    Il presente progetto di ricerca dottorale ha preso spunto dalle esperienze di formazione aziendale che hanno avuto luogo in diverse realtà sanitarie nazionali ed aventi come obiettivo la sensibilizzazione del personale infermieristico alla tematica del contatto, nel contesto di cura.L’oggetto di questa tesi è una ricerca sull’efficacia di un intervento formativo di sensibilizzazione rivolto a infermieri impiegati in varie strutture ospedaliere italiane e sulle loro interpretazioni del contatto interpersonale nel contesto di cura. Il disegno di ricerca prevede un mixed method per andare ad integrare, allo studio e misurazione degli atteggiamenti verso il contatto, attraverso scale di valutazione, anche uno sguardo qualitativo alle interpretazioni degli infermieri nei riguardi del contatto. Gli obiettivi specifici di questo studio sono stati: osservare i possibili cambiamenti o tendenza nei confronti del contatto nell’agire assistenziale, del gruppo degli infermieri formati; esplorare le interpretazioni, descrizioni e concetti degli infermieri (formati e non) sulla tematica del contatto nell’ambito professionale. Nella prima pèarte della tesi viene introdotto il tema del contatto nelle relazioni di cura. In questo capitolo si è voluto partire dal corpo e dal senso del tatto; essendo la professione infermieristica, sia per formazione che per mandato, più vicina al corpo del paziente, è stato fondamentale offrire una prima riflessione sul senso del tatto e, da qui, l’entrare in contatto con l’altro. Da un excursus filosofico e scientifico sulle principali teorie, che rimandano alla multidimensionalità di questo senso, si esplora con una rassegna bibliografica il contatto nel contesto di cura e nello specifico sulle ricerche presenti in letteratura fino ad oggi. Alla ricerca sul contatto interpersonale si aggiunge uno sguardo alla storia e concettualizzazione della professione infermieristica oggi in Italia; partendo dai cambiamenti importanti degli ultimi tre decenni, i nuovi bisogni formativi e nuove figure assistenziali. Uno spazio viene dedicato alle organizzazioni sanitarie, descrivendo il contesto in cui l’infermiere vive e le possibilità di apprendimento che questi contesti offrono. Nel capitolo verrà introdotto il costrutto di ‘comunità di pratica’ come possibile risorsa nelle aziende per implementare progetti formativi e sostenere l’infermiere nella sua vita lavorativa. In Italia esistono realtà dove la comunità di pratica infermieristica è stata implementata, ne è un esempio il gruppo “Affettività delle Cure Infermieristiche” che coltiva la pratica del contatto e del massaggio (offerto ai pazienti) nel Policlinico Tor Vergata di Roma. Il gruppo (ed altri esempi in letteratura, di comunità di pratica) e la pratica del massaggio hanno una funzione protettiva per il professionista nei confronti dello stress lavoro correlato (work-related stress) mentre sta aumentando l’interesse, da parte delle aziende sanitarie, per offrire sostegno e prevenzione al dipendente che si ammala per prendersi cura degli altri. La professione infermieristica è in maniera ricorrente prona alla compassion fatigue (fatica dell’esser compassionevoli) e al burn out e, tra le strategie possibili per migliorarne il coping, esistono in letteratura degli approcci che mirano a far emergere le riflessioni sull’aspetto più umano della cura e un ritorno alle radici del nursing, in modo da restituire il senso della cura all’infermiere. Una possibilità di sostegno all’infermiere viene anche dalle pratiche dove il contatto viene esercitato non esclusivamente come task (come per eseguire un compito) ma come momento d’incontro e di relazione; la pratica del massaggio, e ancor più la preparazione per il massaggio, offrono un buon esempio di pratica infermieristica che cura sia il paziente che il professionista stesso attraverso un entrare in relazione differente, creando occasioni d’incontro con l’altro. Le pratiche di contatto per offrire comfort fisico e supporto anche emotivo sono presenti in letteratura ma sono ancora scarse le ricerche; le evidenze presenti al momento riguardano l’impatto del massaggio nel ridurre il burn-out e la compassion fatigue (Wilson, 2016) mentre ci sono evidenze su quanto l’essere a proprio agio nel contatto con il paziente (e con l’altro) sia un fattore preventivo del burn out (Pedrazza et al, 2015). Da qui emergono riflessioni sulle potenzialità di una formazione che abbia come obiettivo portare sensibilità nel contatto per la professione infermieristica. Lo studio ha portato ad entrare in contatto il ricercatore con diverse realtà ospedaliere italiane. La prima fase (quasi-sperimentale) ha mostrato tra i risultati una tendenza degli infermieri, che hanno partecipato alla formazione, ad avere una maggior attenzione ai propri comportamenti di contatto interpersonale, nel contesto assistenziale, successivamente alla formazione. La seconda fase (Focus group) dello studio ha permesso di approfondire i risultati della prima fase dando anche senso a delle ambiguità emerse tra gli item, nello specifico sulla gestualità di cura più o meno consapevole. La ricchezza delle interpretazioni del contatto emerse nei sei focus group ha offerto possibili risposte ai quesiti iniziali della ricerca e identificato aspetti specifici valorizzati dagli infermieri riguardo al contatto, in particolare alla gestualità di conforto e supporto emotivo agita dall’infermiere. In conclusione, l’intero studio restituisce una rappresentazione del contatto agito dall’infermiere, come ‘gestualità di cura’ che si caratterizza per essere di riconoscimento, di sostegno del mondo emotivo dell’altro e spunto per riflessioni sulla pratica. Una gestualità intesa non più come secondaria rispetto al tocco task-orientated, ma che occupa un suo posto nel repertorio delle competenze professionali infermieristiche e come strumento fondamentale, a disposizione del professionista, nelle relazioni di cura dove, l’entrare in contatto con il paziente, appartiene al set delle pratiche quotidiane. Questo lavoro di tesi porta a ragionare sull’importanza di questo tema e sulle potenzialità di una maggior formazione al contatto interpersonale. In più, apre alla possibilità di andare a riconoscere e costruire competenza e saperi taciti finora poco esplorati ma che possono aiutare nell’impasse vocazione-professione, nel quale spesso si viene a trovare l’infermiere contemporaneo

