    What we know about the scleral profile and its impact on contact lens fitting

    Scleral contact lens fitting has provided practitioners with one of the earliest pieces of evidence that the sclera was more often asymmetric than symmetric. Some preliminary observations such as different haptic alignment patterns over the sclera in 360 degrees, the appearance of fogging in the fluid chamber and excessive tear out-in flow through specific meridians, quadrants, or areas of the haptic provide evidence of some scleral asymmetry. The advent of technologies that allowed measurement of the scleral profile led to formal research confirming that only about 6% of scleras are symmetric, while the rest are toric, quad-specific, or irregular. This has an evident impact on how to focus scleral lens fitting. Measuring the scleral profile also provided us with true ocular sagittal height data for cord diameters beyond the cornea. Although high variability was expected in pathological eyes, healthy eyes also showed a wide range of ocular sagittal heights. Due to this variability among healthy eyes, a discussion has emerged on whether the one-size-fits-all approach to soft lens fitting is a proper strategy to fit the whole spectrum of sagittal heights. The traditional mode of selecting the parameters for custom soft lenses through corneal parameters is also questioned.The author David P.     Intracorneal Ring Segment Implantation for the Management of Keratoconus in Children

    The short-term safety and efficacy of intracorneal ring segment (ICRS) implantation in keratoconus eyes of children are investigated in this study. A retrospective interventional case series study including a total of 33 keratoconus eyes (age 8 to 17 years) that had undergone ICRS (Keraring segments, Mediphacos) implantation was conducted. Information about visual, refractive, pachymetric, corneal topographic and aberrometric, and corneal endothelial changes during a 3-month follow-up were extracted and analysed. A significant improvement was observed in logMAR corrected distance visual acuity (p = 0.005), combined with a statistically significant reduction in keratometric readings (p < 0.001). A reduction in the magnitude of corneal astigmatism of ≥1 D was observed in 52.8% of eyes. No significant changes were observed in corneal endothelial density (p = 0.317). Significant changes were found in the anterior vertical coma component (p = 0.002) as well as in the spherical aberration of the posterior corneal surface (p = 0.004). Only two relevant complications were described: one corneal microperforation with penetration of the ring segment into the anterior chamber (1 eye, 2.8%), and a case of ring extrusion (1 eye, 2.8%).     Short-Term Effect of Wearing of Extended Depth-of-Focus Contact Lenses in Myopic Children: A Pilot Study

    This pseudo-experimental, prospective, and longitudinal pilot study was conducted to characterize the optical and visual changes occurring in the short-term wear of a hydrophilic contact lens (CL) based on extended focus technology (EDOF). A total of 30 eyes of 15 children (age, 6–16 years) were fitted with the EDOF CL Mylo (Mark’ennovy Care SL), performing an exhaustive follow-up for one month evaluating changes in visual acuity (VA), accommodation, binocularity, ocular aberrometry, visual quality, pupillometry, keratometry and biometry. Far and near VA with the CL improved progressively (p < 0.001), obtaining mean final binocular values of −0.08 ± 0.01 and −0.07 ± 0.01 LogMAR, respectively. There was a mean reduction in the accommodative LAG of 0.30 D (p < 0.001), without associated alterations in the magnitude of the phoria and fusional vergences (p ≥ 0.066). A controlled but statistically significant increase (p ≤ 0.005) of ocular high order aberration (HOA) root mean square (RMS), primary coma RMS, primary spherical aberration Zernike term and secondary astigmatism RMS was found with the CL wear. In conclusion, the EDOF CL evaluated provides adequate visual acuity and quality, with associated increased of several HOAs and a trend to reduction in the accommodative LAG that should be confirmed in future studies.The author David P.     Virtual Reality Visual Training in an Adult Patient with Anisometropic Amblyopia: Visual and Functional Magnetic Resonance Outcomes

