A retrospective study of children with multisystemi nflammatory syndrome and intravenous immunoglobulin treatment
Objectives
To compare the outcomes of multisystem inflammatory syndrome in children (MIS-C) associated with severe acute respiratory syndrome coronavirus 2 infection treated with intravenous immunoglobulin (IVIG) and without IVIG.
Patients and methods
A retrospective observational study was conducted over a period of 2 months. We enrolled all children (1–18 years, both sexes) who presented with coronavirus disease 2019-associated MIS-C (as per WHO definition). The patient classification and treatment were done as per the PGIMER Chandigarh protocol (IVIG along with corticosteroids and aspirin was given in all cases with moderate, severe disease, and Kawasaki disease phenotype). Mild cases did not receive IVIG but received corticosteroids and aspirin. Respiratory support was given in cases with breathing difficulty. The primary outcomes of the children were noted in terms of ICU stay, requirement of ventilation, requirement of High flow nasal cannula (HFNC), and nasal oxygen. Response with respect to inflammatory parameters and coronary diameters were also assessed as secondary outcomes.
Results
Among 52 patients enrolled in the study, 25 had mild, 17 had moderate, eight had severe disease, and two had Kawasaki disease phenotype. Among 27 cases requiring IVIG, seven cases did not receive IVIG. In children who received IVIG (n = 20), there were higher odds of ICU stay [odds ratio (OR) 1.185], ventilation (OR 8.784), need for CPAP (OR 5), and HFNC (OR 3.75) but all of these were statistically insignificant (P > 0.05). Among the inflammatory markers, there was a significant reduction in the C-reactive protein levels (23.3 vs. 5, P = 0.004, OR 1.028) with the use of IVIG. There was no significant difference in the change in the D-dimer levels (1480 vs. 449.38, P = 0.148) and coronary artery diameter after the treatment.
Conclusion
The changing evidence about the use of IVIG producing similar outcomes to those not using IVIG suggests that corticosteroids and aspirin can alone be used in cases of MIS-C irrespective of the severity of the disease. However, considering the significant reduction in the inflammatory marker (C-reactive protein) after the use of IVIG in moderate to severe cases, its administration may not totally be negated

Spectroscopic Analysis of Zno Nanocomposite from Green Leaf Extract of Hibiscus Rosa Silensis and Its Biopotentials
Introduction: Zinc oxide (Zno) Nanoparticles have gained significant attention due to their unique physicochemical properties and wide applications in various fields, including medicine, environmental science, and agriculture.
Objectives: The objective of the current research is to synthesize ZnO nanoparticles from Hibiscus rosa- sinensis and characterize them using analytical techniques like UV, FTIR, and XRD, and their bio applications.
Methods: The Synthesis of ZnO nanoparticles is carried out by the precipitation method, and characterization of nanoparticles using analytical techniques like UV (FTIR Bruker) and XRD, and antibacterial activity is carried out by agar-agar method.
Results: The Characterization of Zno Nanoparticles by UV spectroscopy are observed at 250–320 nm and FTIR spectroscopy is strong C=O stretching at 1676 cm⁻¹ suggests key compounds flavonoids, ketones, or carboxylic acids, XRD spectroscopy suggests that the peaks appeared at 2θ value ranging from 31.73°, 34.38°, 36.22°, 47.50°, 56.56°, 62.81°, 66.34°, 67.91°, 69.03,72.6° and 76.90° values
corresponds to pure Zno.
Conclusions: The green synthesis of nanoparticles using plant extracts has emerged as an eco-friendly, cost-effective, and sustainable approach. This research study revealed that the synthesis of Zno nanoparticles using the green leaf extract of Hibiscus rosasinensis as a reducing and stabilizing agent. The synthesized nanoparticles were characterized using various analytical techniques like UV, FTIR, and XRD, and their biological applications, including antibacterial, were investigated
Distribution of ABO blood groups and Rh(D) factor in and around Anatapuramu, Andhra Pradesh
Background: India is a vast country with lot of diversity in race, religion and creed. The same diversity has been observed in geographical distribution of blood groups in population within country. ABO and Rh blood groups are most important blood groups in human beings. Objectives: A retrospective study was conducted at Government Medical College/ Government General Hospital Blood Bank Anantapuramu over a period of one year from January 2014 to December 2014 to determine and compare the distribution of ABO and Rh blood groups.
Materials and Methods: A retrospective study of one year was carried out at our blood bank in Government Medical College & Hospital, Anantapuramu in Andhra Pradesh, South India. Data pertaining to the blood groups of donors was collected from the Blood Bank donor register from January 2014 to December 2014.