    “Awareness to touch”: a qualitative study of nurses’ perceptions of interpersonal professional contact after an experiential training

    Abstract Aim: This study aimed to explore nurses’ perceptions of touch in their professional practice and how these perceptions were articulated in discourse, among participants who attended a specific training on touch and those who did not. Background: Touch is an essential part of nursing practice. Research showed that the use of touch influences patients' general well-being, improving a sense of presence and infusing security and encouragement. Nurses’ attitude towards touch influence positively their job satisfaction and reduces burnout syndrome risks. Nevertheless, there are very few studies describing specifically nurses’ perceptions and opinions about the use of interpersonal physical contact in the clinical setting. Also, an educational perspective focusing on touch seems missing from nursing curricula and research. Design: A qualitative methodology was adopted. Focus groups were organized to explore how nurses define and interpret touch and its relevance in their professional practice. The findings 1 Journal Pre-proof obtained in a previous quantitative step of this research inspired the formulation of the questions posed to nurse participants. Methods: Six focus groups were conducted in three Italian hospitals, which also hosted a one-day training on touch. For each hospital, a purposive sampling approach was used to organize two focus groups: one was made up of nurses who attended the training, and one was made up of nurses who did not. Qualitative thematic analysis was used to identify recurring patterns of meaning by which touch is discussed and interpreted in the focus groups. Results: Registered nurses (N = 39) from different clinical experiences participated in six focus groups. The analysis identified four themes: 1) Touch preferences, 2) Touch and Contact as a natural dimension, 3) Touch as a “praxis” and 4) Reflexivity on Touch. All six focus groups showed a general awareness of interpersonal touch and discussed it as a highly valorized (personal and professional) dimension. Specifically, the trained nurses showed a greater variety and richness of their lexicon, a semantic and imaginary repository by which they described their experience and competence. Conclusions: This study highlights that nurses consider touch as an important, essential part of their practice. Moreover, it suggests that specific training on interpersonal touch in nursing care facilitate nurses to shift their perceptions from a generic, abstract and “personal” dimension, into an articulated, disciplined and specialized practice. This has implications for education on touch in nursing