    A case of an adult with anisometropic amblyopia who underwent a successful vision therapy program playing videogames in a virtual reality environment is described, reporting changes in conventional visual clinical data, as well as in brain activity. The patient was a 22 year old man on baseline examination that never previously wore correction for his anisometropia. After prescribing contact lens correction for the anisometropia and after 44 h of virtual reality-based vision therapy over a period of 1.5 years, the best corrected distance visual acuity (BCDVA) in the amblyopic eye improved from 0.05 to 0.5 (Sloan chart). One year after finishing the visual training, the BCDVA experienced a slight decrease to 0.4 (Sloan chart). Through the visual training, the patient gradually developed stereopsis. Likewise, changes were also detected after visual therapy on functional magnetic resonance imaging while the patient was viewing 2D and 3D stimuli. The preliminary results of this case show the potential of using virtual reality-based visual training as a treatment for adult amblyopia.The author David P.     Optical Impact of Corneal Clearance in Healthy Eyes Fitted with Scleral Contact Lenses: A Pilot Study

    This pilot study was conducted to evaluate the effect on refraction and optical quality of the increase in the corneal clearance after fitting a specific model of scleral contact lens (ScCL) in healthy subjects. A total of 15 eyes from 15 subjects were enrolled in the study, with evaluation of refraction, ocular aberrations and central corneal clearance with the same model of ScCL (ICD Toric, Paragon Vision Science, Gilbert, AZ, USA), but using 3 different sagittal heights: 4200, 4500 and 4800 µm. Mean values of corneal clearance for each ScCL fitted were 418.1 ± 112.1, 706.5 ± 120.3 and 989.9 ± 117.0 µm, respectively. Significant changes were detected in the spherical equivalent and high-order aberrations, especially coma and spherical aberration, when fitting ScCLs of increasing sagittal heights compared to the pre-fitting values. In conclusion, the increase in central corneal clearance when fitting ScCLs affects refraction, leading to a more myopic refractive error, and inducing an increase in different ocular HOAs.     Validation of a modified version of the adult developmental eye movement test

    This study evaluates in terms of reliability, internal consistency, and validity a modification of the Adult Developmental Eye Movement (ADEM) test, ADEM with distractors (ADEMd), designed to analyse oculomotor system, visual processing and visual attentional behaviour. 302 healthy subjects participated in the study (20–86 years old). Intrasession repeatability was evaluated by analysing the correlation between the time needed to read different parts of the test. Inter-session analyses were carried in 40 subjects by calculating intraclass correlation coefficients and using the Bland–Altman method. Validity was assessed in the outcomes obtained according to age as well as investigating the correlation between ADEMd and attentional useful field of vision (UFOV) test. Correlation coefficients between times need to read each sheet were ≥ 0.95 (p < 0.001). The inter-session intraclass correlation coefficient ranged from 0.81 in the horizontal distractor sheet to 0.97 in the vertical sheet. Bland–Altman analysis showed clinically acceptable limits of agreement. Statistically significant correlations were found between age and ADEMd outcomes (r ≥ 0.55, p < 0.001). Processing velocity, divided attention and selective attention measured with the UFOV were correlated with the horizontal distractor times (r ≥ 0.32, p < 0.001). ADEMd test may be a useful clinical tool to evaluate the combined interaction of ocular movements and visual attentional behaviour.The author David P.     Efficacy of Perceptual Learning-Based Vision Training as an Adjuvant to Occlusion Therapy in the Management of Amblyopia: A Pilot Study

    A retrospective study was conducted to evaluate preliminarily the efficacy of perceptual learning (PL) visual training in medium-term follow-up with a specific software (Amblyopia iNET, Home Therapy Systems Inc., Gold Canyon, AZ, USA) for visual acuity (VA) and contrast sensitivity (CS) recovering in a sample of 14 moderate to severe amblyopic subjects with a previously unsuccessful outcome or failure with patching (PL Group). This efficacy was compared with that achieved in a patching control group (13 subjects, Patching 2). At one-month follow-up, a significant VA improvement in the amblyopic eye (AE) was observed in both groups, with no significant differences between them. Additionally, CS was measured in PL Group and exhibited a significant improvement in the AE one month after the beginning of treatment for 3, 6, 12, and 18 cycles/º (p = 0.003). Both groups showed long-lasting retention of visual improvements. A combined therapy of PL-based visual training and patching seems to be effective for improving VA in children with amblyopia who did not recover vision with patching alone or had a poor patching compliance.
