Multimedia Services with QoS Support in an Integrated Terrestrial and Satellite UMTS Network: The IST VIRTUOUS Project
No full textOne of the main challenges of the wireless telecommunication systems which are presently being designed, is an efficient provision of an end-to-end QoS tailored to the specific requirements of each connection. For this purpose, one of the main goals of the European Union IST Research Project VIRTUOUS was the investigation and development of procedures for the QoS support in the UMTS system. This paper deals with the description of the QoS architecture designed for the IST VIRTUOUS system, especially targeting at the design of an efficient UMTS MAC scheduling algorithm that is in charge of managing several parallel active logical channels (i.e. UMTS connections) respecting their relevant QoS constraints. The joined approach to the packet scheduling and Transport Format choice proposed in this paper shows better performances respect to the 3GPP standard solution. The last one gives poor results as the number of connections of different type - i.e. connections with different needs in terms of maximum allowed delay - increases. Furthermore, the innovative MAC design introduced in this paper allows a much better exploitation of the available bandwidth in the UMTS Radio Access Network, increasing also fairness between IP datagrams belonging to different sources

No full textThe analysis of two novels from the 1920s selected as emblematic cases – Le Monde désert by Jouve and La Mort difficile by Crevel – leads the author to point out the connections between the figuration of the subject in literature and the portrait in painting. Indeed, the survival of the portrait itself as a pictorial genre can be seen as a sign of its involvement in the enquiry of identity and subjectivity. This article describes the three main axes that organise subjectivity as a portrait: the fluctuation between stasis and movement, the relational dimension and the fictional one. The portrait becomes the ideal model of a subject conceived as the result of a creative process. Hence, painting is not to be considered a mere narrative theme but a key to the interpretation of the novels and a metaphor of the construction of the subject

Prediction of extubation failure in Intensive Care Unit: systematic review of parameters investigated
No full textINTRODUCTION: Extubation failure (EF) refers to the inability to maintain spontaneous breathing after removal of endotracheal tube. The aim of this review is to identify the best parameter to predict EF in adult intensive care patients. EVIDENCE ACQUISITION: We searched for publications in PubMed (2000-2016). Studies of patients intubated and mechanically ventilated for more than 24 hours were included and divided in groups basing on the extubation method. 2x2 tables were performed to evaluate the sensitivity, specificity and the predictive values only for those parameters investigated in more than three studies. Studies were divided in groups, basing on time required to define EF (72 hours), and EF percentage was calculated for each group. EVIDENCE SYNTHESIS: On 443 potentially studies, 26 were included. Rapid Shallow Breathing Index (RSBI) and cough strength parameters were found in more than three studies. RSBI or cough strength parameter showed a sensitivity of 20-88.8% or 55.5-85.2%, a specificity of 68.5-94.8% or 24-49%, a positive predictive value (PPV) of 39.5-66.6% or 24-49% and a negative predictive value of 98-82% or 89.5-96.4%, respectively. EF rate was 12.5%, 15.3% and 22% in patients evaluated within 24 hours, 72 hours and over 72 hours, respectively. CONCLUSIONS: This review shows that all parameters used to predict EF have a low PPV. Therefore, the limitation of use of such predictive tests may prolong unnecessarily the intubation and increase the unfavorable outcome.
