Os sintomas negativos têm sido apontados como grande causa de sobrecarga nos pacientes esquizofrênicos, apesar dos recentes avanços no tratamento. Este estudo teve por objetivo investigar a correlação entre os sintoma s negativos e a qualidade de vida, em pacientes esquizofrênicos refratários e super-refratários. Cento e dois pacientes que preenchiam os critérios DSM-IV para esquizofrenia foram observados durante seis meses. Os pacientes foram divididos de acordo com critérios pré-estabelecidos, em três grupos: não refratários (N=22), refratários (N= 47) e super-refratários (N= 31), A psicopatologia foi avaliada por meio da Escala para Avaliação da Síndrome Positiva e Negativa (PANSS), Entrevista para Síndrome Deficitária (SDS) e Escala de Calgary para Depressão na Esquizofrenia. A qualidade de vida foi medida pela Escala de Qualidade de Vida (QV) . Os super-refratários tiveram os menores escores de QV, quando comparados ao grupo não refratário (p < 0,05). Não houve diferenças significativas nos escores médios de QV entre os refratários e superrefratários. Pacientes não refratários apresentaram menos sintomas negativos e melhores escores de QV. Os sintomas negativos tiveram uma correlação negativa com a qualidade de vida. Pacientes super-refratários, com mais sintomas negativos, tenderam a ter os menores escores de qualidade de vidaNegative symptoms have been reported as a major cause of burden in schizophrenic patients and often do not respond to drug treatment, in spite of recent advances in antipsychotic therapy. Evidence of efficacy and tolerability of electroconvulsive therapy in clozapine-resistant schizophrenia
Introdução: A literatura mostra que cerca de 30% dos pacientes com esquizofrenia não respondem de forma completa ao tratamento com os antipsicóticos convencionais. Tais pacientes são chamados de refratários, e a medicação de eleição é a clozapina, porém, entre os pacientes refratários e em uso de clozapina na dose e tempo adequados, ainda existe uma parcela, também de 30%, que mantém prejuízo funcional e sintomatologia psicótica incapacitante. Estes pacientes são conhecidos como respondedores parciais à clozapina ou super-refratários, sem suporte na literatura a alternativas realmente eficazes por meio de ensaios clínicos randomizados. Objetivos: Avaliar a eficácia da eletroconvulsoterapia (ECT) como potencializador da clozapina na esquizofrenia super-refratária (ESR) comparada ao placebo do ECT, conhecido como sham ECT. Método: Foram selecionados 20 pacientes em uso de clozapina por tempo e dose adequados que apresentavam ainda prejuízo funcional e sintomas psicóticos. As medidas de desfecho primário seriam a diminuição da pontuação na escala da PANSS (Positive and Negative Syndrome Scale) e CGI (Clinical Global Impression) com base no índice acima de 60 na PANSS e de 4 na CGI. Todos pacientes realizaram a dosagem do nível sérico de clozapina para conferência de nível terapêutico, bateria de testes neuropsicológicos para avaliação da cognição pré-intervenção, avaliação clínica/pré-anestésica, psiquiátrica e odontológica pré-ECT. Após isso, foram randomizados em dois grupos (ECT e sham ECT), sendo encaminhados para o respectivo grupo sem o conhecimento do avaliador final, que permaneceu cego. Ao término das 12 sessões de ECT ou sham ECT, feitas 3 vezes na semana por 4 semanas, os pacientes foram reavaliados por meio da PANSS e da CGI. Resultados: Foram tratados dez pacientes no grupo ECT, seis no grupo sham ECT, e houve quatro drop-outs. Na análise estatística, foi verificado que os grupos eram comparáveis no baseline, exceto para as variáveis de desfecho: o grupo sham ECT apresentou scores médios significativamente maiores na PANSS total e subescalas positiva e geral, além da CGI. Apesar das significativas reduções nas variáveis de desfecho depois da intervenção em ambos os grupos, não houve diferença estatística entre os grupos, mesmo quando a diferença foi estatisticamente corrigida para um nível de significância p<= 0,05. Recomenda-se cautela na interpretação dos resultados, levando-se em consideração o pequeno tamanho da amostra e as limitações da PANSS como medida de desfecho do tratamento da esquizofrenia com ECT, bem como os possíveis efeitos a longo prazo não medidos pelo estudo. O ideal é que este estudo seja replicado com um número maior de pacientes e de medidas de desfecho.Introduction: The prevalence of patients who do not respond to clozapine has rates ranging from 30% to 70%. These patients, despite treatment with adequate doses of clozapine for at least