Results: The total donors studied from January 2014 to December 2014 were 6942. The distribution of blood groups was : blood group ‘A’ 1386 ( 20 % ), ‘B‘ 2489 ( 35.8 % ), ‘AB’ 509 ( 7.3 % ) and ‘ O ‘ 2558 ( 36.9 % ). In both Rh D positive and Rh D negative person’s blood group ‘ O ‘ was the commonest followed by blood group ‘ B ‘ Blood group ‘ AB ‘ is the least common.
Conclusion: The “O” blood group is significantly high in our population and comparatively low “AB” blood group. Every transfusion center should have a record of frequency of blood group system in their population. The study of distribution of blood group is very important for blood banks and transfusion services that could contribute to the patients’ health care

Anti-N-Methyl-D-Aspartate-Receptor Encephalitis Following Herpes Simplex Virus Encephalitis – Presenting as a Pediatric Patient with Abnormal Movements and Psychiatric Manifestation
Herpes simplex virus (HSV) encephalitis remains one of the more severe forms of childhood encephalitis. The clinical course of HSV encephalitis is usually monophasic but occasionally complicated by a clinical relapse, causing biphasic illness. Sometimes relapse may be due to secondary immune mechanisms. Anti-N-methyl-D-aspartate (NMDA) receptor encephalitis is an example of immune-mediated relapse following herpes encephalitis. This is still underrecognized, with probable grave consequences if not treated early. Our index case was an 8-year-old male child with severe generalized choreoathetosis and behavioral disturbances, presented 4 weeks after documented HSV encephalitis. Cerebrospinal fluid (CSF) was positive for anti-NMDA receptor antibodies. The diagnosis was confirmed and treated with intravenous (IV) immunoglobulins (2 g/kg) and IV pulse-dose methylprednisolone (30 mg/kg) for 5 days. Other supportive management included tablet revocon 25 mg (tetrabenazine) ¼ tablet BD, clonazepam 0.25 mg one tablet BD, and syrup sizodon (risperidone) 0.5 mg BD. The patient is on regular follow-up and is recovering well but slowly. Patients with relapsing symptoms following HSV encephalitis or prolonged atypical symptoms, with repeat CSF-negative polymerase chain reaction for HSV should routinely be tested for NMDA receptor immunoglobulin G antibodies in CSF and/or serum. It is important to be aware of this differential diagnosis because patients respond well to immunotherapy
lntestinal health after pelvic radiotherapy - towards understanding persisting pathophysiological mechanisms
Pelvic cancer survivors struggle with life-long symptoms caused by radiotherapy. Many pelvic cancer survivors live with a constant fear of leaking faeces and the need to always have a toilet within reach. Some of these survivors are forced to retire early and live in social isolation. Radiotherapy saves lives, but radiation-induced damage to intestinal tissue close to, or within, the radiation field causes permanent changes in the cancer survivor’s bowel habits. Our goal was to understand the pathogenic mechanisms that continue to cause symptoms even decades after the treatment.
By irradiating the mouse colorectum using the clinic’s linear accelerator for radiotherapy, we have developed a novel model that mimics the human pathophysiological condition in the irradiated colorectal mucosa. The model was used to investigate whether the intake of dietary fiber, known to promote intestinal health, could modulate radiation-induced normal-tissue damage (Paper I). Our findings revealed that dietary fiber modified several intestinal pathophysiological processes and repair mechanisms following irradiation, most notably preventing a late, possibly chronic, bacterial invasion and reducing signs of inflammation. Dietary fiber appeared to also modulate parameters of neurogenesis in the brain following radiotherapy-induced injury to the colorectum, suggesting a connection between intestinal health after radiotherapy and brain health (Paper II). To translate the preclinical findings on inflammation to clinic, a biopsy-study was performed in cancer survivors up to 20 years after pelvic radiotherapy (Paper III). A low-grade chronic intestinal inflammation was observed after pelvic radiotherapy, as evidenced by an elevated neutrophil presence and activity. Inadequate mucus protection was also found, possibly leading to increased pathogen infiltration. These results challenge the notion that pelvic radiotherapy causes acute intestinal inflammation that either heals, or causes tissue to become fibrotic without further inflammation.
In conclusion, this thesis demonstrates that the mucosal resilience to radiotherapy may be modulated by quite simple means, such as dietary approaches. Moreover, protecting intestinal health after radiotherapy may also protect brain health. The thesis also demonstrates the presence of a chronic, low-grade inflammation in the intestinal mucosa after pelvic radiotherapy, possibly driven by infiltrating bacteria due to a poor mucus protection. The findings can have implications for future approaches to safeguarding intestinal health while providing effective pelvic cancer therapy
