The Harmonic scalpel (“Focus“ and the new version “Focus+”) is one of the first devices for surgical simultaneous cutting and tissue coagulation which allows to obtain dissection and hemostasis by direct application of ultrasound and allows minimally invasive surgical procedures with minimal lateral thermal spread and, thus, minimal adjacent tissue destruction. 2014 and December2016 and compare the outcome using the Ultrasonic scalpel versus the device Ligasure in term of safety, operative time, overall drainage volume, complications, hospital stay. materials and methods. 250 patients were randomized into two groups: Group A where Ultracision were used and Group B where the Ligasure device was used. Results. The results of the group A and of the Group B 2014-2016 have been compared with the results of the previous study and we found that they are similar, but asymptomatic hypocalcaemia increased in the patients of the new study. conclusions. We found that the use of Ultrasonic scalpel and Ligasure is effective both in the hemostasis of all vessels and in dissection of tissues and confirm the results of our first study without significant difference in the rate of post-operative morbidity with these two different energy based devices used
DOCIMO: an Online Platform Dedicated to the Construction and Quality Management of Learning and Impact Assessments in the Digital Age
The advent of a digital era (Cazeaux, 2014) and the digitization of pedagogical practices offer a favourable context for R&D deployment in evaluation engineering. Assessment methods and tools adapted to new educational environments appear today (Blais & Gilles, 2011, 2012, 2015, Davis, 2015, Fadel, Honey, & Pasnik, 2007). ICT for evaluation thus responds to the needs of learners, trainers and stakeholders in innovative training systems at local, national and international levels (Blais, Gilles & Tristan Lopez, 2015a, 2015b). Concomitantly, quality management penetrates all spheres of human activity and more and more players who attentively pay attention to the quality of the certification processes. These people ask guarantees of seriousness for evaluations in training and education. In this context, Gilles (2002) proposes a cycle of Construction and quality management of standardized tests (CGQTS). Developed with a quality approach, it comprises eight stages: analysis of the objectives of the evaluation; Design of the assessment; Development of questions databases; Information of the assessed persons; Completion of the evaluation; Correction and analysis of results; Production of feedbacks; Macro-regulation to improve future assessments. The CGQTS cycle is the model used by the online Open Source platform "DOCIMO" developed by the spin off Assess Group (www.assess-group.be), a company resulting from a university centre specialized in evaluation engineering. During this presentation, we will make a short demonstration of DOCIMO currently used by a series of partners such as the Swiss Foundation for Fine Watchmaking (FHH) in Geneva, the Faculty of Medicine of the University of Hasselt in Belgium and the University of Teacher Education of State of Vaud. The anti-transglutaminase autoantibodies in children’s saliva with a suspect coeliac disease: clinical study
The coeliac disease is an immune-mediated enteropathy triggered by an ingestion of gluten in genetically susceptible individuals. Like some other systemic diseases (Crohn’s disease, Sjögren’s syndrome) the celiac disease
is able to alter the oral ecosystem and the composition of the saliva. The aim of this retrospective study has been to examine the incidence of coeliac disease (CD) in paediatric population and to search the presence of
anti-transglutaminase auto-antibodies (anti-tTG) in saliva, comparing and quantifying the concentration regard to the serum values of the anti-tTG auto-antibodies, before and after six months from the beginning of the free gluten diet.
105 children (G0), aged between 5 and 13 years, belonging to the Paediatric Gastroenterology-Endoscopy Unit of PTV Hospital, University of Rome “Tor Vergata”, have been examined for a diagnosis of suspected CD. Of a total of 105 pediatric patients (G0), only the 16.2% (G1) has showed to be positive. About the evaluation of the anti-tTG
auto-antibodies in the serum, obtained from the second blood sample (T1), we can observe that 10 (G2) out of 17 children (G1) show positivity and for this reason they have been subjected to a sampling of intestinal villi to confirm
the diagnosis of CD; in addition the 6.7% has been resulted positive at the first sampling of serum (T0), but negative to the second one (T1). The incidence of the CD has been resulted to be equal to 9.5%. About the evaluation of
anti-tTG in the G1, we can observe that 58.8% of children are “definitely positive” to the salivary anti-tTG, while 11.8% appear to be weakly positive. About the correspondence of serum and salivary anti-tTG in Group G1, we can observe, that children positive to the anti-tTG in the serum have also the anti-tTG in the salivary fluid (sensibility 100%, specificity 71.4%). The results show that the anti-tTG salivary are present in children with CD, even though they have continued to follow the gluten
free diet for 6 months. The presence of anti-tTG in the saliva may be considered, an additional and useful diagnostic dental marker for an initial, reproducible, non invasive, inexpensive and highly sensitive screening of CD having a predictive and precocious value compared to
anti-tTG contained in the serum, as it has been already demonstrated
Immediate dentin sealing in indirect restorations of dental fractures in paediatric dentistry
AIM:
At present, two different clinical procedures to ensure the adherence of indirect tooth restorations to the dental tissues are available: a traditional method based on a delayed dentin sealing (DDS) and an innovative approach that contemplates an immediate dentin sealing (IDS). In this study the authors highlight the advantages of the latter method (IDS), decribing the operating phases of this procedure used in paediatric dentistry to perform indirect restorations of dental fractures.
MATERIALS AND METHODS:
The operating phases of indirect composite restorations of dental fractures in paediatric patients are described, introducing an innovative procedure that recommends the immediate application of the dental adhesive (IDS) on the exposed dentin before the subsequent operating phases of tooth preparation, dental impression and adhesive cementation of the restoration.
RESULTS:
The immediate application of the dental adhesive (IDS) on the freshly cut exposed dentin, before taking the dental impression, protects the dental pulp from bacterial contamination and prevents post-operative sensitivity. At the same time, this procedure provides an ideal substrate for formation of a hybrid layer with excellent adhesion properties.
CONCLUSION:
Both methods (DDS and IDS) allow the formation of an adequate hybrid layer to seal the dentin in the interdiffusion area, although SEM images of samples treated with the two methods reveal clear ultrastructural differences between the different interface
