N07 When the body tires, the mind resists:resilience as a buffer against fatigue-insights into quality of life in Crohn's disease

    No full text
Background: Fatigue is one of the most burdensome and under-recognised symptoms in Crohn’s disease (CD), often persisting even in remission and profoundly affecting physical, psychological, and social functioning (Radford et al., 2021; Regueiro et al., 2023). As fatigue is only partially explained by disease activity, psychological resources such as resilience may play a crucial protective role . Resilience, defined as the ability to adapt and recover from adversity, may help mitigate the negative impact of chronic illness on health-related quality of life (HRQoL) (Jin et al., 2023). This study aimed to explore the relationships between fatigue, resilience, and HRQoL in adults living with CD.Methods: A cross-sectional survey was conducted among 50 adults with a confirmed diagnosis of CD. Three validated instruments were used: WHOQOL-BREF to assess HRQoL, the Inflammatory Bowel Disease Fatigue Scale (IBD-F) to measure fatigue, and the Resilience Scale (RS-14) to evaluate psychological resilience. Descriptive statistics and non-parametric correlations (Spearman’s rho) were performed to explore associations between variables. Results: Fatigue showed a strong negative correlation with both HRQoL and resilience. Patients reporting higher levels of fatigue experienced lower satisfaction with life, poorer psychological and social functioning, and greater difficulties in coping with daily stressors. Conversely, higher resilience scores were positively associated with HRQoL, suggesting that resilience may buffer the adverse effects of fatigue. Conclusion: Fatigue is a key determinant of reduced QoL in CD, while resilience appears to be an important adaptive resource supporting psychological well-being and daily functioning. Incorporating resilience-building interventions and psychosocial assessment into routine nursing practice could enhance patient well-being and complement medical treatment in IBD care.References:Jin, Y., Bhattarai, M., Kuo, W. N40 Stress after an ulcerative colitis diagnosis:examining factors alleviating or exacerbating stress in ulcerative colitis using Interpretative Phenomenological Analysis

    No full text
Background: Stress significantly affects disease flares and quality of life in people with ulcerative colitis (UC), with over 25% experiencing symptoms of anxiety or depression irrespective of disease activity.1–3 However, there is limited research on the specific stressors introduced by a UC diagnosis. This study aimed to explore how stress is experienced following diagnosis and identify contributing factors. Methods: Ten UK-based patients diagnosed with UC within the past six years (seven within the last year) participated in semi-structured online interviews exploring stress and coping during their first post-diagnosis year. Participants (7 female and 3 male; aged 16–76) were predominantly white (n=9). All had elevated scores on Cohen’s Perceived Stress Scale prior to interview.4 The interviews were video-recorded and analysed using Interpretative Phenomenological Analysis to capture both shared and divergent aspects of their unique lived experiences.5Results: Analysis revealed a complex and evolving relationship between stress, resilience, and UC, captured in four themes:I. Rethinking health - Health became effortful and unpredictable, shaped by a tension between personal experience and others’ interpretationsII. Rethinking identity - A lifelong diagnosis disrupted self-identity, social roles, and life plans, often leading to feelings of body estrangementIII. Adapting life - Participants were compelled to adjust their behaviour and daily routines, acquire new knowledge and skills, and rely more on othersIV. Dealing with complex emotions - Emotional and physical health were closely linked, with stress and worry heightening distress but also motivating efforts to conserve emotional energy and seek positive experiencesIllustrative quotations for each theme are provided in Table 1.Conclusion: These findings enhance understanding of the stressors introduced by a UC diagnosis and the process by which individuals respond. While participants developed personal resilience to stressors, the role of others (including healthcare professionals) was pivotal in either alleviating or exacerbating stress. There is a pressing need to improve emotional and practical support for people newly diagnosed with UC and their close networks. Crucially, the collective knowledge and lived experience of people with UC should be actively harnessed, not only to guide and support those newly diagnosed, but also to inform and improve clinical practice and wider support services.References:Black J, Sweeney L, Yuan Y, Singh H, Norton C, Czuber-Dochan W. Review article: description and management of fatigue in inflammatory bowel disease

    No full text
BackgroundFatigue is a common and leading concern for patients with inflammatory bowel disease (IBD). It is managed inadequately in IBD, and there is little evidence to support interventions.AimTo examine patients’ experience of and factors contributing to IBD-fatigue, and to appraise its management by patients and healthcare professionals.MethodsSeven electronic databases were searched. Subject headings and free-text searching were used, with no time limit set. Reference lists of retrieved papers were searched manually.ResultsTwenty-eight papers were reviewed. Researchers used terms including ‘fatigue’, ‘low energy’, ‘tiredness’, ‘decline in vitality and vigour’ and ‘reduced energy and vitality’. Different definitions were used to conceptualise fatigue. None of the reviewed studies asked patients to describe the experience of fatigue in their own words. Numerous physical, psychological and situational factors associated with fatigue were identified. Three small randomised control trials reported a favourable effect of infliximab and adalimumab on fatigue. One intervention study reported benefit from a stress management programme and one from solution-focused therapy.ConclusionsInconsistent use of terminology, lack of data from patients’ perspective on inflammatory bowel disease-fatigue and lack of evidence to support its management contribute to fatigue being largely ignored or overlooked by healthcare professionals. Future research should explore the experience of IBD-fatigue from the individual patient perspective. Further studies are required to fully explore the factors associated with fatigue and to develop patient-centred interventions to reduce fatigue

The author shares his reflections on state of art in housing and urban planning, deficiencies, expectations and possibilities in the Sahel region of Africa. He notices, that the housing problem in Africa is one of the challenges, which should be solved in order to recover life dignity of African people and secure their rights to traditional family life in acceptable conditions. The paper describes the studies on the typical dispersed urban structures and the need to foster this type of settlement structure and proposals of introduction of new on light steel frame housing system in the area of Sahel, combining the traditional way of building houses with modern technology. The particularly analysed case, is the housing problem in the Republic of Chad. The author presents the basic discussion on this topic and his architectural proposals. Unfortunately, the gap between the needs and the financial feasibility of housing construction in this area, makes this project already at the starting point extremely difficult to be realized without external subventions.
