Diagnóstico do doente em últimos dias ou horas de vida em cuidados intensivos
Relatório de Prática Clínica apresentado à Escola Superior de Saúde Dr. Lopes Dias, do Instituto
Politécnico de Castelo Branco para cumprimento dos requisitos necessários à obtenção do grau de
Mestre em Cuidados Paliativos.O presente relatório de prática clínica é concebido no âmbito do 11º Mestrado em
Cuidados Paliativos, da Escola Superior de Saúde Dr. Lopes Dias, do Instituto
Politécnico de Castelo Branco. Surge como requisito para a obtenção do grau de Mestre
em Cuidados Paliativos, e consiste no registo e reflexão do percurso desenvolvido.
Ao longo do documento pretende-se dar resposta aos seguintes objetivos:
desenvolver experiência e capacidade de reflexão crítica das práticas assistenciais
observadas no período de estágio clínico, junto de uma equipa diferenciada em
Cuidados Paliativos; compreender e aplicar os valores e princípios dos Cuidados
Paliativos na prática assistencial; demonstrar competências na implementação de um
plano assistencial de qualidade à pessoa com doença crónica, avançada e progressiva,
à sua família e ao seu grupo social de pertença, maximizando a sua qualidade de vida e
diminuindo o sofrimento em conformidade com a filosofia dos CP, integrada numa
equipa interdisciplinar; demonstrar capacidade de reflexão ética e crítica na análise de
assuntos complexos inerentes à prática dos Cuidados Paliativos. A reflexão crítica da
prática clínica encontra-se estruturada, em concordância com os pilares dos Cuidados
Paliativos – Comunicação, Controlo de Sintomas, Apoio à Família e Trabalho em Equipa.
Este relatório foi sustentado por uma Revisão Sistemática de Métodos Mistos, de
acordo com a metodologia Joanna Briggs Institute, concretizado sobre “As Dificuldades
em Diagnosticar o doente em Últimos dias ou horas vida em Cuidados Intensivos”, com o
objetivo de produzir evidência científica sobre o tema e partilhar conhecimentos junto
dos pares.
Os Últimos dias ou horas vida num contexto de uma doença crónica, avançada e
progressiva, em Cuidados Intensivos são um desafio constante. Reconhece-se a
importância do diagnóstico precoce, no sentido de adequar o plano de cuidados às
verdadeiras necessidades do doente e família, e a redefinição de objetivos e medidas
terapêuticas, como a suspensão de medidas invasivas desnecessárias, fúteis e
desproporcionais que não trazem qualquer benefício, mas contrariamente só
prolongam a vida e aumentam o sofrimento do doente, família e profissional de saúde.
Este projeto surge da necessidade de melhorar a qualidade dos cuidados prestados
ao doente em Últimos dias ou horas vida e familiares, numa Unidade de Cuidados
Intensivos em concordância com os códigos de ética e filosofia dos Cuidados Paliativos,
que recomendam fazer tudo para aliviar o sofrimento do doente e família, aceitando
irreversibilidade da doença e inevitabilidade da morte, proporcionando dignidade e
conforto, aceitando os limites e a morte com quietude e serenidade.Abstract: The present report of the practice clinic is designed in the ambit of 11º master´s
degree in palliative care of the “Escola Superior de Saúde Dr Lopes Dias, of the
“Instituto Politécnico de Castelo Branco.” Arises as a request to obtaining a master´s
degree in Palliative Care, and consists on registration and reflection of the walkthrough
developed.
Throughout the document its supposed to answer at the next objectives: develop
experience and criticism capacity, criticize the care practices observed on clinic stage
period, united to a prestigious team on palliative care, understand and apply the values
and principles of the Palliative cares in care practice; demonstrate skills in order to
implement a assistance plan of quality to the person with chronic disease, advanced
and progressive, to your family and your social group, maximizing your life quality and
decreasing the pain in conformity to the palliative cares philosophy,integrated into an
interdisciplinary team, demonstrate capacity for ethical reflection in the analysis of
complex issues inherent to the practice of palliative care. The criticism reflection of the
clinic practice is structured, in line with the pillars of Palliative Care- Communication,
Symptom Control, Family Support and Teamwork.
This report was sustained by a Systematic Revision of Mixes methods according to
the methodology Joanna Briggs Institute, realized on “The Difficulties in Diagnosing the
Patient in the Last Hours or Days of Life in Intense Care”, as a objective of scientific
evidence producing on the topic and share knowledge with peers.
The Last Days or Hours of Live about a chronic disease context, advanced and
prolonged, in intensive cares is a constant challenge. It is recognized the importance of
early diagnosis is recognized, in the ambit of tailor the care plan to the real needs of the
patient and family and the redefinition of therapeutic objectives and measures, such as
the suspension of unnecessary, futile and disproportionate invasive measures that do
not bring any benefit, but rather only prolong life and increase the suffering of the
patient,family and health professional.
This project arise from the need to improve the quality of care provided to the
patient in Last Hours or Days of Life and family members, in a Intensive Care Unit in
accordance with the codes of ethics and philosophy of Palliative Care, wich recommend
doing everything to alleviate the suffering of patient and family, accepting
irreversibility of the disease and inevitability of death, providing dignity and comfort,
accepting the limits and death with quietness and serenity
Going Beyond Counting First Authors in Author Co-citation Analysis
The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation
counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings
are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that
only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into
account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed. Reasons for these effects are discussed
Variations on the Author
“Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship
Appropriate Similarity Measures for Author Cocitation Analysis
We provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature. Dispelling the Myths Behind First-author Citation Counts
We conducted a full-scale evaluative citation analysis study of scholars in the XML research field to explore just how different from each other author rankings resulting from different citation counting methods actually are, and to demonstrate the capability of emerging data and tools on the Web in supporting more realistic citation counting methods. Our results contest some common arguments for the continued
use of first-author citation counts in the evaluation of scholars, such as high correlations between author rankings by first-author citation counts and other citation
counting methods, and high costs of using more realistic citation counting methods that are not well-supported by the ISI databases. It is argued that increasingly available digital full text research papers make it possible for citation analysis studies to go beyond what the ISI databases have directly supported and to employ more

We have done our best to complete the author checklist relating to the use of animals in the hut study. Note that the objective for the hut study was to evaluate the IRS treatment applications for residual efficacy against Anopheles mosquitoes, including the local An. coluzzii mosquito population. Cows were only used to attract mosquitoes into the huts and no tests were carried out directly on the cows. The author checklist is intended for use with studies where experiments are carried out on animals, which is why we have had such difficulty in completing this for the hut study, as many of the questions do not relate to how the cows were used
