How can targeted therapy influence renal surgery for renal cell carcinoma?
Between the end of 2005 and the beginning of 2006 several new target therapies have been introduced for the treatment of renal cell carcinoma. In this review we aimed to explore and summarize the main findings about the use of systemic treatment and its effect on surgery in patients with renal cell carcinoma. We identified three different settings: neoadjuvant and adjuvant settings as well as the association of systemic therapy with surgery in the metastatic renal cell carcinoma patients. Neoadjuvant target therapy with tyrosine kinase inhibitor may facilitate the tumor resection and reduce the overall tumor diameter and its complexity. However, most of the evidences are from small phase I or II clinical trials and results are often conflicting without determining a relevant change in main parameters investigated, such as tumor complexity. In the adjuvant setting, results from pivotal trials investigating the use of tyrosine kinase inhibitor for patients with nonmetastatic RCC treated with surgery discourage this practice. Indeed, most of the evidences from single clinical trials and pooled results from meta-analysis failed to find a survival advantage with the use of adjuvant systemic treatment. To date, an improvement of clinical outcomes after systemic targeted therapies could be only found in the setting of cytoreductive nephrectomy. However, the CARMENA and SURTIME trials recently confirmed the evidences against a surgical treatment in patients with mRCC and poor prognosis. In the near future significant changes may be introduced by the use of immunotherapies
Progettare le difese: il marchese di Leganés e il padre gesuita Francesco Antonio Camassa esperto di arte miltare
Designing defenses: the Marquis of Leganés and Jesuit Father Francesco Antonio Camassa, expert in military art. Progettare le difese: il marchese di Leganés e il padre gesuita Francesco Antonio Camassa, esperto di arte militareAbstract The "notable campaña del año 1639" sees the Spanish-Lombard troops, led by the Marquis of Leganés, Don Diego Felipe de Guzman, invade the Savoy Piedmont, alley of France, occupy different cities and get dangerously close to Turin. With the Spanish occupation, brief and fleeting, some cities and villages see change its fortified perimeter and the relationship with the surrounding area. Traces of the "Spanish" drawings, if not immediately made, are visible in the rear projects and, in some cases, the execution goes to modify indelibly the urban conformation. The Marquis of Leganés, the soldiers and engineers of his following are undisputed protagonists of "notable campaña": an atlas of drawings preserved in the National Library of Spain allows to identify projects and realizations. Among all, it emerges the figure of the Jesuit Father Francesco Antonio Camassa, professor of mathematics and military art at the Imperial College of Madrid, spiritual father of Leganés, but also designer and consultant in military fortifications

Metabolic syndrome and smoking are associated with an increased risk of nocturia in male patients with benign prostatic enlargement
Background: To evaluate the relationship between cigarette smoking, metabolic syndrome (MetS) and nocturia in patients with lower urinary tract symptoms (LUTS) and benign prostatic enlargement (BPE). Methods: From 2009 onward, a consecutive series of patients with LUTS/BPE were enrolled. Symptoms were assessed using the International Prostate Symptom Score (IPSS). Age, body mass index (BMI), smoker status, prostate volume (PV), prostate-specific antigen (PSA) levels, fasting glucose levels, triglyceride levels, and high-density lipoprotein levels were recorded. MetS was defined according to Adult Treatment Panel III criteria. Moderate/severe nocturia was defined as nocturnal micturition episodes ≥2. Results: Overall 492 patients were enrolled with median age and BMI of 68 years (IQR 61/74) and 26.5 kg/m2 (IQR: 24/29), respectively. Moderate/severe nocturia was reported in 212 (43.1%) patients. MetS was diagnosed in 147 (29.9%) patients and out of them 89 (60.5%) complained moderate/severe nocturia (p = 0.001). Overall 187 (38%) patients were current smokers and out of them 99 (52%) presented moderate/severe nocturia(p = 0.034). Patients with moderate/severe nocturia were older (p = 0.001) and with larger prostate volume (p = 0.003). On multivariate analysis, age (OR: 1.067 per year, 95% CI: 1.036-1.098; p = 0.001), PV (OR: 1.011 per ml, 95% CI: 1.003-1.019; p = 0.006), MetS (OR: 2.509, 95% CI: 1.571-4.007; p = 0.001) and smoking (OR: 1.690, 95% CI: 1.061-2.693; p = 0.027) were associated with nocturia severity. Conclusions: MetS and smoking doubled the risk of moderate/severe nocturia in patients with LUTS and BPE. Assessing smoking and metabolic status in LUTS/BPE patients is suggested
Didattica e ricerca. L’esperienza del Laboratorio aragonese e sforzesco
In the workshop on Diplomatic correspondences in the Italian Renaissance, directed in January-May 2018 by Francesco Storti and Francesco Senatore in the Istituto italiano per gli studi filosofici of Naples, 12 undergraduate and graduate students learned to read and trascribe italian diplomatic dispatches of the 15th cent., producing a specimen of scientific edition. In the opinion of the author, such activities and similar ones are very useful for future school teachers and scholars and they could be introduced in the university and pHD courses of history. Students of humanistic sciences should have concrete contacts with the historical research on primary sources. On the other hand, editing of historical sources could become a marginal activity because it is not enough appreciated in the current research evaluation processes.