six months, have persistent symptoms in at least one of the five dimensions: Positive, Negative, Cognitive, Anxiety-Depression, and Excitation. There are called incomplete responders to clozapine, ultra-resistant, super refractory or, according to the new terminology, carriers of Clozapine Resistant Schizophrenia (CRS). For these patients several potentiation strategies have been proposed, such as the addition of a second antipsychotic, antidepressants, mood stabilizers, cognitive behavioral therapy, transcranial magnetic stimulation, transcranial direct current stimulation and, among them, electroconvulsive therapy (ECT), whose efficacy and tolerability have already been evaluated by some clinical trials and one study, with only one published pilot study of our own, comparing ECT vs Sham in 19 patients with CRS treated with 12 sessions, finding no significant differences between the groups evaluated by the PANSS and CGI scale. However, new work that used symptom reanalysis to evaluate effect sizes as well as individual participant metanalysis showed that ECT was more effective than Sham. Thus the objectives of this thesis are as follows. Objectives: 1- Systematic review of the literature on ECT in resistant schizophrenia in the CRS with emphasis on identifying the studies that used Sham to identify the literature gap that justifies the development of our clinical trials. 2- Submit the original publication of our clinical trial that evaluated the efficacy and tolerability of ECT compared to Sham in ERC. 3- Present work in the form of an article submitted to a journal with an impact factor above 2.5 that presents a reanalysis of our pilot study now using the method and individual item meta analysis of the individual symptoms of PANSS and 4- Present partial study results. randomized controlled trial that is underway comparing ECT-Sham in patients with CRS with a design very similar to the published pilot study, but with 20 sessions. Results: The systematic literature review shows that there are no publications in the ECT vs Sham controlled ransomized clinical trial literature in CRS so this is the first study with these characteristics in the international literature. Reanalysis of the pilot study showed that ECT was more effective than Sham in terms of improving hallucinatory conduct and that patients receiving Sham showed significant worsening in most PANSS items, not identified in the analysis of the original publication. In the ongoing study, the first 17 cases were analyzed, with only 12 completing the protocol and the others remaining performing the procedures. To maintain adequate blindness we present these partial data without identifying the groups. Conclusions: Electroconvulsive therapy may be an effective and safe complementary treatment in patients with CRS, but studies with a larger number of individuals are still necessary for the evidence to have greater statistical power. The pilot study we have published and what is underway are the only ones in the literature comparing active treatment (ECT) with placebo (Sham)
Characterization of the cognitive impairment of clozapine resistant schizophrenia
Introdução: Já está bem estabelecido que pessoas com diagnóstico de esquizofrenia apresentam graus diversos de comprometimento das funções cognitivas. Pacientes com diagnóstico de esquizofrenia também apresentam diferentes tipos de resposta ao tratamento com antipsicóticos de acordo com diferentes critérios, sendo classificados geralmente nos seguintes subtipos: a-Responsivos, b- Resistentes (com indicação para tratamento com clozapina) e c- Resistentes a clozapina (ERC). O estudo do comprometimento cognitivo de pacientes de acordo com a resposta ao tratamento é recente, havendo poucos trabalhos que incluíram pacientes com ERT ou ERC com critérios bem definidos ou instrumentos de avaliação neuropsicológica validados. Objetivos: o objetivo deste trabalho é o da caracterização neuropsicológica de um grupo de pacientes definidos como ERC de acordo om critérios internacionais bem definidos e avaliados por uma bateria válida e sensível para avaliação da alterações cognitivas da esquizofrenia, a Bateria Cognitiva Consensual MATRICS (MCCB), e que participaram de um ensaio clínico randomizado, duplo-cego, controlado com placebo (Sham-ECT) ,que avaliou a eficácia e tolerabilidade e outros desfechos, da Eletroconvulsoterapia (ECT) na potencialização da clozapina em pacientes com ERC. Métodos: 40 pacientes com diagnóstico de esquizofrenia, integrantes do Programa de Esquizofrenia do