    Mapping the osseointegration using immuno-driven biological processes: evidences from a systematic review and a randomized clinical trial with focus on occlusal loading of implant-retained mandibular overdentures wearers

    The dental implant osseointegration occurs due to the process triggered by the host immune response, however the biomarkers that modulate this process have not yet been determined. In order to investigate and understand ossointegration through the biomarkers, two studies were designed: I) Systematic Review (SR) focusing on the evaluation of peri-implant crevicular fluid collection (FCPI) during bone healing after dental implants insertion; II) Longitudinal Randomized Clinical Trial according to the occlusal loading, immediate (IML) or conventional (CL), of implant retained mandibular overdentures. The study II evaluated the clinical and biological behavior of narrow diameter implants (NDR) osseointegration submitted to IML or CC, inserted in 20 totally edentulous patients, with a high edentulism time and limited mandibular bone availability. The SR selected 30 clinical studies and identify 52 biomarkers reported during osseointegration. The most studied biomarkers were interleukin (IL) -1β, tumor necrosis factor alpha (TNF-α) and nitric oxide (NO). PICF collections were performed immediately after implant insertion up to 16 weeks, prior to occlusal loading. Through the data collected, it was not possible to identify the mechanism by which inflammatory and bone biomarkers are released during osseointegration. However, it was possible to associate cellular and molecular events triggered with osseointegration with the results of available clinical studies. The data summarized can guide researchres to design future clinical studies and can help selecting target biomarkers already quantified. The study "II” showed that IML group presented implant stability 8.95% lower than the CL, until week 12 (p <0.05). The calculus presence in IML group was 50% higher than the CL at week 1 (p = 0.006), and 30% lower at week 8 (p = 0.017). The IML probing in depth was about 21.49% lower than in the CL group for all evaluated periods (p = 0.05). The bleeding on probing was 28.9% higher for IML at week 12 (p = 0.044). IML group presented the highest TNF-α concentration by week 4 (p <0.05) and 57.78% more IL-1β after week 4 to 12 than the CL group. The IL-6 concentration was 53.94% lower for IML group until week 8. The IL-10 concentration had a significant progressive and similar increase for both groups until week 8, and at week 12 the IML group had 45.74% higher concentration than the CL group (p = 0.003). The implant survival rate was 90% for IML and CL. The implants immediately loaded showed more stable clinical results, however presented unstable biological resposnes during bone healing. Therefore, the rehabilitation of the population with low bone availability in the anterior region of the mandible using implant retained mandibular overdentures seems to be safer when CL is adopted based on the inflammatory biomarker release.Coordenação de Aperfeiçoamento de Pessoal de Nível Superior - CAPESA osseointegração de implantes dentários ocorre devido ao processo desencadeado pela resposta imune do hospedeiro, entretanto os biomarcadores que modulam esse processo ainda não foram determinados. Com o intuito de investigar e compreender a ossointegração por meio dos biomarcadores foram delineados dois estudos: I) Revisão sistemática (RS) com foco na avaliação de coleta de fluido crevicular peri-implantar (FCPI) durante a cicatrização de óssea após a inserção de implantes dentários; II) Estudo clínico longitudinal randomizado de acordo com o carregamento oclusal, carga imediata (CI) ou convencional (CC), de overdentures mandibulares. O estudo II avaliou o comportamento clínico e biológico da osseointegração de implantes de diâmetro reduzido (IDR) submetidos a CI ou CC, inseridos em 20 pacientes desdentados totais, com elevado tempo de edentulismo e limitada disponibilidade óssea mandibular. Na RS foram selecionados 30 estudos clínicos e identificados 52 biomarcadores durante o período de osseointegração. Os biomarcadores mais estudados foram interleucina (IL) -1β, fator de necrose tumoral alfa (TNF-α) e óxido nítrico (NO). As coletas de FCPI foram realizadas imediatamente após inserção do implante até 16 semanas antes do carregamento oclusal. Através