Biomechanical analysis of tissue engineering construct (TEC) for articular cartilage restoration: a pre-clinical study
INTRODUÇÃO: A lesão condral é uma condição associada a um grande ônus econômico e, se não tratada, pode causar alterações na biomecânica da articulação e resultar em diversas lesões consideradas altamente incapacitantes para o indivíduo, como no caso da osteoartrite. A cartilagem articular é um material viscoelástico que pode sofrer alterações estruturais e de composição, sendo importante avaliar as propriedades físicas do tecido, que variam de acordo com as suas zonas (superficial, média e profunda). As células estromais mesenquimais (CEM) têm a capacidade imunomoduladora e a sinalização parácrina, que são úteis para a engenharia tecidual no tratamento de lesões cartilaginosas. Assim como as propriedades biológicas, a avaliação mecânica do construto formado é importante. OBJETIVO: Portanto, este estudo tem por objetivo descrever a análise biomecânica para restauração de cartilagem por engenharia tecidual em um estudo translacional, com animal de médio porte, pelas técnicas de boas práticas de manipulação. MÉTODOS: Realizou-se um estudo experimental controlado em quatorze porcos em miniatura. Um defeito de cartilagem de 6 mm de diâmetro foi efetuado nos dois joelhos (membros posteriores) de cada animal. Um construto de engenharia tecidual (CET) produzido pelas CEM e livre de arcabouço foi implantado no defeito em um dos joelhos de cada porco. Os porcos foram eutanasiados após seis meses do procedimento experimental e foi realizada a análise biomecânica. Para avaliação mecânica da cartilagem restaurada, o teste de indentação consistiu em três ciclos de força-deslocamento, realizados a uma velocidade constante de 0,5 mm/min, com um deslocamento fixo de 5% da espessura média da cartilagem. Em seguida, foi realizada compressão não confinada máxima, com deslocamento fixo de 50% da altura total do bloco osteocondral, a uma mesma velocidade, com célula de carga de 100 N. RESULTADOS: A partir do teste de indentação, o módulo dinâmico (E*) obtido do segundo ciclo foi de 1,68 ± 1,18 MPa no grupo sem tratamento e de 2,81 ± 2,05 MPa no grupo com tratamento (p<0,05). O módulo de elasticidade reduzido (Er) aos 10% da região superior da segunda curva de descarga, no grupo sem tratamento, foi de 2,56 ± 1,43 MPa, enquanto no grupo tratado com o CET foi de 5,40 ± 3,42 MPa (p<0,05). No teste de compressão não confinada máxima o módulo de elasticidade (E) foi similar entre os grupos com tratamento e sem tratamento. CONCLUSÃO: O tratamento com CET a partir das CEM evidenciou maior capacidade de absorção de choque e resistência à deformação elástica em comparação ao grupo sem tratamentoINTRODUCTION: Chondral injury is a condition associated with a great economic burden, and if left untreated it can cause changes in the biomechanics of the joint and result in several injuries considered highly disabling for the individual, as in the case of osteoarthritis. Articular cartilage is a viscoelastic material that can undergo structural and compositional changes, and it is important to evaluate the physicalproperties of the tissue, which vary according to the zones (superficial, middle, and deep). Mesenchymal stromal cells (MSC) have the immunomodulatory capacity and paracrine signaling that are useful for tissue engineering in the treatment of cartilaginous lesions. As well as biological properties, the mechanical evaluation of the formed construct is important. OBJECTIVE: Therefore, this study aims to describe the biomechanical analysis for cartilage restoration by tissue engineering in a translational study, with a midsize animal, using the good manufacturing practice (GMP) techniques. METHODS: A controlled experimental study was carried out on fourteen miniature pigs. A 6mm diameter cartilage defect was performed in both knees (hind limbs) of each animal. A scaffold-free tissue engineering construct (TEC) produced by MSC was applied to the defect in one of the knees of each pig. The pigs were euthanized six months after the experimental procedure and biomechanical analysis was performed. For mechanical evaluation of the restored cartilage, the indentation test consisted of three force-displacement cycles performed at a constant loading rate of 0.5 mm/min with a fixed displacement of 5% of the mean cartilage thickness. Then, maximum unconfined compression test was performed with a fixed displacement of 50% of the total osteochondral block height, at the same speed, with a load cell of 100N. RESULTS: From the indentation test, the dynamic modulus (E*) obtained from the second cycle was 1.68 ± 1.18 MPa in the untreated group and 2.81 ± 2.05 MPa in the treated group (p<0. 05). The reduced Young modulus at 10% of the upper region of the second unloading curve, in the untreated group, was 2.56 ± 1.43 MPa, while in the group treated with TEC it was 5.40 ± 3 .42 MPa (p<0.05). In the maximum unconfined compression test, the Young modulus was similar between the treated and untreated groups. Imaging evaluation of tissue engineering construct for cartilage repair in a preclinical model