Instituto de Psiquiatria do Hospital das Clínicas da FMUSP (PROJESQ) e de outros serviços em São Paulo foram incluídos no estudo. Os pacientes foram avaliados na linha de base e após 20 sessões de ECT ou placebo (Sham-ECT). O desfecho primário foi avaliado por meio da Positive and Negative Syndrome Scale (PANSS) total e respectivas subescalas. Os desfechos secundários foram avaliados por meio de: fatores da PANSS-6 (negativo, positivo, ansiedade-depressão, cognitivo-desorganização e excitação), PANSS-6 (combinação de 6 sintomas da PANSS); Calgary Depression Rating Scale for Schizophrenia (CDRS), Quality of Life Scale (QoL), Sobrecarga Familiar (FBIS) e o Brain Derived Neurotrophyc Factor (BDNF). Os pacientes foram definidos como ERC de acordo com os critérios do Treatment Response and Resistance in Psychosis (TRRIP) que inclui a avaliação dos níveis plasmáticos de clozapina ( 350ng/mL) na linha de base. A MCCB foi aplicada na linha de base e após 20 sessões de ECT ou Sham-ECT. O estudo foi submetido para publicação por meio de dois artigos cujo resumo encontra-se no corpo desta dissertação e na sua íntegra nos anexos. Resultados: pacientes tratados com ECT ou Sham-ECT não apresentavam diferenças significantes na linha de base exceto em termos da escolaridade, que entrou como covariada em todas as análises. Após o período de intervenção de 20 sessões de ECT/Sham-ECT, ambos os grupos não apresentaram diferenças estatisticamente significantes, tanto em termos do desfecho primário como dos desfechos secundários incluindo as funções cognitivas avaliadas pela MCCB. Do ponto de vista neuropsicológico, a subamostra dos pacientes que completou a MCCB na linha de base e após as intervenções, apresentou importante comprometimento em seis dos sete domínios cognitivos e na pontuação composta total da MCCB (composite), quando comparados a literatura internacional: velocidade de processamento (média=12.9, Desvio Padrão=14.11), atenção (m=23.1, DP=13.11), memória operacional (média=27.2, DP=14.2), aprendizagem verbal (m=31, DP=6.18), aprendizagem visual (m=22, DP=15.22), raciocínio (m=31.6, DP=8.35) e composite (m=13.8, DP=14.04). Apenas o domínio de cognição social mostrou-se preservado (m=46.1, DP=18.5). Além disso, diferentemente dos dados de trabalhos da literatura internacional que utilizaram a MCCB para avaliar a cognição em pacientes com esquizofrenia em geral, no presente trabalho, quando o composite da MCCB foi correlacionado com a PANSS tradicional, PANSS fatorial e PANSS-6, foram obtidas correlações negativas robustas e que permaneceram significantes mesmo quando aplicada a correção de Bonferroni, a saber: correlações entre o composite e a PANSS tradicional: PANSS Negativa (Pearson\'s r=-0,698, p= 0,000), PANSS Geral (Pearson\'s r=-0,616, p=0,002), e PANSS Total (Pearson\'s r=-0,648, p=0,001). Composite e a PANSS Fatorial: Fator Negativo (Pearson\'s r=-0,613, p=0,002), Fator Cognitivo e Desorganização (Pearson\'s r=-0,829, p=0,000) e PANSS-6 (Pearson\'s r=-0,634, p=0,001). Sobre o composite e os componentes da PANSS-6: P2 (Pearson\'s r=-0,679, p=0,000), N5 (Pearson\'s r=-0,672, p=0,000) e G5 (Pearson\'s r=-0,752, p=0,000). Discussão e conclusões: o ensaio clínico não mostrou diferenças entre os grupos ECT e Sham-ECT, tanto sobre os desfechos primários como os secundários, replicando assim os achados do estudo piloto realizado em 2017, cujo número de pacientes era menor bem como o de sessões. No entanto as características neuropsicológicas da subamostra de pacientes avaliados pela MCCB oferecem evidências de que pacientes com ERC apresentam graus significativamente maiores de comprometimento cognitivo do que aqueles descritos na literatura internacional e que apresentam correlação significativa com as alterações psicopatológicas avaliadas pela PANSS. No entanto não é possível inferir que o comprometimento cognitivo dos pacientes desta subamostra esteja relacionado a resposta às intervençõesIntroduction: It is well established that people with schizophrenia suffer from varying degrees of cognitive impairment. Patients with schizophrenia also present different types of response to treatment with antipsychotics medication according to different criteria, generally classified into the following subtypes: a- Treatment Responders (TR), b- Treatment Resistant (TRS - with indication for treatment with clozapine) and c- Clozapine Resistant (CRS). The study of cognitive impairment in patients according to treatment response is recent, with few studies including patients with TRS or CRS and well-defined