dos dados coletados nesta RS, não foi possível identificar qual o mecanismo pelos quais os biomarcadores inflamatórios e ósseos são liberados durante a osseointegração. No entanto, eventos já conhecidos da osseointegração foram associados com os resultados dos estudos clínicos disponíveis, sendo este, um guia aos futuros pesquisadores, devido o mapeamento de todos os biomarcadores já avaliados durante esse processo. No estudo “II”, o grupo CI apresentou estabilidade 8,95% menor que o CC, até a semana 12 (p<0,05). O cálculo no CI foi 50% maior que no CC na semana 1 (p=0.006), e 30% menor na semana 8 (p=0.017). A profundidade a sondagem do CI foi em média 21,49% inferior ao grupo CC em todos os períodos avaliados (p=0.05). O sangramento a sondagem foi 28,9% maior para o CI na semana 12 (p=0,044). Implantes que receberam CI apresentaram a concentração de TNF-α 40,75% maior até a semana 4 (p<0.05) e 57,78% mais IL-1β, após a semana 4 até a 12 que o grupo CC. A concentração de IL-6 foi 53,94% menor para o CI, até a semana 8. A concentração de IL-10 teve aumento progressivo significativo e similar para ambos os grupos até a semana 8, na semana 12 o grupo CI teve 45,74% maior concentração do que o grupo CC (p=0.003). A taxa de sobrevivência foi de 90% para ambos os grupos. Os implantes que receberam CI apresentaram resultados clínicos mais estáveis, mas resultados biológicos mais instáveis durante a cicatrização óssea. Diante disso, a reabilitação da população com baixa disponibilidade óssea em região anterior de mandíbula com overdentures mandibulares é mais segura quando feita com CC, pois a resposta inflamatória foi mais controlada neste grupo

    Factors associated to functional and clinical performance of users of mandibular overdentures during 3 years of function.

    Through the success of osseointegrated implants and concomitant to the need of circumvent patient complaints, such as lack of retention and stability, especially of the mandibular conventional complete denture (CCD), the option for mandibular overdentures (MO) has been widely disseminated as a minimum protocol to rehabilitate total edentulous individuals, due to its excellent costbenefit ratio. In addition, with the option of using narrow diameter implants (NDI), rehabilitation difficulties observed in elderly patients, with moderate and severe levels of mandibular atrophy, has been overcome making treatment with MO predictable and with high success rates. NDI has shown a success and survival rate of around 98%, and maintaining the stability of peri implant bone tissue, showing excellent predictability and success over the years. Still, implant-retained MO promote improvements in the levels of satisfaction with their treatment, quality of life, besides restoring the masticatory function efficiently. Considering all clinical and functional variables related to rehabilitation, which can interfere in maintenance, longevity and quality of life, annual monitoring of MO users is extremely important. This regular monitoring allows the understanding of the influence of individuals' intrinsic factors such as mandibular atrophy, facial pattern, anteroposterior discrepancy and bite force, in addition to occlusal loading, in rehabilitation with MO over time. In view of the above, a cohort of total edentulous patients was assessed annually over 3 years of MO use, in order to investigate possible changes in masticatory function, oral health-related quality of life and satisfaction with their oral condition and monitor variables and bone, which are predictive of the success of implant-supported prosthetic rehabilitation. As a result, this thesis presents, through 6 articles, a compilation of information that determines the predictability and positive impact of MO use retained by narrow diameter implants in the lives 13 of individuals. After 3 years, high success and survival rates and minimal biological and prosthetic complications have been reported, regardless of occlusal loading. In addition, despite reaching the balance of occlusal strength occurring in the first months of function, variations in chewing performance, oral health-related quality of life and clinical and bone variables are still observed during the 3-year follow-up. In addition, intrinsic variables such as facial morphology and gender of individuals continue to influence the clinical and functional behavior of OM.Sem