    Going Beyond Counting First Authors in Author Co-citation Analysis

    The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed. Reasons for these effects are discussed

    “Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship

    Appropriate Similarity Measures for Author Cocitation Analysis

    We provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature.     A Delphi survey of health education system and interprofessional nurse' role

    Abstract Background Interprofessional education in healthcare academic and professional training is renowned to improve collaborative culture. International studies showed the existence of obstacles to establishing interprofessional collaboration and the relevance of Nurses' role in the implementation process. Aim This study was conducted to explore interprofessional collaboration practice and education perceptions, opinions and awareness of healthcare professionals, such as academics, professional bodies representatives and multidisciplinary team managers. Methods A multi-method two-stage approach using: 1) explorative survey and 2) Delphi group technique. A survey questionnaire focusing on interprofessionality in practice and education was administered to a convenience group of students and academics from health degree courses of three universities. Delphi group panellists were selected from a list of experts from three areas (n = 169). The iterative Delphi technique implied three-rounds to reach panel consensus (or not) about the main research topics, starting from expert panel opinions about survey results. Results The study witnessed nurses' overall large participation (60–75%). Survey results (n = 198) showed participants' willingness to implement interprofessional education programs but controversial visions of how to implement interprofessional culture in healthcare settings. The Delphi survey showed experts' (n = 25) convergent opinions about introducing elements of communication skills and interprofessional culture into academic curricula and improving the presence of non-medical professions among the academic body. Nurses showed ambiguous positions towards concepts of autonomy and shared responsibility. Conclusions Our study highlighted interprofessional education implementation obstacles and possible enablers. Nurses' controversial positions may reflect the struggle of the nursing profession to reach permanent academic positions and to support the shift from a medical-centric to a person-centred model of care

    The Fundamentals of Nursing Care Pillar

    Fundamentals of care, the first of seven pillars of learning, is underpinned by a contemporary movement that refocuses attention on nursing and on the essential knowledge, skills, and values required for person-centred care. In 2012, a group of international nurse leaders initiated a debate, an action plan and a fundamentals of care framework. The framework provides nurses with a roadmap to help navigate through the elements of the fundamentals of care model. An anonymised scenario details the journey of a patient, Jay, through intensive care. The application of the fundamentals of care framework illustrates how this is an integral part of nursing practice, including in high-tech healthcare settings. In this chapter, crucial aspects of intensive care nursing, care-recipient nurse relationships and practice-related issues will be illustrated and analysed with the support of evidence. The chapter concludes by offering structured reflections focusing on the integration of the scenario and the framework into a realisable care narrative

    Dispelling the Myths Behind First-author Citation Counts

    We conducted a full-scale evaluative citation analysis study of scholars in the XML research field to explore just how different from each other author rankings resulting from different citation counting methods actually are, and to demonstrate the capability of emerging data and tools on the Web in supporting more realistic citation counting methods. Our results contest some common arguments for the continued use of first-author citation counts in the evaluation of scholars, such as high correlations between author rankings by first-author citation counts and other citation counting methods, and high costs of using more realistic citation counting methods that are not well-supported by the ISI databases. It is argued that increasingly available digital full text research papers make it possible for citation analysis studies to go beyond what the ISI databases have directly supported and to employ more sophisticated methods
