    In this final summary chapter, we explore some basic arguments for developing critical reflection and in this final summary chapter, we explore some basic arguments for developing critical reflection and evaluation in health education, client practices and research. We do not deal with «criticism» in the meaning of faultfinding; but rather with «critique» in the sense of abstract reflection, political commentary, or an in depth discussion of all topics within the health-area. Critique is an exploration into the foundations and limits of scientific and practical knowledge. Our ambition has been to disrupt and challenge attitudes that have been taken for granted, and instead look for what is different, unsettling and thought provoking. In order to manage this we need to embrace criticism at different levels, applying theoretical and historical perspectives on actual empirical issues. This anthology tries to show that critique is a necessary part of all work with health issues, in education, in client work and in scientific activities. For the authors of this book, critique means asking questions about what is taken for granted and what may be understood in a different manner. In all academic, educational and practical reasoning critique is regarded as a necessary too

Studien konkluderar med at: valg av rehabiliteringsarena bør bestemmast ut frå funksjonsnivå framfor diagnose, det kan sjå ut til å vere behov for «mellomstasjonar» mellom første og andrelinjetenesta, ein fare ved å utvikle eit felles tankegods kan vere at ein misser den kimen til framdrift som ligg i at ein ser, og forstår «verda» frå ulike ståstadar og at aktørane på rehabiliteringsområdet bør ta eit felles ansvar slik at brukarane ikkje blir skadelidande på grunn av uklare ansvarsforhold.Rehabilitation activity can be viewed as a diverse, interprofessional, interdepartmental, complex and challenging professional crossfield. This qualitative study looks into the challenges connected to the interaction and coordination between the primary and secondary health services as described by healthcare professionals. The purpose is to clarify what really happens in the crossfield between clients, professionals, institutions, profession and economy. Data is obtained through interviewing 19 participants from private rehabilitation institutions, primary and secondary health services, from three different interprofessional focus groups. Results indicate that: choice of rehabilitation arena can be random, that there is a need for rehabilitation institutions in between the primary and secondary health services, professionals call for a common rehabilitation philosophy and economy seems to overrule professional assessment. The study concludes that: choice of rehabilitation arena should preferably be determined by level of functioning rather than diagnosis, there seems to be a need for rehabilitation institutions in between primary and secondary health services, by developing a common rehabilitation philosophy healthcare professionals can lose the advantage of seeing and understanding the world from different perspectives, and that the professionals within the field of rehabilitation should take/share a common responsibility so that clients/users don´t end up as casualties due to unclear responsibilities

    Challenges with collectivistic culture in home care reablement

    Purpose : The aim of this study is to investigate the challenges that health professionals face in rehabilitation with Adults' minorities in home care reablement and how these experiences can be understood. Background: The study indicates there has been an increase of immigrant in Norway, due to labour immigration from the EU's new member states east of Europe, and to the many refugees who have found their way to Norway due to war or poverty. Meeting this group of people from different culture with lots of family involvement in clients' rehabilitation, limited knowledge about their health and public information about rehabilitation has and is challenging for health professionals. Method: Qualitative method has been used to answer the study's research questions. Semi- structured interviews were carried out with six health professionals. The sample consisted of 2 physiotherapist (both were Norwegians), 3 were occupational therapist (one Norwegian, one from Bosnia and one from Zimbabwe) and 1 nurse from Somalia. A systematic text condensation that consists of four steps was used to analyse the raw data. Results (analyses and discussion): The health professionals identified several features of the challenges they face in home care reablement that they believed affected the rehabilitation of adults´ minorities. These were diversity culture, interdependence, expectation on recovery, problematic with goal setting and cultural awareness. Conclusion: It appears to be a need for increased multicultural competence and more people with minority backgrounds in health and social services with better prerequisites for understanding the cultural and religious background of adult´ minorities

    Kommunal planlegging av rehabiliteringstjenester - en deskriptiv analyse av fire nordtrønderske kommuner