bolsaFrente ao sucesso dos implantes osseointegráveis e concomitante a necessidade de contornar queixas do paciente, como falta de retenção e estabilidade, principalmente da prótese total convencional (PTC) mandibular, a opção por overdentures mandibulares (OM) tem sido amplamente difundida como protocolo mínimo para reabilitar indivíduos desdentados totais,devido sua excelente relação custo-benefício. Em adição, com a opção de uso de implantes de diâmetro reduzido (IDR), dificuldades reabilitadoras observadas em pacientes idosos, com níveis de atrofia mandibular moderada e severa, tem sido superadas tornando o tratamento com OM previsível e com altos índices de sucesso. IDR tem apresentando índice de sucesso e sobrevivência em torno de 98%, e mostram- se capazes de manter a estabilidade do tecido ósseo peri implantar,o que evidencia excelente previsibilidade e sucesso ao longo dos anos. Ainda, OM implanto-retidas promovem aos indivíduos melhorias nos níveis de satisfação com o seu tratamento, qualidade de vida, além de reestabelecer a função mastigatória de forma mais eficiente em relação a PTC. Considerando todas as variáveis clínicas e funcionais relacionadas à reabilitação,que podem interferir na manutenção, longevidade e qualidade de vida, torna- se de extrema importância o acompanhamento anual de usuários de OM. Esse acompanhamento regular permite a compreensão da influência de fatores intrínsecos dos indivíduos como atrofia mandibular, padrão facial, classificação ântero-posterior e força de mordida,além do carregamento oclusal,na reabilitação com OM ao longo do tempo. Diante do exposto, uma coorte de pacientes desdentados totais foi avaliada anualmente ao longo de 3 anos de uso de OM, com a finalidade de investigar possíveis mudanças na 11 função mastigatória, qualidade de vida relacionada à saúde bucal e satisfação com sua condição oral e monitorar variáveis clínicas e ósseas, que são preditivas do sucesso da reabilitação protética implantos suportada. Como resultado, esta tese apresenta através de 6 artigos um compilado de informações que determinam a previsibilidade e o impacto positivo do uso OM retidas por implantes de diâmetro reduzido na vida dos indivíduos. Após 3 anos, altas taxas de sucesso e sobrevivência e mínimas complicações biológicas e protéticas foram reportadas, independente do carregamento oclusal. Em adição, apesar do alcance do equilíbrio da força oclusal ocorrer já nos primeiros meses de função, variações no desempenho mastigatório, qualidade de vida relacionada à saúde bucal e das variáveis clínicas e ósseas ainda são observadas durante os 3 anos de acompanhamento.     Functional performance of wearers of conventional full dentures and of mandibular overdentures retained by 2 and 3 implants: a parallel observational study of 3 groups.

    Conventional full dentures wearers show a reduction of masticatory capacity of around 55% compared to mandibular overdentures wearers; however, it is still uncertain whether the increase in the number of implants for retention of mandibular overdentures can positively influence functional parameters that interfere in obtaining an efficient masticatory function. Studies indicate that retained mandibular overdentures by three implants promote superior functional performance when compared to retention by 2 implants. This parallel cross-sectional observational study compared differences in bite strength, swallowing threshold and masticatory performance between 3 treatment strategies for mandibular edentulism: conventional complete denture, mandibular overdentures retained by 2 and 3 implants. A total of 36 edentulous patients were allocated to 3 study groups to receive the following rehabilitation treatments: I) conventional full prosthesis; II) mandibular overdenture retained by 2 implants and III) mandibular overdenture retained by 3 implants. The implants (Facility, 2.9x10mm) were installed using single-stage surgery and conventional loading and the prosthetic retention system used was of the button type (Attachment Equator). After 3 months of using the prosthesis, the functional evaluation was performed by i) recording the bilateral bite force using the gnatodynamometer device, ii) and chewing function tests: masticatory performance and swallowing threshold using artificial test food (Optocal) , processed quantitatively by the fractionation method in sieves. The Mann-Whitney test was