Management of a Solid Pseudopapillary Tumor of the Pancreas in a Country with Limited Resources: About a Case
INTRODUCTION: Pseudo-papillary and solid tumors of the pancreas are rare tumours, of uncertain etiopathogenes is. We report the difficulty of managing a pseudopapillary tumor in our country, where resources were limited. Our objective was to discuss the therapeutic management in relation to the literature.
OBSERVATION: This is a 19 year old young women. View in consultation for chronic abdominal pain, without family neoplastic history. Abdominal palpation revealed an intra-abdominal mass syndrom. Abdominal computed tomography had shown a heterogeneous solidocystic tumor mass in the corporeo-caudal region, well encapsulated, well limited, without infiltration, without peripheral lymph nodes with homogeneous contrast uptake, without vascular invasion or remote secondary location. This allowed us to decide on pancreatic enucleation by laparotomy. The diagnosis was confirmed by histological study of the surgical specimen. The evolution was favorable without remote metastasis or recurrence after two years of follow-up
Severity Factor in the First Forty-Eight Hours of Thoracic Trauma in University Hospital Center Joseph Ravoahangy Andrianavalona Antananarivo
Introduction: Severe thoracic trauma is the second leading cause of death in multiple traumas after head trauma. The objective of this study is to research the serious factors of thoracic trauma at the Joseph Ravoahangy Andrianavalona Antananarivo university hospital center.
Patients and methods: This is a retrospective cohort study on patients with closed and open chest trauma hospitalized in the thoracic surgery department, from January 1, 2015, to December 31, 2022.
Results: We had collected 331 patients with a male predominance (82.2%), with the median age of 29 years, many of our patients were treated by chest drainage (62.8%), and 17.2% having received a thoracotomy. The mortality rate is 36.2%. Several factors were found: age ? 55 years (RR=2.7 [2.2-3.4]), chronic respiratory disease including : obstructive pulmonary disease, asthma (RR=2.8 [2.3-3 ,4]), the comatose state (RR=2.1[1.6 -2.8], hemodynamic instability and respiratory distress syndrome; patient victim of penetrating trauma and associate neurological lesion ; l severe anemia ? 70g/l (RR=2.4 [1.7-3.1]); chest flap, compressive pneumothorax (RR=5.5 [2.5-6.7]) and large hemothorax ? 1500 ml (RR=3.6 [2.4-5.4]; delay in management beyond six hours (RR=2.1 [1.6-2.8]), massive blood transfusion; Chest tube flow ? 250 ml/h.
Conclusion: Serious thoracic trauma leads to a high mortality rate, several factors have been implicated, clinical, biological, scannographic and therapeutic factors
Primary Pulmonary Resection in Madagascar: Indication and Results
Introduction:� Primary pulmonary resection is defined as the surgical ablating or endoscopic of an entire lobe called lobectomy or an entire lung called pneumonectomy. Our objective is to describe the indication for a primary pulmonary resection and to research the morbidity and mortality factors of primary pulmonary resection at the Joseph Ravoahangy Andrianavalona Antananarivo University Hospital.�
Method: This is a retrospective descriptive and analytical study of 216 patients hospitalized in the thoracic surgery department at the Joseph Ravoahangy Andrianavalona Antananarivo University Hospital (CHU-JRA), from January 1, 2015, to December 31, 2023, who underwent primary pulmonary resection.�
Results:�We collected 216 patients with a male predominance (73,6%) and a median age of 33. Post-tuberculosis pulmonary destruction is the main indication for resection in 53,24% cases, followed by a cavernous lesion of pulmonary aspergillosis in 24,07% cases then tumor mass in 18,06% and the nodular lesion in 4,63% cases. We realized a lobectomy in 71,29% of cases and a pneumonectomy in 28,71%. The complications found are dominated by pneumothorax, prolonged bubbling, pleural empyema, bronchopleural fistula, bleeding, recurrent paralysis, septic shock and cardiac rhythm disorder. The mortality rate is 8,33% cases. severe factors were found notably: undernutrition with IMC ? 18 kg/m2 (RR= 5[2,1-11,7]), hemorrhagic shock (RR=9,3[3,7-13,3]), septic shock (RR=13,3 [4,6-28,4]), cardiogenic shock (RR=8,5[3,7-12,3]), pleural empyema� (RR=8,5[3,8 -13,6], bronchopleural fistula (RR=6,7[2,9-15,3]).
Going Beyond Counting First Authors in Author Co-citation Analysis
The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation
counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings
are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that
only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into
account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed. Reasons for these effects are discussed
Variations on the Author
“Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship
Appropriate Similarity Measures for Author Cocitation Analysis
We provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature. Dispelling the Myths Behind First-author Citation Counts
We conducted a full-scale evaluative citation analysis study of scholars in the XML research field to explore just how different from each other author rankings resulting from different citation counting methods actually are, and to demonstrate the capability of emerging data and tools on the Web in supporting more realistic citation counting methods. Our results contest some common arguments for the continued
use of first-author citation counts in the evaluation of scholars, such as high correlations between author rankings by first-author citation counts and other citation
counting methods, and high costs of using more realistic citation counting methods that are not well-supported by the ISI databases. We have done our best to complete the author checklist relating to the use of animals in the hut study. Note that the objective for the hut study was to evaluate the IRS treatment applications for residual efficacy against Anopheles mosquitoes, including the local An. coluzzii mosquito population. Cows were only used to attract mosquitoes into the huts and no tests were carried out directly on the cows. The author checklist is intended for use with studies where experiments are carried out on animals, which is why we have had such difficulty in completing this for the hut study, as many of the questions do not relate to how the cows were used
