    Systematic review of functional outcomes and quality of life after transanal endoscopic microsurgery and transanal minimally invasive surgery: a word of caution

    PURPOSE: The introduction of transanal endoscopic or minimally invasive surgery has allowed organ preservation for rectal tumors with good oncological results. Data on functional and quality-of-life (QoL) outcomes are scarce and controversial. This systematic review sought to synthesize fecal continence, QoL, and manometric outcomes after transanal endoscopic microsurgery (TEM) or transanal minimally invasive surgery (TAMIS). METHODS: A systematic review of the literature including Medline, Embase, and the Cochrane Library databases was conducted searching for articles reporting on functional outcomes after TEM or TAMIS between January 1995 and June 2018. The evaluated outcome parameters were pre- and postoperative fecal continence (primary endpoint), QoL, and manometric results. Data were extracted using the same scales and measurement units as from the original study. RESULTS: A total of 29 studies comprising 1297 patients were included. Fecal continence outcomes were evaluated in 23 (79%) studies with a wide variety of assessment tools and divergent results. Ten studies (34%) analyzed QoL changes, and manometric variables were assessed in 15 studies (51%). Most studies reported some deterioration in manometric scores without major QoL impairment. Due to the heterogeneity of the data, it was not possible to perform any pooled analysis or meta-analysis. CONCLUSIONS: These techniques do not seem to affect continence by themselves except in minor cases. The possibility of worsened function after TEM and TAMIS should not be underestimated. There is a need to homogenize or standardize functional and manometric outcomes assessment after TEM or TAMIS

    Wide temperature range mesomorphic behaviour of highly fluorinated 15-membered macrocycles and their open trisulphonamide precursor

    The mesomorphic properties of 15-membered azamacrocycles containing long hydrocarbon or highly fluorinated long chains have been explored along with those of their open precursors. The liquid crystalline behaviour is determined by the presence of different heteroatoms or long fluorinated chains and the substitution pattern of the aryl units of the sulphonamides. The presence of highly fluorinated long chains in the para position with regard to the sulphonamide group was found to favour the formation of smectic mesophases with wide temperature ranges. All of the mesophases were studied by polarizing optical microscopy and differential scanning calorimetry. © 2007 Taylor & Francis

    Perfluoro-tagged phosphine-free palladium nanoparticles supported on silica gel: application to alkynylation of aryl halides, Suzuki–Miyaura cross-coupling, and Heck reactions under aerobic conditions

    The utilization of perfluoro-tagged palladium nanoparticles immobilized on fluorous silica gel through fluorous–fluorous interactions (Pdnp–A/FSG) or through covalent bonding to silica gel (Pdnp–B) in the alkynylation of aryl halides, in the Suzuki–Miyaura cross-coupling, as well as in the Heck reaction between methyl acrylate and aryl iodides is described. The reactions are carried out under aerobic and phosphine-free conditions with excellent to quantitative product yields in each case. The catalysts are easily recovered and reused several times without significant loss of activity. The alkynylation of aryl halides (under copper-free conditions) and the Suzuki–Miyaura cross-coupling are carried out in water. The Heck reaction of methyl acrylate with aryl iodides is best performed in MeCN. The utilization of Pdnp–B in the synthesis of 2,3-disubstituted indoles from 2-(alkynyl)trifluoroacetanilides and aryl halides is also reported

    Descripció del recurs: el 1 setembre 2011Els compostos altament fluorats o polifluorats es caracteritzen per tenir propietats físiques i químiques inusuals i úniques molt diferents a les dels hidrocarburs. Donades aquestes propietats, els compostos altament fluorats han estat utilitzats en diverses aplicacions tant en química farmacèutica com en ciència dels materials. En el nostre grup d'investigació, es va descobrir que alguns compostos altament fluorats estabilitzaven nanopartícules metàl·liques, partícules aïllades entre 1 i 100 nm de diàmetre. Aquests materials presenten aplicacions en biologia, física i química, sent una de les aplicacions més importants en el camp de la catàlisis, on el seu us ha permès crear catalitzadors que es poden recuperar i reutilitzar. En la present Tesi Doctoral s'han preparat nous compostos aromàtics altament fluorats mitjançant reaccions de substitució nucleòfila aromàtica. Els nous compostos han mostrat ser excel·lents estabilitzants de nanopartícules de Pd(0), Ru(0), i Rh(0). Per altra banda, s'ha preparat un tiol aromàtic altament fluorat capaç d'estabilitzar nanopartícules de Au(0). Les nanopartícules de Pd(0) s'han mostrat actives en catàlisi en reaccions de formació d'enllaços C-C (acoblament creuat). Per tal de facilitar la recuperació i reutilització del catalitzador, les nanopartícules de Pd(0) estabilitzades pels compostos altament fluorats s'han immobilitzat en un gel de sílice fluorat mitjançant interaccions febles F-F. Per altra banda, s'ha preparat un material més robust capaç d'estabilitzar nanopartícules metál·liques on l'estabilitzant està unit a la matriu de sílice mitjançant enllaç covalent. Tots dos materials han mostrat ser bons catalitzadors. Les nanopartícules de Au(0) s'han immobilitzat en un gel de sílice fluorat i s'han usat com a catalitzadors reutilitzables en l'oxidació d'alcohols primaris i secundaris en presència d'oxigen. També, han mostrat activitat en l'esterificació oxidativa d'alcohols. Finalment, les nanopartícules de Ru(0) i Rh(0) s'han fet servir com a catalitzadors en reaccions d'hidrogenació.Fluoroorganic compounds have a unique set of physical and chemical properties. Due to these properties, the polyfluorinated compounds have been used in pharmaceutical chemistry and in material science. In our research group, we have discovered that some heavily fluorinated compounds stabilized metal nanoparticles, a term referring to isolated particles with a size between 1 and 100 nm. Among the many potential applications of metal nanoparticles, catalysis has perhaps been one of the most intensely investigated. High surface area of nanoparticles makes them attractive candidates for the development of efficient recoverable catalysts. In the present Doctoral thesis, two heavily fluorinated compounds have been prepared and used in the preparation of Pd(0), Ru(0) and Rh(0) nanoparticles. In addition, a heavily fluorinated thiol has been synthesised and used in the stabilization of gold nanoparticles. Palladium nanoparticles have been used as reusable catalysts in cross-coupling reaction. In addition, some of these nanoparticles have been supported on fluorous reversed-phase silica gel through fluorous-fluorous interactions and used as catalysts. In an effort to create a more robust catalyst, we have prepared an organic-inorganic fluorinated hybrid material using the sol-gel process. This new material was found to act as an efficient stabilizer of catalytically active metal nanoparticles. Gold nanoparticles have been immobilized on reversed-phase silica gel and successfully used in the oxidation of primary and secondary alcohols. Moreover, the material was shown to be active in the direct oxidative conversion of alcohols into ethers. Finally, ruthenium and rhodium nanoparticles have been used as catalyst in hydrogenation reactions