INTRODUÇÃO: A lesão condral articular causa limitações funcionais, dor e pode progredir para osteoartrite, o que causa elevado custo financeiro para o tratamento. As células-tronco mesenquimais (CTM) apresentam capacidade de expansão, diferenciação condrogênica, e de imunorregulação local, mostrandose úteis para o tratamento das lesões condrais. O composto desenvolvido por engenharia tecidual (CET) é formado por células mesenquimais e matriz extracelular, sendo uma estratégia que evita a necessidade de arcabouço sintético ou de origem animal. O tratamento da lesão condral pode ser avaliado de forma não invasiva com a ressonância magnética (RM), que permite a análise da morfologia e da composição do tecido de reparo. OBJETIVOS: o objetivo primário desse estudo foi avaliar por ressonância magnética o reparo da lesão da cartilagem, utilizando o CET. E o secundário foi avaliar o tratamento por histologia e correlacionar com a avaliação por ressonância. MÉTODOS: O CET foi desenvolvido em centro de tecnologia celular tipo 2, certificado pela ANVISA, com células-tronco mesenquimais, seguindo as boas práticas em laboratório. Um defeito de cartilagem de 6mm de diâmetro foi realizado nos dois joelhos (membros posteriores) em 14 porcos em miniatura (BR1). O CET foi aplicado no defeito em um dos joelhos de cada animal. Após 6 meses, foi realizada eutanásia e ressonância magnética dos joelhos operados para avaliação da morfologia, com o sistema de pontuação MOCART 3D, e da composição de água e fibras de colágeno do tecido de reparo, pelo mapeamento em T2. O valor médio do T2 foi calculado na região em que foi realizado o defeito e em região adjacente com cartilagem saudável. Em cada região, o valor médio do T2 foi calculado na zona profunda e superficial. Por último, o reparo da cartilagem foi avaliado através de histologia pelo escore ICRS-2. RESULTADOS: O valor médio do MOCART no grupo submetido ao defeito sem tratamento foi de 46,2 ± 13,4, enquanto o grupo tratado com o CET teve valor médio da pontuação de 62,3 ± 12,3 (p < 0,001). A avaliação por mapeamento em T2 no grupo sem tratamento mostrou haver diferença estatística entre o valor médio do T2 da região do defeito condral (50,9 ± 2,4) e da região da cartilagem saudável (54,4 ± 1,7), (p < 0,05) o que indica diferença na composição entre as duas regiões. No grupo com tratamento, o valor médio do T2 na região que recebeu o CET foi de 53,4 ± 2,1 e não apresentou diferença em relação ao valor médio do T2 da cartilagem adjacente saudável, que foi de 54,7 ± 2,2 (p=0,7). A avaliação histológica do reparo da cartilagem com o sistema ICRS-2 mostrou um valor médio de 43,5 ± 4,3 na avaliação geral do grupo submetido apenas ao defeito da cartilagem e um valor médio de 57,8 ± 5,1 no grupo submetido ao tratamento com o CET (p < 0,05). CONCLUSÃO: A avaliação da morfologia e composição por ressonância magnética e análise histológica mostraram que o defeito de cartilagem tratado por composto de engenharia tecidual foi responsável por uma cobertura maior e de melhor qualidade do defeito em relação ao grupo sem tratamentoINTRODUCTION: The chondral joint damage causes functional limitations, pain and can progress to osteoarthritis, which causes a high financial cost for the treatment. Mesenchymal stem cells (MSC) are capable of expansion, chondrogenic differentiation, and local immunoregulation, proving to be useful for the treatment of chondral lesions. The compound developed by tissue engineering (TEC) is formed by mesenchymal cells and extracellular matrix, being a strategy that avoids the need for a synthetic framework or of animal origin. The treatment of chondral injury can be evaluated non-invasively with magnetic resonance imaging (MRI), which allows the analysis of the morphology and composition of the repair tissue. OBJECTIVES: The primary objective of this study was to evaluate by magnetic resonance the repair of cartilage damage using TEC. And the secondary was to evaluate the treatment by histology and correlate with the evaluation by resonance. METHODS: The TEC