criteria or validated neuropsychological assessment instruments. Objectives: the objective of this dissertation is the neuropsychological characterization of a group of CRS patients diagnosed as such according to well-defined international criteria and assessed by a valid and sensitive test battery able to measure cognitive changes in schizophrenia, the MATRICS Consensus Cognitive Battery (MCCB), and who participated in a randomized, double-blind, placebo-controlled clinical trial (Sham-ECT), which evaluated the efficacy, tolerability and other outcomes of Electroconvulsive Therapy treatment (ECT) in augmenting clozapine in patients with CRS. Methods: 40 patients diagnosed with schizophrenia, members of the Schizophrenia Program at the Institute of Psychiatry at HCFMUSP (PROJESQ) and other services in São Paulo were included in the study. Patients were assessed at baseline and after 20 sessions of ECT or placebo (Sham-ECT). The primary outcome was assessed using the total Positive and Negative Syndrome Scale (PANSS) and respective subscales. Secondary outcomes were assessed using: PANSS-6 factors (negative, positive, anxiety-depression, cognitive-disorganization, and excitement), PANSS-6 (combination of 6 PANSS symptoms); Calgary Depression Rating Scale for Schizophrenia (CDRS), Quality of Life Scale (QoL), Family Burden (FBIS) and the Brain Derived Neurotrophic Factor (BDNF). Patients were defined as CRS according to the Treatment Response and Resistance in Psychosis (TRRIP) criteria, which includes assessment of clozapine plasma levels ( 350ng/mL) at baseline. MCCB assessment was done at baseline and after 20 sessions of ECT or Sham-ECT. The study was submitted for publication through two articles whose summary is found in the body of this dissertation and in its entirety in the annexes. Results: patients treated with ECT or Sham-ECT showed no significant differences at baseline except in terms of years of education, which was included as a covariate in all analyses. After the intervention period of 20 ECT/Sham-ECT treatment sessions, both groups did not show statistically significant differences, both in terms of the primary outcome and secondary outcomes including cognitive functioning assessed by the MCCB. From a neuropsychological point of view, the subsample of patients who completed the MCCB at baseline and after the interventions, showed significant impairment in six of the seven cognitive domains and in the MCCB composite score, when compared to data from international literature: speed processing (mean=12.9, Standard Deviation=14.11), attention (m=23.1, SD=13.11), working memory (m=27.2, SD=14.2), verbal learning (m=31, SD=6.18), visual learning (m=22, SD=15.22), reasoning (m=31.6, SD=8.35) and composite (m=13.8, SD=14.04). Only social cognition was preserved (m=46.1, SD=18.5). Furthermore, unlike data from studies in the international community that used the MCCB to assess cognition in patients with schizophrenia in general, in our present study, when the MCCB composite was correlated with the traditional PANSS, Factorial PANSS and PANSS-6, we observed robust negative correlations which remained significant even after the Bonferroni correction was applied, namely: correlations between the MCCB composite score and the traditional PANSS: Negative PANSS (Pearson\'s r=-0.698, p= 0.000), General PANSS (Pearson\'s r=-0.616 , p=0.002), and Total PANSS (Pearson\'s r=-0.648, p=0.001). Composite and the Factorial PANSS: Negative Factor (Pearson\'s r=-0.613, p=0.002), Cognitive and Disorganization Factor (Pearson\'s r=-0.829, p=0.000) and PANSS-6 (Pearson\'s r=-0.634, p=0.001). MCCB composite and PANSS-6 components: P2 (Pearson\'s r=-0.679, p=0.000), N5 (Pearson\'s r=-0.672, p=0.000) and G5 (Pearson\'s r=-0.752, p=0.000). Discussion and conclusions: the clinical trial showed no differences between the ECT and Sham-ECT groups, both in terms of primary and secondary outcomes, thus replicating the findings of the pilot study carried out in 2017, in which the number of patients was smaller as well as the number of treatment sessions. However, the neuropsychological characteristics of the subsample of patients assessed by the MCCB provide evidence that patients with CRS have significantly higher degrees of cognitive impairment than those described in the international literature, also presenting a significant negative correlation with the psychopathological changes assessed by PANSS. However, it is not possible to infer that the cognitive impairment of this subsample is related to the response to intervention

