    Race and Ethnicity-Adjusted Age Recommendation for Initiating Breast Cancer Screening.

    IMPORTANCE Breast cancer (BC) is the second leading cause of cancer death in women, and there is a substantial disparity in BC mortality by race, especially for early-onset BC in Black women. Many guidelines recommend starting BC screening from age 50 years; however, the current one-size-fits-all policy to start screening all women from a certain age may not be fair, equitable, or optimal. OBJECTIVE To provide race and ethnicity-adapted starting ages of BC screening based on data on current racial and ethnic disparities in BC mortality. DESIGN, SETTING, AND PARTICIPANTS This nationwide population-based cross-sectional study was conducted using data on BC mortality in female patients in the US who died of BC in 2011 to 2020. EXPOSURES Proxy-reported race and ethnicity information was used. The risk-adapted starting age of BC screening by race and ethnicity was measured based on 10-year cumulative risk of BC-specific death. Age-specific 10-year cumulative risk was calculated based on age group-specific mortality data without modeling or adjustment. MAIN OUTCOMES AND MEASURES Disease-specific mortality due to invasive BC in female patients. RESULTS There were BC-specific deaths among 415 277 female patients (1880 American Indian or Alaska Native [0.5%], 12 086 Asian or Pacific Islander [2.9%], 62 695 Black [15.1%], 28 747 Hispanic [6.9%], and 309 869 White [74.6%]; 115 214 patients died before age 60 years [27.7%]) of any age in the US in 2011 to 2020. BC mortality per 100 000 person-years for ages 40 to 49 years was 27 deaths in Black females, 15 deaths in White females, and 11 deaths in American Indian or Alaska Native, Hispanic, and Asian or Pacific Islander females. When BC screening was recommended to start at age 50 years for all females with a 10-year cumulative risk of BC death of 0.329%, Black females reached this risk threshold level 8 years earlier, at age 42 years, whereas White females reached it at age 51 years, American Indian or Alaska Native and Hispanic females at age 57 years, and Asian or Pacific Islander females 11 years later, at age 61 years. Race and ethnicity-adapted starting ages for Black females were 6 years earlier for mass screening at age 40 years and 7 years earlier for mass screening at age 45 years. CONCLUSIONS AND RELEVANCE This study provides evidence-based race-adapted starting ages for BC screening. These findings suggest that health policy makers may consider a risk-adapted approach to BC screening in which individuals who are at high risk are screened earlier to address mortality due to early-onset BC before the recommended age of mass screening

