    Long-term results in quality of life after surgery of early adenocarcinoma of the distal esophagus

    Zur Behandlung des Ösophagusfrühkarzinoms gibt es die radikale Ösophagektomie im Gegensatz zur limitierten onkologischen Resektion des ösophago-gastralen Überganges. 64 Patienten unterzogen sich einer radikalen Ösophagektomie und bei 53 Patienten wurde eine limitierte Resektion des gastroösophagealen Übergangs durchgeführt. Beide Patientengruppen wurden hinsichtlich Rezidivfreiheit, Komplikationen und Lebensqualität mit Hilfe des Gastrointestinalen Lebensqualitätsindexes (GIQLI) verglichen. Mit beiden Verfahren ist eine komplette Tumorresektion möglich. In der Gruppe der radikalen Resektion traten zwei Rezidive auf. Die Lebensqualität war nach limitierter Resektion höher als nach radikaler Operation (102 vs. 95 Punkte im GIQLI), ebenso die Langzeit-Überlebensraten (5-JÜR 89,6% vs. 78,7%). Die Komplikationsrate war bei der limitierten Resektion niedriger (24,5% vs. 29,7%).There is the radical esophagectomy to treat an early esophageal adenocarcinoma in contrast to the limited oncologic resection of the esophagogastric junction. 64 patients were treated with a radical esophagectomy and a limited resection of the gastroesophageal junction was performed in 53 patients. Both groups were compared as to relapse, complications and quality of life with the Gastrointestinal Quality of Life Index (GIQLI). With both procedures a complete tumour resection is possible. In the radical resection group there were two relapses. Quality of life was higher after limited resection compared to after radical operation (102 vs. 95 points in the GIQLI), as well as longterm survival (5-year survival rate 89,6% vs. 78,7%).     Going Beyond Counting First Authors in Author Co-citation Analysis

    The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed.     “Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship

    Diese Studie untersucht die Prävalenz, die prognostischen Relevanz und die zu ziehenden therapeutischen Konsequenzen bei einer postoperativ festgestellten R1 Situation eines Adenokarzinoms des ösophagogastralen Übergangs. Hierzu identifizierten wir Risikofaktoren und Rezidivmuster für und bei einer R1 Resektion und führten Überlebensanalysen durch. In dem untersuchten Patientengut manifestierte sich das Tumorrezidiv hauptsächlich in Form von Fernmetastasen., was auf eine frühe und aggressive systemische Tumorstreuung hindeutet. Unsere Studie sieht somit den größten therapeutischen Vorteil in der Anwendung einer systemisch wirkenden additiven reinen Chemotherapie für fortgeschrittene Stadien eines AEGs.The goal of this thesis was the outcome analysis of patients with adenocarcinoma of the esophagogastric junction (AEG) and a postresection R1 status. We tried to identify risk factors for and prevalence of R1 resection, their recurrence and prognosis, and efficacy of postoperative therapy. The association of R1-resections with poor tumor characteristics allows for identification of patients at risk for R1-resection. The main challenge of this tumor entity is the very early sytemic spread as tumor recurrence in R1-resections was mainly systemic, not local. The potential benefit of additive local postoperative therapies after R1-resections must be balanced against overall prognosis and therapy-specific morbidity and mortality

    Appropriate Similarity Measures for Author Cocitation Analysis

    We provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature.     Dispelling the Myths Behind First-author Citation Counts

    We conducted a full-scale evaluative citation analysis study of scholars in the XML research field to explore just how different from each other author rankings resulting from different citation counting methods actually are, and to demonstrate the capability of emerging data and tools on the Web in supporting more realistic citation counting methods. Our results contest some common arguments for the continued use of first-author citation counts in the evaluation of scholars, such as high correlations between author rankings by first-author citation counts and other citation counting methods, and high costs of using more realistic citation counting methods that are not well-supported by the ISI databases. It is argued that increasingly available digital full text research papers make it possible for citation analysis studies to go beyond what the ISI databases have directly supported and to employ more sophisticated methods

    We have done our best to complete the author checklist relating to the use of animals in the hut study. Note that the objective for the hut study was to evaluate the IRS treatment applications for residual efficacy against Anopheles mosquitoes, including the local An. coluzzii mosquito population. Cows were only used to attract mosquitoes into the huts and no tests were carried out directly on the cows. The author checklist is intended for use with studies where experiments are carried out on animals, which is why we have had such difficulty in completing this for the hut study, as many of the questions do not relate to how the cows were used

    In folgender Arbeit wurde der onkologische Langzeitverlauf des Plattenepithelkarzinoms des Ösophagus untersucht. Bei 1051 Patienten, die im Zeitraum von 1982 bis 2009 in der chirurgischen Klinik des Klinikums rechts der Isar behandelt wurden, wurde das Auftreten, die Lokalisation und der zeitliche Zusammenhang des Rezidivs bei verschiedenen Therapiemodalitäten und diversen Faktoren bezüglich der Patienten und des Tumors in einer retrospektiven Studie ausgewertet. Bei 37,8% der tumorfrei resezierten Patienten wurde ein Rezidiv diagnostiziert. Über die Hälfte erlitt dies im ersten postoperativen Jahr. Am häufigsten wurden Lokalrezidive und Lymphknotenmetastasen beobachtet. T-, N- und G-Status korrelieren mit der Rezidivinzidenz. Es wurde kein „Tumor-Downstaging“ durch neoadjuvante Therapie nachgewiesen.This study investigates the oncological long-term results of patients with squamous cell carcinoma of the oesophagus. 1051 Patients, who were treated from 1982 to 2009 in the Department of Surgery at Klinikum rechts der Isar, were enrolled in a retrospective study and analysed concerning the incidence, the localisation and the temporal correlation of the recurrence with different types of therapy and various characteristics of the patients and of the tumour. 37,8% of the tumour-free patients developed recurrence. Over 50% of the recurrences occurred in the first postoperative year. The most frequent localisations were local recurrence and lymph node metastases. T-, N- and G-Status correlate with the incidence of recurrence. “Tumor-downstaging” after neoadjuvant therapy was not observed
