Abstract
There is no agreement on the ideal type of surgical management for Achilles tendon rupture. The present randomized prospective study was performed to compare outcome data of open and percutaneous repair in the treatment of Achilles tendon rupture. Forty consecutive patients with acute rupture of Achilles tendon were recruited. Patients were randomized to receive open (group A) or percutaneous repair with Tenolig (group B). All patients followed the same rehabilitation protocol except for slight differences in the duration of immobilization. Follow-up included objective evaluation (at 4 and 12 months), subjective evaluation using the SF-12 questionnaire (at 24 months), and bilateral ultrasound scanning and isokinetic testing (at 12 months). The differences in the parameters evaluated clinically were not significant except for ankle circumference, which was significantly greater in group B. There were two minor complications in the open repair group and one case of failed repair in the percutaneous group. SF-12 questionnaire, ultrasound and isokinetic test data did not show significant differences between the groups. The present study demonstrates that the open and the percutaneous technique are both safe and effective in repairing the ruptured Achilles tendon and that both afford the same degree of restoration of clinical, ultrasound and isokinetic patterns. Medium-term results were substantially comparable. Percutaneous repair is performed on a day-surgery basis, it reduces cutaneous complications and operation times, and enables faster recovery, enhancing overall patient compliance. To us, these characteristics make it preferable to open repair in managing subcutaneous ruptures of Achilles tendon in non-professional sports practicing adults

Un itinerario, condotto con metodo comparativo, al confine tra contratto e torto. Nello scritto si ricostruiscono la nascita e l’evoluzione dell’obbligazione “determinata” di sécurité e della stipulation pour autrui tacite, in Francia, degli obblighi di protezione (Schutzpflichten) e del contratto con effetti di protezione per terzi (Vertrag mit Schutzwirkung für Dritte) nel diritto tedesco, dai primi tentativi in materia di infortuni sul lavoro e di contratto di trasporto alla loro estensione ad ipotesi sempre più varie e diversificate. L’affermazione delle obbligazioni di sécurité, degli obblighi di protezione e di inediti rimedi contrattuali “a favore" di terzi testimoniano una tendenza comune della giurisprudenza europea volta ad attrarre, all’interno di una struttura complessa del rapporto obbligatorio, interessi diversi rispetto alla prestazione, ai quali la disciplina della responsabilità contrattuale può conferire un più intenso livello di tutela
The use of local flaps in the one-step nose reconstruction after cancer resection
PURPOSE: Purpose of this study is to assess different surgical techniques, comparing them in order to remove nasal skin cancer by restoring the anatomy and the function of the respiratory system without neglecting the aesthetic result.
METHODS: A total of 107 patients were enrolled in the study between January 2006 and December 2012. We compared the results obtained using 23 front-glabellar flaps, 30 bilobed flaps, 27 nose-cheek flaps, 15 sliding flaps, 12 frontal island flaps.
RESULTS: We obtained the oncological radicality with good aesthetic results. There were no infections and no sensory (numbness and tenderness) and motor abnormalities of the facial nerve. None of the used techniques has lead to scarring ectropion or to alteration of the nasal filter.
CONCLUSIONS: The best nasal reconstruction mostly depends on skin cancer location and on the amount of tissue to be removed to definitely eradicate the neoplastic lesion; during the planning of a nasal reconstruction we also must assess adjacent tissue characteristics, the presence of fixed structures and the donor skin area (color, thickness, pils, etc) . Using these parameters we have chosen for the ala the nose-cheek flap, for the dorsum the front-glabellar flap, the bilobed flap and the sliding flap (lateral region) and for the tip both the frontal island flap and the bilobed flap
CORRUGATED FEED-HORN ARRAYS FOR FUTURE CMB POLARIZATION EXPERIMENTS.
This work presents the design realization and performance of a 7-element prototype array of corrugated feedhorns
in W-band. The module is fabricated using a so-called “platelet” technique by milling Aluminum
plates. This technique is suitable for low-cost and scalable high performance applications.
Room temperature Return Loss measurements show a low (<-30 dB) reflection over a 20% bandwidth
with a maximum matching of -40 dB at 105 GHz for all seven antennas. Beam pattern measurements
indicate good repeatability and a low (-30 dB) sidelobe levels.
This work is particularly relevant for future Cosmic Microwave Background polarisation measurements,
which require large microwave cryogenic detector arrays coupled to high performance corrugated
feed horns