was developed in a type 2 cell technology center, certified by ANVISA, with mesenchymal stem cells, following good laboratory practices. A 6mm diameter cartilage defect was performed in both knees (hind limbs) in 14 miniature pigs (BR1). TEC was applied to the defect in one of the knees of each animal. After 6 months, euthanasia and MRI of the operated knees were performed to assess the morphology, with the MOCART 3D scoring system, and the composition of water and collagen fibers of the repair tissue, by T2 mapping. The mean T2 value was calculated in the region where the defect was made and in an adjacent region with healthy cartilage. In each region, the mean T2 value was calculated in the deep and superficial zone. Finally, cartilage repair was evaluated through histology using the ICRS-2 score. RESULTS: The mean value of MOCART in the group submitted to the defect without treatment was 46.2 ± 13.4, while the group treated with CET had a mean score of 62.3 ± 12.3 (p < 0.001). The T2 mapping evaluation in the untreated group showed a statistical difference between the mean T2 value of the chondral defect region (50.9 ± 2.4) and the healthy cartilage region (54, 4 ± 1.7), (p < 0.05) which indicates a difference in composition between the two regions. In the treatment group, the mean T2 value in the region that received the TSC was 53.4 ± 2.1 and showed no difference in relation to the mean T2 value of the healthy adjacent cartilage, which was of 54.7 ± 2.2 (p=0.7). The histological evaluation of cartilage repair with the ICRS-2 system showed a mean value of 43.5 ± 4.3 in the general evaluation of the group submitted only to the cartilage defect and a mean value of 57.8 ± 5.1 in the group submitted to treatment with the CET (p < 0.05). CONCLUSION: The evaluation of morphology and composition by magnetic resonance and histological analysis showed that the cartilage defect treated with tissue engineering construct was responsible for greater coverage and better quality of the defect compared to the untreated grou
Advanced cell therapy with mesenchymal stromal cells for chondral lesions and osteoarthritis: systematic review and meta-analysis of randomized clinical trials
Esta tese teve como objetivo avaliar a eficácia e a segurança da terapia avançada com células estromais mesenquimais (CEM) para o tratamento de osteoartrite (OA) e de lesões focais da cartilagem do joelho. O método empregado em nossa pesquisa foi uma revisão sistemática e metanálise de ensaios clínicos randomizados (ECR), conduzida de acordo com os preceitos do Cochrane Handbook e relatada seguindo as recomendações do protocolo PRISMA. A abordagem GRADE foi implementada para a avaliação da certeza da evidência. Nós realizamos uma ampla busca na literatura, sem restrição de idioma, local ou data. A data da última busca foi 31 de outubro de 2022. A leitura e análise de 6.372 resultados levou à inclusão de 133 registros, sendo 43 registros correspondentes a 25 estudos completos e os 90 restantes correspondentes a estudos em andamento. Os estudos completos incluídos foram publicados entre 2002 e 2022 e somaram um total de 1.048 participantes. Foram 22 estudos a respeito do tratamento da osteoartrite e 3 a respeito das lesões de cartilagem. Os dados das metanálises executadas indicaram que, no tratamento da OA de joelho, a terapia avançada com CEM foi mais eficaz do que o placebo e a viscossuplementação (VS) para reduzir a dor, especialmente após 12 meses de tratamento. Além disso, ao excluir estudos com alto risco de viés, observou-se um efeito semelhante, com maior certeza de evidência. Quanto à ocorrência de eventos adversos graves, não foram identificadas diferenças significativas entre a terapia avançada com CEM e o placebo ou a VS.The objective of this thesis was to evaluate the efficacy and safety of advanced mesenchymal stromal cell (MSC) therapy for the treatment of osteoarthritis (OA) and knee focal chondral lesions. The method employed was a systematic review and meta-analysis of randomized controlled trials (RCTs), conducted in accordance with the Cochrane Handbook and reported following the PRISMA 2020 statement. The GRADE approach was implemented to assess the certainty of the evidence. We conducted a broad literature search, with no restrictions on language, location or date. The date of the last search was on October 31, 2022. The reading and analysis of 6,372 results led to the inclusion of 133 records, of which 43 records corresponded to 25 complete studies and the remaining 90 corresponded to ongoing studies. The complete studies were published between 2002 and 2022 and included 1,048 participants. There were 22 studies regarding the treatment of osteoarthritis and 3 on chondral lesions. Data from the meta-analyses performed indicated that, for the treatment of knee OA, advanced MSC therapy was more effective than placebo and viscosupplementation (VS) in reducing pain, especially after 12 months of treatment. Furthermore, when studies with high risk of bias were excluded, a similar effect was observed, with greater certainty of evidence. Evaluation of knee stability in central and anteromedial anatomic anterior cruciate ligament reconstruction in cadaver: a randomized study
