; Bundesärztekammer (BÄK) 2020). Rhinovirus ist ein häufiger Auslöser von Infekten der oberen Atemwege und gehört zur Familie der Picornaviridae (Jacobs et al. 2013). Gelangt das Rhinovirus in eine Zelle, wird es von sogenannten Toll-like Rezeptoren, den Toll-like Rezeptoren (TLR) 7,8,9 und 3, erkannt (Thompson and Locarnini 2007). Dies führt zu einer Aktivierung von „myeloid differentiation primary response protein“ (MyD88) -abhängigen intrazellulären Signalkaskaden, welche zu einer Phosphorylierung des Interferonregulierenden Faktors (IRF) 7 und 3 und daraufhin zur Produktion von Interferon Typ I (Interferon-alpha und Interferon-beta) und Interferon Typ III (Interferon-lambda) (Bergauer et al. 2017; Thompson and Locarnini 2007) führt. Interferon-alpha bindet dann den Interferon-alpha-Rezeptor, welcher aus 2 Transmembranglykoproteinen, Interferon-alpha-Rezeptor 1 und Interferon-alpha-Rezeptor 2 besteht (Platanias 2005). Interferon-lambda bindet an den sogenannten Interferon-lambda Rezeptor, dieser besteht aus Interferon-lambda-Rezeptor-alpha und Interleukin-10-Rezeptor 2 (Sopel et al. 2017; Donnelly and Kotenko 2010). Die Bindung an diese Rezeptoren führt intrazellulär zu einer Aktivierung der Transkriptionsfaktoren „signal transducer and activator of transcription protein“ (STAT) 1 und STAT2. Die nun phosphorylierten STAT1 und STAT2 bilden mit IRF9 einen wichtigen Transkriptionskomplex, den sogenannten „IFN-stimulated gene factor 3“ (ISGF3) Komplex. Dieser Komplex translokiert in den Zellkern und bindet spezifische Elemente, die als IFN-stimulierte Antwortelemente (ISREs) bekannt sind. Diese führen dann zur Transkription IFN-stimulierter Gene (Platanias 2005).Verschieden Studien zeigten eine fehlende oder reduzierte Interferon-alpha Antwort bei asthmatischen Kindern bei Virusinfektionen (Sykes et al. 2012; Wark et al. 2005; Edwards et al. 2013), allerdings konnte Bergauer et al. erhöhte Interferon-alpha Spiegel in der akuten Exazerbation nachweisen (Bergauer et al. 2017). Ziel dieser Arbeit war es, die Effekte von Rhinovirus auf die 2 verschiedenen Rezeptoren (Interferon Typ I Rezeptor und Interferon Typ III Rezeptor), sowie den Einfluss von TLR7/8 Agonist (R848) auf die Regulation dieser Rezeptoren zu untersuchen. Methoden: In dieser Arbeit wurden Interferon-alpha und Interferon-lambda sowie deren Rezeptoren in peripheren mononuklären Zellen (PBMC) sowie deren Überstände von Kindern mit und ohne Asthma bronchiale gemessen. Hierzu wurden 2 Kohorten genutzt. Die Kohorte der europäischen PREDICTA (Post-infectious immune reprogramming and its association with persistence and chronicity of respiratory allergic disease) Studie umfasste Kinder im Alter von 4-6 Jahren. Die Kohorte der AGENDAS (Gender, age and environmental factors that modify immune-response and the development of allergic asthma during the school age in childhood) Studie schloss Kinder im Alter von 6-12 Jahren ein. Die Zellen wurden dann in vitro mit Rhinovirus 1b infiziert oder mit R848, einem Toll-like-Rezeptor 7 und 8 Agonisten, stimuliert. Zudem untersuchten wir murine Lungenzellen, welche ebenfalls in vitro mit Rhinovirus 1b infiziert oder mit R848 stimuliert wurden. Ergebnisse und Schlussfolgerungen: Es zeigte sich eine Induktion der mRNA von Interferon-lambda Rezeptor und eine Reduktion der mRNA von Interferon-alpha Rezeptor nach R848 Stimulation in den PBMC der untersuchten Kinder. Ebenso zeigte sich im murinen Versuchsaufbau eine verminderte Expression des Interferon-alpha Rezeptors auf der Oberfläche von T-Lymphozyten nach Rhinovirusinfektion und Rhinovirusinfektion und R848 Stimulation in vitro. Die Induktion des Interferon-lambda Rezeptors durch R848 Stimulation in vitro könnte eine neue Möglichkeit der Induktion einer antiviralen Immunantwort in der Therapie des pädiatrischen Asthma bronchiale sein

    Neoplasms of the lungs are the leading cause of cancer incidence and mortality worldwide. Although immunotherapy has increased the overall survival of patients with lung cancer, there is the need to improve this treatment. At this regard, blood lipid levels are thought to be linked to cancer risk and thus a preventive intervention through regulation of the nutrition of patients with lung cancer is gaining much attention. In this study, we therefore asked about the contribution of serum lipids and cholesterol cellular metabolism in lung cancer development and progression. We measured different serum lipids and analyzed cholesterol synthesis enzymes 3-hydroxy-3-methyl-glutaryl CoA reductase (HMGCR) and acetyl-coenzyme A cholesterol acetyltransferase 1 (ACAT1) as well as the cholesterol cellular export protein ATP-binding cassette (ABC) A-1 mRNA by quantitative PCR (qPCR) in the control and tumoral regions of post-surgery lung tissues to analyze the accumulation of cholesterol in cancer cells in a cohort of patients with lung adenocarcinoma (LUAD). We found that triglycerides in serum directly correlated with the body mass index (BMI) in patients with LUAD. By contrast, we found that high-density lipoprotein (HDL) cholesterol inversely correlated with the BMI, C-reactive protein (CRP) and overall survival and total cholesterol inversely correlated with the tumor diameter, serum CRP and overall survival in these LUAD patients. Functionally, the role of cholesterol is indispensable for the growth and development of normal animal cells where it is tightly regulated. Excess of cellular cholesterol regulated by HMGCR is converted to cholesteryl esters by the enzyme ACAT1 and exported extracellularly by the cholesterol transporter ABCA1. Here we found HMGCR and ACAT1 upregulated and ABCA1 downregulated in the lung’s tumoral region of our LUAD cohort, indicating cholesterol dysregulated cellular export in lung tumor cells.     Going Beyond Counting First Authors in Author Co-citation Analysis

    The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed. Reasons for these effects are discussed
