    Obiettivo: analisi dei dati epidemiologici, degli aspetti clinici e dell'appropriatezza della gestione di una popolazione pediatrica ricoverata in 2 centri Italiani per Disturbo da Sintomi Somatici (DSS), diagnosi posta secondo i criteri del DMS-5 Risultati: i DSS rappresentano il 0,9 % dei ricoveri pediatrici; l'età media è di 11.7 , con una prevalenza del sesso femminile. L'esordio della sintomatologia precede di molti anni il ricovero, con sintomi multipli e un alto assenteismo scolastico. La maggior parte dei pazienti ha una storia di numerosi pregressi interventi terapeutici non risolutivi ed di indagini strumentali alla ricerca di una patologia organica. Un trattamento Neuropsichiatrico specifico è stato iniziato nel 51% dei casi ; la diagnosi è avvenuta in 1/3 dei casi. Conclusioni: la diagnosi di DSS deve essere posta precocemente per evitare indagini strumentali e terapia inutili ed iniziare prontamente un percorso psichiatric

    In patients with inflammatory bowel disease (IBD) there is an increased incidence of thromboembolic events. We report a case of a female, age 14, with a 6 months history of diarrhea with occasional presence of red blood and with a very distended abdomen with evident ascites. The diagnosis was Budd-Chiari syndrome (BCS) in ulcerative colitis (UC). Therapy with subcutaneous low molecular weight heparin, methylprednisolone, mesalazine was started. Clinical and radiological features quickly improved. No thrombophilia abnormality nor other risk factor for thrombosis were detected. BCS is a rare condition caused by obstruction of the supra-hepatic veins and causes liver congestion, portal hypertension, ascites, esophageal varices, and in some cases, acute onset of severe liver failure. Only six pediatric cases of BCS with concurrent UC are reported. Of these, four had no thromboembolic risk factor. IBD should always be carefully evaluated as a possible underlying trigger of an acute thrombotic event

    Going Beyond Counting First Authors in Author Co-citation Analysis

    The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed. Reasons for these effects are discussed

    A very smart little girl looking for her pain

    Osteoid osteoma is a condition that causes pain and limping in the lower extremity. Therefore, a thorough investigation is necessary in preschool children with prolonged pain and limping. The paper presents the case of a 6-year-old girl with severe nocturnal pain in her tibia. X-ray and MRI did not show any abnormalities, but a technetium bone scan revealed an osteoid osteoma in her femoral neck. CT scan confirmed the diagnosis. The patient underwent ablative treatment with focused ultrasound guided by MRI. When osteoid osteoma is suspected, standard radiology should be the initial diagnostic imaging. However, it is important to extend the imaging to the entire femur if the pain radiates from the femur to the leg. If the standard x-ray is inconclusive, a CT scan or technetium bone scan should be considered

    : Kawasaki disease is a systemic vasculitis that presents with stable vital signs. Although it is well known that Kawasaki disease can cause myocarditis, tachycardia and heart failure during the acute stage, Kawasaki disease shock syndrome (KDSS) has recently been described. It is characterized by hypotension, signs and symptoms of poor perfusion and a shock-like state. We report the case of a 7-year-old boy with KDSS who presented with persistent fever, conjunctival injection, laterocervical lymphadenitis, and echocardiography showing signs of myocardial dysfunction and shock. The patient's hemodynamic status markedly improved with immunoglobulin therapy. Early recognition of KDSS can be challenging; however, delay in diagnosis may increase the risk of coronary artery abnormalities and death