INTRODUÇÃO: A localização ideal dos túneis do enxerto para a reconstrução do ligamento cruzado anterior (LCA) está na área da impressão original do ligamento. Entretanto, há pacientes que mesmo após a reconstrução anatômica do LCA se queixam de falseio. O objetivo do presente estudo foi comparar a estabilidade do joelho com túneis em duas posições diferentes da área de impressão original do LCA. MÉTODOS: Trinta reconstruções anatômicas do LCA foram realizadas em 15 joelhos de cadáver, com preservação do quadril até o tornozelo. Não foram criadas lesões para intensificar a instabilidade do joelho. O protocolo possuía quatro grupos: (1) LCA intacto, (2) deficiência completa e isolada do LCA, (3) reconstrução do LCA na posição anteromedial (AM REC) e (4) central (C REC) da impressão original do ligamento (ordem randômica). A estabilidade do joelho foi testada pelo teste de Lachman (68 N) e pelo teste do \"pivot shift\" mecanizado (CPM de 0° a 55° + valgo e rotação interna de 20 Nm) e avaliada por sistema óptico de navegação e tomografia 3D. A descrição da cinemática do joelho com 6 graus de liberdade foi realizada por sistema de coordenadas com eixo no centro geométrico dos côndilos. Estatística: rotação interna (°) e translação anterior (mm) foram avaliados nos testes do \"pivot shift\" e Lachman com 2 e 1- way RM ANOVA, respectivamente (alfa 0,05) e de Lachman (1,2 mm ± 0,2 mm vs. 1,1 mm ± 0,2 mm, respectivamente, P > 0,05). Os grupos C REC e AM REC não mostraram diferença quando comparados ao grupo LCA intacto no teste de Lachman para translação anterior (2,1 mm ± 0,5 mm) e rotação interna (3,0° ± 0,5°) (para ambos, P > 0,05) e mostraram diferença no teste do \"pivot shift\" (6,4 mm ± 0,4 mm e 5,6° ± 0,3°, respectivamente, P 0,05) or Lachman (1,2 mm ± 0,2 mm vs. 1,1 mm ± 0,2 mm, respectively, P > 0,05) test. Central REC and AM REC groups showed no differences compared to intact ACL group in Lachman test for anterior translation (2,1 mm ± 0,5 mm) and internal rotation (3,0° ± 0,5°) (for both, P > 0,05) but did show significant differences in the pivot-shift test (6,4 mm ± 0,4 mm and 5,6° ± 0,3°, respectively, P < 0.05). DISCUSSION: This was the first study to demonstrate knee kinematic changes in a hip-to-toe cadaver model without associated lesions to amplify knee instability with a mechanized pivot shift in the complete sequence of intact, ACL-deficient and anatomic reconstructions. CONCLUSION: Central anatomic ACL reconstruction permitted greater restriction of internal rotation than did anteromedial anatomic ACL reconstruction based on mechanized pivot shift and Lachman tests. Going Beyond Counting First Authors in Author Co-citation Analysis
The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation
counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings
are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that
only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into
account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed. Variations on the Author
“Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. Appropriate Similarity Measures for Author Cocitation Analysis
We provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature. Dispelling the Myths Behind First-author Citation Counts
We conducted a full-scale evaluative citation analysis study of scholars in the XML research field to explore just how different from each other author rankings resulting from different citation counting methods actually are, and to demonstrate the capability of emerging data and tools on the Web in supporting more realistic citation counting methods. Our results contest some common arguments for the continued
use of first-author citation counts in the evaluation of scholars, such as high correlations between author rankings by first-author citation counts and other citation
counting methods, and high costs of using more realistic citation counting methods that are not well-supported by the ISI databases.