    Background: National guidelines for rehabilitation are presented in laws, regulations, and different reports. A new evaluation of the national rehabilitation development plan exposes that the municipalities lack a uniform approach to the rehabilitation field. Research question: How does four municipalities in North Trøndelag plan and organize their rehabilitation services? Are organizational placement of rehabilitation services and type of plan coherent factors? How is the rehabilitation service organized in sectors and what effect does that show on the professional orientation of the plan? What coherence is there between professionals` participation in the planning, and the specificity of the plan? Material and method: The study design is a qualitative analysis of four plan documents. The plans are of two different types, two overall health care plans, and two executive rehabilitation plans. Results: The two municipalities that have placed their rehabilitation services in a unit called Home care services, have an overall health care plan as the governing document for the rehabilitation field. The two municipalities that use the term Rehabilitation and/or Prevention in the name of the unit, have their own rehabilitation plan. The municipalities who organize rehabilitation in two different agencies have a wider professional orientation than the municipalities who gathers their rehabilitation services in one agency. The type of plan has an obvious significance for how overall or specific the goals and measures are described. Conclusion: The four municipalities have differences in plan types and professional orientation. To have an executive plan, visualization of rehabilitation high up in the organization, and dedicated spokespersons involved in decision-making processes can provide power to put rehabilitation on the agenda and ensure development of the field. An inter-agency organization of rehabilitation services seems to contribute to a broader understanding of rehabilitation than one-agency organization. An executive plan with specific goals and measures appears to be a more useful and integrative tool for rehabilitation professionals - rather than an overall plan with overall goals and measures.publishedVersio

    Bakgrunn: Mitt formål med masteroppgaven er å belyse hvordan minoritetsforeldre som har barn med nedsatt funksjonsevne opplever sin situasjon. Det å inneha minoritetsbakgrunn og funksjonsnedsettelse er i politiske dokumenter beskrevet som en «dobbel minoritetsstatus». Det er lite forskningsbasert kunnskap om dette feltet og med denne oppgaven ønsker jeg å bidra med å synliggjøre disse foreldrenes erfaringer og opplevelser. Problemstilling: Studiens problemstilling er tredelt. Jeg har en hovedproblemstilling og to underspørsmål.1) Hvordan opplever foreldre med minoritetsbakgrunn det å ha et barn med nedsatt funksjonsevne? Som utdypende spørsmål har jeg følgende; 1.1. Hvordan forholder de seg til egne følelser og til barnet? 1.2. Hvordan forholder de seg til andre nære relasjoner og hjelpeapparatet? Metode: Jeg har valgt kvalitativ studie med intervju av åtte familier fra fem ulike land. Intervjuene ble tatt opp på bånd og analysert inspirert av prinsippene i Grounded Theory. Analysemetoden er diskutert i forskerteam med spesialkompetanse innen GT. Resultat: I studien laget jeg fire hovedkategorier og en kjernekategori. Hovedkategoriene peker på fire ulike aspekt ved kjernekategorien; «å møte en helt ny verden». Hovedkategoriene «å legge egne reaksjoner på is» og «å forstå hva funksjonshemming innebærer» belyser samtidig den ene underproblemstillingen. I de to siste hovedkategoriene «å håndtere den praktiske hverdagen» og «å forholde seg til et hjelpeapparat» belyses også den andre underproblemstillingen. Diskusjon: Problemstillingen besvares med kjernekategorien «å møte en helt ny verden» og i diskusjonsdelen drøftes fremtredende trekk ved denne. Sentralt for den nye situasjonen familien befinner seg i er at den i stor grad oppleves som en ensom og isolert tilværelse. I forhold til egne følelser og til barnet så formidler mange at de ikke har behov for å snakke om hvordan de har det, å gå inn i egne følelser ville ikke hjelpe barnet, som var deres hovedfokus. Å bli møtt med varme og engasjement fra fagpersoner og sosialt nettverk er vesentlig for foreldrene, og er av betydning for hvordan de håndterer hverdagen. For at vi som fagpersoner kan bidra til at møte med den nye verden blir mer forståelig er kommunikasjon en viktig faktor, med kommunikasjon kan vi skape et meningsfullt samhandlingsrom. Konklusjon: Minoritetsforeldre som har barn med nedsatt funksjonsevne har med sin doble minoritetsstatus noen tilleggsutfordringer når de møter «en helt ny verden». De er i et nytt land, men mestrer ikke å ta ut sine egne følelser, men setter inn all oppmerksomhet på barnet. Mange av disse reaksjonene vil de dele med foreldre som er etnisk norske. Men det å ha et begrenset nettverk, kommunikasjonsutfordringer og liten oversikt over offentlige tilbud er eksempel på at de har det ekstra vanskelig. En fast kontaktperson som de kan ha tillit til og som bistår og veileder dem i forhold til hjelpeapparatet og som har oversikt over familiens behov er noe som mange av dem har etterspurt