used to compare central trends for independent samples. Values of p ≤0.05 were considered statistically significant. Compared to of conventional complete denture wearers, mandibular overdentures retained by 2 implants wearers showed increased bilateral bite strength, dominance on the right side and significantly better masticatory performance. Women obtained improved chewing performance and improved bilateral bite strength. Mandibular overdentures retained by 3 implants wearers showed decreased swallowing threshold time and improved masticatory performance. Compared to the group of overdentures retained by 2 implants, the group of overdentures retained by 3 implants did not show significant differences in all parameters. Compared to conventional complete denture wearers, overdentures retained by 2 implants wearers show better chewing performance and bite strength, while the group of overdentures retained by 3 implants showed a reduction in chewing time only. O teste de MannWhitney foi usado para comparar tendências centrais de amostras independentes. Valores de p ≤0,05 foram considerados estatisticamente significativos. Comparativamente aos usuários de prótese total convencional, os usuários de overdentures mandibulares retidas por 2 implantes apresentaram aumento da força de mordida bilateral, dominância do lado direito e performance mastigatória significativamente melhor. Mulheres obtiveram melhoria da performance mastigatória e melhoria da força de mordida bilateral. Usuários de overdentures mandibulares retidas por 3 implantes apresentaram tempo de limiar de deglutição diminuído e melhoria da performance mastigatória. Comparativamente ao grupo de overdentures retidas por 2 implantes, o grupo de overdentures retidas por 3 implantes não apresentou diferenças significativas em todos os parâmetros.     Going Beyond Counting First Authors in Author Co-citation Analysis

    The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed.     “Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films.     The assessment of Oral health-related quality of life (OHRQoL) is an important patientcentered outcome to measure and predict the treatment with implant-retained mandibular overdentures (MO). However, the OHRQoL aspects of fully edentulous patients with mandibular bone atrophy rehabilitated with MO are still not well understood, as there are currently no paired or randomized clinical trials available. Thus, the objective of this thesis was to investigate the influence of MO rehabilitation in the patients OHRQoL. As well as verifying the possible influences of the independent variables: age, time since edentulism, different types of occlusal loading (conventional loading - CL and immediate - IML); and prosthetic intercurrences resulting from rehabilitation with MO retained by 2 narrow diameter implants (NDI) in the OHRQoL. or this, four studies were planned: i) A prospective short-term clinical study that evaluated the impact of treatment with MO retained by 2 NDI on OHRQoL using three questionnaires (OHIPEDENT, DIDL, and GOHAI) completed by 25 fully edentulous patients before and after 3 months of MO loading; ii) A 1-year prospective paired longitudinal clinical study that investigated the influence of age (≤ 65 and > 65 years) and time since edentulism (< 25 years and ≥ 25 years) of 33 patients on masticatory function (MF) and OHRQoL in edentulous individuals, before and during the first year of MO usage. The MF was evaluated through the masticatory performance (MP) and swallowing threshold (ST) tests before and after 1, 3, 6, and 12 months of MO loading. The OHRQoL was evaluated through the OHIP-EDENT and GOHAI questionnaires, before and after 3, 6, and 12 months. iii) A 1-year randomized clinical trial that randomized the occlusal load protocol of IML (10 patients) or CL (10 patients) and investigated the influence of the occlusal loading type on NDI as MO retainers through clinical, biological, functional, and OHRQoL, along with the incidence of the prosthetic intercurrences in each group. The outcomes were: peri-implant health indices (plaque index - PI, calculus presence - CP, gingival index - GI, probing depth - PD and bleeding on probing - BOP), primary and secondary stability of implants (ISQ), survival and success rates, marginal bone loss (MBL), marginal bone change (MBC), concentration of IL-1β and TNF-α cytokines in the peri-implant