Randomized and controlled trial to evaluate the efficacy of occupational therapy in the rehabilitation of executive functions in patients with treatment-resistant schizophrenia
A esquizofrenia é uma das 25 principais causas de incapacidade em todo o mundo. Indivíduos que desenvolvem a doença apresentam prejuízos cognitivos relevantes ao longo vida. A combinação de tratamentos farmacológico e psicossocial é eficaz na melhora das dimensões psicopatológicas da esquizofrenia, porém a maioria dos pacientes apresenta importantes déficits cognitivos, principalmente em funções executivas (FE) com impacto na funcionalidade, impossibilitando uma vida independente. O método Occupational Goal Intervention (OGI) é eficaz na melhora de FE, porém não foi testado em pacientes com esquizofrenia resistente (ER). Realizamos estudo piloto cujos resultados foram descritos na dissertação de mestrado e publicados no periódico Psychiatry Research. O presente trabalho inclui o estudo completo com follow-up de 6 meses. Objetivos: Trata-se de um ensaio clínico comparando a intervenção de Terapia Ocupacional (TO), baseada no método OGI para a reabilitação de FE, com um grupo-controle de indivíduos com ER. Método: O estudo desenvolveu-se em 3 tempos: baseline, pós-tratamento e follow-up. Cinquenta e quatro pacientes com diagnóstico de ER foram randomizados e distribuídos em dois grupos: experimental e controle (placebo). O grupo experimental recebeu 30 sessões de TO com o método OGI e o grupo controle (GC) recebeu 30 sessões de atividades artesanais de livre escolha, sem intervenção ativa do terapeuta (placebo). As medidas de desfecho primário foram avaliadas pela BADS, e as de desfecho secundário pela DAFS-BR e ILSS-BR O impacto sobre a cognição foi avaliado por uma bateria neuropsicológica padrão, que mediu as seguintes funções: atenção, fluência verbal, FE, memória e o QI estimado. As escalas PANSS e o CGI foram utilizadas para monitorar a gravidade dos sintomas psicopatológicos. A eficácia foi avaliada nos três tempos do estudo por meio da análise linear de efeitos mistos e obtido o tamanho de efeito por meio d de Cohen. Resultados: Observou-se melhora significativa com tamanhos de efeito de médios a altos na BADS total (d=0,73), com pequena redução no follow-up (d=0,69).Na DAFS-BR total os tamanhos de efeito foram médios (d=0,58) porém não se sustentou no follow-up (d=0,39). Os resultados de tamanhos de efeito na ILSS-BR total foi alto (d=1,09) e com aumento de melhora clínica no follow-up (d=1,19). Não foram observadas mudanças nas funções cognitivas avaliadas pela bateria neuropsicológica. Conclusão: o método OGI mostrou-se eficaz na melhora da maioria das FE relacionadas à capacidade de controle inibitório, planejamento, resolução de problemas e flexibilidade mental de pacientes com ER quando comparado ao placebo, melhora esta que foi mantida 6 meses de seguimentoSchizophrenia is one of the top 25 causes of disability worldwide. Individuals who develop the disease have relevant cognitive impairments over a long period of time. The combination of pharmacological and psychosocial treatments is effective in improving the psychopathological dimensions of schizophrenia, but the majority of patients present significant cognitive deficits, especially in executive functions (EF) with an impact on functionality, making life independent. The Occupational Goal Intervention (OGI) method is effective in improving EF but has not been tested in patients with resistant schizophrenia (RE). We conducted a pilot study whose results were described in the master\'s thesis and published in the journal Psychiatry Research. The present study includes the complete 6-month follow-up study. Objectives: This is a clinical trial comparing Occupational Therapy (OT), based on the OGI method for the rehabilitation of EF, with a control group of individuals with RE. Method: The study was developed in 3 stages: baseline, post-treatment, and follow-up. Fifty-four patients diagnosed with RE were randomized and divided into two groups: experimental and control (placebo). The experimental group received 30 OT sessions with the OGI method and the control group received 30 sessions of free-choice craft activities without active therapist intervention (placebo). Primary outcome measures were evaluated by the BADS and secondary endpoints by DAFS-BR and ILSS-BR. The impact on cognition was assessed by a standard neuropsychological battery, which measured the following functions: attention, verbal fluency, EF, memory, and