Resectoscope or Versapoint for hysteroscopic metroplasty
To assess the feasibility, safety, and reproductive outcome of hysteroscopic metroplasty using the Versapoint device compared with the resectoscope using the Collins loop. Sixty-three women diagnosed with partial septate uterus were included in the study. Forty-two women underwent hysteroscopic metroplasty using Versapoint and 21 women had the procedure using the resectoscope. Operating time, complications, pregnancy rate, and mode of delivery were recorded. Data were analyzed with the Mann-Whitney test. Operating time was 20.5 min for the resectoscope group and 15.4 min for the Versapoint group (P<0.05). Pregnancy rate, delivery rate, and spontaneous abortions were similar in both groups. Operative hysteroscopy with Versapoint does not require cervical dilation, thus avoiding cervical incompetence, cervical lacerations, and uterine perforation. The Versapoint technique is a safe and effective alternative to the resectoscope. It could be used predominantly in nulligravida women, especially in those with cervical canal stenosis
Accidental fetal lacerations during cesarean delivery: Experience in an Italian level III university hospital
Objective The purpose of this study was to investigate the incidence, type, location, and risk factors of accidental fetal lacerations during cesarean delivery. Study design Total deliveries, cesarean deliveries, and neonatal records for documented accidental fetal lacerations were reviewed retrospectively in our level III university hospital. The gestational age, the presenting part of the fetus, the cesarean delivery indication, the type of incision, and the surgeon who performed the procedure were recorded. Cesarean deliveries were divided into scheduled, unscheduled, and emergency procedures. Fetal lacerations were divided into mild, moderate, and severe. Neonatal follow-up examinations regarding laceration sequelae were available for 6 months. Results Of 14926 deliveries, 3108 women were delivered by cesarean birth (20.82%). Neonatal records documented 97 accidental fetal lacerations. Of these accidental lacerations, 94 were mild; 2 were moderate, and 1 was severe. The overall rate of accidental fetal laceration per cesarean delivery was 3.12%; the accidental laceration rate in the cohort of fetuses was 2.46%. The crude odds ratios were 0.34 for scheduled procedures, 0.57 for unscheduled procedures, and 1.7 for emergency procedures. The risk for fetal accidental lacerations was higher in fetuses who underwent emergency cesarean birth and lower for unscheduled and scheduled cesarean births (P < .001). Conclusion Fetal accidental laceration may occur during cesarean delivery; the incidence is significantly higher during emergency cesarean delivery compared with elective procedures. The patient should be counseled about the occurrence of fetal laceration during cesarean delivery to avoid litigation. © 2004 Elsevier Inc. All rights reserved
Computerized cardiotocography parameters in pregnant women affected by pregestational diabetes mellitus
Aim: To evaluate whether computerized CTG (cCTG) is a reliable method of predicting neonatal outcome in pregnancies complicated by pregestational diabetes at term. Patients and methods: We considered 27 pregnant women affected by pregestational diabetes and 46 normal pregnancies as controls that fulfilled the following criteria: singleton, Caucasian, euglycemic pregnancies at term (>37 weeks gestational age). All women delivered by cesarean section (CS), with an antepartum cCTG performed within one hour before the CS and an UBGA available at birth. No patient was in labor during FHR monitoring. Results: Among cCTG parameters, accelerations 15 bpm, HV min, HV ms and STV were significantly lower in comparison to controls. We observed that in the diabetic pregnant women the parameter STV was not able to predict or to linearly regress with the most important UBGA parameters: pH and pCO(2). Contrarily, in normal pregnancies, the STV linearly regressed with both the pH (p<0.03) and pCO(2) (p<0.04). Conclusions: Computerized FHR criteria may not be applicable to fetuses in pregestational diabetic pregnancies at term. Therefore some criteria should perhaps be modified for a correct interpretation of cCTG in these pregnancies
Going Beyond Counting First Authors in Author Co-citation Analysis
The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation
counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings
are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that
only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into
account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed. Variations on the Author
“Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship
Eficacia del estriol intravaginal de dosis baja en el envejecimiento urogenital de las mujeres posmenopáusicas
Objetivo: evaluar la eficacia y la seguridad del estriol intravaginal en la incontinencia urinaria, la atrofia urogenital y las infecciones recurrentes
del conducto urinario en las mujeres posmenopáusicas.
Diseño: se reclutaron en este estudio prospectivo, con asignación al azar y controlado con placebo, 88 mujeres posmenopáusicas con síntomas de envejecimiento urogenital. Las participantes se asignaron al azar a uno de dos grupos. Las mujeres en el grupo de tratamiento (n = 44) recibieron un óvulo (1 mg) intravaginal una vez al día durante dos semanas, y después dos óvulos cada semana durante seis meses como terapia de mantenimiento. A las pacientes del grupo control (n = 44) se les dio supositorios vaginales con placebo inerte en un régimen similar. Se evaluaron los síntomas urogenitales, los cultivos urinarios, los hallazgos colposcópicos y citológicos uretrales, los perfiles de presión uretral y la uretrocistometría antes y a los seis meses del tratamiento.
Resultados: después de la terapia, los síntomas y signos de atrofia urogenital se atenuaron significativamente en el grupo de tratamiento en comparación con el grupo control. Treinta (68%) de las pacientes tratadas y sólo siete (16%) de las del grupo control registraron una mejoría subjetiva de su incontinencia. En las tratadas, las mejorías se observaron colposcópicamente, e incluso aumentó la presión uretral máxima media, la presión de cierre uretral media así como la proporción de transmisión de la presión abdominal a la uretra proximal. La uretrocistometría mostró modificaciones estadísticamente positivas, pero no estadísticamente significativas.
Conclusiones: estos resultados revelan que la administración intravaginal de estriol puede ser una elección terapéutica satisfactoria para las mujeres posmenopáusicas con trastornos del conducto urogenital que tienen contraindicaciones o que no quieren seguir una terapia hormonal estándar.

Appropriate Similarity Measures for Author Cocitation Analysis
We provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature. Dispelling the Myths Behind First-author Citation Counts
We conducted a full-scale evaluative citation analysis study of scholars in the XML research field to explore just how different from each other author rankings resulting from different citation counting methods actually are, and to demonstrate the capability of emerging data and tools on the Web in supporting more realistic citation counting methods. Our results contest some common arguments for the continued
use of first-author citation counts in the evaluation of scholars, such as high correlations between author rankings by first-author citation counts and other citation
counting methods, and high costs of using more realistic citation counting methods that are not well-supported by the ISI databases. It is argued that increasingly available digital full text research papers make it possible for citation analysis studies to go beyond what the ISI databases have directly supported and to employ more