    Marketing mix has an impact on increasing product sales volume. The marketing mix provides strategies and efforts for marketing activities so as to obtain maximum results. Increasing sales volume through marketing activities cannot be separated from the provisions of Products, Prices, Places, Promotions so as to increase sales results. This study aims to determine the effect of the marketing mix on the sales volume of Riau's specialty food products Bolu Cermai Al-Mahdi. The method used is quantitative using a descriptive survey, which is a type of research that describes the variables studied, this study determines 50 respondents and 2 informants. Data collection techniques Observation, questionnaires, interviews, and documentation with descriptive data analysis through a quantitative approach. From the results of the study it is known that the marketing mix has not affected sales volume, this is because the product, price, place, promotional activities have not been carried out in accordance with market demand so that the sales results are not appropriate. . not in accordance with the revenue target. This study concludes that based on the results of statistical tests, it can be concluded that the marketing mix on the Sales Volume of Riau Special Food Products Bolu Cermai Al-Mahdi with fairly good results because the marketing mix affects sales volume so as to increase sales results Bolu Cermai Al-Mahdi.     ABSTRAK Judul penelitian ini dilatarbelakangi oleh struktur doktrinal Shi’ah Ithna ‘Asyariyah terhadap Imam Mahdi yang sangat fundamental. Dasar keyakinan makna Imam yang sangat dilandasi oleh kekuatan magic, kekuatan dimensi spiritual Sesungguhnya Masalah doktrin Imamah yang sangat fundamental bisa didefinisikan sebagai sesuatu konsep Imamah yang secara esoterik tidak rasional “esoteric nonrational Imamism”, tidak terkecuali konsep Imam Mahdi Shi’ah Ithna ‘Asyariyah. Tetapi apa yang sudah dilakukan oleh pelaku tradisi mitis pada hakikatnya adalah proses penghayatan holistik dan upaya memperjelas nilai–nilai abstrak. Karena sesungguhnya tradisi mitis merupakan tradisi yang secara eksoteris adalah abstrak, tidak gampang dinilai dengan satu mata tapi harus mendalami juga apa hakikat atau aspek esoteris dari tradisi mitis tersebut. Meskipun dalam pandangan orang lain, itu sangatlah tidak rasional dan tidak logis. Sesuatu yang nampak jelas di depan mata bisa menjadi dimensi kekuatan penuh dan stimulus kekuatan hidup. Seperti pernyataan Elilade bahwa mitos memiliki peranan penting karena melukiskan lintasan supranatural ke dalam dunia, atau berusaha membuat dunia yang natural menjadi supranatural. Unsur mitos itulah ekspektasi manusia religius terhadap supranatural. Selain itu terdapat unsur lain yang menambahkan pola mitos dari tanda–tanda (simbol) waktu dan sejarah yang sakral. ketiga pokok dasar mitos ini sangat mempengaruhi substansi supranatural dunia. Penelitian ini dilakukan dengan menggunakan dua macam teknik yang dipakai yakni pertama, teknik penggalian data dan kedua teknik analisa data. Untuk metode penggalian data, penulis menggunakan metode Library Research (riset kepustakaan) terhadap literatur–literatur primer maupun skunder. Literatur–literatur primer terdiri dari karya–karya penulis dari pihak Shi’ah sementara yang lain dari penulis Shi’ah tentang konsep Imam Mahdi Shi’ah Ithna ‘Asyariyah. Selain itu juga buku–buku yang ditulis oleh Mircea Eliade. Pendekatan analisa penelitian ini menggunakan antropologi positivisme. Dengannya berguna menghasilkan kemungkinan pemahaman obyektif, selain itu mendapatkan data–data atau perspektif yang tepat, kritis dan obyektif atas kesejarahan Shi’ah Ithna ‘Asyariyah, selain itu tanpa harus memberikan pertanyaan atau wawancara kepada informan. Tetapi hanya melihat fakta sosial dan tulisan tulisan pelaku dari fakta sosial tersebut. Metode ini penulis mencoba memberikan interpretasi atau penafsiran sesuai dengan kecendrungan teks yang diinginkan oleh penulis Shi’a. Hasil penelitian ini menyimpulkan bahwa mitos yang dikatakan oleh Mircea Eliade dimiliki juga oleh keyakinan sakralitas Shi’ah Ithna ‘Asyariyah terhdap Imam Mahdi. Kekuatan power sakralitas Imam Mahdi membentuk proses eksistensi dan ekspektasi kehidupan Shi’ah di dunia. Karena sesungguhnya itulah (mitos) hal utama yang dijadikan iman dan landasan hidup. Supranatural dari unsur natural menjadikannya dimensi kekuatan spiritual penuh. ABSTRACT The title of the study is motivated by the structure of the fundamental Imam Mahdi Shiite doctrinal. Basic beliefs meaning Priest was based on the power of magic, the power of the spiritual dimension. Indeed, problems of fundamental doctrine of Imamate can be defined as something that is esoteric concept of Imamate irrational "nonrational Imamism esoteric" does not mention the concept of Imam Mahdi Shi'ah Ithna 'Asyariyah. Indeed, what has been done by the perpetrators of the mythic tradition is essentially a holistic appreciation of the process and attempt to clarify the value of the abstract. Because mythical tradition is a tradition that is exoterically abstract, not easily assessed with one eye but should also explore what nature or esoteric aspects of the mythical tradition. Although in the eyes of others, it is irrational and illogical. Something clearly visible in front of the eyes can be a stimulus dimension in full force and life force. Elilade states that the myth has an important role because it depicts a supernatural path into the world, or trying to make the natural into the supernatural world. The research was carried out using two different techniques are used: first, data mining techniques and second data analysis techniques. For data mining method, the writer used the method Library Research of the primary and secondary literature. The primary literature consists of works of writer from the Shiites, while others from the author of the concept of Imam Mahdi Shi'ite Shia Ithna 'Asyariyah. There was also books written by Mircea Eliade about the sacred, profane, reality and myth. The works are the primary sources of this study. While the approach to analyze the data, writer use the method positivism of antropology. It is useful to understand the objective result, in addition to get data or perspective, critically and objectively the historical Shia Ithna 'Asyariyah. Without giving any question or interview the informant. But just look at the facts of social actors and the writing paper of the social facts. The writer tries to give interpretations in accordance with the trend of the desired text by the author Shi'a. Finally, the results of this study concluded that the myth which is said by Mircea Eliade also owned by the belief the sacred of Shia Ithna 'Asyariyah towards Imam Mahdi.     Heritage Strikes Back: The Al Mahdi Case, ICC’s Policy on Cultural Heritage and the Pushing of Law’s Boundaries