estimated IQ. PANSS scales and CGI were used to monitor the severity of psychopathological symptoms. Efficacy was assessed in the three study times by means of the linear analysis of mixed effects and the effect size obtained by means d of Cohen. Results: A significant improvement was observed with mean to high effect sizes in the total BADS (d = 0.73), with a small reduction in follow-up (d = 0.69). In DAFS-BR total effect sizes were medium (d = 0.58) but did not sustained at the follow-up (d = 0.39). The results of effect sizes in the total ILSS-BR were high (d = 1.09) and with an increase in clinical followup at follow-up (d = 1.19). No changes were observed in the cognitive functions evaluated by the neuropsychological battery. Psychotic symptoms in older people without dementia from a Brazilian community-based sample: Incidence, risk factors and cognitive impairment development
Pouco se sabe sobre a presença de sintomas psicóticos em idosos sem diagnóstico de demência, levando a dificuldades no esclarecimento da etiologia, na ausência de informações quanto a possíveis diagnósticos e no manejo clínico. Os estudos sobre a incidência de sintomas psicóticos nessa população são ainda mais escassos e limitados. Os dados disponíveis na literatura são todos provenientes de estudos em populações de países desenvolvidos. A prevalência de sintomas psicóticos em idosos sem diagnóstico de demência varia de 0,9 a 10,5%, já a incidência na literatura varia entre 4,8 e 8,0%. É possível especular sobre alguns fatores de risco para o desenvolvimento de sintomas psicóticos em indivíduos sem demência: idade avançada, gênero feminino, comprometimento sensorial, pior desempenho cognitivo, isolamento social, pior funcionalidade. Dados recentes sugerem os sintomas psicóticos como uma expressão prodrômica da demência, devido a sintomas como alucinações, delírios e ideação paranoide estarem associados ao aumento da incidência de demência no acompanhamento, à maior presença deles à medida que a faixa etária sobe e à menor média no escore do Miniexame do Estado Mental (MEEM) em indivíduos com tais sintomas. Objetivamos determinar a incidência de sintomas psicóticos, correlacioná-los com características clínicas e estabelecer uma taxa de conversão em idosos sem comprometimento cognitivo. Este estudo foi realizado em uma amostra de idosos de comunidade de São Paulo, sendo a amostra inicial composta por 1.125 indivíduos acima de 60 anos. Destes, 547 foram reavaliados em 2011 e submetidos ao mesmo protocolo inicial. Não tinham sintomas psicóticos na primeira fase 199 e 64 já possuíam em 2006. A incidência de ao menos um sintoma psicótico em 7 anos foi 8,0% (alucinações visuais/táteis: 4,5%; ideias persecutórias: 3,0%; alucinações auditivas: 2,5%). A incidência esteve relacionada à epilepsia (OR: 7,75 e 15,83), baixa pontuação no MEEM (OR: 0,72) e depressão referida (OR: 6,48). 57,8% dos indivíduos com sintomas psicóticos, mas sem demência na fase I, desenvolveram comprometimento cognitivo em 7 anos (alucinações visuais/táteis foram preditivas - OR: 5,66), o que estava relacionado a baixo MEEM e comprometimento funcional. A incidência de sintomas psicóticos e a taxa de conversão em comprometimento cognitivo estão no limite superior dos dados da literatura. Alucinações visuais/táteis foram os sintomas mais incidentes e os únicos preditivos para a evolução para comprometimento cognitivo em 5 anos. This is a seven year follow up study aiming to determine the incidence of psychotic symptoms and their correlation with other clinical aspects as well as conversion rates to cognitive impairment. Objectives: To determine the incidence of psychotic symptoms, correlate these symptoms with clinical characteristics and establish the conversion rate to cognitively impaired individuals. Design: Cross-sectional study of a community-based sample of elderly subjects. Setting: City of Sao Paulo, State of Sao Paulo, Brazil. Participants: The original sample was composed of 1,125 individuals aged 60 years and older from a community. Among this sample, 547 subjects were re-evaluated in 2011 and submitted to the same protocol. Of these, 199 did not have psychotic symptoms at phase I and 64 already had psychotic symptoms in 2006. The costs of schizophrenia in a university tertiary center: a microcosting study