crevicular fluid, MF was evaluated using the MP test and OHRQoL was analyzed via the OHIP-EDENT questionnaire and via description of intercurrences during treatment. iv) A 2-year longitudinal clinical study that evaluated the performance of the10 Equator abutment as MO retainer in 24 patients from the prosthetic perspective during 2 years of prosthesis loading and investigated whether maintenance events and prosthetic complications can affect OHRQoL. OHRQoL was assessed through the OHIP-EDENT and GOHAI questionnaires. The results of study I showed that the OHRQoL of conventional denture (CD) wearers was significantly improved after 3 months of treatment with MO, especially in the domains of questionnaires that include functional and pain-related aspects. The second clinical study showed that the age and time since edentulism did not influence the MF of MO wearers after 1 year. However, MO significantly improved the MF of all patients, and this improvement was more evident in younger patients (≤ 65 years) with a shorter time of mandibular edentulism (< 25 years). The OHRQoL was influenced only by age and time since edentulism in CD wearers, and treatment with MO eliminated these differences. The randomized clinical trial (study III) showed that IML generates a better adaptation of the peri-implant tissues around the prosthetic components, as indicated by a lower probing depth and fewer prosthetic intercurrences. Significant differences in TNF-α and IL-1β levels between the CL and IML groups at 6 months of healing likely relate different time under function in both protocols. The improvement in MF and OHRQoL was observed regardless of the type of occlusal loading used. However, the IML group perceived OHRQoL improvements faster, yet this group needed more time to attain comparable MF improvements. The results of the fourth study show that certain complications and prosthetic maintenance events, such as fracturing of the prosthesis, Equator dislodgment, prosthesis rebase, and confection of new overdentures, affect patients’ OHRQoL. The physical pain and discomfort domains were most affected and the effects were largest during the first year of MO usage. In addition, all domains of the GOHAI and OHIP-EDENT questionnaires showed a significant improvement when comparing the baseline and the final evaluations after 1 and 2 years of MO loading, except for the social disability and psychological disability domains of OHIP-EDENT after 1 year. In conclusion, the four studies demonstrate that OHRQoL is significantly improved as early as 3 months after treatment with MO retained by 2 NDI. Considerable improvement can thus be achieved even in a population with low bone availability in the anterior region of the mandible, independent of patient’s age, time since edentulism, and occlusal loading protocol. However, prosthetic complications and maintenance events can affect the patients’ OHRQoL up to 2 years after loading.Coordenação de Aperfeiçoamento de Pessoal de Nível Superior - CAPESA avaliação da qualidade de vida relacionada a saúde bucal (QVRSB) é um importante desfecho centrado no paciente que pode ser utilizado como indicador de sucesso do tratamento com overdentures mandibulares implanto-retidas (OM). No entanto, estudos pareados e randomizados que investiguem a relação entre a QVRSB e melhorias funcionais advindas de diferentes interevenções em pacientes desdentados totais com atrofia óssea mandibular e que passam pela reabilitação com OM ainda são escassos. Assim, o objetivo desta tese foi investigar a influência da reabilitação com OM na QVRSB. Bem como verificar as possíveis influências das variáveis independentes: idade, tempo de edentulismo dos pacientes, diferentes tipos de carregamento oclusal (carregamento convencional – CC e imediato - CI); e intercorrências protéticas resultantes da reabilitação com OM retidas por 2 implantes de diâmetro reduzido (IDR) não esplintados na QVRSB. Para isto, foram delineados quatro estudos: i) Estudo clínico prospectivo de curta duração que avaliou o impacto do tratamento com OM retidas por 2 IDR na QVRSB através de três questionários (OHIPEDENT, DIDL e GOHAI) em 25 pacientes antes e após 3 meses do carregamento oclusal das próteses; ii) Estudo clínico pareado prospectivo longitudinal com 12 meses de acompanhamento que investigou a influência da idade (divisão em dois grupos: ≤65 e >65 anos) e do tempo de