    Management of Mycosis Fungoides With Chlormethine Hydrochloride Gel in Combination With Systemic Therapies: A Case Series

    Background Mycosis fungoides (MF) is a subtype of T‐cell lymphoma that is characterised by the infiltration of malignant T cells into the skin. Treatment approaches usually include skin‐directed therapies for early‐stage disease and, additionally, systemic therapies for advanced stages. Chlormethine hydrochloride gel is recommended as a first‐line treatment option for adult patients with MF, with previous studies demonstrating its efficacy. Due to the rarity of this disease, available literature on the optimal treatment of MF with chlormethine gel is limited, creating challenges for making informed clinical decisions. Thus, we share our individual clinical experiences of selected patients on chlormethine combination treatments to increase the real‐world evidence for chlormethine gel in patients with MF. Case Presentation We present the cases of five male and two female Caucasian patients above the age of 48 with Stage I–IV MF, presenting with symptoms of skin plaques and lesions. All patients were treated with chlormethine hydrochloride gel in combination with other skin‐directed and systemic therapies, including bexarotene, methotrexate, topical steroids, extracorporeal photopheresis, donor lymphocyte infusion and interferon‐α (IFN‐α) 2a. In most cases, chlormethine combination treatment resulted in disease control, e.g., plaque reduction, stable disease, and partial or complete response. The combination regimens were generally well tolerated, with associated adverse events being inflammation, pruritus and erythema. Conclusions This case series reports on the efficacy and safety of chlormethine hydrochloride gel in combination with other topical and systemic therapies in reducing the skin lesion severity in patients with Stage I–IV MF in different real‐world settings.Recordati Rare Diseases https://doi.org/10.13039/10001755

    Going Beyond Counting First Authors in Author Co-citation Analysis

    The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed.     “Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship

    Appropriate Similarity Measures for Author Cocitation Analysis

    We provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature.     Pegylated interferon‐α2a in cutaneous T‐cell lymphoma – a multicenter retrospective data analysis with 70 patients

    Summary Background Interferon‐alpha is an important therapeutic option for the treatment of the cutaneous T‐cell lymphomas (CTCL). Since the approved recombinant interferon‐α‐2a (IFN‐α2a) has no longer been produced since January 2020, pegylated interferon‐α2a (pegIFN‐α2a) can be used as an alternative treatment, even though it is not approved for the treatment of CTCL. The aim of this multicentre study was to generate comprehensive data on the efficacy and tolerability of pegIFN‐α2a in the treatment of CTCL. Patients and Methods A multicenter retrospective study was conducted with 70 patients with CTCL from twelve German skin centers. Results In total, 70 patients were included in the study, with 57.2% male and a mean age of 58.8 ± 14.9 years. Mycosis fungoides was present in 71.4% of cases and Sézary Syndrome in 28.6%. An overall response rate of 55.2% was observed with pegIFNα‐2a therapy. In 50% of cases, therapy was discontinued after 63.6 ± 33.5 weeks. The most common reason for discontinuation was adverse events, which occurred in 68.6% of cases and which were classified as severe in 29.2%. Blood count changes, fatigue and liver toxicity occurred most frequently. Conclusions Our analysis provides comprehensive data on the efficacy and tolerability of pegIFNα‐2a therapy in patients with CTCL. In terms of response rates and side effect profile, pegIFNα‐2a appears to be comparable to IFN‐α2a therapy

    Dispelling the Myths Behind First-author Citation Counts

    We conducted a full-scale evaluative citation analysis study of scholars in the XML research field to explore just how different from each other author rankings resulting from different citation counting methods actually are, and to demonstrate the capability of emerging data and tools on the Web in supporting more realistic citation counting methods. Our results contest some common arguments for the continued use of first-author citation counts in the evaluation of scholars, such as high correlations between author rankings by first-author citation counts and other citation counting methods, and high costs of using more realistic citation counting methods that are not well-supported by the ISI databases. It is argued that increasingly available digital full text research papers make it possible for citation analysis studies to go beyond what the ISI databases have directly supported and to employ more sophisticated methods

    Praxisempfehlungen für die Therapie und Überwachung von Mogamulizumab‐Patienten in Deutschland

    Zusammenfassung Mycosis fungoides (MF) und Sézary‐Syndrom (SS) sind die häufigsten Subtypen der heterogenen Gruppe der kutanen T‐Zell‐Lymphome (CTCL). Mit der Erweiterung der Therapielandschaft sind in den letzten Jahren neue Behandlungsoptionen verfügbar geworden, allen voran der gegen den C‐C‐Chemokinrezeptor 4 (CCR4) gerichtete monoklonale Antikörper Mogamulizumab. Auf der Grundlage der Phase‐III‐Zulassungsstudie wird Mogamulizumab in den deutschen S2k‐Leitlinien für die Zweitlinienbehandlung von SS und MF ab Stadium IB nach mindestens einer vorherigen systemischen Therapie empfohlen. Seitdem wurden durch Post‐hoc‐Analysen und Praxiserfahrungen neue Erkenntnisse zur Sicherheit und Wirksamkeit von Mogamulizumab gewonnen. CTCL‐Experten haben die verfügbare Literatur und eigene Erfahrungen diskutiert und entsprechende Empfehlungen für den Einsatz von Mogamulizumab in der klinischen Praxis in Deutschland entwickelt. Die Empfehlungen umfassen Patientenkriterien, vorherige Therapien, den Einsatz von Mogamulizumab als Mono‐ oder Kombinationstherapie, das Management von Nebenwirkungen, die Dauer der Therapie und die Überwachung von Patienten. Ziel dieser klinischen Empfehlungen ist es, das medizinische Fachpersonal bei der Entscheidungsfindung und dem Einsatz von Mogamulizumab in der täglichen Praxis zu unterstützen

    We have done our best to complete the author checklist relating to the use of animals in the hut study. Note that the objective for the hut study was to evaluate the IRS treatment applications for residual efficacy against Anopheles mosquitoes, including the local An. coluzzii mosquito population. Cows were only used to attract mosquitoes into the huts and no tests were carried out directly on the cows. The author checklist is intended for use with studies where experiments are carried out on animals, which is why we have had such difficulty in completing this for the hut study, as many of the questions do not relate to how the cows were used
