Noninvasive monitoring of lung recruitment maneuvers in morbidly obese patients: the role of pulse oximetry and volumetric capnography

    No full text
BACKGROUND: We conducted this study to determine whether pulse oximetry and volumetric capnography (VCap) can determine the opening and closing pressures of lungs of anesthetized morbidly obese patients. METHODS: Twenty morbidly obese patients undergoing laparoscopic bariatric surgery with capnoperitoneum were studied. A lung recruitment maneuver was performed in pressure control ventilation as follows: (1) During an ascending limb, the lungs? opening pressure was detected. After increasing positive end-expiratory pressure (PEEP) from 8 to 16 cm H2O, fraction of inspired oxygen (FIO2) was decreased until pulse oximetric arterial saturation (SpO2) was <92%. Thereafter, end-inspiratory pressure was increased in steps of 2 cm H2O, from 36 to a maximum of 50 cm H2O. The opening pressure was attained when SpO2 exceeded 97%. (2) During a subsequent decreasing limb, the lungs? closing pressure was identified. PEEP was decreased from 22 to 10 cm H2O in steps of 2 cm H2O. The closing pressure was determined as the PEEP value at which respiratory compliance decreased from its maximum value. We continuously recorded lung mechanics, SpO2, and VCap. RESULTS: The lungs' opening pressures were detected at 44 (4) cm H2O (median and interquartile range) and the closing pressure at 14 (2) cm H2O. Therefore, the level of PEEP that kept the lungs without collapse was found to be 16 (3) cm H2O. Using respiratory compliance as a reference, receiver operating characteristic analysis showed that SpO2 (area under the curve [AUC] 0.80 [SE 0.07], sensitivity 0.65, and specificity 0.94), the elimination of CO2 per breath (AUC 0.91 [SE 0.05], sensitivity 0.85, and specificity 0.98), and Bohr's dead space (AUC 0.83 [SE 0.06], sensitivity 0.70, and specificity 0.95] were relatively accurate for detecting lung collapse during the decreasing limb of a recruitment maneuver. CONCLUSIONS: Lung recruitment in morbidly obese patients could be effectively monitored by combining noninvasive pulse oximetry and VCap. SpO2, the elimination of CO2, and Bohr's dead space detected the individual's opening and closing pressures.Fil: Tusman, Gerardo. Fundacion Medica de Mar del Plata. The eigenvalues of limits of radial Toeplitz operators

    No full text
Let A2 be the Bergman space on the unit disk. A bounded operator S on A2 is called radial if Szn = λnzn for all n 0, where λn is a bounded sequence of complex numbers. We characterize the eigenvalues of radial operators that belong to the Toeplitz algebra.Fil: Suarez, Fernando Daniel. Universitat Autònoma de Barcelona; España. Let D be the unit disk. We show that for some relatively closed set F ⊂ D there is a function f that can be uniformly approximated on F by functions of H∞, but such that f cannot be written as f = h + g, with h ∈ H∞ and g uniformly continuous on F. This answers a question of Stray.Fil: Suarez, Fernando Daniel. Consejo Nacional de Investigaciones Científicas y Técnicas. Oficina de Coordinación Administrativa Saavedra 15. Localisation techniques for division in Douglas algebras

In this paper we use uniform algebra techniques and recent results on separation properties to prove several division theorems for closed subalgebras of L∞ containing H∞. We also study ideals having the local approximation property and we show that not every ideal in such algebras is local.Fil: Suarez, Fernando Daniel. Let L^2-a be the Bergman space of the unit disk and T(L^2_a) be the Banach algebra generated by Toeplitz operators T_f , with f ∈ L∞. We prove that the closed bilateral ideal of T(L2a) generated by operators of the form T_fT_g − T_gT_f coincides with T(L^2_a).Fil: Suarez, Fernando Daniel. The present degree work tries to define in a methodological way the influence of child labor in the levels of schooling of students of the municipality of Suarez Tolima, a municipality that traditionally with high concentration in agricultural production, starting from this rush, tries to stimulate the debate on child labor guidelines issued in large proportion by the International Labor Organization, hereinafter, OTI, which states take as a reference in the design of public policies to mitigate this problem, but in evidence found, legal regulations are very distant of the social reality that children and adolescents face in everyday life in many regions of Colombia. In order to address the legal and social problems of this work, the general objective was to develop a legal and social case study referring to the conditions in which minors under immersion in child labor in Suarez - Tolima perform their duties. To obtain and process the data, a qualitative method with a case study approach was used, with a probabilistic sample composed of 17 teachers of grades 9,10 and 11 in the educational institution of the municipality.Introducción. -- 1. The Essential Norm of Operators in the Toeplitz Algebra on Ap(Bn)

    No full text
Let Ap be the Bergman space on the unit ball Bn of Cn for 1 <p< ∞, and Tp be the corresponding Toeplitz algebra. We show that every S ∈ Tp can be approximated by operators that are specially suited for the study of local behavior. This is used to obtain several estimates for the essential norm of S ∈ Tp, an estimate for the essential spectral radius of S ∈ T2, and a localization result for its essential spectrum. Finally, we characterize compactness in terms of the Berezin transform for operators in Tp.Fil: Suarez, Fernando Daniel. Consejo Nacional de Investigaciones Científicas y Técnicas.
