Comparative Characteristics of Metabolic Markers in The Assessment of Bone Health in Postmenopausal Age // Сравнителна характеристика на метаболитни маркери при оценка на костното здраве в постменопаузална възраст
Метаболитният синдром и остеопорозата са два социално-значими здравни проблема, чието разпространение в световен мащаб непрекъснато нараства. За потенциалните връзките между тях отдавна се говори, но резултатите в литературата от клиничните проучвания остават противоречиви.
Оценените в дисертационния труд взаимовръзки на някои кардио-метаболитни и инфламаторни маркери с костния интегритет в постменопаузална възраст разкриват определени зависимости, които могат да бъдат от полза за по-комплексна превенция, профилактика и лечение на остеопорозата. Проведените комплексни анализи предполагат, че сумарната връзка на метаболитния синдром с костното здраве все пак зависи от броя, комбинацията, степента на изява на компонентите на синдрома, както и от съпътстващите го антропометрични и метаболитни промени. От друга се поставя акцент върху необходимостта от своевременното идентифициране и коригиране на негативно свързаните с костния интегритет параметри чрез подходящ хигиенно-диетичен режим и добавяне на медикаменти, които освен кардио-метаболитни ползи притежават и предполагаеми плеотропни скелетни ефекти.Metabolic syndrome and osteoporosis are two socially important health problems. Their worldwide distribution is constantly growing. The potential association between metabolic syndrome and osteoporosis has long been discussed, but the results of the clinical trials remain controversial.
This thesis analyzes the relationships of some cardiometabolic and inflammatory markers with bone integrity in postmenopausal age. The obtained results reveal specific correlations that may be useful for more complex prevention, prophylaxis and treatment of osteoporosis. Furthermore, the conducted analyses suggest that the complex association of the metabolic syndrome with bone health depends on the number, combination, degree of manifestation of each component of the syndrome, and the accompanying anthropometric and metabolic changes. On the other hand, emphasis is placed on the need for timely identification and correction of parameters that are negatively related to bone integrity through an appropriate lifestyle change and initiation of appropriate drugs that have presumed pleiotropic skeletal effects in addition to cardiometabolic benefits
Muñecos históricos sobre la escena literaria.: Las tempranas creaciones poéticas sobre la “Historia de Li y Yang”
The article discusses the early poetic creations on the "Story of Li and Yang", the romance between Emperor Xuanzong of the Tang Dynasty and his concubine Yang Guifei. It analyzes the different perspectives on this issue in some works of the three great poets of Tang: Li Bai, Du Fu and Bai Juyi. The transformations of the "Story…" remind the different facets of the "puppet within the system of culture" as the semiotician Yuri Lotman defines the "cultural object". The puppet serves as a toy, a model and finally as a work of art. Bai Juyi’s creation on the "Story ..." is crucial because he takes it up and engages it in a literary game, in order to save it from oblivion and give it a new life. Through numerous poems, he manages to recreate the romance in a multifaceted way and trace a path to immortality for its two protagonists.El artículo aborda las tempranas creaciones poéticas sobre la “Historia de Li y Yang”, el romance entre el emperador Xuanzong de la dinastía Tang y su concubina Yang Guifei. Se analizan las distintas perspectivas con respecto a este tema en algunas obras de los tres grandes poetas de Tang: Li Bai, Du Fu y Bai Juyi. Las transformaciones de la “Historia…” recuerdan las distintas facetas del “muñeco dentro del sistema de la cultura”, como el semiótico Yuri Lotman denomina al “objeto cultural”. El muñeco se desempeña como juguete, modelo y, finalmente, obra de arte. Resulta clave la labor de Bai Juyi, quien retoma la “Historia…” y la involucra en un juego literario, con el objetivo de salvarla del olvido y darle una nueva vida. Mediante numerosos poemas, él logra recrear el romance de una manera polifacética y traza el camino de sus dos protagonistas hacia la inmortalidad
Bone and visceral adipose tissue - a literature review
The potential associations between obesity and bone health have long been discussed, but the results of the clinical trials remain controversial.It is known that adipose tissue affects bone metabolism through mechanical, hormonal and inflammatory factors, but it is still debated whether the total effect is positive or negative for the bone. Higher body mass is associated with greater mechanical load, which in turn can stimulate the increase in bone mass. Fat accumulation is also associated with increased production of estrogen, which has a positive effect on bone metabolism. Increased secretion of insulin and amylin further suggests a greater anabolic effect on bone tissue. On the other hand, obesity is associated with low-grade inflammation and increased secretion of cytokines such as interleukin-6 (IL-6) and tumor necrosis factor-alpha (TNF-α). However, these cytokines have a negative effect on bone tissue. In addition, decreased circulating levels of 25-hydroxyvitamin D (25OHD) have been observed in obese individuals, which may lead to impaired bone formation with altered quantitative and qualitative bone characteristics. Lower levels of vitamin D further lead to increased levels of parathyroid hormone (PTH), which can separately adversely affect bone metabolism. Additional discussions arise in connection with weight reduction, as dietary restrictions and mechanical changes caused by skeletal unloading during weight loss, as well as the resulting hormonal changes can increase the risk of fractures.

