    Diclofenac vs indomethacin in reducing postoperative inflammation : a randomized clinical trial

    Purpose. To compare the efficacy and tolerability of diclofenac vs indomethacin eyedrops in reducing postoperative inflammation after phaco-extraction and IOL implantation. Methods. 100 patients undergoing phaco through a standard corneal tunnel procedure: 50 were given diclofenac 0.1% eyedrops, while 50 received indomethacin 0.1% eyedrops and were examined 1, 7 and 14 days after surgery. Postop inflammation was graded according to a dedicated scoring system that included evaluation of conjunctival hyperaemia, flare and cells count in the anterior chamber, corneal edema, and ciliary flush. Burning, stinging and blurred vision symptoms at drops instillation were recorded through a questionnaire. Other variables such as IOP, occurrence of surgical complications, duration of surgery, duration and intensity of phaco ultrasounds that might be related to study outcomes were also recorded. Results. Mean inflammatory scores were 4.00 (sd=1.05) and 5.32 (sd=1.64) at day 1, 1.14 (sd=0.45) and 1.61 (sd=0.51) at day 7, and 0.29 (sd=0.16) and 0.33 (sd=0.18) at day 14 for diclofenac and indomethacin respectively. After adjusting for factors related to surgery (i.e. duration of phacoemulsification, duration and intensity of ultrasounds, type of IOL, postop IOP etc.) the difference between the two groups failed to reach statistical significance. Tolerability was good both for diclofenac and indomethacin and no significant difference arised. Conclusions. Diclofenac and indomethacin seem to be both effective in reducing postop inflammation and provide a safe alternative to steroids for the management of the patients undergoing phacoemulsification

    LAUREA MAGISTRALEQuesto lavoro di Tesi Magistrale riguarda la descrizione e l’analisi di un caso studio svolto in Fratelli Branca Distillerie S.r.l., una storica azienda italiana che opera nel campo delle bevande alcoliche dal 1845. Il progetto è stato svolto seguendo la metodologia A3 (uno strumento capace di dare un approccio strutturato all’impostazione e risoluzione dei problemi) e si è focalizzato su due temi principali: il primo riguarda lo studio del processo di apertura delle anagrafiche dei prodotti finiti, il secondo invece è focalizzato sul processo di gestione degli ordini. Lo studio empirico ha previsto lo sviluppo e l’implementazione di contromisure atte a risolvere i problemi emersi durante le analisi iniziali: per quanto riguarda il problema della creazione delle anagrafiche, la proposta fatta dal nostro team è stata quella di creare un modulo standard da condividere attraverso il cloud aziendale per raccogliere tutte le informazioni necessarie alla creazione di nuove SKUs in SAP. In questo modo, è stato possibile standardizzare le operazioni, diminuire il tempo di processo e facilitare l’inserimento di un nuovo responsabile. Per affrontare in maniera alternativa gli stessi argomenti, nel documento è presente uno studio circa la razionalizzazione delle SKUs. L’analisi del processo di gestione degli ordini ha permesso invece di evidenziare come il tempo necessario a controlli di tipo creditizio rallentassero l’intero processo e inoltre, come la mancanza di informazioni scambiate portasse ad un basso livello di servizio al cliente. La contromisura proposta è stata quella di simulare un sistema di tracking che permettesse all’utilizzatore finale (agente) di conoscere lo stato del proprio ordine dal momento in cui questo viene inserito a sistema. Nel report viene inoltre fornito uno studio puramente teorico circa i benefici derivanti dall’anticipazione dei controlli finanziari a monte dell’inserimento ordine. Tutte le proposte sono state testate con progetti pilota, i quali hanno dato prova della propria validità raggiungendo gli obbiettivi precedentemente concordati con l’azienda. In conclusione, l’utilità di questo lavoro è duplice: da un lato è un termine di paragone che potrà essere sfruttato da Branca per lo sviluppo di progetti sperimentali simili, dall’altro potrà servire all’azienda come “testimonianza” e punto di partenza per implementazioni future in questi campi.This Master Thesis concerns the description and analysis of a case study carried out in Fratelli Branca Distillerie S.r.l., an historical Italian company operating in the spirits industry. This project has been developed following the A3 methodology (a tool that provides a structured approach to problem-setting and problem-solving) and it is focused on a customer-centred BPR about two main themes: the first relates to the registry creation process of finished products, the latter is about the order management process. The empirical study has required the development and the implementation of countermeasures to solve problems arose during the initial analyses: as regards to the registry creation topic, the proposal made by our team was to create a standard module to be shared within the company in order to collect all the necessary information to compile the new products registry. In this way, it was possible to standardize the tasks, to reduce the LT process and also to simplify the integration of a new non-expert responsible. In this paper it is also proposed a theoretical study on the SKUs rationalization which only aims to highlight another way to cope these aspects. The study of the order management process has allowed to highlight that the time needed to financial checks slowed down the entire process while the lack of information exchanged with clients led to a low customer service level. The countermeasure we proposed consisted in a tracking simulation that allows customers to know the status of their order from the moment it was placed in the system. Furthermore, to decrease LT process, in this paper is presented an estimation of benefits which may be generated by moving checks before the order uploading. All proposals were tested with pilot projects which have successfully proved the effectiveness of the countermeasure devised by achieving targets previously set. In conclusion, the usefulness of this Thesis work is twofold: on one hand it is a term of comparison that can be used by Branca for the development of similar experimental projects, on the other it may serve to the Company as “evidence” or as starting point for further improvements in these fields

    O presente trabalho investigou como a telemedicina se desenvolveu no Brasil, expondo os obstáculos normativos para essa prática e as demandas sociais e culturais da intersecção entre as áreas da saúde e da tecnologia. Neste estudo foi apontado como os países no globo integram as práticas de telesaúde nas ações profissionais, os limites normativos, a regulação e as ferramentas que permitem o contato entre pacientes e médicos. A segurança de dados nas práticas de telemedicina, a estruturação das teleconsultas e o certificado digital que valida as práticas médicas no ciberespaço também foram analisadas. Através deste trabalho foi possível entender que o mundo contemporâneo está demarcado pela tecnologia de modo que influencia as ações institucionais de cuidado para com a saúde. Novas demandas de atendimento médico mediante o distanciamento social e a mediação tecnológica em vista da pandemia do COVID-19 foram apresentadas. As novas perspectivas da vida social impuseram, seja por seguridade ou por inovação, a intersecção entre a atuação médica e as ferramentas da Tecnologia da Informação no cuidado com o ser humanoThe present work investigated how telemedicine developed in Brazil, exposing the normative obstacles to this practice and the social and cultural demands of the intersection between the health and technology areas. In this study, it was pointed out how countries around the globe integrate the practices of telessaúde in professional actions, normative limits, regulation and the tools that allow contact between patients and physicians. Data security in telemedicine practices, the structuring of teleconsultations and the digital certificate that validates medical practices in cyberspace were also analyzed. Through this work it was possible to understand that the contemporary world is demarcated by technology in a way that influences the institutional actions of health care. New demands for medical care through social distancing and technological mediation in view of the COVID-19 pandemic were presented.