INTRODUÇÃO: A esquizofrenia é um transtorno psiquiátrico com alta morbidade que se inicia na fase adulto jovem. Apesar de ter uma prevalência relativamente baixa (0,3%), a doença é responsável globalmente por mais de 13 milhões de anos de vida ajustados pela incapacidade (DALYs). A esquizofrenia está associada a altos custos, tanto diretos como indiretos, por causa da gravidade, cronicidade e perda de produtividade. Apesar do alto impacto da doença no sistema de saúde, há poucos estudos de custos conduzidos em países de baixa e média renda, incluindo o Brasil. O objetivo deste estudo é avaliar os custos da esquizofrenia em um centro terciário especializado, e identificar as variáveis com maior impacto nos custos. MÉTODOS: Estudo transversal, de microcusteio, na perspectiva do pagador público e da sociedade, para analisar os custos anuais diretos e indiretos dos pacientes. As variáveis dependentes foram o custo direto anual e o custo total anual. As variáveis preditivas avaliadas foram o perfil demográfico, características clínicas, funcionamento global, medicamentos, profissionais de saúde e tratamento com eletroconvulsoterapia (ECT). O custo indireto foi calculado pelo método do capital humano, com base na perda de produtividade. RESULTADOS: Foram incluídos 37 pacientes, sendo que 81% (n= 30) faziam uso do medicamento clozapina, que é indicado para esquizofrenia resistente a tratamento, e 10 pacientes (27%) faziam tratamento com ECT de manutenção, refletindo a gravidade dos pacientes acompanhados neste serviço. O custo total anual médio dos pacientes foi de R24.419,10 a R 9.173,68 (variação R 34.985,82), sendo os maiores custos associados aos pacientes que faziam uso de ECT, ou seja, pacientes resistentes à clozapina. O custo adicional da ECT variou de R 27.801,72 (36 sessões por ano). A ECT foi a variável de maior impacto no custo direto, enquanto o custo indireto foi determinado pelo nível de escolaridade do paciente. Os custos indiretos corresponderam a 80% do custo total da doença. CONCLUSÕES: Pacientes resistentes a clozapina apresentam maior custo direto. É necessário conduzir estudos de custo-efetividade da ECT de manutenção para uma alocação eficiente de recursos.INTRODUCTION: Schizophrenia is a psychiatric disorder with high morbidity that begins in young adulthood. Despite its relatively low prevalence (0.3%), the disease is responsible for more than 13 million disability-adjusted life years (DALYs) globally. Schizophrenia is associated with high costs, both direct and indirect, due to its severity, chronicity, and loss of productivity. Despite the high impact of the disease on the health system, there are few cost studies conducted in low- and middle-income countries, including Brazil. This study aims to assess the costs of schizophrenia in a specialized tertiary center and to identify the variables with the most significant impact on costs. METHODS: We conducted a cross-sectional, microcosting study from the perspective of the public payer and society to analyze the annual direct and indirect costs of the illness. The dependent variables were the annual direct cost and the total annual cost. The predictive variables evaluated were the demographic profile, clinical characteristics, global functioning, medications, health professionals, and treatment with electroconvulsive therapy (ECT). The indirect cost was calculated using the human capital approach, based on the loss of productivity. RESULTS: Thirty-seven patients were included, of whom 81% (n = 30) were taking clozapine, which is indicated for treatmentresistant schizophrenia, and 10 patients (27%) were undergoing maintenance ECT treatment, reflecting the severity of the patients followed in this service, as the ECT is indicated for clozapine-resistant patients. The average annual total cost of patients was R 24,419.10 to R9,173.68 (range: R34,985.82), with the highest costs associated with patients undergoing ECT. The additional cost of ECT ranged from R 27,801.72 (36 sessions per year). ECT was the variable with the greatest impact on direct costs, while indirect costs were determined by the patient\'s level of education. Indirect costs accounted for 80% of the total cost of the disease. CONCLUSIONS: Patients resistant to clozapine have higher direct costs. It is necessary to conduct cost-effectiveness studies of maintenance ECT for efficient allocation of resources
Going Beyond Counting First Authors in Author Co-citation Analysis
The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation
counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings
are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that
only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into
account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed.
