SVILLUPPO DI UN MODELLO SPERIMENTALE PER LA VALUTAZIONE DEL RISCHIO PROGNOSTICO DI MORTE IN PAZIENTI CANDIDABILI A RESEZIONE EPATICA E TRAPIANTO DI FEGATO PER EPATOCARCINOMA
Lo scopo di questo studio è quello di individuare un modello prognostico in grado di valutare
una classe di rischio per i pazienti sottoposti a resezione epatica (LR) ed il loro beneficio di
sopravvivenza rispetto al trapianto di fegato (LT) come trattamento alternativo, inoltre di
creare uno strumento mirato ad aiutare i medici nella strategia chirurgica per la cura di
carcinoma epatocellulare (HCC).
Si tratta di uno studio retrospettivo bi-centrale. Novecentoventisette pazienti sottoposti a LR
per HCC nei 2 centri sono stati collezionati e i loro dati sono stati raccolti prospetticamente in
un database condiviso. Cinquecentoventiquattro pazienti sottoposti a resezione (56,5%) sono
stati considerati anche trapiantabili a causa di età <70 anni e assenza di invasione
macrovascolare pre-operatoria. Analizzando questa serie di pazienti, abbiamo sviluppato un
modello statistico per identificare i pazienti a basso rischio di mortalità dopo LR e pazienti ad
alto basandosi esclusivamente sulle variabili pre-operatorie. Inoltre, una volta i pazienti a
basso rischio sono stati asportati, abbiamo ri-valutato il proprio rischio di mortalità basandosi
su un nuovo modello integrato con le variabili intra e post-operatorie.
La sopravvivenza globale dei pazienti a 5 e 10 anni è stata rispettivamente del 53% (95% CI
48% -58%) e il 30% (95% CI 23% -36%); mentre il 5 e 10 anni la sopravvivenza libera da
recidive è stata rispettivamente del 29% (95% CI 24% -34%) e il 22% (95% CI 17% -27%). La
presenza di cirrosi, AST, alfa-fetoproteina, MELD il punteggio, il numero di noduli, il diametro
del nodulo più grande al momento della diagnosi di tumore sono stati in grado di identificare
una classe a basso rischio e una ad alto rischio pre-operatorio di mortalità dopo la LR. Il
gruppo a basso rischio pre-operatorio conta 262 pazienti con 91 casi di morte dopo un tempo
mediano di 7,4 anni (5.7-11.0 anni) dalla diagnosi di tumore. Il gruppo ad alto rischio preoperatorio
include 262 pazienti con 155 casi di morte dopo un tempo mediano di 3,7 anni
(2.9-5.0 anni) (c-index: 0,67, 95% CI 0,63-0,71). Il gruppo a basso rischio post-operatorio
conta 142 pazienti con 36 casi di morte dopo un tempo mediano di 11,0 anni (anni 9,0-NYR)
dalla diagnosi di tumore. Il gruppo ad alto post-operatorio comprendeva 120 pazienti con 55
casi di morte dopo un tempo mediano di 4,7 anni (4.1-6.7 anni) (c-index: 0,65, 95% CI 0,60-
0,71).
I modelli elaborati risultano fortemente predittivi della probabilità di sopravvivenza dei
pazienti dopo LR per HCC su cirrosi epatica. I pazienti ad alto rischio di morte valutato prima
della LR dovrebbero essere considerati per LT, a causa del loro notevole beneficio di
sopravvivenza a lungo termine. I pazienti che rimangono a basso rischio di morte dopo la
resezione possono essere considerati guariti e la loro sopravvivenza probabilmente sarà
simile a quella ottenibile dopo LT; i pazienti che vedono un rischio convertito da una classe a
basso rischio ad una ad alto post resezione dovrebbero essere considerati per il trapianto abinitio

Going Beyond Counting First Authors in Author Co-citation Analysis
The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation
counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings
are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that
only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into
account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed. Variations on the Author
“Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship
Appropriate Similarity Measures for Author Cocitation Analysis
We provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature. Dispelling the Myths Behind First-author Citation Counts
We conducted a full-scale evaluative citation analysis study of scholars in the XML research field to explore just how different from each other author rankings resulting from different citation counting methods actually are, and to demonstrate the capability of emerging data and tools on the Web in supporting more realistic citation counting methods. Our results contest some common arguments for the continued
use of first-author citation counts in the evaluation of scholars, such as high correlations between author rankings by first-author citation counts and other citation
counting methods, and high costs of using more realistic citation counting methods that are not well-supported by the ISI databases. It is argued that increasingly available digital full text research papers make it possible for citation analysis studies to go beyond what the ISI databases have directly supported and to employ more

Successful Transplant of a Liver Graft After Giant Hepatic Artery Aneurysm Resection and Reconstruction
The shortage of organs has pushed transplant surgeons to accept liver grafts with extended criteria, but severe vascular abnormalities may still discourage the use of otherwise acceptable organs. We report herein the case of a liver graft with a 64-mm aneurysm of the proper hepatic artery extended to the origin of the right and left hepatic branches. The graft was deemed unsuitable for transplant by all other centers in the region. However, liver function tests were normal, and there was no evidence of compromised arterial supply. At back table, we resected the aneurysm and anastomosed the right and left hepatic arteries to a vascular graft obtained from the distal tract of the donor's superior mesenteric artery. After portal reperfusion, we anastomosed the mesenteric graft to the recipient's hepatic artery at the origin of the gastroduodenal artery. The postoperative course and the subsequent 6-month follow-up were uneventful. In conclusion, the presence of a hepatic artery aneurysm should not be an absolute contraindication to the use of a liver graft. The present case emphasizes the possibility to utilize an organ that would have been otherwise discarded