There is little knowledge on this field and with this task, I want to contribute these parents' experiences. Both minority and disabled are in policy documents described as a "double minority status." Problem: The issue is divided into three. The main issue is: “How parents from minority groups experience having a child with disabilities?” The questions are: “How do they relate to their own feelings and the child”, and, “How do they relate to other close relationships and support system?” Method: A qualitative interview method has been used where 8 families from 5 different countries, have been interviewed. In the study, Grounded Theory was applied as the main research method. The analysis method is discussed in a research team with expertise in GT The results: The core category of the study became “meeting a whole new world”. This emerges from the four main categories. The main categories point to four different aspects of the core category. The main categories "to add their own reactions on ice" and "understanding what disability means" illuminate the first question. In the last two main categories "to deal with the everyday practice" and "to deal with the supporting service" also point to the second question. Discussion: This issue is answered with the core category "to meet a whole new world" and in this section prominent features will be discussed. Central to the new situation the family is in is that it is largely perceived as a lonely and isolated existence. In relation to their own feelings and the child they explain that they do not need to talk about how they feel, to enter into their own feelings would not help the child, which was their main focus. Being greeted with warmth and commitment from professionals and social networks are important for the parents, and its importance for how they handle their everyday life. As professionals we can contribute the families to meet and understand the new world better. Communication is an important factor; with communication we can create a meaningful interaction. Conclusion: Minority Parents who have children with disabilities have some additional challenges when facing "a whole new world." They are in a new country, and they don`t manage to take out their own feelings, but inserting all the attention on the child. Many of these reactions they share with parents who are ethnically Norwegian. But having a limited network, communication challenges and little information on public services give them problems. A coordinator who can assist and guide is something that many of them want

    Bakgrunnen for masteroppgaven er kliniske og forskningsbaserte erfaringer som tyder på at pårørende til brukere av rehabiliteringstjenester har en uklar rolle og blir lite involvert i rehabilitering. Gjennom juridiske og politiske føringer vektlegges betydningen av samarbeid med pårørende i helsetjenester, men det synes å være et gap mellom teori og praksis. Hensikten med studien er å finne ut mer om hvordan pårørende til eldre rehabiliteringsbrukere opplever og utøver rollen som pårørende gjennom samhandling med eget familiemedlem og rehabiliteringspersonell i en kommunal rehabiliteringskontekst. Studiens problemstilling er: ”Hvordan opplever og utøver pårørende til eldre brukere sin rolle i rehabiliteringsprosessen?” Teoretisk forankring: Masteroppgaven er inspirert av symbolsk interaksjonisme og Goffmans teori om hvordan roller blir formet gjennom hverdagslig samhandling. I tillegg belyses intervjupersonenes arbeid med å gjenopprette kontinuitet i hverdagspraksis ved hjelp av teorier om mestring (Bury og Antonovsky). Legal and political guidance emphasize the importance of collaboration with relatives in health care, but there seem to be a gap between theory and practice. The purpose of this study is to explore how relatives to elderly people who have undergone community-rehabilitation manage the role as informal carers, through interaction with their family members and health care workers within a rehabilitation context in the community. Problem: How do informal carers of elderly people experience and manage their role in the rehabilitation process? Literature review: The master thesis is inspired by symbolic interaction and Erving Goffmans theory of how roles are formed through everyday interactions. How the informal carers try to restore their everyday practice is discussed by theories of coping (Bury and Antonovsky). Method: The study has an interpreting design including collecting of qualitative data using half structured interviews. Results: The study shows how six women in various ways get their established everyday life changed, and get new role responsibilities after their family member becomes ill or injured. They manage their new role in various ways in different situations, depending of the constraints and opportunities they have. Informal carer –professional carer interaction reveals the ways they negotiate a new social order, and that there is unequal power in the relationship between informal carers and healthcare workers. It requires competence and development of coping strategies to succeed in establishing a partnership and setting limits in a way that their family member and the professional carers agree and find acceptable. Healthcare workers ought to be aware of the context in which informal carers design and manage their roles, and show respect in the meeting with them and their coping strategies