U radu se analizira pristup Europskog suda za ljudska prava predmetima koji se tiču izdvajanja djeteta iz obitelji. Mjera izdvajanja djeteta iz obitelji najintenzivniji je oblik intervencije nadležnih tijela u obiteljski život djece i roditelja koja, s obzirom na relativno široko područje primjene, utječe na različita prava i interese. U radu se razmatraju načelna stajališta Suda o pojedinim aspektima predmeta koji se tiču te mjere kao i njihova primjena u konkretnim slučajevima. Autorica drži da analiza sudske prakse u ovom vrlo osjetljivom području pokazuje da ona može pozitivno utjecati na razvoj pravne kulture, ali samo ako joj se pristupa kritičkiThis paper analyses the European Court of Human Rights' approach to the applications regarding the removal of the child from the family. The measure of removing the child from the family is the most intrusive form of interference of the competent authorities into family life that, considering its broad area of application, affects different rights and interests. This paper considers the Court's general principles on certain aspects of the cases regarding this measure, as well as their application in the concrete cases. The author is of the opinion that the analysis of the Court's case-law in this extremely sensitive area shows that it can have positive effect on the development of the legal culture, but only if it is approached critically
Bone and insulin resistance - a literature review
Insulin resistance (IR) is defined as a decreased tissue response to insulin-mediated cellular activity. Decreased insulin sensitivity and concomitant hyperinsulinemia are known to play a central role in metabolic disorders associated with obesity, metabolic syndrome (MetS) and type 2 diabetes mellitus (DM). In recent years, more and more research has been focused on the relationship between IR and bone health. In vitro, physiological concentrations of insulin have been shown to increase osteoblast proliferation rate, collagen synthesis, alkaline phosphatase production, and glucose uptake and to inhibit osteoclast activity. This evidence indicates that insulin acts as an anabolic agent on bone. It remains to be discussed whether osteoblasts retain their sensitivity to insulin in states of IR. On the one hand, a large number of clinical studies have established that IR and concomitant hyperinsulinemia are positively associated with bone mineral density (BMD), both in individuals with or without overt DM. On the other hand, a number of researchers have found suppressed bone metabolism and decreased bone volume in an IR state. The data on the effect of IR on bone microarchitectonics and bone strength are also controversial. In relation to these data, it remains unclear whether the net effect of IR is favorable or unfavorable for bone health and fracture risk

Kosovo 1989: The (Ab)use of the Kosovo Myth in Media and Popular Culture
The author explores the creation of public opinion in Serbia in the late
1980s and the (ab)use of the 600th anniversary of the Battle of Kosovo on 28 June
1989. As a result of Serbian president Slobodan Milošević’s carefully planned
propaganda, a negative image of Albanians as well as a positive perception of
Serbian nationalism were enforced. The media and popular culture played a
particularly important role in reviving the Kosovo Myth, together with the leading
Serbian (academic) institutions and influential intellectuals. Thirty-some years
after 1989, the Kosovo Myth is presented in the media in a largely unchanged
manner, while for Serbia the Kosovo problem remains unsolved
Application of optic coherence tomography as a screening in prevention and control of diabetic retinopathy
Background. Diabetes mellitus is a leading cause of death, disability, and adult blindness from diabetic retinopathy worldwide. Recent studies have provided a better understanding of the disease process and have opened new doors for prevention and treatment. The aim of our study is to evaluate the effectiveness of the current screening methods for prevention and control of dia-betic retinopathy and to create a program for optimization. Material and methods. We included 123 patients with type 1 and type 2 diabetes mellitus. Clinical characteristics of the population were collected from patients’ files. The HbA1c level was measured. Results. Among examined cohort group – 45% are with diabetes type 1 and 55% with diabetes type 2. In distribution by gender the prevalence is for males. The biggest part of the participants in our study is in the age group 45–66 years old. In 94% of the patients the duration of the disease is between 3–10 years. 22% of the pa-tients are with non-proliferative diabetic retinopathy (NPDR), 45% with proliferative diabetic retinopathy (PDR) and 33% with diabetic macular edema (DME). Conclusion. Our opinion is that the current ophthalmologic devices, such as fundus cameras and OCT (multimodal imaging), should be incorporated in the process of screening for diabetic retinopathy due to the detailed information they provide and the short time to perform the procedure