    Images of genocide, mass graves and torn families come to mind when one hears the term ‘war crime’. But does cultural heritage have similar legal rights? Is it protected by the Rome Statute? What lays in the future of cultural heritage protection against destruction? And where do the boundaries of law lie with regards to the rights of cultural objects? The purpose of this paper is to answer these questions by focusing on the 2016 International Criminal Court’s (ICC) judgement in the Al Mahdi case and the analysis of the 2021 ICC’s Policy on Cultural Heritage born in its wake, which will shape our perception of the cultural heritage protection in the years to come. In the first, introductory part of the paper the author ponders upon the concept of cultural heritage, trying to understand why it matters. In turn, the second part of the article focuses on the investigation of the many faces of interactions between cultural heritage and law. The third part of the paper is devoted to the analysis of the Al-Mahdi case heard before the ICC. The author explains how the case was brought before the ICC and the way in which the Court reached its now precedential decision, showing the various ways in which it pushed the boundaries of law and our understanding of what constitutes a war crime. In the fourth part of the paper the author turns his attention to the Policy on Cultural Heritage proposed by the ICC in June 2021 in close collaboration with UNESCO, looking into the new paths it puts forward for cultural heritage. The concluding part of the paper is focused on the question of what the ICC’s Policy means for the future of the prosecution of the crimes against cultural heritage, with the author asking whether it may be an effective tool and deterrent in fighting against the destruction of world’s heritage, and wondering how the rights of monuments may be further broadened in the coming years

    Risk of Gynecological Cancers in Cholecystectomized Women: A Large Nationwide Cohort Study.

    Background: Gallstones affect women more frequently than men, and symptomatic gallstones are increasingly treated with surgical removal of the gallbladder (cholecystectomy). Breast, endometrial, and ovarian cancer share several risk factors with gallstones, including overweight, obesity, and exposure to female sex hormones. We intended to assess the association between cholecystectomy and female cancer risk, which has not been comprehensively investigated. Methods: We investigated the risk of female cancers after cholecystectomy leveraging the Swedish Cancer, Population, Patient, and Death registries. Standardized incidence ratios (SIRs) adjusted for age, calendar period, socioeconomic status, and residential area were used to compare cancer risk in cholecystectomized and non-cholecystectomized women. Results: During a median follow-up of 11 years, 325,106 cholecystectomized women developed 10,431 primary breast, 2888 endometrial, 1577 ovarian, and 705 cervical cancers. The risk of ovarian cancer was increased by 35% (95% confidence interval (CI) 2% to 77%) in the first 6 months after cholecystectomy. The exclusion of cancers diagnosed in the first 6 months still resulted in an increased risk of endometrial (19%, 95%CI 14% to 23%) and breast (5%, 95%CI 3% to 7%) cancer, especially in women cholecystectomized after age 50 years. By contrast, cholecystectomized women showed decreased risks of cervical (-13%, 95%CI -20% to -7%) and ovarian (-6%, 95%CI -10% to -1%) cancer. Conclusions: The risk of ovarian cancer increased by 35% in a just short period of time (6 months) following the surgery. Therefore, it is worth ruling out ovarian cancer before cholecystectomy. Women undergoing cholecystectomy showed an increased risk of breast and endometrial cancer up to 30 years after surgery. Further evaluation of the association between gallstones or gallbladder removal on female cancer risk would allow for the assessment of the need to intensify cancer screening in cholecystectomized women