edentulismo (divisão em dois grupos <25 anos e ≥25 anos) de 33 pacientes na função mastigatória (FM) e na QVRSB em indivíduos edêntulos, antes e durante o primeiro ano de reabilitação com OM. A FM foi avaliada através dos testes de performance mastigatória (PM) e limiar de deglutição (LD) antes e após 1, 3, 6 e 12 meses do carregamento das OM. A QVRSB foi avaliada através dos questionários OHIP-EDENT e GOHAI, antes e após 3, 6 e 12 meses. iii) Ensaio clínico randomizado de acordo com o carregamento oclusal, CI (10 pacientes) ou CC (10 pacientes), com 1 ano de acompanhamento em que se investigou a influência do tipo de carregamento oclusal em IDR como retentores de OM através do comportamento clínico, biológico, funcional, e percepção subjetiva do paciente, além da observação da incidência das intercorrências protéticas de cada grupo. As variáveis de desfechos foram: saúde peri-implantar (índice de placa visível – IPV, cálculo - C, grau de inflamação - GI, índice de profundidade de sondagem – IPS e índice de sangramento gengival – ISG), estabilidade primária e secundária dos implantes (ISQ), sobrevivência, sucesso, perda óssea marginal (POM), remodelação óssea (RO), concentração de citocinas IL-1β e TNF-α presentes no fluído crevicular periimplantar, avaliação da FM através do teste de PM e da QVRSB pelo questionário OHIP-EDENT e descrição de complicações/manutenções durante o tratamento; iv) Estudo clinico longitudinal que avaliou o desempenho do componente protético Equator como retentor de OM em 24 pacientes através da perspectiva protética em 2 anos de carregamento das próteses; e investigou se os eventos de manutenções e complicações protéticas afetariam a QVRSB. A QVRSB foi acessada através dos questionários OHIP-EDENT e GOHAI. Os resultados do estudo I mostraram que a QVRSB de usuários de próteses totais convencionais (PT) foi significativamente melhorada já após 3 meses de tratamento com OM, especialmente em relação aos domínios dos questionários que englobam aspectos funcionais e relacionados à dor com o uso das próteses. No estudo clínico II observamos que a idade e o tempo de edentulismo não influenciaram a FM de pacientes usuários de OM após 1 ano da instalação das próteses. No entanto, a OM melhorou significativamente a FM de todos os pacientes, sendo esta melhora mais evidente em pacientes mais jovens (≤65 anos) e com menor tempo de edentulismo mandibular (<25 anos). Já a QVRSB foi influenciada pela idade e tempo de edentulismo em somente enquanto os pacientes eram usuários de PT, sendo que o tratamento com OM eliminou essas diferenças. De acordo com os resultados do artigo III, foi evidenciado que o CI promoveu melhor adaptação dos tecidos periimplantes ao redor dos componentes protéticos em decorrência da observação de menor profundidade de sondagem e menor número de intercorrências protéticas. Diferença significativa entre CC e CI foi observada para o desfecho IPS aos 12 meses, e para a concentração de TNF-α e IL-1β em 6 meses de cicatrização. Ainda, a melhoria na FM e na QVRSB foi observada independentemente do tipo de carregamento utilizado. Os resultados do artigo IV revelaram que complicações relacionadas à manutenção protética, como fratura da prótese, soltura do componente Equador, necessidade de reembasamento de prótese e confecção de nova overdenture, afetaram a QVRSB dos usuários de OM, principalmente nos domínios dor física e desconforto, com maior significância no primeiro ano de uso das OM. Além disso, todos os domínios dos questionários GOHAI e OHIP-EDENT apresentaram diferença significativa entre as avaliações iniciais e após 1 e 2 anos do carregamento das próteses, exceto para o domínio de incapacidade social e desconforto psicológico do OHIP-EDENT após 1 ano. Através dos resultados dos 4 estudos, pode-se concluir que a QVRSB é significativamente melhorada já em 3 meses após a instalação de OM retidas por 2 IDR em uma população com baixa disponibilidade óssea na região anterior da mandíbula, sendo independentemente da idade, tempo de edentulismo dos pacientes e do tipo de carregamento oclusal utilizado. No entanto, os eventos de complicações protéticas advindas deste tratamento podem ainda continuar interferindo na QVRSB de usuários de OM até 2 anos após o carregamento das próteses

    Appropriate Similarity Measures for Author Cocitation Analysis

    We provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature.
