A Unified Protocol for the Transdiagnostic Psychodynamic Treatment of Anxiety Disorders: An Evidence-Based Approach
Although there is evidence for the efficacy of psychodynamic therapy (PDT) in anxiety disorders, results are not yet satisfactory, for example, if rates of remission and response are considered. To address this problem, a unified psychodynamic protocol for anxiety disorders (UPP-ANXIETY) is proposed that integrates the treatment principles of those methods of PDT that have proven to be efficacious in anxiety disorders. In addition, this protocol is transdiagnostic, implying that is it is applicable to various forms of anxiety disorders and related disorders (generalized anxiety disorder, social phobia, panic disorders, avoidant personality disorder). Based on supportive-expressive therapy, the UPP-ANXIETY represents an integrated form of psychodynamic therapy that allows for a flexible use of empirically supported treatment principles. UPP-ANXIETY encompasses the following 9 treatment principles (modules): (1) socializing the patient for psychotherapy, (2) motivating and setting treatment goals, (3) establishing a secure helping alliance, (4) identifying the core conflict underlying anxiety, (5) focusing on the warded-off wish/affect, (6) modifying underlying internalized object relations, (7) changing underlying defenses and avoidance, (8) modifying underlying response of self, and (9) termination and relapse prevention. Some principles are regarded as core components to be used in every treatment (principles 3-8). A unified protocol for the psychodynamic treatment of anxiety disorders has several advantages, that is (1) integrating the most effective treatment principles of empirically supported psychodynamic treatments for anxiety disorders can be expected to further improve the efficacy of PDT; (2) using a unified protocol in efficacy studies has the potential to enhance the evidence-based status of PDT by aggregating the evidence; (3) a unified protocol will facilitate both training in PDT and transfer of research to clinical practice; and (4) thus, a unified protocol can be expected to have a significant impact on the health care system. La coordination rythmique entre locuteurs lors d’une tâche de lecture alternée: Tâche verbale d’une étude sur l’entrainement rythmique
Le présent mémoire propose une étude pilote concernant les capacités d’alignement conversationnel de participants âgés entre 9 et 12 ans. La tâche présentée dans ce mémoire fait partie d’une étude plus large tentant d’évaluer l’effet de l’entrainement rythmique sur le langage des enfants présentant un trouble langagier, plus précisément un bégaiement. Une tâche verbale a donc été créée afin de vérifier les effets de cet entrainement sur un groupe contrôle. L’étude tente de voir les effets qu’une conversation pourrait engendrer sur le rythme ou le débit de l’enfant. Vingt participants ont été recrutés de manière volontaire afin de participer à une étude dirigée par Simone Falk au laboratoire BRAMS de l’Université de Montréal. Les participants devaient accomplir quatre lectures différentes d’un texte créé spécialement pour l’étude, impliquant des tours de parole à intervalles fixes. Les données ressorties de l’étude avaient pour but de vérifier les différences dans le débit de parole, le nombre d’erreurs de prononciation ou disfluences et le débit articulatoire entre les différentes conditions de lecture. Les deux contextes de lecture consistaient à une lecture seule et une autre, lue en alternance avec un interlocuteur adulte. L’étude tente d’évaluer la tâche afin de vérifier si des résultats préliminaires font surface afin d’explorer davantage les effets de l’alignement conversationnel chez les enfants en fin de développement langagier. L’étude a fait ressortir plusieurs résultats. Aucun résultat significatif au niveau du débit de la parole et au niveau du nombre d’erreurs de prononciation ou disfluences n’est ressorti. Quant au débit d’articulation, il a été constaté que les participants semblaient modifier leur débit d’articulation de manière significative en contexte d’alternance avec un adulte. L’étude abordera donc l’élaboration de la tâche et l’analyse des données accumulées lors de l’étude.The present thesis proposes a pilot study concerning the interactive alignment abilities of children aged between 9 and 12 years old. The task presented in this thesis is part of a larger study aiming to evaluate the effect of rhythmic training on the language of children with a language disorder, specifically stuttering. A verbal task was therefore created to assess the effects of this training on a control group. The study attempts to see the dynamics of a conversation between an adult and a child, and how this interaction might affect the child’s rhythm and flow. Twenty participants voluntarily took part of the study led by Simone Falk in BRAMS laboratory at the Université de Montréal. A reading task was specifically created for this research; participants were asked to take turns while reading the same text four times. To ensure conclusive result, both parties were assigned the same length of sentences to read. The two reading contexts consisted of a solo reading and another, read alternately with an adult interlocutor. The aim of the study was to investigate the differences in speech rate, the frequency of pronunciation errors or disfluencies and the articulation rate between the different reading conditions. This research aims to evaluate the task to verify whether the preliminary results are sufficient to further explore the effects of the alignment conversation in a child at the end of their language development. The study highlighted several results. No significant results were found regarding speech rate and the number of pronunciation errors or disfluencies. As for articulation rate, it was noted that participants seemed to significantly modify their articulation rate in the context of alternating with an adult. The study will therefore address the task development and the analysis of the data collected during the study

THE EFFICACY OF PSYCHODYNAMIC PSYCHOTHERAPY IN SPECIFIC MENTAL DISORDERS: A 2013 UPDATE OF EMPIRICAL EVIDENCE
This article reviews the empirical evidence for psychodynamic therapy for specific mental disorders in adults. The focus is on randomized controlled trials (RCTs). However, this does not imply that RCTs are uncritically accepted as the gold standard for demonstrating that a treatment works. According to the results presented here, there is evidence from RCTs that psychodynamic therapy is efficacious in common mental disorders, that is, depressive disorders, anxiety disorders, somatic symptom disorders, personality disorders, eating disorders, complicated grief, posttraumatic stress disorder (PTSD), and substance-related disorder. These results clearly contradict assertions repeatedly made by representatives of other psychotherapeutic approaches claiming psychodynamic psychotherapy is not empirically supported. However, further research is needed, both on outcome and processes of psychodynamic psychotherapy. There is a need, for example, for RCTs of psychodynamic psychotherapy of PTSD. Furthermore, research on long-term psychotherapy for specific mental disorders is required
Sound field control with hemi-cylindrical loudspeaker arrays
An acoustical model for the sound field generated by hemi-cylindrical loudspeaker arrays is presented and a method for beamforming with said arrays is derived. The sound field model is obtained by introducing two independent boundary conditions for the sound field of a single impinging plane wave. The model for the radiation from a single loudspeaker in the array is then obtained from the reciprocity principle. Various beam patterns are presented and the theoretically predicted sound field is evaluated as a function of frequency. The results are discussed and an experimental array prototype is presented
